Helicobacter pylori의 생육과 각종 유기산의 식품 보존료로서의 효과

Helicobacter pylori의 생육과 각종 유기산의 식품보존료로서의 효과를 검토하였다. 우선 보존료로 현재 식품첨가물로 사용되고 있는 소르빈산(sorbic acid), 프로피온산(propionic), 안식향산(benzoic acid) 등의 H. pylori에 대한 항균능력을 검토한 결과 소르빈산, 프로피온산, 안식향산의 H. pylori에 대한 항균효과가 관찰되었으며 안식향산은 소르빈산이나 프로피온산과 비교했을 때는 별로 큰 효과를 나타내지 못하였다. 보존료의 조합에 대한 H. pylori의 생육억제효과는 0.5% 소르빈산과 0.5% 프로피온산을 조합하는 경우 나타났으며 다른 조합군과 비교할 때 큰 차이를 나타내지 못하였다. 유기산과 H. pylori의 생육관계에서는 단독 첨가시에는 항균력이 크게 나타나지 않고 조합하는 경우에만 좋은 항균력을 나타내었다. 유기산, 보존료와 H. pylori 생육관계는 유기산류는 어떤 보존료와 혼합하여도 항균력에 상승효과가 있는 것으로 나타났다. 특히 구연산의 경우 소르빈산과 조합한 경우에는 H. pylori생육이 많이 억제되었으며 그 다음이 구연산+안식향산, 구연산+프로피온산 순으로 균에 대한 항균력이 감소되었다. 또 사과산과 숙실산은 구연산과 비슷한 경향을 나타내었다. The purpose of this study was to examine the effect of various organic acids as food preservatives on the growth of Helicobacter pylori(H.pylor). The antimicrobial effects of sorbic acid, propionic acid and benzoic acid currently used as food preservatives were examined in light of the growth of Helicobacter pylori. Each of them was observed to have positive antimicrobial effect on Helicobacter pylori. However, benzoic acid wasn't higher than sorbic acid and propionic acid in antimicrobial effect. As for the combination of preservatives, antimicrobial activity of H. pylori was shown in the combination of sorbic acid(0.5%) and propionic acid(0.5%). However, its effect was not significantly different in comparison with other combinations. In the relationship of organic acids and H. pylori's growth, organic acid had no effect on antimicrobial activity when it was used alone, whereas antimicrobial effect was shown to be better in case of combination. If intermix of organic acids and preservatives and addition to the medium for suppression of H. pylori growth were achieved, organic acid exhibited synergistic effect on suppression of H. pylori's growth, even in combination of any other preservatives. In particular, in combination of citric acid and sorbic acid, the growth of H. pylori was shown to be remarkable suppression. And the effectiveness on suppression of H. pylori's growth was lowered in order of citric acid + bezoic acid and citric acid + propionic. acid. It was also found that the antimicrobial effects of malic acid and succinic acid were similar to those of citric acid.

One of the contributory causes to poor pregnancy rates in assisted reproduction has been the decreased viability of transferred embryos and the transfer of four-cell embryos into and environment that naturally would be receptive only to 5-day-old blastocysts. In this paper, we have reviewed on the role of helper cells(cocultures) to mimic the in vivo, environment to bring about improved embryo quality and an increased number of blastocysts for replacement in IVF patients. The behavior of helper cell is first presented, followed by their use as cocultures for fertilization and cleavage of embryos. The mode of action and specificity of helper cells are also discussed. The currently available results of pregnancies after cocultures are presented together with future aspects of research that are necessary to refine the coculture system, The ultimate aim is to mimic in vivo conditions in vitro, so that at least the pregnancy rates of assisted conception can be parallel to normal fecundity in the human. Therefore, a very attractive future includes the freezing of blastocysts generated from coculture, thawing, and replacing them in natural cycles.

