      • 신경외과 중환자실에서 치료받은 환자들의 합병증 및 사망율에 대한 임상적 분석 : A Prospective Study of 200 Consecutive Patients

        저자들은 본원 신경외과 중환자 관리의 개선을 위하여 1993년 2월부터 12월까지 중환자실에 입원하여 치료받은 환자 200명을 대상으로 합병증 및 사망율을 일정한 분석양식에 의하여 후향성 연구를 시행하였다. 전체 합병증 발생률은 16.5% 였으며, 환자의 의식 상태가 나쁘고 재원 기간이 길수록 빈도가 증가하였다. 합병증중에서 감염이 66.8%로 합병증 중 가장 많은 빈도를 보였으며, 감염 부위별로는 요도 감염(18.8%), 상처 감염(18.8%) 그리고 폐렴(16.9%) 순으로 많았다. 그외 합병증은 욕창(9.4%), 위장관 질환(7.5%), 피부 질환(7.5%) 순이었다. 사망자는 18명으로 전체 환자의 9%를 차지하였으며 그중 외상 환자가 11명(61%)으로 가장 많았다. 사망 원인은 패혈성 쇼크로 사망한 1명을 제외하고 나머지 모두는 두개강 내압 항진으로 중환자실에서 치료상 합병증에 의하여 사망한 것은 아니었다. 따라서 본 연구 결과 적합한 약물 요법, 적절한 인원 배치, 시설의 개선 및 종사자들에 대한 적극적인 교육 등을 통하여 중환자실에서의 환자 관리를 개선함으로써 합병증 및 사망율을 감소시킬 수 있을 것으로 사료된다. In order to improve the management outcome of patients in neurosurgical intensive care unit(NICU), the authors analyzed complications and mortality of 200 consecutive patients between February and December 1993. Complications were occurred in 33 patients (16.5%) among 200 patients who had been treated at NICU. Infection(66.8%) was the most common complication in our complication series and their detailed sites were urinary tract infection(18.8%), wound infection (18.8%) and pneumonia (16.9%). The motality rate was 9%. Increased intracranial pressure was the most common cause of death. Based on this study, the authors conclude that appropriate medical treatment, proper management and education for personnel and excellent equipments were key to prevention of complication and mortality.

        GSIS is a system that contains spatially referenced data that can be analyzed and converted to information for a specific set of purpose,or application. The key of feature of a GSIS is the analysis of data produce new information. The history of using computers for mapping and spatial analysis shows that there have been paralled developments in automated data capture, data analysis, and presentation in several broadly related fields. Geo-spatial information systems do have a lot in common with auto computer aided drafting(ACAD) systems used for drafting a wide range of technical objects. This study aims to convert of GSIS database model by ACAD system.

        This paper presented surge protective method and devices for shipboard electrical and electronics devices. Parallel type surge protective devices (SPD) for the 440 [V] and a series type SPD for the 220 [V] rated voltage were designed and fabricated. The performance were evaluated by an un-powered test which applies the 8/20 [㎲] surge current Lip to 10 [㎄]. The maximum clamping voltage of the parallel and the series type SPD were 1,032 [V] and 875 [V], respectively.

        Pancreatic cystadenoma is very rare, slow growing tumor and generally preoperative differential diagnosis may be difficult. A 43-year-old female was admitted to our hospital with the complaint of abdominal discomfort in the epigastrium for about preceeding 5 years. Ultrasonography and Computed tomography were performed. When endoscopic retrograde cholangio-pancreatogram was performed, displacement of distal pancreatic duct by huge mass was noted and injected dye was drained irregulary into the cavity of mass. Distal pancreatectomy was performed with splenectomy. The mass size of distal pancreas was 7X4cm, and the cyst contained with yellowish thick mucoid fluid. Diagnosis was confirmed the mutinous cystadenoma of the pancreas on histologic examination.

