We have encountered a rare case of symmetrical peripheral gangrene complicating Escherichia coli sepsis in a 47-years-old male. He was successfully treated with antibiotics, anticoagulants, and vasodilator. To our knowledge, this is the first report on symmetrical peripheral gangrene complicating E. coli sepsis in Korea.

Skeletal cryptococcosis is an uncommon infection. Cryptococcus is a common cause of meningitis and infects 7∼10% of patients with AIDS. As well as AIDS, the infection may be seen in association with leukemia, lymphoma, Hodgkin's disease, sarcoidosis, tuberculosis and diabetes, also in patients on steroid medication. But there is no case report of skeletal cryptococcosis following allogeneic hematopoietic stem cell transplantation. A 40-year-old woman was admitted to the hospital because of low back pain. She had chronic myelogenous leukemia for 2 years and underwent allogeneic hematopoietic stem cell transplantation 8 months ago. She have been treated with steroid and cyclosporine orally because of chronic graft versus host disease. On examination she was afebrile and had posterior lower lumbar tenderness. But, she had no reduced strength of low extremities. Open biopsy was underwent. Histology demonstrated budding, round-to-oval, refractile yeast-like organisms within debris. The results of a lumbar puncture were unremarkable and cerebrospinal fluid culture failed to grow bacteria and yeast. The patient was treated with amphotericin B (1 gram) and AmBisome□ (2.8 gram) over 6 weeks. Three months after cessation of therapy, the patient was doing well.(Korean J Infect Dis 33:298∼301, 2001)
우리나라 일부 도시와 농촌지역 주민의 혈중 납 및 혈중 카드뮴 농도

본 연구에서는 도시 지역 주민들과 농촌 지역 주민들을 대상으로 지리적 환경 차이에 따른 혈중 납 및 혈중 카드뮴농도를 조사하여 노출 정도를 알아보고자 하였다. 연구 대상은 2007년 7월 1일에서 8월 30일 까지 경남의 전형적인 농촌마을(진주) 주민 84명과 부산 인근의 반농촌마을(기장) 주민 64명, 대도시 부산지역 주민 100명을 선정하여 연구를 수행하였으며, Graphite furnace를 갖춘 비불꽃 원자 흡광기를 이용하여 분석하였다. 평균 혈중 납 농도는 대도시 부산지역 주민이 6.38±2.86 ㎍/㎗이었으며, 진주 농촌지역 주민들은 5.41±2.16 ㎍/㎗, 부산 인근의 기장지역 주민들은 4.50±2.87 ㎍/㎗으로 부산지역 주민들의 평균 혈중 납 농도가 가장 높았으며 부산지역 주민의 혈중 납 농도와 부산 인근 기장지역 주민의 혈중 납 농도간 유의한 차이(p<0.01)를 보였다. 평균 혈중 카드뮴 농도는 대도시 부산지역 주민이 0.85±0.44 ㎍/ℓ이었고, 진주 농촌지역 주민이 1.57±0.78 ㎍/ℓ, 부산 인근의 기장지역 주민 0.95±0.54 ㎍/ℓ으로 진주 농촌지역 주민들의 평균 혈중 카드뮴 농도가 가장 높았으며 부산지역 주민과 진주 농촌지역 주민들 간에 통계적 유의한 차이(p<0.01)가 관찰되었다. 본 연구에서는 도시 지역과 농촌 지역 간에 따라 혈중 납 및 혈중 카드뮴의 농도에 차이가 있음을 알 수 있었으나, 일부 지역 주민들의 결과만으로 우리나라 전체 도시와 농촌지역 주민의 혈중 납 및 카드뮴 농도에 대한 차이를 단언하기는 어렵다고 생각된다. 하지만 작업장 내의 직업적 폭로가 아닌 도시와 농촌 간의 지역적 차이에 관한 연구로써 의미 있는 연구라 생각되며, 향후 혈중 납과 카드뮴의 체내축척을 줄이기 위하여 보다 더 많은 대상자들의 정밀한 조사가 이루어져야 될 것으로 사료된다. The purpose of this study was to evaluate the levels of blood lead and cadmium in urban and rural populations. Blood samples were collected from 100 urban (Busan) residents and 150 rural residents (Jinju-84, Gijang-66) from July 1 to August 30, 2007. The blood lead and cadmium levels were analyzed using flameless atomic absorption spectrophotometry and were compared by age, gender and smoking status. The mean levels of blood lead in urban-Busan, rural-Jinju and rural-Gijang residents were 6.38±2.86 ㎍/㎗, 5.41±2.16 ㎍/㎗ and 4.50±2.87 ㎍/㎗, respectively. There was a significant difference in the level of blood lead between urban-Busan residents and rural-Gijang residents. The mean levels of blood cadmium in urban-Busan, rural-Jinju and rural-Gijang residents were 0.85±0.44 ㎍/ℓ, 1.57±0.78 ㎍/ℓ and 0.95±0.54 ㎍/ℓ, respectively. There was a significant difference in the level of blood cadmium between urban-Busan residents and rural-Jinju residents. This study showed that the levels of blood lead and cadmium were significantly different between urban and rural populations. The level of blood lead was highest in urban-Busan residents, but the level of blood cadmium was highest in rural-Jinju residents. Further studies are needed to define the cause of high levels of blood lead and cadmium related to area of residence and personal habits.

