Objective We hypothesized that a multi-tier response (MTR) will provide high-quality cardiopulmonary resuscitation including airway management. However, the type of tier response system and airway management will have different interactive effects resulting in varying outcomes following out-of-hospital cardiac arrest (OHCA). This study aimed to determine whether the advanced airway management method has an effect on OHCA outcomes and to compare the size of the effect across MTR types. Methods This is a retrospective population-based observational study using the Korea OHCA Registry. Airway management methods were categorized into endotracheal intubation (ETI) and supraglottic airway (SGA) groups. The tier system was categorized into single-tier response (STR) or two types of MTR: ambulance-ambulance MTR or fire engine-ambulance MTR. Results In total, 45,264 patients were analyzed among the 89,087 emergency medical service assessed OHCAs. The SGA group was significantly associated with a lower prehospital return of spontaneous circulation (ROSC) rate compared to the ETI group (adjusted odds ratio [aOR], 0.79; 95% confidence interval [CI], 0.72–0.88). Both MTR with an ambulance or fire engine were significantly associated with higher prehospital ROSC rates compared to STR (STR vs. MTR with an ambulance: aOR, 1.33; 95% CI, 1.21–1.47; STR vs. MTR with a fire engine: aOR, 1.43; 95% CI, 1.20–1.71). Prehospital SGA was significantly associated with poor neurological outcomes in MTR with fire engine (aOR, 0.71; 95% CI, 0.53–0.96). Conclusion In this nationwide observational study, we observed that MTR was associated with higher prehospital ROSC than STR. Moreover, SGA is associated with a lower prehospital ROSC rate regardless of tier response type compared to ETI.
Korean version of the Cough Symptom Score: clinical utility and validity for chronic cough

Background/Aims: The Cough Symptom Score (CSS) is a simple, useful tool for measuring cough severity. However, there is no standard Korean version of the CSS. We developed a Korean version of the CSS and evaluated its clinical utility and validity for assessing chronic cough severity. Methods: The CSS was adapted for Korean use following a forward-backward translation procedure. Patients with chronic cough enrolled from five university hospitals were graded using the CSS and a 100-mm linear visual analog scale (VAS) of cough severity at each visit. Patients completed the Leicester Cough Ques-tionnaire (LCQ) upon presentation and completed the LCQ and Global Rating of Change at follow-up visits after 2 to 4 weeks. The concurrent validity, repeatabili-ty, and responsiveness of the Korean version of the CSS were determined. Results: Correlation coefficients between the CSS and LCQ, and between the CSS and VAS, were -0.66 and 0.52, respectively. There was a weak correlation between the scores for night and day symptoms (r = 0.24, p = 0.0006). The repeatability of the CSS in patients with no change in cough (n = 23) was high (intra-class cor-relation coefficient, 0.75; 95% confidence interval [CI], 0.51 to 0.88). Patients who reported an improvement in cough (n = 30) at follow-up visits had a significant improvement in the CSS (median, -2; 95% CI, -3 to -1; p = 0.0003). Conclusions: The Korean version of the CSS correlated well with other tools for accessing cough severity in chronic cough patients. Therefore, it could be a reli-able method for measuring chronic cough severity.
AKAP12 regulates vascular integrity in zebrafish

The integrity of blood vessels controls vascular permeability and extravasation of blood cells, across the endothelium. Thus, the impairment of endothelial integrity leads to hemorrhage, edema, and inflammatory infiltration. However, the molecular mechanism underlying vascular integrity has not been fully understood. Here, we demonstrate an essential role for A-kinase anchoring protein 12 (AKAP12) in the maintenance of endothelial integrity during vascular development. Zebrafish embryos depleted of akap12(akap12 morphants) exhibited severe hemorrhages. In vivo time-lapse analyses suggested that disorganized interendothelial cell-cell adhesions in akap12 morphants might be the cause of hemorrhage. To clarify the molecular mechanism by which the cell-cell adhesions are impaired, we examined the cell-cell adhesion molecules and their regulators using cultured endothelial cells. The expression of PAK2, an actin cytoskeletal regulator, and AF6, a connector of intercellular adhesion molecules and actin cytoskeleton, was reduced in AKAP12-depleted cells. Depletion of either PAK2 or AF6 phenocopied AKAP12-depleted cells,suggesting the reduction of PAK2 and AF6 results in the loosening of intercellular junctions. Consistent with this, overexpression of PAK2 and AF6 rescued the abnormal hemorrhage in akap12 morphants. We conclude that AKAP12 is essential for integrity of endothelium by maintaining the expression of PAK2 and AF6during vascular development.
노인 환자에서 Dapsone 투여 후 호전된 IgA-associated Leukocytoclastic Vasculitis