        cDNA array 방법을 이용한 망간에 노출된 흰쥐 뇌기저핵의 유전자발현 분석

        Objectives: This study investigated the gene expression profile in basal ganglia of manganese-exposed rats based on cDNA array analysis. Methods: For cDNA array, 25 male Sprague-Dawley rats (250±25 g) were intraperitoneally injected with 25 ㎎/㎏ B. W./day of MnCl2 (0.3 ㎖) for 10 days. For dose-related gene expression analysis, rats were intraperitoneally injected with 0.2, 1.0, and 5.0 ㎎/㎏ B. W/day of MnCl2 for 10 days. Control rats were injected with an equal volume of saline. RNA samples were extracted from brain tissue and reverse-transcribed in the presence of [α^(32)P]-dATP. Membrane sets of the Atlas Rat 1.2 array Ⅱ and Toxicology array 1.2 kit (Clontech, Palo Alto, CA) were hybridized with cDNA probe sets. Northern blot hybridization method was employed to assess the dose-related gene expression. Results: Fifty-two genes showed significant changes in expression of more than two-fold. Twenty-eight were up-regulated and 24 were down-regulated in the manganese-exposed group compared to the control. Among the 52 genes, 28 genes including nuclear factor I-X1 (NF1-X1), neuroligin 2 and 3, mitochondrial stress-70 protein (MTHSP70), neurodegeneration-associated protein 1 (Neurodap 1), multidrug resistance protein (MDR), and endoplasmic reticulum stress protein 72 (ERP72), were reported for the first time related to the manganese-induced neurotoxic-metabolism in the rat basal ganglia. According to the dose-related gene expression analyses, MTHSP70, Neurodap 1 and ERP72 genes were up-regulated compared to the control even in the group exposed to low manganese dose ( 0.2 ㎎/㎏ B.W./day). Conclusions: Twenty-eight genes detected for the first time in this study were closely related to the manganese-induced neurotoxic-metabolism in the rat basal ganglia and further study of these genes can give some more useful information about the manganese metabolism.

      • 부산지역 모 공단 및 인근지역 사업체의 산업보건 실태에 관한 조사연구

        부산지역 공단 및 인근지역 사업체의 산업보건실태를 파악하기 위하여 205개 사업체의 보건담당자를 대상으로 1990년도의 보건실태에 관한 설문조사를 하였다. 조사내용을 일반사항, 보건관리실태에 관한 사항, 작업장 유해환경 및 보호구 지급사항, 건강관련 사항 및 근무조건에 관한 사항 등으로 대별하여 분석ㆍ관찰하였다. In order to obtain the fundamental data for developing more effective and acceptable programs of public health manegement in an industrial complex, 205 health manager in the plants answered the questionaire of the status of industrial health during 1990. The questionaire consisted of general charateristics, health care status, environmental problems, health problems, and others. 72.4% of 205 plants had less than 100 workers. Fabricated metal manufacture was 32.4%. Although Footwear manufacture was 6.4%, the percentage of workers was 33.1% of total workers. The percentage(56.2) of retired women was higher than that(38.2) of men. 51.9% of total plants has recieving group health care services. 85.4% of plants was exposed to noise, 66.5% was exposed to dust. Ear plug and mask were given to prevent noise and dust in 69.2% and 68.6%, respectively. Among 22,299 workers, 618(2.8%) were diagnosed as suspected non-occupatinal disease. 85.9% of them worked on continuously and had their disease treated. The cases suspected for occupational disease was 100 workers. 82.5% of them changed their post. 45% had the compensation for their occuaptional disease. The rate of working: accidents was 2.3%.

        Toluene, Xylene, Trichloroethylene 투여가 흰쥐의 시상하부와 뇌하수체의 GnRH, GnRH Receptor, Pit-1 유전자 발현에 미치는 영향

        The workplace exposure of chemicals has steadily increased, therefore the concern for subsequent effect on reproductive outcome has been an important issue in occupational medicine. In previous studies, higher rates of spontaneous abortion, reduced fertility and menstrual disorder among women, and an impairement of sperm quantity and quality among men have been associated with a wide variety of chemical agents. This study was conducted to evaluate the effects of toluene, xylene and trichloroethylene(TCE) injection on the mRNA levels of GnRH, GnRH receptor and Pit-1 genes in male rats hypothalamus and pituitary and the effects on the plasma levels of FSH, LH, prolactin and testosterone. Sprague-Dawley male rats were divided into five groups of five each according to concentration of toluene, xylene and TCE. The rats were injected subcutaneously to 0, 50, 100, 200, 400 mg/kg body weight/day of toluene, xylene and TCE, respectively for 6 days. Rat brains were excised and hypothalamus and pituitary were separated. Reverse transcription-polymerase chain reaction (RT-PCR) and RNase protection assay(RPA) were used to evaluate the GnRH, GnRH receptor and Pit-1 mRNA levels. Plasma concentrations of FSH, LH, prolactin and testosterone were assayed by radioimmunoassay(RIA). The results were as follows; 1. GnRH, GnRH receptor and Pit-1 mRNA levels in toluene and xylene injected groups, and GnRH receptor mRNA levels in TCE injected group were lowered dose-dependently. Especially, GnRH receptor and Pit-1 mRNA levels in 200mg/kg of toluene injected group, and GnRH, GnRH receptor and Pit-1 mRNA levels in 400mg/kg of toluene injected group were significantly lower than control group (p<0.05). GnRH receptor and Pit-1 mRNA levels in 400mg/kg of xylene injected group, and GnRH receptor mRNA levels in 400mg/kg of TCE injected group were significantly lower than control group(p<0.05). 2. The plasma levels of prolactin and testosterone in 400mg/kg of toluene injected group, and LH in 100, 200 and 400mg/kg of xylene injected group, and testosterone in 400 mg/kg of TCE injected group were significantly lower than control group(p<0.05). In conclusion, we speculated that toluene and xylene affected reproductive system secondarily through hypothalamus-pituitary axis, and TCE affected directly through steroidogenesis. And we recommended that further study for assessment of the reproductive toxicity of mixed organic solvent exposures should be conducted.

