Numerical optimization of two dimensional high lift configuration was performed with flow solver and optimization method based on RSM(Response Surface Model). Navier-Stokes solver with Spalart-Allmaras turbulence model was selected for the simulation of highly complex and separated flows on the flap. For the simultaneous optimization of both flap shape and setting (gap/overlap), 10 design variables (eight variables for flap shape variation and two variables for flap setting) were chosen. In order to generate the response surface model, 128 experimental points were selected for 10 design variables. The objective function considering maximum lift coefficient, lift to drag ratio and lift coefficient at specific angle of attack was selected to reduce flow separation on the flap surface. The present method was applied to two dimensional fowler flap in landing configuration. After applying the present method, it was shown that the optimized high lift configuration had less flow separation on the flap surface and lift to drag ratio was suppressed over entire angle of attack range.

Purpose: To report a rare case of Acanthamoeba keratitis in both eyes related to cosmetic contact lenses. Methods: A 17-year-old girl with a history of wearing cosmetic contact lenses presented with keratitis. She purchased cosmetic contact lenses on the internet, and used contact lens care system irregularly with tap water. Results: After analysis of the corneal scraping, the contact lenses and the storage solution, the organism Acanthamoeba was identified. The patient was treated with polyhexamethylene biguanide (PHMB) and chlorhexidine for 3 months, and recovered with normal visual acuity. Conclusions: Poor hygiene and insufficient disinfection may be major risk factors for Acanthameoba keratitis in cosmetic contact lens wearers. The cosmetic contact lens user should receive professional advice before using these lenses, and this precaution must be communicated to the public.
청폐사간탕(淸肺瀉肝湯)이 급성기 중풍환자의 동맥경직도 및 맥압에 미치는 영향

Objectives : Arterial stiffness and pulse pressure are related to cardiovascular and cerebrovascular survival and longevity. This study is aimed at examining the effects of Chungpyesagan-tang on arterial stiffness and Pulse pressure in acute stroke Patients. Methods: The subject of this study was acute strike Patients within 1 week after ictus, with Cardio-ankle vascular index(CAVI) higher than 9.0. They were divided into two groups: A treatment group (n=44) and a control group(n=46). For two weeks, Chungpyesagan-tang was given to the former, other herbal medicines to the latter. used for stroke patients for the control group for 2 weeks. At the end of first and second week, CAVI, pulse Pressure, National Institute of Health stroke scale(NIHSS), Modified Barthel Index(MBI) were measured. Serum lipid Profile, aspartate transaminase(AST), alanine transaminase(ALT). blood urea nitrogen(BUN), creatinine were also measured at the end of the study. Results : After 2 weeks, CAVI and Pulse Pressure in Chungpyesagan-tane group were significantly tower than those in the control group(P<0.05). NIHSS and MBI were improved in both groups. But there was no significant difference between the treatment group and the control group in terms of the NIHSS and MBI. Conclusions : We suggest Chungpyesagan-tanghas desirable effects on arterial stiffness and Pulse Pressure of acute stroke patients. It can improve morbidity and mortality of patients on the basis of influencing vascular stiffness and increased pulse pressure.

Purpose: To analyze the morphology and density of corneal tissue in patients with bullous keratopathy using in vivo confocal microscopy (IVCM).<br /> Case summary: Four eyes of 4 patients with clinically diagnosed bullous keratopathy were examined using IVCM. Cross-sectioned corneal images of the corneal epithelium, Bowman’s layer, stromal layer, Descemet’s membrane, and endothelium were evaluated. In bullous keratopathy patients, the outline of corneal epithelial wing cells was blurred. Several hyper-reflective whitish dots were found at the corneal epithelial level and a reduction in the density and thickness of the corneal nerves was observed. There was also a reduction in the density of the stromal keratocytes. The outline of the stromal keratocytes was blurred. Some keratocytes resembled myofibroblast. The hypo-reflective vertical strands were observed in the stroma adjacent to the Descemet’s membrane. Endothelium showed hyper-reflectivity with associated increased endothelial pleomorphism and polymegathism.<br /> Conclusions: In patients with bullous keratopathy, IVCM showed tissue edema in the corneal epithelium, stroma, and endothelium. Hyper-reflectivity appearances were noted due to the increase in the metabolic rate of the cells or extracellular matrix accumulation. During the conceptual design of turboprop aircraft, the power effect driven from rotating propeller is typically obtained from empirical data. In the present paper, propeller power effect was obtained by using unsteady three-dimensional Navier-Stokes solver with $k-{\omega}$ turbulence model for the accurate prediction of turboprop aircraft performance. In order to simulate the relative motion between propeller and fuselage, unsteady sliding mesh method was used. During simulation, three flow conditions such as climb, cruise and descending flight were selected considering the flight envelop of the real turboprop aircraft. For the correction of aerodynamic coefficients, the thrust effect of engine exhaust gas was included based on the engine manufacturer's data. Using the computational results, the correction table for the aerodynamic coefficient of turboprop aircraft was suggested for the performance analysis of turboprop aircraft.

