Quality Indicators and Outcome Measures of Endoscopy in the National Cancer Screening Program

Purpose: Quality indicators of the National Endoscopy Quality Improvement Program (NEQIP) and outcome measures of endoscopyin the National Cancer Screening Program (NCSP) in Korea are not clear. We evaluated the quality indicators of the revisedNEQIP and outcome measures of endoscopy at different types of healthcare facilities participating in the NCSP. Materials and Methods: This study was conducted between March and August 2018 in primary, secondary, and tertiary healthcarefacilities that perform endoscopy as a part of the NCSP. Representative endoscopists completed a questionnaire for qualityindicators of the NEQIP and provided data on outcome measures for endoscopy. Results: Quality indicators of the NEQIP were mostly acceptable. However, the quality indicators for annual volume of esophagogastroduodenoscopy(EGD) and colonoscopy, training for endoscopy quality improvement by endoscopy nursing staff, colonoscopyreports, documentation of pathologic lesions, quality of endoscopy reprocessing areas, and completion of endoscopy reprocessingeducation programs were suboptimal. For outcome measures of EGD, the number of photo-documentations and totalprocedure time were higher at tertiary healthcare facilities than at other facilities (p<0.001 and p=0.023, respectively). For the outcomemeasures of colonoscopy, colonoscopy completion rate and waiting times for colonoscopy were significantly higher at tertiaryhealthcare facilities than at other facilities (both p<0.001). Conclusion: Outcome measures of endoscopy should be included as quality indicators of NCSP. However, universal outcomemeasures for all types of healthcare facilities should be established because performance levels of some outcome measures differamong individual healthcare facility types.

The spot weldability of coated steels for automobile has been investigated. Coated steels of SPC-Zn DC, SPC Zn-Ni SC, SPC Zn-Ni DC, SPC Zn-Fe DC and OCCS were welded under different conditions of welding current, force and time. Coating thickness at the welded surface was reduced as increased welding current. Tensile shear strength(TSS) and cross tensile strength (CTS) were increased up to expulsion began, then dropped as increased current. Optimum conditions of welding force and time were different, however 200~250kgf and 15~20cycle were optimum for coated SPC (Steel Plate Cold). Weldability lobes were measured for each coated steel and they showed narrow range of working welding current. The organic composite coated steel (OCCS) had the highest current to get $\sqrt5{t}$ nugget size and narrowest working welding current range.

본 논문은 김대중 정부의 개혁을 평가하기 위한 것이다. 이를 위해, 경제, 정치영역, 대북정책에서의 개혁의제들과 실제 이뤄졌던 개혁 내용의 한계를 짚어보았다. 우선, IMF라는 위기상황을 극복하기 위해 왜곡된 시장의 개혁에 대한 과제로 재벌개혁을 진행했다. 그러나 재벌개혁은 치밀한 중장기적인 목표없이 정부의 단기적인 효과만 가져오는 일방적 정책으로 부작용을 야기시켰다. 또한, IMF식의 구조조정은 한국사회의 중산층을 붕괴시켰을 뿐만 아니라, 개방과 자유화라는 규제없는 시장에 대한 혼란을 고려하지 못하고 취약한 경제구조를 생산해냈다. 다음으로 정치개혁에 있어서는 정치적 소수파의 한계를 극복하지 못하고 국정운영의 안정적 기반 확보에 주력함으로써 개혁에 소극적인 자세를 지속했다. 그로인해, 정치개혁이란 이름으로 여당의 안정 의석확보라는 선거제도의 개정이 수행됐을 뿐이다. 마지막으로, 김대중 정부의 햇볕정책은 무엇보다 남북관계의 평화적 정착과 남한 내 반공이데올로기의 극복이라는 차원에서 그 일반적 의의를 가진 것이었다. 하지만 이에 대한 과도한 집착은 그 수행방식이 굳어져 버리는 결과를 초래하였다. 정상회담이라는 가시적인 성과에 치중한 나머지 평화체제에 대한 대안 형성을 격감시켰고, 남한 내부의 국민들 간의 갈등을 포용하지 못한 나머지 정책에 대한 지속적인 효과를 격감시키고 말았다고 평가된다.

분산 트랜잭션 워크플로우 모니터링 시스템의 설계 및 구현

본 논문에서는 분산 트랜잭션 워크플로우 모니터링 시스템의 설계 및 구현에 관하여 기술한다. 워크플로우 인스턴스들의 실행 상태 관리를 주요 목적으로 하는 전통적인 워크플로우 시스템의 모니터링 기능은 워크플로우의 초대형화와 트랜잭션화를 특징으로 하는 분산형 트랜잭션 워크플로우 시스템에서는 워크플로우의 처리 상태 관리뿐만 아니라 구조적인 정보의 수집, 통계 정보 제공, 사용자들의 처리 상태 정보 등과 같은 다양한 부가적인 정보 처리 및 관리 기능을 제공하는 분산형 모니터링 서비스를 필요로 한다. 본 논문에서는 이러한 분산형 트랜잭션 워크플로우 시스템에서 필수적으로 요구되는 새로운 워크플로우 모니터링의 특징을 확장 정의하고, 이를 지원하는 점 기반의 분산 트랜잭션 워크플로우 모니터링 시스템의 상세 설계 및 이의 구현 방안을 소개한다. This paper describes the design and implementation details of a distributed transactional workflow monitoring system. There have been prevalent research and development trends in the workflow literature - workflow systems tend to be completely distributed architectures to support very large-scale workflow applications on object-oriented and internet-based infrastructures. That is, the active (object), distributed (architecture), system-oriented (transaction), and large-scale (application) workflow systems are the key targets in terms of the research and development aspects. While the passive, centralized, human-oriented, and small/medium scale workflow systems are the typical instances of the traditional workflow systems. Unlike in the traditional (the client-server architecture) workflow systems, the workflow monitoring features should not be easily supported in the recent (the fully distributed architecture) workflow systems. At the same time, they need a set of additional monitoring features, such as gathering and displaying statistical (or overload status) information of the workflow architectural components dispersed on the internet. We, in this paper, introduce the additional workflow monitoring features that are necessarily required for the recent workflow systems, and show how to embed those features into a web-based distributed workflow system.