High Throughput Ultralong (20 cm) Nanowire Fabrication Using a Wafer-Scale Nanograting Template

<P>Nanowires are being actively explored as promising nanostructured materials for high performance flexible electronics, biochemical sensors, photonic applications, solar cells, and secondary batteries. In particular, ultralong (centimeter-long) nanowires are highly attractive from the perspective of electronic performance, device throughput (or productivity), and the possibility of novel applications. However, most previous works on ultralong nanowires have issues related to limited length, productivity, difficult alignment, and deploying onto the planar substrate complying with well-matured device fabrication technologies. Here, we demonstrate a highly ordered ultralong (up to 20 cm) nanowire array, with a diameter of 50 nm (aspect ratio of up to 4 000 000:1), in an unprecedented large (8 in.) scale (2 000 000 strands on a wafer). We first devised a perfectly connected ultralong nanograting master template on the whole area of an 8 in. substrate using a top-down approach, with a density equivalent to that achieved with e-beam lithography (100 nm). Using this large-area, ultralong, high-density nanograting template, we developed a fast and effective method for fabricating up to 20 cm long nanowire arrays on a plastic substrate, composed of metal, dielectric, oxide, and ferroelectric materials. The Role of Transglutaminase in Double-stranded DNA-Triggered Antiviral Innate Immune Response

Cellular uptake of double-stranded DNA (dsDNA) triggers strong innate immune responses via activation of NF-${\kappa}B$ transcription factor. However, the detailed mechanism of dsDNA-mediated innate immune response remains yet to be elucidated. Here, we show that the expression of tazarotene-induced gene 3 (TIG3) is dramatically induced by dsDNA stimulation, and the siRNA-mediated down-regulation of TIG3 mRNA results in significant suppression of dsDNA-triggered cytokine expression. Because TIG3 has been previously shown to physically interact with transglutaminase (TG) 1 to activate TG activity, and TG2 has been shown to induce NF-${\kappa}B$ activity by inducing $I{\kappa}B{\alpha}$ polymerization, we tested whether TG also plays a role in dsDNA-mediated innate immune response. Pre-treatment of TG inhibitors dramatically reduces dsDNA-triggered cytokine induction. We also show that, in HeLa cells, TG2 is the major TG, and TIG3 physically interacts with TG2. Combined together, our results suggest a novel mechanism of dsDNA-triggered innate immune response which is critically dependent on TIG3 and TG2.

<P>In order to investigate the combined effect of expansive and shrinkage-reducing admixtures on shrinkage and mechanical properties of ultra high performance fiber-reinforced concrete, a series of experiments including three types of shrinkage tests, compressive and flexural tests, and penetration-resistance test were performed. Test results showed that about 51–53% of free shrinkage and 35–49% of restrained steel strains were reduced by adding 1% shrinkage-reducing admixture along with 7.5% expansive admixture with slight change of strength. Elastic stress, residual interface pressure and degree of restraint from ring specimens were also significantly reduced by using expansive and shrinkage-reducing admixtures. Although the elastic stress of the ring specimen without expansive and shrinkage-reducing admixtures was higher than the tensile strength, no shrinkage crack was observed for all test specimens due to the relaxation effect.</P>

Aims: Renal subcapsular hematoma has been reported in patients undergoing urological intervention or in patients with renal malignancy. Renal subcapsular hematoma after abdominal angiography has been rarely reported. We report two patients with hepatocellular carcinoma (HCC) who experienced fatal renal subcapsular hematoma after transcatheter arterial chemoembolization (TACE). Methods: Case 1: A 77-year-old female was admitted to treat HCC for the 6th TACE. The patient had hypertension, diabetes, liver cirrhosis associated with HBV. After the TACE, decrease in awareness occurred. Vital signs were unstable with blood pressure 57/35 mmHg and pulse rate 118 times/minute. Portable abdominal sonography showed right renal subcapsular hematoma. Contrast media leakage was observed at the branch of the upper polar division of renal artery on the angiography. Case 2: An 84-year-old female was admitted to treat HCC with the first TACE. The patient had hypertension, diabetes, liver cirrhosis associated with HBV. Decrease in consciousness occurred immediately after TACE and vital signs were unstable with blood pressure 80/40 mmHg, pulse rate 79 times/minute. Hematoma around the right kidney was detected in the non-contrast abdominal CT scan (Figure 1). Results: Case 1: Embolization was performed for bleeding control. Even though successful embolization and aggressive supportive care with transfusion, she died due to hypovolemic shock and multi-organ failure. Case 2: Embolization was performed on the bleeding lesion identified by angiography. Afterwards, the patient continued to show lowered blood pressure and decreased consciousness in spite of aggressive transfusion and resuscitation. She died due to metabolic acidosis, hypovolemic shock, and multi-organ failure. Conclusions: Thrombocytopenia and hypertension are known as risk factors for hematoma formation, and vascular intervention may require careful manipulation in patients with HCC who have cirrhosis and hypertension. We report these cases to keep in mind the occurrence of renal subcapsular hematoma, a rare but serious complication of vascular intervention for treatment of HCC.
