      • 중소병원의 처방전달시스템 도입효과분석에 관한 연구 : S병원의 사례를 중심으로 Focused on the case of S Hospital

        The purpose of this study is to ascertain whether the effect of introduction of OCS(Order Communication System) to the hospital is satisfied or not comparing the anticipated effect with the actual effect. For this purpose, a domestic hospital which has introduced and has been operating OCS for several years was chosen. Based on the internal data of S Hospital prepared before introducing OCS, researcher has analyzed the basic direction, design standard and status of operation after the introduction of OCS, etc. After analyzing the status of operations of several departments using OCS and interviewing with the chiefs of pertinent departments, a survey form was designed. Actual survey and interviews were conducted by the researcher for weeks to know whether doctors, nurses, m e d d technicians and clerks of the patient management dept. were satisfied with OCS and to find if they have any recommendations to improve OCS. Based on the analysis of survey, the effect of OCS was evaluated whether it has satisfied the anticipated effectiveness. For the question if they feel convenient in using OCS, doctors, nursing staffs in charge of ward and the staffs of billing dept. has answered that they were all satisfied(lOO%) The answers for the same question were relatively hgh in the case of nurses in charge of outpatient and staffs of radiography. Of course, there have been some nurses and staffs who complained for the inconvenience. However, overall satisfaction was high on the average. Some common problems occurred after the introduction of OCS were frequent errors due to instability of OCS system, paralysis of function of hardware on data back-up system and redundant investment due to erroneous choice of DB program in seting DB. It was also pointed out that lack of computer education and low participation of medical staffs has resulted in failure of developing effective software. As a result, it has lowered the efficiency of OCS. For example, some works have to be done by hands even after OCS. Based on the result of this research, recommendations to maximize the effect of OCS were presented as follows. First, strong leadershp of CEO and active cooperation of doctors are mandatory. Second, all the process of hospital work should be analyzed and be redesigned in more efficient ways. Third, OCS should be designed to be user-based system which can be used efficiently by all staffs of the hospital. Forth, prior to the operation of OCS, proper tests of the program and trainings of the pertinent staff are required. Fifth, prior to the selection of hardware, BMT(Bench Marking Test) should be conducted. Sixth, before introducing OCS, staffs in charge of OCS should visit many hospitals operating the OCS system and take their cases into account.

        처방전달체계에 대한 사용자 만족도에 영향을 미치는 요인

        Objectives : To identify the factors affecting users' satisfaction with the Order Communicating System(OCS) and to highlight the factors important for the successful establishment of OCS. Methods : A Users Satisfaction survey was sent to 4,513 people, consisting of 1,503 doctors, 2,379 nurses, 255 pharmacists and 370 administrative workers in 16 hospitals which had introduced OCS. The response rate was 63.9%. Measurement of users' satisfaction was peformed with the instrument which was used in Doll's study. Some aspects of Doll's instrument were adjusted according to the aims of this study. The classifying sections of this survey included age, job and status classification, computer experience, OCS education, duration of daily OCS use, type of order entering, number of personnel in the Hospital Information System's department, cost of OCS, problem frequency, proportion of work managed by hand, OCS type, and Hospital establishment type. Results : There was a positive correlation between satisfaction level and managerial status throughout all job classifications. Irrespective of the importance of OCS education as a factor relating to users' satisfaction, the additional work load caused by OCS lowered users' satisfaction. Different factors affected users' satisfaction according to job and status classification. The composition of factors affecting the pharmacist and administrative worker satisfaction levels was simpler than that of the doctor and nurse levels. There were no statistically significant differences between the actual computer experience duration of daily OCS use and users' satisfaction with OCS. Conclusions : There was an understandable relationship between users' attitude to OCS and factors affecting users' satisfaction. The results of this study could be used as a basis for the successful expansion of the operation of OCS. But more detailed studies on users' satisfaction and further improvements of methodologies are required for the successful establishment of OCS.

