Purpose: Robotic surgery is known to provide an improved technical ability as compared to laparoscopic surgery. We aimed to compare the efficiency of surgical skills by performing the same experimental tasks using both laparoscopic and robotic systems in an attempt to determine if a robotic system has an advantage over laparoscopic system. Methods: Twenty participants without any robotic experience, 10 laparoscopic novices (LN: medical students) and 10 laparoscopically-experienced surgeons (LE: surgical trainees and fellows), performed 3 laparoscopic and robotic training-box-based tasks. This entire set of tasks was performed twice. Results: Compared with LN, LEs showed significantly better performances in all laparoscopic tasks and in robotic task 3 during the 2 trials. Within the LN group, better performances were shown in all robotic tasks compared with the same laparoscopic tasks. However, in the LE group, compared with the same laparoscopic tasks, significantly better performance was seen only in robotic task 1. When we compared the 2 sets of trials, in the second trial, LN showed better performances in laparoscopic task 2 and robotic task 3; LE showed significantly better performance only in robotic task 3. Conclusion: Robotic surgery had better performance than laparoscopic surgery in all tasks during the two trials. However, these results were more noticeable for LN. These results suggest that robotic surgery can be easily learned without laparoscopic experience because of its technical advantages. However, further experimental trials are needed to investigate the advantages of robotic surgery in more detail.
금속고정판의 제거시기가 악교정 수술후 조기 골격성 회귀에 미치는 영향에 대한 연구

The early skeletal relapse after surgery was much influenced by clockwise rotation of the proximal segment. The purpose of this study is to prove statistcally that the removal of fixation plate at postoperative 2 weeks with active excerise of month opening could reposition not only both proximal and distal segment to its original position but also reduce skeletal relaspse. All subject comprised 35 patients underwent mandibular setback via BSSRO in Kyunghee medical center by a surgeon. We divided the sample into two groups. In group 1,the fixation plate was maintained and that was removed at postoperative 2 weeks in groups 2. Cepalometric radiograph were obtained before surgery, within a week after surgeru, two months postoperatively. All radiograph were traced and analyzed by the same investigator and were serially superimposed on the reference lines, anterior and posterior cranial base structures.We measured the changes of both proximal & distal segment and performed statistical analysis used paired t test(p<0.05). The results obstained are as follows that the proximal segment was displaced posteroinferiorly(clockwise fashion)at postsurgery in both group and repositioned toward its orgional position during follow-up periods,especially in group 2. And the distal segment was more displaced anterirly in group 1 that didn't remove the fixation plate. In conclusion, the clockwise rotation of proximal segment is a sugnificant factor reflecting early skeletal relapse. The early removal of fixation plate after BSSRO could reposition the proximal segment which displaced postoinferiorly to physiologic position and reduce early skeletal replapse.
