        항정신병약물 사용 중인 정신분열병 환자에서 올란자판으로의 교체 방법에 관한 연구(II) : Comparison of Safety 안전성 비교

        연구목적: 이 다기관 공동임상연구는 사용 중인 항정신병약물을 ’직접 교체 방법’또는 ’시작-감량 교체 방법’중 한 가지 방법으로 올란자핀으로 교체한 후, 안정성 측면에서 두 교체 방법 간의 비교와 교체후의 변화를 관찰하기 위한 것이다. 방법: 국내 13개 병원의 입원 및 외래에 내원한 환자들 중 ICD-10 지단기준으로 정신분열병에 해당되며, 임상적으로 항정신병약물 교체가 필요한 환자를 대상으로 하였다. 두 가지 교체 방법 중 한 가지를 무작위로 피험자에 적용하였으며, ’직접 교체 방법’에 배정된 경우에는 사용중인 항정신병약물을 일시에 중단하고 10㎎의 올란자핀을 바로 투여하였고, ’시작-감량 교체 방법’에 배정된 경우는 10㎎의 올란자핀 투여하고 2주에 걸쳐서 기존 약물을 감량하여 중단하였다. 올란자핀 사용기간은 총 6주이며, 용량은 5∼20㎎ 범위로 제한하였다. 한정성 평가를 위해서 체중, 생명징후, 자발적인 이상반응 복, 실험실 검사 그리고 Simpson-Angus Scale(SAS), Barnes akathisia rating scale(BARS), Abnormal involuntary movement scale(AIMS). Liverpool University neuroleptic side effect rating scale(LUNSERS)등을 이용하였다. 결과: 총 103명의 정신분열병 환자를 대상으로 하였다. 사용한 올란자핀의 용량, 벤조디아제핀의 병용률, 탈락률과 탈락 사유, 자발적인 이상반응 보고, 생명징후, 실험실 검사 그리고 대부분의 부작용 척도 상에서 임상적으로 의미 있는 차이를 두 교체 방법간에 발견하지 못하였다. 다만 AIMS의 감소는 ’직접 교체 방법’군에서 보다 적었고, 항콜린제의 병용률은 ’시작-감량 교체 방법’군에서 보다 많았다. 기저 상태에서 전체 피험자의 SAS와 BARS 점수는 각각 3.5점과 1.8점이었으며 70% 이상의 피험자가 고프로락틴 혈증을 보였다. 올란자핀으로 교체한 후, SAS, BARS, AIMS 점수의 유의한 감소가 있었으며 고프로락틴 혈증을 보인 피험자 분율도 약 30%이하로 감소하였다. 그러나 교체 방법과 상관없이 올란자핀 교체 후 유의한 체중 증가가 있었다. 결론: 이 연구를 통해 교체 방법에 관계없이 비교적 안전하고 용이하게 올란자핀으로 교체 할 수 있음을 알 수 있었다. 그리고 기존 항정신병약물을 올란자핀으로 교체함으로써 일부 부작용들을 줄일 수 있음을 간접적으로 관찰할 수 있었다. 하지만 이 연구는 여러 제한점과 문제점을 지니고 있기 때문에 보다 체계적인 연구를 통해 검정이 필요하리라 생각된다. Objectives: This multicenter clinical trial involving 13 hospital sites compared the safely of switching to olanzapine between ’direct switching method’ and ’start-tapering switching method’. Method: This study included both inpatients and outpatients who fulfilled the criteria for schizophrenia as defined in the ICD-10, and were in need to be appropriate for switching antipsychotics. Subjects were randomly assigned to one of the two switching methods. For ’direct switching method’group, previous antipsychotics were abruptly discontinued and 10㎎ of olanzapine was administered, and previous antipsychotics was gradually tapered for 2 weeks. Olanzapine was used for 6 weeks and the dose was adjusted within the range of 5-20㎎. The safety of switching to olanzapine was measured with vital sings including body weight, adverse events reported spontaneously, laboratory tests, and various scales such as Simpson-Angus Scale(SAS), Barnes Akathisia Rating Scale(BARS). Abnormal Involuntary Movement Scale(AIMS), and Liverpool University Neuroleptic Side Effect Rating Scale(LUNSERS). Results: 103 patients were switched to olanzapine in this study. The comparison between two switching methods did not show any significant difference in the dosage of olanzapine used, the concomitant use of benzodiazepine, the rate and reasons of drop-out, the adverse events, vital signs, laboratory tests, and most scales for measuring side-effects. However, the decrease in AIMS scores was significantly lower in ’direct switching method’ group, and the concomitant use of anticholinergics was comparatively greater in ’start-tapering switching method’ group. At baseline, SAS and BARS scores were 3.5 and 1.8 points respectively, and more than 70% of the subjects showed hyperprolactinemia. After switching to olanzapine, SAS, BARS, and AIMS scores were significantly decreased and the proportion of the patients with hyperprolactinemia was also decreased to less than 30%. However significant weight gain after the treatment of olanzapine was observed regardless of switching method. Conclusion: This study may suggest that switching to olanzapine can be done with relatively high safety regardless of switching methods and olanzapine can significantly decrease some side-effects induced by other antipsychotics.

