        Correlations Between Fasciology and Yin Yang Doctrine

        The aim of this study is to explore the correlations between fasciology and yin yang doctrine. Professor Yuan developed fasciology by three-dimensional reconstruction of connective tissue (fascia) in the trunk and limbs of the human body and tracing back to tissue origins in light of biological evolution and developmental biology. Fasciology states that the human body can be divided into two systems: the supporting-storing system and the functional system. This article elaborates on the roles of the two systems and their mutual relationship. The two systems are used to analyze the yin,the yang, and their relationship. The two systems are promoted but also restricted in different contexts. The supporting-storing system is formed by undifferentiated connective tissue and provides undifferentiated cells and nutrients for differentiated cells of the functional system. Thus, the supporting-storing system could be classified as quiet, similar to yin. The functional system continuously maintains the various functional activities of the human body. Thus, the functional system could be classified as active, similar to yang. In interpreting the yin yang doctrine from the point of view of fasciology, yin can be compared with the supporting-storing system and yang can be compared with the functional system.

        Efficacy and Safety of Ultrasound-Guided Radiofrequency Ablation for Primary Hyperparathyroidism: A Prospective Study

        Objective: To assess the efficacy and safety of ultrasound (US)-guided radiofrequency ablation (RFA) in patients with primary hyperparathyroidism (PHPT). Materials and Methods: This prospective study enrolled 39 participants (14 male, 25 female; mean age, 59.5 ± 15.3 [range, 18–87] years) between September 1, 2018, and January 31, 2021. All participants had parathyroid lesions causing PHPT, proven biochemically and through imaging. The imaging features of the PHPT nodules, including the shape, margin, size, composition, and location, were evaluated before treatment. Serum intact parathyroid hormone, calcium, and phosphorus levels; parathyroid nodule volume; and PHPT-related symptoms were recorded before and after treatment. We calculated the technical success, biochemical cure, and clinical cure rates for these patients. Complications were evaluated during and after the ablation. Results: Complete ablation was achieved in 38 of the 39 nodules in the 39 enrolled participants. All the patients were treated in one session. The technical success rate was 97.4% (38/39). The mean follow-up duration was 13.2 ± 4.6 (range, 6.0–24.9) months. At 6 and 12 months post-RFA, the biochemical cure rates were 82.1% (32/39) and 84.4% (27/32), respectively, and the clinical cure rates were 100% (39/39) and 96.9% (31/32), respectively. Only 2.6% (1/39) of the patients had recurrent PHPT. At 1, 3, 6, and 12 months after technically successful RFA, 44.7% (17/38), 34.3% (12/35), 15.8% (6/38), and 12.5% (4/32) of participants, respectively, had elevated eucalcemic parathyroid hormone levels. Recurrent laryngeal nerve paralysis occurred in 5.1% (2/39) of the patients, who recovered spontaneously within 1–3 months. Conclusion: US-guided RFA was effective and safe for PHPT patients. RFA may be an alternative treatment tool for patients who cannot tolerate or refuse to undergo surgery.

        Interfacial micromechanics modeling for bolted joints in ultra-precision machine tools

        Bolted joints are widely adopted in various mechanical structures. However, the uncertainty and complexity of the contact surfaces affect accurate modeling. Herein, an interfacial micromechanics model is proposed to characterize the contact behaviors of bolted joints in an ultra-precision machine tool (UMT). The bolted joint is modeled as a virtual material layer with transversely isotropic behaviors, whose constitutive relation is identified by fractal theory. The proposed model accurately formulates the multi-scale contact characteristics and allows convenient integration with the structural dynamic modeling of UMT. Dynamic analysis of the simulation results and the experimental measurements in the time and frequency domains was conducted to identify the most sensitive factor affecting the tool tip vibration. On this basis, the structural optimization for the UMT was further implemented, which effectively reduced vibration by 15.5 %. The dynamic predictions were validated by modal and vibration response tests.

