Left main coronary artery (LMCA) thrombosis is rare and the cause should be determined. A previously healthy young man presented with severe chest pain and dyspnea. The electrocardiogram showed typical ST-segment elevation myocardial infarction with clinical instability. Emergency coronary angiography revealed complete LMCA occlusion by thrombosis. After reperfusion, the patient was admitted to the cardiac care unit. He was diagnosed with hemolytic anemia and tested positive for antinuclear antibodies. Systemic lupus erythematosus (SLE) and LMCA disease due to systemic thrombosis were diagnosed. Steroids were started and the patient was discharged without complications. We report this rare case of LMCA thrombosis as an initial presentation of SLE.
Bronchogenic cyst masquerading as malignant pericardial effusion with tamponade

We report on a rare case involving a 23-year-old female patient with mediastinal cystic mass complicated with acute pericarditis and cardiac tamponade. Pericardial fluid demonstrated lymphocyte-predominant exudate and the level of carcinoembryonic antigen (CEA) was unexpectedly elevated. Successive aspiration of mediastinal cystic mass revealed a very high level of CEA (>100,000 U/mL) and carbohydrate antigen 19-9 (>15,000 ng/mL). This patient was clinically diagnosed as an infected bronchogenic cyst complicated with pericarditis and cardiac tamponade. The treatment resulted in alleviation of her symptoms.

AMI is a large-scale IoT system that provides various customer services such as demand response and load analysis service by collecting and analyzing customers" electricity consumption in semi-real time using bidirectional communication. Since the existing AMI system used a non-standard private protocol, standardization of the data model and transmission method is required. In this paper, the data model and transmission method based on the LwM2M IoT protocol were designed to be suitable for the AMI system. In addition, a prototype was developed and the performance and stability were tested in the field. As a result of the field test, it was confirmed that the timely data collection rate of more that 99% was achieved, and the collection time of AMI information was greatly reduced by more that 95% compared to the existing one. Therefore, it is considered that it can be used for future distribution system prediction and demand management service.
특발성 척추측만증의 한의학적 치료에 대한 국내 연구 동향: 국내 학술지를 중심으로

Objectives This study aimed to analyze the trends of Korean traditional medicine treatment for idiopathic scoliosis. Methods We searched clinical studies used Korean traditional medicine for idiopathic scoliosis in 4 Korean online databases ‘Koreanstudies Information Service System’, ‘Earticle’, ‘Oriental Medicine Advanced Searching Integrated System’, ‘Research Information Sharing Service’. We selected 18 studies and analyzed their characteristics according to author, publication year, number/sex/age of patient, treatment period, intervention, outcome measure and result. Results Eighteen case report studies were selected by criteria and randomized controlled trials were not found. The most frequently used therapy was Chuna therapy. Cobb's angle was most commonly used outcome measure. Conclusions In this study, we reviewed studies used Korean traditional medicine treatments for idiopathic scoliosis. However, evidence is limited because all selected studies were case reports. This study suggests that further clinical studies are needed to provide higher evidence about Korean traditional medicine treatment for idiopathic scoliosis. (J Korean Med Rehabil 2020;30(4):55-64)

그람 음성이며 긴 막대 모양의 betaproteobacteria에 속하는 GR16-43을 한강 발원지 검룡소에서 분리하였다. GR16-43 균주에 대한 유전체분석을 실시하였으며, G + C 비율이 67.12%인 4,806,848 bp 크기의 염기서열을 얻었다. 유전체 특징은 황산화와 관련된 다량의 유전자를 보유하고 있어 균주의 잠재적 중요성을 보여준다. 이러한 결과는 GR16-43 균주가 빈영양 담수 환경에서의 적응 연구를 위한 유전체 정보를 제공한다. A betaproteobacterium strain GR16-43 was isolated from a surface layer of the Geomnyong Pond in Republic of Korea by a dilution-to-extinction culturing method. We report the whole genome sequence of the strain GR16-43, which contains 4,806,848 bp with a G + C content 67.12%, and to include 4,424 protein-coding genes and 47 transfer RNA genes. The genome was determined to contain the genes encoding carbon monoxide dehydrogenase, nitrate reductase, nitrite reductase, nitric oxide reductase, and the sulfur oxidation (sox) gene cluster, highlighting the potential importance of the bacterial group represented by the strain in the cycling of inorganic elements. These results indicate that strain GR16-43 genome showed several traits indicating adaptation of the bacteria to living in freshwater environments.

Background: Hemorheologic indices are known to be related to vascular complications in variable clinical settings. However, little is known about the associations between hemorheologic parameters and acute myocardial infarction (AMI) in type 2 diabetes mellitus (T2DM). The purpose of this study was to demonstrate the changes of hemorheologic environment inside of blood using hemorheologic parameters, especially the elongation index (EI) and critical shear stress (CSS) in diabetics with versus without AMI. Methods: A total of 195 patients with T2DM were enrolled. Patients were divided into the study group with AMI (AMI+, n=77) and control group (AMI–, n=118) who had no history of coronary artery disease. Hemorheologic parameters such as EI and CSS were measured and compared between the two groups. Results: The EI was lower (30.44%±1.77% in AMI+ and 31.47%±1.48% in AMI–, P<0.001) but the level of CSS was higher (316.13±108.20 mPa in AMI+ and 286.80±85.34 mPa in AMI–, P=0.040) in the AMI+. The CSS was significantly related to the erythrocyte sedimentation rate (R2=0.497, P<0.001) and use of dipeptidyl peptidase-4 inhibitors (R2=0.574, P=0.048). Conclusion: Diabetics with AMI resulted in adverse hemorheologic changes with lower EI and higher CSS compared to diabetic subjects without AMI. Evaluation of the hemorheologic parameters may provide valuable supplementary information for managing patients with AMI and T2DM.