Sleep-related eating disorder (SRED) is a newly recognized parasomnia that describes a clinical condition of compulsive eating under an altered level of consciousness during sleep. Recently, it is increasingly recognized in clinical practice. The exact etiology of SRED is unclear, but it is assumed that SRED might share features of both sleepwalking and eating disorder. There have been also accumulating reports of SRED related to the administration of various psychotropic drugs, such as zolpidem, triazolam, olanzapine, and combinations of psychotropics. Especially, zolpidem in patients with underlying sleep disorders that cause frequent arousals, may cause or augment sleep related eating behavior. A thorough sleep history is essential to recognition and diagnosis of SRED. The timing, frequency, and description of food ingested during eating episodes should be elicited, and a history of concurrent psychiatric, medical, sleep disorders must also be sought and evaluated. Interestingly, dopaminergic agents as monotherapy were effective in some trials. Success with combinations of dopaminergic and opioid drugs, with the addition of sedatives, has also been reported in some case reports.
피부과 외래환자에서 아토피피부염의 상대 빈도와 임상적 특징 및 검사 소견에 관한 연구

Background : Atopic dermatitis(AD) is a common disease, and population-based studies indicate that the frequency of AD has increased substantially during recent decades. However, there have been few studies on the hospital based relative frequency, and clinical and laborat,ory findings of AD in Korea. Objective : The presenl study was aimed to elucidate the relative frequency of AD on the outpatient base, and to investigate the clinical manifestations and immunological laboratory findings. Methods : Our study was designed by analysing outpatients with AD via physical examination, questionnaires and immunological investigations such as skin prick tests, Multi test CMI, and measurernent of eosinophil counts, total IgE, Derrnatophagoides pteronyssinus-specific IgE and eosinophil cationic protein(ECP). Results : The results were as follows : 1. The relat,ive frequen:y of AD on the hospital base in 1995-1996 was 8.6%. 2. The mean age of onset was 3.36 years. 57.3% of the patients had their onset of eczema by the age of 1 year, 65.5% by 2 years and 83% by 5 years. The most common site at the onset was the face followed by antecubital area, popliteal area, neck, and trunk. 3. The predilection site on physical examination were the face, neck, antecubit,al area, popliteal area and trunk in decreesing order. 4. Concomitant allergic rhinitis, keratoconjunctivitis and asthma were present in 2~3.4%, 14.1%, and 9.5% of patients with AD, respect.ively, whereas pure AD was present in 34.1%. 5. Family history of atopy was observed in 60.5% of AD patient.s. 6. The most frequent ninor fea~ture of AD was xerosis followed by perifollicular accentuation, allergic shiner, cheilitis, imitation allergy, palrnar hyperlinearity and molluscum cantagiosum. 7. Seasonal aggravation was found in 86.8% of patients with AD, especially in summer and winter, The rnost significant aggravating factors were wools followed by sweating, heating, dryness, excessive bat.hing and emotional stress. 8. Abnormal levels of .osinophils, total IgE and ECP were seen in 44.8%, 78.9% and 31.7% Of patients with AD, respec.ively. 9. Cell mediated immunty was decreased in 68.2% of patients with AD. 10. Positive reactivity( at least rnore than one alle~rgen) in the skin prick t,est was noted in 76% of pat,ients with AD and the most popular 8 allergens were D. pteronyssinus, D. farinae, cat fur, M4 mixed threshing dust., dog hair, mugwort pollen, H3 tree pollen and birch pollen. 11. The specific IgE aritibodies to D. pteronyssinus(>+1) were found in 72.3% of AD patients and correlated with the r esults of the skin prick test in 76%. Conclusion : The hospi al-based relative frequency of AD investigated in our study tends to be increased. As the clinica featu~res and laboratory findings of AD might be influenced by genetic, environrnental and psychological factors, the systernic evaluation of patients with AD will be needed (Kor J Dermatol 1997;35(1): 96-106)
