      • 최근 5년간 급성 A형 간염으로 입원한 환자들에 대한 입원 내용 분석

        Abstract Background : Due to improvements in Korea's sanitary conditions, antibody rates to the hepatitis A virus is decreasing rapidly and admission due to the disease is increasing, Patients who were admitted for hepatitis A during the last 5 years were selected and yearly differences in admission, clinical picture, medical fees, and differences according to age and gender were analyzed. Methods : Patients who were admitted to the Seoul Soonchunhyang University Hospital during the 5 year period between May of 2003 and April of 2008 and tested positive for IgM anti-HAV were selected and their medical records retrospectively analyzed. Results : 156 subjects [91 male (58%), 65 female (42%) were selected. The average age was 29.2 years (range; 21.4-37), average admission period 12.6 days (range; 12.5-1 7.7) days, and average medical fees 2,295,151 (range; 932,097-3,658,205) KRW(Korean Won), Yearly incidence rates were as follows : 10 for May, 2003 to April, 2004, 14 for May, 2004 to April, 2005, 23 for May, 2005 to April, 2006, 69 for May, 2006 to April, 2007, and 41 for May, 2007 to April, 2008, The maximum value median of AST, ALT, and bilirubin during admission was 989 IU/L, 1999 IU/L, and 5.6 mg/dL each, Each factors were all positively correlated with the patient's age. Conclusions : Recent decrease in antibody positivity of the hepatitis A virus and subsequent surge of acute hepatitis A is a cause of rising socioeconomic costs. A large scale epidemiologic research on whether HAV vaccination should be made mandatory and health insurance applied seems to be needed. Key words : Hepatitis A, Age, Medical fees, Vaccination