      • 응급의료센터에서의 처방전달시스템(OCS)사용 실태 및 평가

        병원정보시스템은 현재 의료기관에서 다방면의 목적 및 용도로 사용을 하고 있으며 그 유용성이 널리 알려졌다. 또한 일부 의료기관에서는 처방전달시스템(OCS)을 사용함으로써 병원업무의 획일화, 간편성, 연속성, 효율성 등을 높히고 있으며 사용자들이 매우 편리하게 활용을 하고 있는 추세이다. 한편 응급의료센터에서도 이런 시스템의 도입으로 진료업무 및 간호업무, 원무업무 등에서 간편하게 사용을 할 수 있으며 업무시간 단축 및 경제성, 효율성을 높힐 수 있다고 사료되며 이를 위한 프로그램 개발 및 보완을 위해 많은 노력을 하고 잇다. 영남대학교의료원 응급의료센터에서도 적극적으로 OCS를 도입하여 사용을 함으로써 그 효용성을 알 수가 있었고 사용자에 대한 면담조사 결과에서 나타난 바와 같이 몇몇 수정, 보완을 하고 사용자의 적극적인 노력과 훈련이 계속되면 더욱 발전된 OCS를 구축할 수 있을 것으로 기대하며 향후 더욱 편리하고 유용한 시스템의 개발과 보급으로 타 의료기관의 응급의료센터와도 호환을 할 수 있고 환자정보교환 등을 가능케 하여 응급의료체계 구축과 확립에 이바지할 수 있을 것으로 사료된다 Hospital information system has been widely used and increased recently for a variety of many aspects. And Order-Communicating system(OCS), as like as hospital information sytem , has been used in many medical care facilities, which is simple and easily accessible, useful system. Also then the use of OCS in emergency care center in YUMC has been introduced since 1996. 10, above 70% of availability is noticed at present and increasing in the use rate, in considered that it is very simple and accurate, time-saving , widely applicable system. So authors say that, after the use of OCS in emergency care center, interhospital exchange of the patient's information and also accomplishment of EMSS can be possible.

      • 인터넷 기반 제어 시스템 구현에 관한 연구

        오늘날 제어시스템들은 자동화가 요구되는 분야가 날로 광범위해지고 기능도 전문화됨에 따라서 제어 시스템 통합을 단순화할 수 있는 개방형 시스템이 등장하고 있다. 본 연구에서는 제어시스템의 개방화, 즉 HMI, 제어기, 그리고 제어 통신망의 이더넷 도입 경향에 따라 HMI는 제어용 언어 SFC와 Visual C++, OS는 소프트 실시간을 보증하고 네트워킹에서 가장 안정적인 Window NT, 제어기는 펜티엄 PC, 통신망은 산업용 이더넷을 이용한 개방형 제어시스템(OCS)을 설계하고 구현한다. OCS는 FA과 PA에서 많이 이용되고 있는 레벨제어 시스템과 온도제어 시스템으로 구성되며, 제안된 제어구조의 유효성을 나타내기 위하여 제어알고리즘과 실험적 결과를 제시한다. These days, most of control system requires automazation in the all fields. Therefore OCS which can simplify control system integration is appearing. In this study, OCS consists of HMI, OS, Controller, and Communication network. (SFC and visual C++ are used as HMI, window NT is used as OS, pentium PC is used as Controller, and industrial Ethernet is used as Communication network.) Finally, We applied OCS to Level control system and Temperature control system, and verified its efficiency.

      • 외부의료영상의 PACS 등록 시 OCS상 FTP Module의 유용성

        This study was conducted to improve performance flow of PACS registration and output of external medical images by developing or adding File Transfer Protocol(FTP) Module on Order Communication System(OCS) and to upgrade efficiency of performance. The numbers of requests for examination of external medical images from January 2004 to December 2007 were investigated and the numbers of cases of examinations and losses were done respectively from January to June, 2007 as a period before the improvement and from July to December, 2007 as a period after it. Satisfaction of persons inputting external medical images of CD was asked directly before and after the improvement. When the numbers of requests for examination of external medical images from 2004 to 2007 2004 were investigated, they were 12,783, 16,774, 21,613 and 27,487 cases in 2004, 2005, 2005 and 2007 respectively. For the numbers of the requests from January to December, 2007 it was the lowest in February by recording 1846 and it was 2800 in October. While the number of loss of external medical images was averagely 3.6 before using FTP Module on OCS, it reduced to 0 and satisfaction increased to 98% after it. By utilizing FTP Module on OSC for PACS registration of external medical images, the images were delivered simultaneously with occurrence of order, PACS registration of many patients was conducted together and CDs were returned to patients immediately after sending images from outpatient clinics or wards to remove possibility of losing them. In addition, because the module was established on OCS and data were sent associated with order, problems related with order were decreased. By eliminating movement distance it was turned out to be very effective in terms of time and space.

