      • '98 경주 세계 문화 엑스포를 중심으로 한 경주지역의 관광상품 개발에 관한 연구

        The purpose of this study is to make a plan for the development of new tour commodities for '98 Kyongju World Culture Exposition. The result of this study is as follow: First, cultural assets in Kyongju area are located at Inkyo - dong(16.8%), at Bohwang - dong(9.4%), at Topjong - dong(8.1%), at Mt. Namsan area(11.8% ; 6.9% for Naenam - myon, and 4.9% for Dodong - dong), and at the vicinity of Kyongju City(5.9%). Second, tourism enterprises in Kyongju Area are located mostly at one place ; Pomun Lake Resort area. This fact have had bad influence on the influx of tourists. Third, over 40% of tourists from Kyongsangbuk - do Province vote for Kyongju Area as the most favorite tourist - oriented city in this province. And they are visiting Kyongju mostly in April(22.6%) and in May(17.8%). Lastly, current tour routes are limited mostly to the heart of Kyongju City. So, other attractive tour routes must be developed also in other tourism circles such as the East Sea Tourism Circle and the Northern Province Tourism Circle, which belong to the 6 tourism circles designated by Kyongju City but are now excluded from the current tour routes. In conclusion, this study proposes "'98 Kyongju World Culture Exposition with Themes" related to the 6 tourism circles. The themes proposed are as follows: 1. The exploration of the historical places of the Shilla Kingdom in the heart of Kyongju City including Mt. Seoak area and Pulkuksa Temple area. 2. Smile with a new millenium in Pomun Lake Resort area. 3. The exploration of the Buddhist culture in Mt. Namsan. 4. The harmony between nature and culture in the East Sea Tourism Circle and the Northem Culture Circle.

        병식 평가 척도-한국판(SUMD-K)의 신뢰도와 타당도 검증

        Objectives : This study was conducted to evaluate the reliability and the validity of the Korean version of the Scale to Assessment Unawareness of Mental Disorder (SUMD-K). Methods : The subjects were 55 schizophrenic patients between ages of 26 and 58 who were recruited from the psychiatric unit of the university hospital and the out-patient clinic. The SUMD-K and insight scale of PANSS were applied to the patients. Results : The range of inter-rater (Spearman's rho) of the general items of SUMD-K were .97-.98 on the current illness and .58-.98 on the past illness. The correlation coefficient between the sum and the general items of the SUMD-K showed high scores. The concurrent validity between SUMD-K and insight item of PANSS was high (scores between .59-.96). However,the reliability and the validity of the attribution items of symptoms showed low scores compared to the current symptoms. Conclusion : The SUMD-K was confirmed in its reliability and validity. It can be a useful clinician-rating scale for evaluating insight in schizophrenic patients as well as patients with other neuropsychiatric conditions.