        제1형 당뇨병 환자에서 췌도세포 동종이식의 반복시행

        연구배경:췌도세포 이식은 시술이 간편하고 안전하기 때문에 반복이식이 가능하여 그 동안 여러 센터에서 반복이식의 성공을 보고한 바 있다. 국내에서도 본 병원을 비롯하여 몇몇 센터에서 췌도세포 이식이 활발히 시도되고 있으나 사람에서의 동종이식 및 동종 이식의 반복시행은 보고된 바 없는 실정이다. 저자 등은 국내 최초로 췌도세포 동종이식을 시행하여, 혈청 C­펩타이드가 증가하고 인슐린 요구량이 감소하였으며, 혈당농도와 당화혈색소의 안정을 보였다. 그러나 이식 70일 후 다시 인슐린 요구량이 증가하고 C­펩타이드가 감소하여 췌도세포 이식을 다시 시행한 바 있다. 2차 이식 후 70여일이 지난 현재 다시 혈청 C­펩타이드가 증가하고, 인슐린 요구량이 더 감소되어 췌도세포 이식에서 반복 시행의 유용성을 보고하는 바이다. 방법:환자는 32세 남자로 17년전 당뇨병 진단 후 인슐린 치료 중이었으며, 3년전 부터는 만성신부전증으로 혈액툭석을 하고있었다. 1999년 12월 25일 신장이식을 하였고 3일 후 췌도이식을 하였으며, 두번째 이식은 70일 후 시행하였다. 췌도분리는 변형된 Recordi방법과 비연속성 자당 농도차(discontinuous density gradient)를 이용하였으며 분리한 췌도는 배양 후 환자의 신장기능이 정상화되고 미생물학 검사에서 음성을 확인한 후 경피경간으로 간문맥을 접근하여 16G 폴리 에틸렌 카테터를 이용하여 간실질에 이식하였다. 결과:1차 췌도이식시 순수분리전 췌도수는 210,000개, 순수분리후 획득한 췌도수는 90,000개, 순도 95%, 세포양 1.0mL 이었고, 2차 췌도이식시 순수분리전 췌도수는 420,000개, 순수분리후 획득한 췌도수는 370,000개, 순도 95%, 세포양 1.5mL 이었다. 1,2차 모두 간문맥을 통해 약 20분에 걸쳐 간실질에 주입하였다. 췌도이식전 인슐린요구량은 75∼75U/일, HbA1e 8∼10%, C­펩타이드 0.6ng/mL 였으며, 1차 췌도이식수 7일째 인슐린 요구량은 40U/일, C­펩타이드 1.5ng/mL, FPS 109mg/mL 였고, 40일 추적관찰 후 인슐린 요구량이 36U/일, C­펩타이드 1.8ng/mL, HbA1e 6.5∼7.0%로 안정되었다. 그러나 이식 50일째부터 인슐린 요구량이 50∼56U/일, C­펩타이드 0.6ng/mL, FPS 130∼200mg/dL로 혈당 조절이 불안정하여 다시 췌도이식을 시행하였다. 2차 이식후 50일이 경과한 현재, 인슐린 요구량은 26U/일, C­펩타이드 1.8ng/mL, FPS 90∼120mg/dL로 다시 안정되었다. 결론:췌도이식은 반복이식이 가능하며, 본 환자의 경우 인슐린요구량 감소, 혈당의 안정화 및 C­펩타이드가 상승하여 이식한 췌도의 기능을 확인할 수 있었고, 향후 스테로이드 등 면역억제가 유지 용량으로 감량되면 인슐린 요구량은 더 감소될 것으로 기대된다. Over the past 20 years, significant advances have been made in human islet transplantation. However, cases of prolonged insulin independence after islet allotransplantation have rarely been reported and over time, a slight, gradual decrease in insulin secretion appears to occur, as suggested by the lower C-peptide. Although preliminary clinical success achieved over the past few years has been considerably higher with whole pancreatic transplant than with isolated islet grafts, both approaches remain experimental. Islet grafts might gain, over time, increasing credibility and might eventually provide an easier alternative in terms of grafting procedures and patient management, as compared with the more "traumatizing" whole-pancreas transplantation. Also, using islet, re-transplantation is possible. But it is not known whether re-transplantation of islet could be suitable for those patients who lost grafted islet function. The aim of the present study was to investigate the benefits of re-transplantation of islet in previously simultaneous islets-kidney transplant (SIK) patient who have lost graft function. Methods : The recipient was a 32 year old male. First islet transplantation was underwent at December 25, 1999. However, the grafted islets lost function after 70 days. So we performed re-transplantation of islets. The isolation of islet was conducted sterilely on a laminarflow hood and isolated by a modified Recordimethod. The islet was injected slowly into the liver via a cannular placed in the potalvein for 20 minutes. Results : Transplanted islets were 90,000 IEq at first islet transplantation, 370,000 IEq at second islet transplantation. The insulin requirement was reduced from 75-85 to 35-40 U/day, the basal C-peptide level was 1.5 ng/mL at 7 days posttransplant Unfortunately, the grafted islets lost function after 70 days. After second transplantation, the insulin requirement was reduced to 26 U/day. Conclusions : Despite the continuous need for exogenous insulin therapy, islet transplantation can prevent wide glucose fluctuations, thus resulting in normalization of glycemic control and improvement in HbAlc, and also, show that islets can be successfully and safely re-transplanted intraportally in patients who have lost previously grafted islet function (J Kor Diabetes Asso 457~466, 2000).

        <P>Germanium sulfide (GeS and GeS<SUB>2</SUB>) nanoparticles were synthesized by novel gas-phase laser photolysis and subsequent thermal annealing. They showed excellent cycling performance for lithium ion batteries, with a maximum capacity of 1010 mA h g<SUP>−1</SUP> after 100 cycles.         Effect of Separation on the Fracture Surface of Pipeline Steels with Ferrite–Bainite Dual Phases During Drop Weight Tear Test

        The effect of separation on the fracture surface of ferrite–bainite dual-phase pipeline steels during a drop weight tear test(DWTT) was investigated via microstructural analysis and fracture surface examination. Two specimens (ST1 and ST2) weredesigned to have a dual-phase microstructure, with different ferrite and bainite fractions, by controlling the reduction ratioand holding time in the ferrite–austenite two-phase region during the thermomechanical process. Notably, ST2, which had alonger holding time for the retained austenite to absorb carbon, exhibited a harder bainite but softer ferrite phase comparedto ST1. The greater hardness difference between those phases in ST2 induced the strain incompatibility frequently evidentat the phase boundaries, resulting in lower ductility during the tensile test. Owing to the strain incompatibility, ST2 alsogenerated more separation over a wider temperature range during DWTT. At low temperatures, where the brittle fracture wasprominent, it was observed that ST2 generated separations on its fracture surface, whereas ST1 did not. These separationsformed a local shear lip around themselves, thereby obstructing cleavage fracture propagation from the notch. Hence, it wasconfirmed that ST2 had a higher DWTT shear area than ST1 over a temperature range near the ductile–brittle transitiontemperature (DBTT). As a result, the DBTT of ferrite–bainite dual phase steel could be improved by increasing the amountof separation during DWTT.