        동결 건조법을 이용한 옥천산 캡슐 제형 개발에 관한 연구

        Objective : Okchun-San(OCS) is known as an effective herbal medicine on Type 2 diabetes. We performed to change OCS formulation for freeze drying capsulation. Methods The mixtures of OCS were extracted with water. finally, the filtered solution were evaporated and lyophilized to dry granules. The various ratio of excipients were studied to determine the formation for capsulizing. The samples were inspected for any difference in color, taste and appearance. Results: The prepared form of OCS were dried and weighted 260kg. The lyophilized dry powder yielding 40kg. The suitable ratio of OCS-dry powder and excipient was 10:1. The average weights of On and placebo capsules were $440{\pm}5.28mg$, $465{\pm}7.95mg$, respectively. There was no notable change in color and appearance for both capsuled samples throughout the study period. Conclusions: Therefore in can be concluded that freeze drying capsulation is appropriate form of OCS.

        Stability and Fragmentation of OCS2+ Studied by Using Auger-Electron-Ion Coincidence Measurement

        An Auger-electron-ion coincidence method has been applied to investigate dissociation of OCS^2+ states with two valence holes, which are populated via Auger decay following S 2p photoionization of OCS. We found that OCS^2+ states lying around a binding energy of 32 eV are mostly stable against dissociation, which indicates that the potential energy surfaces of these OCS^2+ states have deep wells around the Franck-Condon region. In contrast, the OCS^2+ states lying around 34 eV and 42 eV dissociate predominantly into CO^+ + S^+ and O + C^+ + S^+, respectively.

        OCS 운영병원에서의 병동 약반납 업무 현황분석과 개선방향

        Information about returned-medications after preparing as prescribed may serve as a part medication history, and to patients that information may be associated with increased medical expenses. Therefore, it is very important that such information is handled rapidly and accurately among departments involved in a hospital. Furthermore, accurate medication administration history serves as a basic information pharmacists needs in order to expand the role of a pharmacist as patient-oriented drug therapy specialist. In Samsung Medical Center (SMC), the information about returned medications is done by written communication and the information delivered to pharmacists is inaccurate. So the purpose of this study was to analyze work related to returned medications in hospitals with Order Communication System (OCS). First the problems with work related to processing returned-medications were identified. Based on this result, questionnaires were sent by mail to 50 tertiary hospitals all over the country. Finally, based on these, a new model for handling returned-mediations has been proposed. In SMC, the biggest problem in work associated with returned-medications was inaccurate information on slips for returned-medications. This problem could not be solved by short-term education in such matter. Of 50 hospitals where questionnaires were mailed 50% replied. and among these hospitals, 60% were with OCS. But in most hospitals with OCS, inaccuracy of slip was not a problem, because slip was done by on-line program and not a hand-written one. However, accurate patient information was not shared between wards and pharmacy as in SMC. The workflow related to returned medications could be divided into two types; however they presented with different patterns of cost loss. A poor appreciation by pharmacist regarding workflow associated with returned-medications and a difficulty in sharing information between wards and pharmacy were listed as common problems. The workflow related to returned medications must be managed by pharmacists. Thus we need to develop a new model for processing returned medications with accuracy and appropriate information.

        As occupant injuries induced by airbag deployment had become a critical issue, revisions to FMVSS 208 were made to mandate the adoption of advanced airbag which can protect occupants of varying statures. As a result, OCS(occupant classification system) has become an important part of advanced airbag technologies. In this paper, we review the existing OCS technologies briefly and list the details of development issues and solutions for weight-based OCS. As an effort to enhance performance, a study on design optimization for the current system using Genetic algorithm is used and mainly discussed.
