        문화체험을 위한 민박 서비스디자인 연구 -부산광역시 사하구 감천문화마을 민박을 중심으로-

        본 연구는 도시민박업 활성화에 따라 도시재생 지역에서 지역의 문화를 체험하는 체류형 관광을 유도하기 위한 보다 효과적인 문화체험 민박서비스 모델 제안을 목적으로 한다. 주민을 위한 수익창출을 기대하는 감천문화마을의 민박서비스 디자인을 통해 지역의 문화체험 콘텐츠와 민박서비스가 원활하게 융합되어 방문자들이 숙박을 통해 차별화 된 예술적 문화체험을 하게하는 콘텐츠를 도출한다. 민박서비스 이용자에게 보다 나은 경험을 제공하기 위해서는 이용자의 요구와 함께 이용자의 행동 등을 예측하고 이에 적합한 서비스를 제공하는 것이 중요하다. 따라서 주요 이용자를 종합 분석하여 적합한 서비스 시나리오 모델을 도출하기 위해 가상의 이용자 모델인 퍼소나를 기반으로 한 시나리오 디자인을 하였다. 감천문화마을의 현황분석에 따른 민박이용 정황시나리오를 구성하고, 민박서비스의 이해관계자 분석을 통해 민박이용자와 운영자뿐만 아니라 마을의 문화예술 관련자 그룹을 도출하였다. 감천문화마을 방문객 유형분석을 통한 민박서비스 체험 퍼소나를 외국인과 내국인으로 설정하여, 민박서비스와 체험요소가 전달되는 과정을 담은 고객여정맵이 작성되었다. 다음단계로 민박서비스의 문화체험 요소의 주요 경로시나리오를 작성하였고, 이를 바탕으로 문화체험 민박서비스 커뮤니케이션 모델을 구축하였다. 문화체험 민박서비스의 점검시나리오를 기반으로 각 서비스와 관련 이해관계자의 원활한 서비스 흐름을 가시화하기 위해 서비스청사진을 작성하였다. 서비스디자인은 단순히 아름답게 작품을 만드는 것이 아니라 사람들로 하여 어떻게 생각하고, 행동하게 하는가의 문제를 발견하고 해결하는 것이다. 감천문화마을의 가장 큰 매력은 마을 특유의 역사와 경관 그리고 조형작품에 의한 문화적 분위기이다. 따라서 민박서비스의 개발은 마을의 문화와 충분히 결합시켜야만 민박을 중심으로 한 문화체험 시스템을 형성할 수 있다. 민박이 자리 잡고 있는 지리적 위치와 결합된 역사생활의 흔적을 발굴하여, 방문자들로 하여금 볼거리, 먹거리, 예술작품 및 마을이야기체험 등 감천의 특별한 분위기를 체험할 수 있게 하여 좋은 마을 이미지를 형성할 수 있다. This study is to proposals put forward an effectual culture experience model of the local culture experience for lodging tourism, that is invigorate from urban home-stay industry depending on urban regeneration local. By the Gamcheon Culture Village``s home-stay service design that is expectant benefit creation for the local residents, local culture experience contents and home-stay service melting together smoothly, sustainable home-stay service contents let satisfaction improve, it is not only for the users and also includes the service provider as local residents. It is important to provide the suitable service from user``s needs and behavior prediction to provide the user a better experience. Thus the main users aggregate analysis, suitable service scenario model derive that oder to scenario design stand on the basis of the imagine user personal model. With the analysis of the Gamcheon Culture Village``s actuality circumstance to constitute the context scenario, through the Stakeholder analysis, users and operators, export the related groups of village``s culture and art. By the type analysis of the Gamcheon Culture Village``s visitor, the home-stay service experience personal was setting as foreign student and domestic employee, make the journey-map pass on the transmit process of the home-stay service and experience element. The next step is make the culture experience home-stay service communication model by the key-path scenario through home-stay service culture experience element. Finally, base of the culture experience home-stay service validation scenario and the related stakeholder of service process flow smoothly , in order to visualization to made the service blueprint. Service design is not only for made a beautiful products, but also is to find and solve the problems according people how to thinking or behavior. The most attractive point of the Gamcheon Culture Village is the unique cultural atmosphere that is from Village``s history, landscape and arts works. Therefor, develop the home-stay service is building the culture experience system with the centre of home-stay that is must combined regional culture fully. Gamcheon Culture Village``s home-stay pass on the geographical position and related traditional life trace excavate, to create special cultural experience for visitors through the Gamcheon Culture Village``s seeing, eating, arts works and history story etc.