      • 페니실린 내성 폐렴구균 균혈증의 임상적 특성

        목적: 지역사회 획득 감염의 매우 흔한 원인균인 폐렴구균의 페니실린 및 다른 항균제에 대한 내성의 증가는 세계적으로 문제가 될 뿐 아니라 국내에서 더욱 큰 문제로 대두되고 있다. 본 연구는 항균제 내성 여부에 따른 폐렴구균 균혈증의 임상적 특성 및 위험 요인를 분석하여 항균제 내성의 임상적 의미를 조사하고자 시행되었다. 방법:1989년부터 1994년까지의 폐렴구균 균혈증 41례를 대상으로 하여 의무기록지를 조사하였다. 균주의 항균제 감수성 검사는 oxacillin 디스크 확산법으로 페니실린 내성 여부를 거사한 후, 한천 희석법을 이용하여 페니실린을 비롯한 11개의 항균제의 MIC (minimal inhibitory concentration)를 구하고 감수성 여부를 판정하였다. 결과: 41균주 중 68.3%가 페니실린에 대하여 내성(중등도 12.2%, 고도 56.1%)을 보였으며 다제내성률은 61%였다. 특히 소아에서는 100%(고도 88.2%)의 내성률을 보여 성인보다 내성률이 더 높았다. 페니실린 내성 균주는 대부분 다른 β-lactam 항균제에도 내성을 보였으며 특히 페니실린 고도 내성 균주는 cefotaxime, ceftriaxone에 대하여 100% 내성을 보였고 imipenem에 대해서도 95.5%의 중등도 내성률을 보였다. 혈청형은 19와 23이 많았고 그 외에 6이 있었다. 임상적 특성의 분석상 환자들의 연령분포는 3개월부터 82세까지였으며 기저질환을 가지고 있는 환자가 56%이었으며 지역사회 획득 감염이 71%였다. 균혈증의 일차감염원은 폐렴(22례, 54%)과 수막염(5례, 12%)이 가장 많았다. 페니실린 내성의 위험요인으로는 소아 연령만이 유의한 요인으로 분석되었다. 항균요법에 대한 반응을 볼 때 실패한 경우는 고도 내성 군의 20%에서만 있었으며 병원내 총사망률은 페니실린 감수성 군이 30%, 중등도 내성 군이 20%, 고도 내성 군이 13.6%로서 유의한 차이는 없었다. 결론: 페니실린 내성 폐렴구균은 국내에 만연하고 있으며 높은 다제내성률을 보였다. 이들은 대개 지역 사회 획득 감염을 일으키는데 균혈증의 경우 그 사망률은 내성 여부에 따른 차이를 보이지 않았다. 소아연령만이 페니실린 내성의 위험요인으로 분석되었다. Background: The emergence of penicillin-resistant Streptococcus pneumoniae (PRSP) poses serious therapeutic problem in clinical practice, especially in cases with bacteremia or meningitis. Methods: To investigate clinical characteristics of bacteremic pneumococcal diseases due to PRSP, we retrospectively analyzed 41 cases with cocumented bacteremic diseases seen in a tertiary care hospital between 1989 and 1994. Results: Agar dilution test of 41 strains isolated showed that 68.3% of S. pneumoniae were PRSP [high-level resistance(R) 56.1%, intermediate resistance(I) 12.2%]. High-level resistant strains were not susceptible to other β-lactam agents, whereas isolates of penicillin-susceptible S. pneumoniae(PSSP) were uniformly susceptible to all β-lactam agents. Predominant serogroups of PRSP were 19, 23, and 6. Bacteremic diseases caused by PRSP included pneumonia (22), meningitis (5), peritonitis (3), acute otitis media (2), acute tonsillitis (2), endocarditis (1), pyelonephritis(1), and primary bacteremia(5). Children were more likely to be infected with PRSP with high-level or intermediately resistant strains than were adults(P=0.0001), but no differences were seen between PRSP and PSSP regarding sex. previous antibiotic history, previous hospitalization, and underlying immunocompromised conditions. Most cases were community-acquired (R 78.3%, I 60%, S 61.5%). Fatality rates of patients infected with PRSP were not different significantly from PSSP (22.6% vs. 30%). However, most fatal cases had underlying immunocompromised conditions. Conclusion: PRSP is widely prevalent in Korea and shows resistance to most antibiotics. It causes community-acquired bacteremic diseases with poor outcomes, but there was no difference in mortality between patients infected with PRSP and PSSP. Pediatric age was the only risk factor for penicillin resistance.

        A 7-month old, castrated male Shih-tzu dog was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with history of depression, dyspnea, mild scale, tachypnea, exercise intolerance and severe leanness. On physical examination, cardiac arrhythmias was heard and heart worm ELISA test was positive. Microfilaria was found on the direct blood smear. Survey radiographs showed loss of diaphragmatic line and cardiac silhouette, displacement of lung fields and presence of gas filled intestines in the thoracic cavity. Rib fracture and pelvic fracture with callus formation were found incidentally. On the basis of physical examination and radiographic findings, it was diagnosed as traumatic diaphragmatic hernia and heart worm disease. Herniorrhaphy was performed. Postoperatively, it was treated for heart worm disease and scale. There is no evidence of recurrence of diaphragmatic hernia and heart worm disease over 1 year.

        Amplification of the UQCRFS1 Gene in Gastric Cancers

        Purpose: The specific aim of this study is to unravel a DNA copy number alterations, and to search for novel genes that are associated with the development of Korean gastric cancer. Materials and Methods: We investigated a DNA copy number changes in 23 gastric adenocarcinomas by array-comparative genomic hybridization and quantitative real-time polymerase chain reaction analyses. Besides, the expression of UQCRFS1, which shows amplification in array-CGH, was examined in 186 gastric cancer tissues by an immunohistochemistry, and in 9 gastric cancer cell lines, as well as 24 gastric cancer tissues by immunoblotting. Results: We found common gains at 48 different loci, and a common loss at 19 different loci. Amplification of UQCRFS1 gene at 19q12 was found in 5 (21.7%) of the 23 gastric cancers in an array-comparative genomic hybridization and DNA copy number were increased in 5 (20.0%) out of the 25 gastric cancer in quantitative real-time polymerase chain reaction. In immunohistochemistry, the overexpression of the protein was detected in 105 (56.5%) out of the 186 gastric cancer tissues. Statistically, there was no significant relationship between the overexpression of UQCRFS1 and clinicopathologic parameters (P>0.05). In parallel, the overexpression of UQCRFS1 protein was confirmed in 6 (66.7%) of the 9 gastric cancer cell lines, and 12 (50.0%) of the 24 gastric cancer tissues by immunoblotting. Conclusions: These results suggest that the overexpression of UQCRFS1 gene may contribute to the development and/or progression of gastric cancer, and further supported that mitochondrial change may serve as a potential cancer biomarker.