Background: The aim of the study was to describe the characteristics, treatments, and outcomes of critically ill patients with pandemic Influenza A/H1N1 2009 at a major medical center in Korea. Methods: This retrospective observational study examined critically ill adult patients with pandemic Influenza A/H1N1 2009, who were admitted to the AMC between August and December 2009. Results: 27 patients with confirmed pandemic Influenza A/H1N1 2009 were admitted to the intensive care unit (ICU) at the Asan Medical Center (AMC). The median age (IQR) was 59 years (41~67), and 66.7% of the patients were older than 51 years. A total of 81.5% of the patients had 2 or more co-morbidities. The median time (IQR) from symptom onset to presentation was 2 days (1~4), and the median time from presentation to ICU admission was 0 days (0~1.5). All patients received oseltamivir (300 mg/day) and 13 patients received triple combination therapy (oseltamivir, amantadine, ribavirin). Twelve patients required mechanical ventilation on the first day of ICU admission. A total of 6 patients (22.2%) died within 28 days of admission. The patients who died had significantly higher acute physiology and chronic health evaluation (APACHE) II and sequential organ failure assessment (SOFA) scores at presentation. There were no significant differences in age, co-morbidities, or antiviral regimens between survivors and non-survivors. Conclusion: Critical illness related to pandemic Influenza A/H1N1 2009 was common in elderly patients with chronic co-morbidities. All patients were given high-dose oseltamivir or triple combination antiviral therapy. Nonetheless, patients with critical illnesses associated with pandemic Influenza A/H1N1 2009 had a death rate of 22.2%.

<P>We have shown that the bacterial iron chelator, deferoxamine (DFO), triggers inflammatory signals including the production of CXC chemokine IL-8, in human intestinal epithelial cells (IECs) by activating the ERK1/2 and p38 kinase pathways. In this study we investigated the mechanisms involved in IL-8 generation by DFO, focusing on the transcription factors involved and the roles of both mitogen-activated protein kinases (MAPKs) in the transcription factor activation. Treatment of human epithelial HT-29 cells with DFO markedly up-regulated the expression of the essential components of the transcription factor AP-1 at a transcriptional level, while it minimally affected the expression of the NF-κB subunits. DFO also induced AP-1-dependent transcriptional activity in HT-29 cells, and this activity was further augmented by the wild-type c-Jun transfection. In contrast, the AP-1 activity by DFO was markedly decreased by the dominant-negative c-Jun transfection. Electrophoretic mobility shift assays revealed that DFO increases the specific binding of AP-1 but not of NF-κB. Such AP-1 binding and transcriptional activities were blocked by the inhibitors of the ERK1/2 and p38 kinase pathways, suggesting that both mitogen-activated protein kinases (MAPKs) lie upstream of AP-1. Besides its action on AP-1, DFO also induced the specific binding of other transcription factors such as CREB and Egr-1. In summary, our results indicate that iron chelator-induced IL-8 generation in IECs involves activation of ERK1/2 and p38 kinase and downstream activation of AP-1. A possible link between iron status and two additional transcription factors, that is, CREB and Egr-1, rather than NF-κB, was also suggested. J. Cell. Based on published guidelines on the management of chronic constipation, secondary causes should be excluded and then patientsshould be told to increase their dietary fiber intake to 20-25 g per day. If these measures do not improve the symptoms, conventionallaxatives are generally the next choice. Although there is limited evidence for the efficacy of these older laxatives due toa lack of well-designed clinical trials, most clinicians agree that they are effective at relieving the symptoms of constipation. Conventional laxatives include bulk-forming, osmotic, and stimulant laxatives. Bulking laxatives consist of fiber such as psyllium,cellulose, and bran. Osmotic laxatives are classified into sugar-based laxatives and polyethylene glycol. Bisacodyl and sodium picosulfateare stimulant laxatives. Understanding their mechanisms of action, efficacy, and side effects might improve the quality oflife of patients suffering from chronic constipation.
시청각자료를 이용한 환자교육이 대장내시경 검사에 미치는 영향

Background/Aims: The purpose of this study is to investigate how effectively audiovisual aids education can improve patients``bowel preparation and their satisfaction during colonoscopic examination. Methods: The control group consisted of 116 patients and the experimental group consisted of 109 patients. The control group was given the existing test guidebooks and the experimental group was offered audiovisual aids education immediately before the test. After the test, the patients assessed. Results: In regard to the organization and consistency of colonoscopic education, the experimental group showed a statistically significant score (p=0.000). In regard to satisfaction with the colonoscopy, the average score of the control group was 7.50±2.02, while that of the experimental group was 8.63±1.76 (p=0.000). In regard to bowel preparation, the experimental group showed a statistically meaningful score compared to that of the control group (p=0.002). In terms of examination time comparison, the experimental group returned a statistically meaningful score (p=0.005). In regard to the nurses`` task satisfaction, according to the information offered, the experimental group showed a statistically significant score (8.47±1.32) compared to the control group (6.49±2.23)(p=0.000). Conclusions: The audiovisual aids education enhanced the patients`` satisfaction. Therefore these aids are worth incorporating within nursing methods. (Intest Res 2007;5:52-59)