        부산.경남지역의 일부 고등학교 교사들의 물리치료에 대한 인식도 조사

        This study randomly chose 10 humanity high schools, which are located in Busan and KyoungNam province. We then examined the recognition rate of physical therapy among 378 teachers who were and still are working in the schools. 1. The recognition grade of physical therapeutics between men and women was significantly different with results of 80.7% of men and 69.7% of women(p<0.05). In the case of experiencing physical therapy, 83.6% of people said they knew the physical therapeutics and in the case of the opposite side, the result was also significantly different with a result of 67.8%(p<0.05). 2. We ranked the people on how much they knew about the physical therapeutics among men and women. With men, the actual experience of physical therapeutic came before what they heard from other people. In the case of women, what they heard from other people came first and the actual experience of physical therapeutic came after. According to the public information from Inje University, the recognition grade is recorded in the 3rd grade department as 21.5% while in the case of experiencing treatment from a physical therapist, the result was 39.1%. In the other department (the 1st grade department), what men and women heard from mass media was recorded at 17.5% and what they heard from other people, it was recorded at 33.8%(p<0.05). 3. The research about the recognition of academic ability of being a physical therapist was that 55.6% of teachers thought that 3-year-college was the best, which made the results different from the recognition grade of the experience of education(p<0.05). 4. In the recognition grade of physical therapeutics, there were significantly differences from 50.2% of men and 45.2% of women were expecting about 1mil ion won to 1.99million won. In case of 25.2% of people who have experience less than 15years at work, they expected about 2million to 2.99million won. The figures were higher than 10.1% of people who have experiences over 15years. About the recognition grade of physical therapists social level in the future is 48.8% of people who had experienced of physical therapy and 63.8% of people who never had experienced of it said it would be better in the future. From these results, the recognition about physical therapeutics and the view of its social status are optimistic and they know well about the level of academic ability of physical therapists. On the other hand, most teachers don't know exactly about physical therapists work. So we have to inform them aggressively for enhancing the recognition of physical therapy because high school students are well affected by their teachers when they face their future or major in university.