      • Tuning the nanostructure of nitrogen-doped graphene laminates for forward osmosis desalination

        <P>Studies have concentrated on the physicochemical properties of graphene-based membranes that can replace polymeric membranes for use in forward osmosis (FO) systems. However, recent research studies have focused on mixtures of two or more different materials (<I>e.g.</I>, graphene oxide and polymers) due to the need to reinforce underwater stability. Alternatives include reduced forms such as reduced graphene oxide to improve the stability and size-based selectivity, which have resulted in a narrow nanochannel that restricts water permeability. Herein, we propose the use of a novel nitrogen-doped graphene (NG) membrane to solve a trade-off between permeability and selectivity, investigating the nanostructure <I>via</I> N-doping reaction time. In an FO process, NG membranes achieved an outstanding specific salt flux of 0.18 g L<SUP>−1</SUP>, compared to commercial membranes (0.55 g L<SUP>−1</SUP>). The pyridinic-N bonding structure improved the permeability and selectivity under a similar nanochannel size because of its negatively polarized hole defects with the moderate energy barrier enabling water passage while blocking ions. Our results confirm the possibility of fabricating novel graphene-based FO membranes by tailoring the nitrogen-bonding structure, which will significantly help develop a process for improving the scalability of membrane materials.</P>

        Endoscopic Submucosal Dissection for Treatment of Localized Gastric Mucosa-associated Lymphoid Tissue Lymphoma: A Case Series

        Background/Aims: The treatment for gastric mucosa-associated lymphoid tissue lymphoma (MALToma) generally involves eradication of Helicobacter pylori. However, MALToma lesions may recur even without H. pylori re-infection. Furthermore, the remission rate of H. pylori-negative MALToma after eradication is low. Therefore, herein, we report on endoscopic submucosal dissection (ESD) as a treatment strategy for gastric MALToma. Methods: We retrospectively reviewed the data of all patients of gastric MALToma who underwent endoscopic resection at our institution between January 2000 and December 2021. Clinical remission was defined as complete histological remission or probable minimal residual disease according to the GELA grading system for post-treatment evaluation of gastric MALToma. Results: Six patients with gastric MALToma underwent ESD. Two patients were diagnosed with gastric MALToma, which improved after eradication treatment and relapsed approximately 36 and 41 months later, respectively. These patients had singular lesions localized to the mucosa and did not experience H. pylori re-infection. The lesions were successfully removed via ESD. The remaining four patients had H. pylori-negative gastric MALToma. These patients also had single, localized lesions that were removed via ESD. All the patients remained in clinical remission until the final follow-up. Conclusions: ESD is a safe and effective intervention for H. pylori-negative gastric MALToma when the lesion is single and confined to the mucosal layer.

        Predicting Helicobacter pylori infection from endoscopic features

        Background: Helicobacter pylori infection, prevalent in more than half of the global population, is associated with various gastrointestinal diseases, including peptic ulcers and gastric cancer. The effectiveness of early diagnosis and treatment in preventing gastric cancer highlights the need for improved diagnostic methods. This study aimed to develop a simple scoring system based on endoscopic findings to predict H. pylori infection. Methods: A retrospective analysis was conducted on 1,007 patients who underwent upper gastrointestinal endoscopy at Asan Medical Center from January 2019 to December 2021. Exclusion criteria included prior H. pylori treatment, gastric surgery, or gastric malignancies. Diagnostic techniques included rapid urease and 13C-urea breath tests, H. pylori culture, and assessment of endoscopic features following the Kyoto gastritis classification. A new scoring system based on endoscopic findings including regular arrangement of collecting venules (RAC), nodularity, and diffuse or spotty redness was developed for predicting H. pylori infection, utilizing logistic regression analysis in the development set. Results: The scoring system demonstrated high predictive accuracy for H. pylori infection in the validation set. Scores of 2 and 3 were associated with 96% and 99% infection risk, respectively. Additionally, there was a higher prevalence of diffuse redness and sticky mucus in cases where the initial H. pylori eradication treatment failed. Conclusions: Our scoring system showed potential for improving diagnostic accuracy in H. pylori infection. H. pylori testing should be considered upon spotty redness, diffuse redness, nodularity, and RAC absence on endoscopic findings as determined by the predictive scoring system.