      • 소규모사업장 용접근로자들의 소음·용접흄·망간 노출에 관한 연구

        Objective : This study was conducted for the purpose of investigating the level exposure of welders to noise, welding fume and Manganese(Mn) related to this process. Methods and Material : All subjects in this study were welders engaged in CO2 arc and coated arc welding process especially in small-scale enterprises classified by type of manufacture of ship building, repairing and of other fabricated metal products, which is located in Gimhae and Milyang of Kyeongsang province in south Korea. Air sampling of welding fume was measured, shipped and analyzed on the basis of national institute for occupational safety and health(NIOSH) method in U.S. and work environment measurement method established in south Korea. To measure Noise in the work environment, Noise dosimeter was used on the basis of work environment measurement method established in south Korea. Result : This study was performed during a period from January, 2000 to December, 2001 and Results were as follows: 1. Noise level measured in this work environment was higher than national criteria for permissible exposure limit on health examinations related to hearing loss. 2. The concentration of welding fume and Mn in manufacture of ship building and repairing was higher than those in manufacture of other fabricated metal products. 3. In conditions of existing in ventilation system installed and operated, the concentration of welding fume and Mn was slightly lower than those of not existing. 4. In conditions of existing in sufficient air supply from out door into the work place relied on natural exhaust system, the concentration of welding fume and Mn was slightly lower than those of in not sufficient. 5. In all subjects, a rate of wearing ear plug and respiratory mask for dust and fume was lower than that of not wearing. Conclusion : According to above results, this study suggests that more positive work environment managements such as the use of portable local ventilation system, the supply of highly qualified personal protective equipment, and consecutive education and training related to health protection should be need to acquire suitable health protection program for welders engaged in small-scale enterprises in Korea.

      • 퍼머제의 중금속 농도에 관한 연구

        Objective : This study was conducted to prepare the fundamental data on heavy metal concentration in hair permanents agents and to prevent the health impairment due to heavy metal. Methods and Material : The author determined of 5 heavy metals(Pb, Cr, Mn, Ni, and Cu) concentration hair permanent agents in the market with flameless atomic absorption spectrophotometer. Results : The results were as follows: 1. The geometic mean concentrations of total subjects were 0.04㎍/g for Pb, 0.09㎍/g for Cr, 0.06㎍/g for Mn, 0.03㎍/g for Ni, and 0.04㎍/g for Cu. 2. The most highest mean concentrations of heavy metals in hair permanent agents by using type were Cr for wave agent and straight cream agent, Cu for setting iron agent, Cr and Cu for coating perm agent. 3. Metal most hlghest mean concentrations of heavy in hair permanent agents by type of color was green for Cu. 4. The mean concentration of Cu among 5 heavy metals in hair permanent agents by marker was most highest. 5. There was most statistically significant difference on mean concentration in hair permanent agents between domestic and foreign products. 6. The exposure amount of heavy metal concentration by one time using the permanent agent were 4.O㎍ for Pb, 9.O㎍ for Cr, 6.0㎍ for Mn, 3.O㎍ for Ni and 4.0㎍ for Cu. Conclusion : As above results author suggest to prepare the preventive program of health impairment due to heavy metal by long term chronic exposure to who were using the hair permanent agents hair designer and customers.