        노인에서의 지역사회획득 폐렴

        입원을 필요로 하는 지역사회폐렴의 대다수는 노인 환자들이며 젊은 환자들에 비하여, 입원기간이 길고, 동반질환이 많으며, 중환자실 치료가 필요한 경우가 많다. 노인 인구에서 폐렴은 중요한 사망원인이다. 노인들의 경우 급성 호흡기 증상이 아닌 혼돈이나 무기력증, 기저질환에 의한 기능부전으로 폐렴 증상이 표현될 수 있어 진단되기까지 걸리는 시간이 길어지게 되며 사망률을 높이는 요인이 된다. 따라서 노인 환자에서 폐렴의 중증도 평가를 시행하여 질병의 예후를 예측하고 입원여부 등을 결정하여 치료방침을 세우는 것이 필요하겠다. Community-acquired pneumonia (CAP) is an important threat to the health of older adults. CAP is one of the leading causes of overall mortality and the most common cause of infection-related death in Korea, especially in older adults. As the incidence and mortality of CAP is increasing recently, the economic and clinical burden is also increasing consistently. The clinical presentation in older adults is different from that in younger patients. Classic symptoms are often absent in older adults. Therefore, appropriate diagnosis and treatment can be delayed.

        The Beneficial Effect of Chronic Graft-versus-Host Disease on the Clinical Outcome of Transplantation with Fludarabine/Busulfan-Based Reduced-Intensity Conditioning for Patients with De Novo Myelodysplastic Syndrome

        <P>Allogeneic hematopoietic stem cell transplantation following reduced-intensity stem cell transplantation (RIST) has enabled the treatment of older or medically infirm patients with myeloid malignancies; however, determining the value of RIST outcomes for myelodysplastic syndrome (MDS) is difficult because of the heterogeneity of the diseases included in most trials. To define the role of RIST in MDS, we performed RIST for 22 consecutive patients who had de novo MDS as classified by World Health Organization (WHO) criteria and who received an allograft with fludarabine/busulfan (Busulfex) or fludarabine/Busulfex/antithymocyte globulin (ATG) conditioning. Nineteen patients (86.4%) achieved engraftment. At a median follow-up of 18.9 months (range, 13.1-24.8 months), the estimated 2-year rates of overall survival, event-free survival (EFS), transplantation-related mortality, and relapse were 78.7%, 67.7%, 12.6%, and 22.5%, respectively. Acute graft-versus-host disease (GVHD) greater than grade II developed in 3 patients (15.8%). Chronic GVHD developed in 10 patients (55.6%), none of whom received ATG as a conditioning regimen. Variables influencing EFS were chronic GVHD, marrow blasts before transplantation, and the WHO criteria. The present study clarifies the benefits of the fludarabine/Busulfex-based conditioning regimen for de novo MDS diagnosed according to the WHO criteria and shows that chronic GVHD appears to have a beneficial effect on survival rates, which are strongly associated with graft-versus-tumor effects.</P>

        A 2.5–5.6 GHz Subharmonically Injection-Locked All-Digital PLL With Dual-Edge Complementary Switched Injection

        <P>A 2.5–5.6 GHz low-phase-noise subharmonically injection-locked sub-sampling all-digital phase-locked loop with a dual-edge complementary switched injection technique is presented. While previously reported injection-locked phase-locked loops (ILPLLs) require additional circuitry for resolving a phase alignment mismatch between the PLL loop and injection path, the presented ILPLL exhibits a simplified architecture owing to the proposed injection technique and sub-sampling bang-bang phase detector (SSBBPD). Because the proposed injection technique exploits dual-edge injection, we analyze the performance impact of dual-edge injection when inaccurate injection timing occurs. This paper also offers an analysis of the injection technique based on the charge transfer and derives the realignment factor of the injection. With the proposed injection technique and the direct connection of the digitally controlled oscillator (DCO) clock to the SSBBPD, the timing mismatch between the PLL loop and injection path becomes insensitive to voltage and temperature drift. The proposed ILPLL prototype is fabricated in a 65-nm CMOS process and achieves a 168-fs integrated rms jitter over 1 kHz to 40 MHz at a 5-GHz output frequency with 156.25-MHz reference clock while consuming 15.4 mW with an active area of 0.06 mm<SUP>2</SUP>.</P>