      • Annual Financial Impact of Thyroidectomies for Nodular Thyroid Disease in China

        A large proportion of patients with thyroid nodules in China undergo thyroidectomy in order to get confirmatory histology diagnosis. The financial impact of this modality remains to be investigated. To evaluate rationality of performing thyroidectomy without a routine FNA preoperatively from the economic perspective, we conducted a retrospective, observational study of all archival thyroidectomies with records of cost per stay (CPS), cost per day (CPD) and length of stay (LOS) from 2008 to 2013 in the First Affiliated Hospital of Nanjing Medical University. We compared all the parameters between cancer and non-cancer thyroidectomies. We recruited 6, 140 thyroidectomies with valid records of CPS, CPD and LOS in this period. The CPS of cancer thyroidectomy was significantly higher than non-cancer thyroidectomy. The percentage of cancer thyroidectomy increased from 26.5% to 41.6%. The percentage of annual cost of cancer thyroidectomies rose from 30.2% to 45.2%. The LOS for cancer and non-cancer thyroidectomy decreased while the CPD increased in the past six years. The estimated national cost in 2012 for all thyroidectomies would be USD 1.86 billion with USD 1.09 billion for non-cancer thyroidectomies. We have witnessed great improvement in the healthcare for patients with thyroid nodules in China. However, given limited healthcare resources, currently thyroid FNA for more precise preoperative diagnosis may help to curb the rapidly increasing demand in healthcare costs in the future for nodular thyroid disease in China.

        Predictors for Better Blood-Flow Restoration of Long-Segmental Below-the-Knee Chronic Total Occlusions after Endovascular Therapy in Diabetic Patients

        Objective: To prospectively investigate predictors for good restoration of blood flow of below-the-knee (BTK) chronic total occlusions (CTOs) after endovascular therapy in diabetes mellitus (DM) patients. Materials and Methods: A total of 120 long-segmental (≥ 5 cm) BTK, CTOs in 81 patients who underwent recanalization were included in this study. After angioplasty, blood-flow restoration was assessed using modified thrombolysis in myocardial ischemia grades and classified as good flow (grade 3) and poor flow (grade 1/2). One hundred and six CTOs with successful recanalization were divided into a good flow group (GFG; n = 68) and poor flow group (PFG; n = 38). Multivariate logistic regression analyses were undertaken to determine independent predictors of blood-flow restoration. Receiver operating characteristic curves were constructed to determine the best cutoff value. The prevalence of target-lesion restenosis during follow-up was compared between two groups. Results: Univariate analyses suggested that CTOs in GFG were characterized by lighter limb ischemia (p = 0.03), shorter course of ischemic symptoms (p < 0.01) and lesion length (p = 0.04), more frequent use of intraluminal angioplasty (p = 0.03), and higher runoff score (p < 0.01) than those in PFG. Multivariate regression analyses suggested that distal runoffs (p = 0.001; odds ratio [OR], 10.32; 95% confidence interval [CI]: 4.082–26.071) and lesion length (p < 0.001; OR, 1.26; 95% CI: 1.091–1.449) were independent predictors for good flow restoration. Kaplan-Meier analyses at 12 months showed a higher prevalence of non-restenosis in GFG (p < 0.01). Conclusion: Distal runoffs and lesion length are independent predictors for good flow restoration for long-segmental BTK, CTOs in DM patients who receive endovascular therapy.

        Background: Sentinel lymph node (SLN) mapping has been recommended as an alternative staging approach to lymphadenectomy for apparent uterine-confined endometrial cancer (EC). However, the prognostic value of SLN mapping alone instead of systematic lymphadenectomy on EC patients remains unclear. Methods: A multi-center, open label, non-inferiority randomized controlled trial has been designed to identify if SLN mapping alone is not inferior to pelvic lymphadenectomy on prognosis of patients with intermediate-high-risk EC clinically confined to uterus. Eligible patients will be 1:1 randomly assigned to accept SLN mapping or pelvic lymphadenectomy. The primary endpoint is the 2-year progression-free survival (PFS). The second points are the 5-year PFS, 5-year overall survival, surgery-related adverse events and life quality. A total of 780 patients will be enrolled from 6 hospitals in China within 3-year period and followed up for 5 years. Trial Registration: ClinicalTrials.gov Identifier: NCT04276532

      • Impact of a Clinical Pathway on Hospital Costs, Length of Stay and Early Outcomes after Hepatectomy for Hepatocellular Carcinoma

        Background: A clinical pathway (CP) can standardize and improve perioperative care for a number of interventions. In hepatic surgery, however, pertinent evidence is very limited. This study was conducted to implement a CP for hepatocellular carcinoma (HCC) patients undergoing hepatectomy, and to evaluate its effects on hospital costs, length of hospital stay (LOHS) and early clinical outcomes. Materials and Methods: Medical records for HCC patients undergoing hepatectomy were retrospectively reviewed before implementation of a CP (the non-CP group) from March 2012 to August 2012. This information was compared with the data collected prospectively from patients after implementation of the CP (the CP group) between September 2012 and April 2013. Hospital costs, LOHS and early clinical outcomes were evaluated and compared between groups. Results: There were no significant differences in terms of patient clinical characteristics between the two groups. For clinical outcome measures, no significant differences were found in postoperative complications, mortality and readmission rate. The hospital costs were significantly reduced from 24,844 RMB in the non-CP group to 19,761 RMB in the CP group (p<0.01). In addition, patients of the CP group also had shorter LOHS compared with the non-CP group (8.3 versus 12.3 days, p<0.001). Conclusions: The CP proved to be an effective approach to minimize hospital costs and LOHS with hepatectomy for HCC without compromising patient care.