      • 실적공사비에 의한 개산견적모델의 개발 : 골프장 공사를 중심으로

        이 연구는 골프장 건설공사에 대해, 실적공사자료를 공사규모나 시간 등의 조건에 따라 분석함으로서, 공사비가 어떻게 변동하는지를 추정하기 위한 기본자료를 제시하고자 한다. 이 연구의 목적은 국내 골프장 건설공사를 하는데 실제 투입된 실적공사자료들을 비교·분석함으로서, 공사비지수를 이용하여 골프장 건설공사비를 추정할 수 있는 개략적인 공사비예측모형 개발을 시도하는데 목적이 있다. 공사비예측을 위한 비용-규모 지수를 구하기 위하여 사용한 식은 다음과 같다. log y = log A + nlogx 여기서, y: 공사비, x: 공사규모, n: 비용-규모 지수, A: 상수 연구의 결과, 7개의 골프장자료를 이용하여 회귀분석으로 골프장 공사비예측을 위한 비용-규모 지수 n을 구한 결과, 전체공사에 대해서는 0.75, 코스조성공사는 0.73, 조경공사 0.74, 부대공사 0.47, 배수공사 1.15로 도출되었다. This study for construction of golf courses is to present basic information and pattern of change of construction cost by looking at the capacity of construction, requiring time and other aspects. The propose of this study is to develop the model of brief cost expected by using cost index and analyzing the actual work cost data gathered in golf course construction industry in Korea. The equation used for the cost-capacity index in other to deliver the expected construction cost is followed next. log y = log A + nlogx where, y: the construction cost, x: the construction capacity, n: the cost-capacity index, A: the constant The results of this study, by regression analysis for the information of 7 golf course, it could be found the cost-capacity index n, such as 0.75 for whole construction, 0.73 for construction of building up courses, 0.51 for landscaping construction, 0.45 for appurtenant works, 1.15 for drainage construction.

      • 종목별 운동특성이 체급경기 선수들의 신체구성과 체력에 미치는 영향

        This study compares and analyzes body fat, subcutaneous fat, cardiopulmonary function and physical strength of 25 body builders, 25 weight lifters and 25 wrestlers who have exercise career more than 10 years and ranked high at national matches, aims to offer the materials to improve competitive performance with scientific training by events, calculates the mean and standard deviation using SPSS 11.0 statistical program, conducts one-way ANOVA for differential verification of the mean among groups and then obtains the following conclusions using Scheffe as posttest. 1. In comparison of body fat rates, body-builder group showed the least body fat rate compared to weight lifter and wrestler groups and in comparison of heart rate and vital capacity at stable state, wrestler group ranked higher than other two groups and it was statistically significant at the level of p<.001. 2. In comparison of physical strength, wrestler group showed higher physical strength in muscular strength (grip), muscular endurance (push-up, raising upper body), flexibility (bending upper body forward), equilibrium (standing on one leg with closed eyes), agility (side step, whole body reaction), cardiopulmonary endurance (maximum oxygen intake) than other groups and in power, weight lifter group was higher and in muscular strength, body-builder group was higher. Then the comparison of push-up showed statistically significant difference at the level of p<.05 and other physical strengths were statistically significant at the level of p<.001. 3. In comparison of subcutaneous fat thickness, body-builder group showed the least thickness at below jaw(p<.05), Triceps(p<.001), Biceps(p<.05), mid-axillary (p<.01), Subscapular(p<.05), Abdominal(p<.001) and Iliac crest(p<.05). Back neck part (p<.001) showed the least subcutaneous fat thickness at weight lifter group and chest part had the least subcutaneous fat thickness at weight lifter group, but there was no statistically difference. Subcutaneous fat of body-builder group was least at the parts of Front thigh(p<.01), Medial Calf(p<.001) and there was statistically significant difference.

      • 투기운동별 대학 선수들의 요추, 대퇴골밀도 비교

        In this study, the bone mineral density (BMD) of the femoral neck and lumbar vertebrae were measured using dual photon absorptiometry. The results were as follows. 1. In the priority of order among combat sport players, the lumbar vertebra BMD was highest in those engaged in wrestling(mean 1.5654±0.1518)g/㎠ and 1.3230±0.0804g/㎠ in taekwondo, 1.2107±0.0805g/㎠ in kumdo.(P<01) 2. In the priority of order among college combat sport players, the femoral neck BMD was highest in those engaged in wrestling(mean 1.4696±0.1485)g/㎠ and 1.4297±0.0663g/㎠ in taekwondo, 1.2976±0.1283g/㎠ in kumdo respectively.(P<5) 3. In the priority of order among college combat sport players, the Ward's Triangle BMD was highest in those engaged in wrestling(mean 1.4107±0.1459)g/㎠ and 1,3644±0.0565g/㎠ in taekwondo, 1.2280±0.1355g/㎠ in kumdo respectively.(P<5) 4. In the prioroty of order among college combat sport players, the trochanter BMD was highest in those engaged in wrestling(mean1.3517±0.1416)g/㎠ and 1.2891g/㎠ in taekwondo, 1.1597±0.1351g/㎠ in kumdo respectively.(P<5)