        Circular RNA cFAM210A, degradable by HBx, inhibits HCC tumorigenesis by suppressing YBX1 transactivation

        Hepatitis B protein x (HBx) has been reported to promote tumorigenesis in hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC), but the mechanism awaits further investigation. In this study, we found that cFAM210A (a circular RNA derived from the third exon of transcript NM_001098801 of the FAM210A gene; CircBase ID: hsa_circ_0003979) can be silenced by HBx. cFAM210A expression was downregulated and negatively correlated with tumorigenesis in patients with HBV-related HCC. Furthermore, cFAM210A reduced the proliferation, stemness, and tumorigenicity of HCC cells. Mechanistically, HBx increased the N6-methyladenosine (m6A) level of cFAM210A by promoting the expression of RBM15 (an m6A methyltransferase), thus inducing the degradation of cFAM210A via the YTHDF2-HRSP12-RNase P/MRP pathway. cFAM210A bound to YBX1 and inhibited its phosphorylation, suppressing its transactivation function toward MET. These findings suggest the important role of circular RNAs in HBx-induced hepatocarcinogenesis and identify cFAM210A a potential target in the prevention and treatment of HBV-related HCC.

        Effect of Surface Waviness on the Performances of an Aerostatic Thrust Bearing with Orifice-Type Restrictor

        In this paper, four waviness types are presented to investigate the effect of surface waviness on the static performance, dynamic performance and stability of an aerostatic thrust bearing with orifice-type restrictor. The influences of wave amplitude and rotational speed on the bearing performances are studied. The Reynolds equation is first solved through small perturbation method to obtain the static and dynamic characteristics. Subsequently, by adopting an iterative method based on the acquired dynamic coefficients, a numerical method is proposed to analyze the bearing stability. The calculated critical supply pressure is compared with that obtained by the traditional orbit method and it indicates that the proposed method has higher computational efficiency. The results show that the surface waviness has great influence on the bearing performances and the effect becomes increasingly pronounced with the growth of wave amplitude. The performances are more sensitive to the waviness arranged in bearing radial direction than circumferential direction. The enhancement of certain performance is always accompanied by the decline of another performance for the bearing with wave pattern on the surface, which leads to the fact that the bearing performances cannot be comprehensively improved in static characteristic, dynamic characteristic and stability. Besides, high rotational speed will cause pneumatic instability more readily. Finally, a stiff ness measuring experiment and a modal test are performed to verify the accuracy of computational analysis and results.

        The role of ginsenoside Rb1, a potential natural glutathione reductase agonist, in preventing oxidative stress-induced apoptosis of H9C2 cells

        Background: Oxidative stress-induced cardiomyocytes apoptosis is a key pathological process inischemic heart disease. Glutathione reductase (GR) reduces glutathione disulfide to glutathione (GSH) toalleviate oxidative stress. Ginsenoside Rb1 (GRb1) prevents the apoptosis of cardiomyocytes; however,the role of GR in this process is unclear. Therefore, the effects of GRb1 on GR were investigated in thisstudy. Methods: The antiapoptotic effects of GRb1 were evaluated in H9C2 cells by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide, annexin V/propidium iodide staining, and Western blotting. Theantioxidative effects were measured by a reactive oxygen species assay, and GSH levels and GR activitywere examined in the presence and absence of the GR inhibitor 1,3-bis-(2-chloroethyl)-1-nitrosourea. Molecular docking and molecular dynamics simulations were used to investigate the binding of GRb1 toGR. The direct influence of GRb1 on GR was confirmed by recombinant human GR protein. Results: GRb1 pretreatment caused dose-dependent inhibition of tert-butyl hydroperoxide-induced cellapoptosis, at a level comparable to that of the positive control N-acetyl-L-cysteine. The binding energybetween GRb1 and GR was positive ( 6.426 kcal/mol), and the binding was stable. GRb1 significantlyreduced reactive oxygen species production and increased GSH level and GR activity without altering GRprotein expression in H9C2 cells. Moreover, GRb1 enhanced the recombinant human GR protein activityin vitro, with a half-maximal effective concentration of z2.317 mM. Conversely, 1,3-bis-(2-chloroethyl)-1-nitrosourea co-treatment significantly abolished the GRb1’s apoptotic and antioxidative effects of GRb1in H9C2 cells. Conclusion: GRb1 is a potential natural GR agonist that protects against oxidative stresseinducedapoptosis of H9C2 cells.