        Most materials exhibit different tensile and compressive strains given the same stress applied in tension or compression. These materials are known as bimodular materials. An important model of bimodular materials is the criterion of positive-negative signs of principal stress proposed by Ambartsumyan. This model is mainly applicable to isotropic materials and deals with the principal stress state in a point. However, due to the inherent complexity of the constitutive relation, FEM based on iterative strategy and analytical methods based on a simplified mechanical model are required. In this paper, we review the basic assumptions of this model and its development, several innovative computational methods, and some important engineering applications. We also discuss the sequent key problems in this field.

        Pu-Erh Tea Hot-Water Extract Activates Akt and Induces Insulin-Independent Glucose Transport in Rat Skeletal Muscle

        Skeletal muscle is a major organ that is important for whole-body glucose metabolism. We found that when isolated rat epitrochlearis muscle was incubated with a Pu-erh tea hot-water extract (PTE) for 30 min, the rate of 3-O-methyl-D-glucose (3MG) transport increased in the absence of insulin. This activation was associated with an increase in Ser473 phosphorylation of Akt, a signaling intermediary leading to insulin-dependent glucose transport, but not Tyr458 phosphorylation of phosphoinositide 3-kinase p85, an upstream molecule of Akt. PTE-stimulated 3MG transport was also not accompanied by Thr172 phosphorylation of the catalytic α-subunit of 5′-AMP-activated protein kinase (AMPK). Gallic acid, a water-soluble ingredient in Pu-erh tea, stimulated Akt phosphorylation, but not AMPK phosphorylation. These results suggest that Pu-erh tea potentially promotes skeletal muscle glucose transport at least in part by activating Akt.

        <P><B>Abstract</B></P> <P>Plasmonic thin film solar cells deposited on periodically textured photonic crystal substrates have been extensively studied since the substantially enhanced light absorption. The reduction of parasitic absorption losses in the metal and spacer layers becomes one of the key issues to achieve high efficiency solar cells. Herein, plasmonic amorphous silicon (a-Si:H) flexible thin film solar cells with different thickness of oxide spacer layers are systematically investigated. An increase of the spacer layer thickness leads to an evolution in surface morphology of AZO and final devices. More intriguingly, the increase of spacer layer thickness reduces the absorption in Ag layer while induces more absorption in spacer layer. The highest light absorption in silicon layer is observed as applying 100nm spacer layer, which is further verified by electrical measurements.         pH-sensitive drug controlled release core/shell fibers fabricated by combination of electrospinning and photopolymerization

        pH-sensitive drug loaded core shellfibers were fabricated by a combination of electrospinning and UVphoto-polymerization. Combretastatin A4 (CA4) was selected as the model drug loaded in PLA to test thepH-sensitivity property of the core shellfibers. The morphology of thefibers was studied by scanningelectron microscopy (SEM), and the core shell structure of thefibers was confirmed by transmissionelectron microscopy (TEM), and X-ray photoelectron spectroscopy (XPS). The drug release assay wastested via the UV–vis spectrophotometer, and the pH-sensitivity of core shellfibers was tested by drugrelease assay under pH 5.0 and 7.4.

        Reduction of the contact resistance in copper phthalocyanine thin film transistor with UV/ozone treated Au electrodes

        Bottom-contact (BC) copper phthalocyanine (CuPc) thin film transistor with UV/ozone treated Au as a source/drain electrode was fabricated and the contact resistance was estimated from the transmission line method (TLM). Comparing the properties of OTFT with untreated Au electrode, the performance of the BC CuPc-TFT with the UV/ozone treated Au electrodes was significantly improved: saturation mobility increased from 4.69 × 10−3 to 2.37 × 10−2 ㎠/V s, threshold voltage reduced from −29.1 to −6.4 V, and threshold swing varied from 5.08 to 2.25 V/decade. The contact resistance of the device with UV/ozone treated Au electrodes was nearly 20 times smaller than that of the device with untreated Au electrodes at the gate voltage of −20 V. This result indicated that using the UV/ozone treated Au electrode is an effective method to reduce the contact resistance. The present BC configuration with UV/ozone treated Au electrodes could be a significant step towards the commercialization of OTFT technology.