      • 방전관용 전극개발 및 특성에 관한 연구

        This paper describes the development of sputtered tungsten electrode for discharge tube by making use of Sputtering method using plasma. The Xe discharge tube was designed and fabricated by using sputtered tungsten electrode, and the ICCD(Intensified Charged Coupled Device) and monochromator were used in order to observe the discharge image distribution and to measure the signal intensity. As a result, the emission light form the electrode was spreaded almost uniformly over a wide wavelength band, and the discharge image distribution and the signal intensity show quite a uniform distribution. The emission light from the Xe discharge tube was peaked at 488[nm] and 648[nm].

      • 새로운 연노출의 효과지표로서의 혈장 δ-aminolevulinic acid 측정의 유용성

        To evaluate the usefulness of the measurement of plasma δ - aminolevulinic acid (ALAP) as a new effect indicator of lead exposure, the association of ALAP with blood lead, DMSA chelatable lead and bone lead with a special emphasis of genetic interaction of ALAD polymorphism was studied with 419 lead exposed workers and 85 non-lead exposed workers. Lead workers were recruited from storage battery industries, secondary smelting industries and other lead using industries and non-lead workers selected from electronic refrigerator manufacturing industry. Blood lead, DMSA chelatable lead and bone lead were chosen for parameters of lead exposure. Plasma and urinary ALA were selected for the effect parameter of lead intoxication. Information for age, smoking, drinking habit and body mass index were also obtained. Job duration for lead workers was also collected. The overall prevalence of the variant allele, ALAD type 1-2 or 2-2(ALAD2) in lead exposed workers was 10.0% (42 out of 419 lead workers) which was not differed from control workers (9.4% : 8 out of 85). The means blood lead and DMSA chelatable lead of lead workers with ALAD 1(ALAD type 1-1) were lower than those of lead workers with ALAD2, but the differences were not statistically significant. On the other hand, the mean of tibia lead of lead workers with ALADI was higher than that of lead workers with ALAD2, but the difference was not statistically significant. DMSA chelatable lead showed highest correlation with log transformed plasma ALA(LALAP)(r=0.682) and blood lead and tibia lead had r=0.650, r=0,402 correlation coefficient with LALAP respectively On the other hand. log transformed urinary ALA(LALAU) had lower correlation coefficient with all three parameters of lead exposure than LALAP. The equation of robustic simple linear regression of blood lead on LALAP was LALAP=2.0439+0.0165PbB in ALADI lead workers and LALAP=2.031+0.0121PbB in ALAD2 lead workers. ALAD genotype in the multiple regression analysis of blood lead with LALAP after adjusting for possible confounders(sex, ago, BMI, smoking and drinking status and job duration) showed significant main effect on LALAP resulting lower LALAP in ALAD2 lead workers than ALADI lead workers. On the other hand, effect modification of ALAD genotype was observed in the multiple regression analysis of tibia lead with LALAP after adjusting for possible confounders, but there was no significant effect of ALAD genotype in the multiple regression analysis of DMSA chelatable lead with LALAP after covariate adjustment. With above results, plasma ALA was found to be a useful effect parameter of lead exposure compared with urinary ALA. It was also observed that ALAD polymorphism made significant effect on the association of blood and tibia lead with ALAP. Thc lower plasma ALA in ALAD2 lead workers suggested supportive effect of ALAD2 in lead exposure.

        고등학교 지구과학 실험·실습 요목 비교 분석

        The purpose of this study is to investigate the status of the experimental and practical education in high school earth. The present status and reasonable management of the experimental and practical education in high school earth science have been grasped from the questionaires. To do this, eighty eight earth science teachers in Korea are administered questionaires. The frequency of the experimental and practical items in the seven kind of earth science text books were investigated. The problems and the reasonable management for experimental and practical education were proposed in this paper.