We report a case of elderly patient with IgA-associated leukocytoclastic vasculitis who was successfully treated with dapsone. A 75-year-old Korean female patient who had suffered from asthma visited because of multiple purpura, vesicle, and bullae on her extremities. She also complained of pitting edema, especially on the lower extremities. She was diagnosed as IgA-associated leukocytoclastic vasculitis by skin biopsy and treated successfully with steroid. How-ever, skin lesions recurred after tapering steroid. We restarted high-dose steroid and the lesions showed improvement. However, skin lesions were refractory to lower-dose steroids, and also refractory to colchicine treatment. Dapsone (50 mg daily) induced dramatic improvement of her cutaneous vasculitis, proteinuria and edema. (Korean J Asthma Allergy Clin Immunol 2007;27:186-189)

In this study, we investigated how to fabricate flexible silicon memory using thinned die. Thinning of silicon wafers has become common practice in the packaging industry for use in stacked die packages. Wafer thinning is also being used in the development of 3-D wafer stacking. this technology can also include building very thin and flexible electronics. Silicon wafer thinned to 30 ㎛ and less is flexible allowing the fabrication of flexible and conformable electronics. thus, we would like to suggest how to fabricate flexible silicon memory by using this technology. A key for achieving these techniques is the thinning of wafer to a thickness of 30 ㎛ or thinner, the handling of thinned die and the electrical interconnection between thinned die and flexible substrate.
Fast Mobile IPv6 에서의 mSCTP를 이용한 handoff 지연 감소 기법

무선 네트워크와 모바일 기술의 발달로 무선상에서의 통신뿐만 아니라, 모바일 기기의 이동성에 대한 요구가 점점 더 커지고 있다. 모바일 기기의 이동성을 지원하기 위해Fast Mobile IPv6 (FMIPv6), Hierarchical Mobile IPv6 (HMIPv6)와 같은IPv6기반의 네트워크 계층 프로토콜이 제안되어 왔으며, 최근에는 트랜스포트 계층에서 동작하는 mobile Stream Control Transport Protocol (mSCTP) 가 제안 되었다. 네트워크 계층에서의 이동성을 지원하기 위한 방안 중 하나인 FMIPv6 의 핸드오프 과정은 엑세스 라우터간 양방향 터널링을 설정 함으로서 엑세스 라우터의 버퍼링 오버헤드가 발생하고, 터널링과정을 위한 메시지 교환으로 인해 핸드오프 지연이 발생하게 된다. 본 논문에서는 FMIPv6 의 이러한 단점을 해결하기 위해 트랜스포트 계층의 이동성 지원 방안인 mSCTP 를 이용한 통합 핸드오프기법을 제안한다. 양단의 세션을 유지한 채 새로운 IP 주소를 등록 할 수 있는 mSCTP 를 사용 함으로서 FMIPv6 의 터널링으로 인해서 발생하는 엑세스 라우터의 오버헤드와 핸드오프 지연 시간을 줄이고 전체적인 핸드오프 성능을 향상 시킨다.

The Korean Association of Medical Colleges (KAMC) developed graduate outcomes based on “The role of Korean doctor, 2014” to serve as guidelines regarding outcome-based education in Korea. The working group in this study analyzed 65 competencies proposed in “The role of Korean doctor, 2014” according to the developmental principle that certain outcomes should be demonstrated at the point of entry into the graduate medical education. We established 34 competencies as “preliminary graduate outcomes” (PGOs). The advisory committee consisted of 11 professors, who reviewed the validity of PGOs. Ultimately, a total of 19 “revised graduate outcomes” (RGOs) were selected. We modified the RGOs based on opinions from medical schools and a public hearing. In November 2017, the KAMC announced the “graduate outcomes for basic medical education,” which serves as a guide for basic medical education for the 40 medical schools throughout Korea. Medical schools can expand the graduate outcomes according to their educational goals and modify them according to their own context. We believe that graduate outcomes can be a starting point for connecting basic medical education to graduate medical education.