        Changes in Glucose Metabolism after Adrenalectomy or Treatment with a Mineralocorticoid Receptor Antagonist for Primary Aldosteronism

        Background: Data on the effects of excess aldosterone on glucose metabolism are inconsistent. This study compared the changes in glucose metabolism in patients with primary aldosteronism (PA) after adrenalectomy or treatment with a mineralocorticoid receptor antagonist (MRA). Methods: Overall, 241 patients were enrolled; 153 underwent adrenalectomy and 88 received an MRA. Fasting glucose, homeostatic model assessment of insulin resistance (HOMA-IR), and homeostatic model assessment of β-cell function (HOMA-β) were compared between the treatment groups after 1 year. Plasma aldosterone concentration (PAC) and factors determining HOMA-IR and PAC were evaluated. Results: No baseline differences were observed between the groups. Fasting insulin, HOMA-IR, and HOMA-β increased in both groups and there were no significant differences in fasting glucose following treatment. Multiple regression analysis showed associations between PAC and HOMA-IR (β=0.172, P=0.017) after treatment. Treatment with spironolactone was the only risk factor associated with PAC >30 ng/dL (odds ratio, 5.2; 95% confidence interval [CI], 2.7 to 10; P<0.001) and conferred a 2.48-fold risk of insulin resistance after 1 year compared with surgery (95% CI, 1.3 to 4.8; P=0.007). Conclusion: Spironolactone treatment might increase insulin resistance in patients with PA. This strengthened the current recommendation that adrenalectomy is the preferred strategy for patient with positive lateralization test. Achieving a post-treatment PAC of <30 ng/dL for improved insulin sensitivity may be appropriate.

        Recent advances on Pestalotiopsis genus: chemistry, biological activities, structure–activity relationship, and biosynthesis

        Strains of the fungal genus Pestalotiopsis are reported as large promising sources of structurally varied biologically active metabolites. Many bioactive secondary metabolites with diverse structural features have been derived from Pestalotiopsis. Moreover, some of these compounds can potentially be developed into lead compounds. Herein, we have systematically reviewed the chemical constituents and bioactivities of the fungal genus Pestalotiopsis, covering a period ranging from January 2016 to December 2022. As many as 307 compounds, including terpenoids, coumarins, lactones, polyketides, and alkaloids, were isolated during this period. Furthermore, for the benefit of readers, the biosynthesis and potential medicinal value of these new compounds are also discussed in this review. Finally, the perspectives and directions for future research and the potential applications of the new compounds are summarized in various tables.

        Post-Acute Care as a Key Component in a Healthcare System for Older Adults

        Older adults often experience functional decline following acute medical care. This func-tional decline may lead to permanent disability, which will increase the burden on the medical and long-term care systems, families, and society as a whole. Post-acute care aims to promote the functional recovery of older adults, prevent unnecessary hospital readmission, and avoid premature admission to a long-term care facility. Research has shown that post-acute care is a cost-effective service model, with both the hospital-at-home and community hospital post-acute care models being highly effective. This paper describes the post-acute care models of the United States and the United Kingdom and uses the example of Taiwan’s highly effective post-acute care system to explain the bene-fits and importance of post-acute care. In the face of rapid demographic aging and smaller household size, a post-acute care system can lower medical costs and improve the health of older adults after hospitalization.
