Objective: Advances in surgical technique, anesthesia and perioperative patient management, coupled with the rapid increase in the population of patients reaching their 60s or older, have led to a signiticant rise in spinal precedures. The objective of this study was to evaluate frequency of the disease, surgical results and postoperative complications of the aged patients who underwent spinal surgeries. Methods : We reviewed all spinal procedures for spinal lesions including degenerative diseases, trauma, tumor and others in patients aged 60 years or older between January 1 1994 and October 1 1998(56 men and 26 women), retrospectively. Final outcome was assessed by telephone interview with the patient or the family in 67.1%(55/82). Results : There were 56 male patients and 26 female patients. There were 57 cases of degenerative diseases, 14 cases of trauma, 5 cases of metastaisis, 4 cases of infection and 2 cases of others. According to the level of surgical procedures, there were 20 cases fo cervical spine, 8 cases of thoracic spine, 54 cases of lumbar spine. Postoperaive complications included the following(17/82): 7 dural tears, 2 sepsis, 2 nerve root injury and 6 others. There were no operation-related deaths. During follow up period, ten patients(12.2%) had died, 45 patients(54.9%) were contacted and 27 patients(32.9%) could not be located. Mean follow up period for the 45 patients contacted was 26.1±15.7 months. Of the 45 persons contacted, 34(75.6%) reported improvement, 6(13.3%) no change and 5(11.1%) aggravation of their preoperative symptoms. Conclusion: Spine surgery in patients aged 60 years or older has higher complication rate compare to the younger but it is reasonable to consider the surgical procedures even in the elderly if it is needed. We should give attention not to tear the dura during the surgical procedures, especially in the elderly.

The sickle cell disease are a group of hemoglobin disorders characterized by red cells that undergo sickle shape transformation when they are deoxygenated. Sickle cell disease is transmit-ted as an autosomal recessive trait. This unusual property, due to the polymerization of sickle hemoglobin results in anemia and vasoocclusive complication. 1) The most clinically significant of these disease are sickle cell anemia, sickle cell hemoglobin C disease, and sickle cell beta thalassemia. Symptoms of pallor, fever, abdominal and joint pain, enlargement of the liver and spleen, swelling of hands and feet first appear near the latter part of the first year of life. Intravascular sickling affects all organs. For clinical and therapeutic purposes, exacerbations may be classified as vasoocclusive or pain, aplastic, hemolytic or sequestration crisis. we experienced a 22 year old female patient who suffered severe multiple joint pain and back pain thought to be caused by vasoocclussive phenomena.

객체 지향 데이타베이스 시스템의 질의 및 객체 관리기 인터페이스

객체 지향 데이타 모델을 지원하는 데이타베이스 시스템은 기존의 데이타베이스의 단점을 보완하며, 다양하고 복잡한 실세계의 정보를 처리할 수 있다. 객체 지향 데이타베이스 시스템은 사용자의 접근을 위해 질의 및 객체 관리 인터페이스를 필요로 한다. 본 논문에서는 사용자를 위한 질의 인터페이스와 질의 계획을 수행하기 위한 객체 관리 인터페이스를 설계함으로써 질의 목적을 달성토록 하였다. 설계된 객체 관리 인터페이스는 응용을 위해 "C"언어 함수로 구성하였고, 성능 항상을 위한 확장성을 제공할 수 있도록 설계 하였다. The database system that supports object oriented data model can complements conventional DBMS's drawbacks and processes complex and intensive applications. In object oriented database system, the access and management of all objects are manipulated by object manager. Therefore, a query interface as well as an Object Manager Interface(OMI) is required for interfacing between a user and an object store. In this paper, we propose the query interface which can support object oriented paradigm and design rules of OMI, and implement OMI based on the rules. The OMI have been implemented by using C language on UNIX OS machine.

SMEDDS를 이용한 난용성 약물의 용출율 향상

ABSTRACT-A self-microemulsifying drug delivery system (SMEDDS) was developed to enhance the solubility and dissolution rate of poorly water soluble drug, biphenyl dimethyl dicarboxylate, DDB. The system was optimized by evaluating the solubility of DDB and the microemulsion existence range after the preparation of microemulsions with varying compositions of triacetin and surfactant-cosurfactant mixtures (Labrasol as surfactant (S) and the combination of Transcutol, Cremophor RH 40 and Plurol oleique as cosurfactant (CoS)). SMEDDS in this study markedly improved the solubility of DDB in water up to 10 mg/ml and the size of the o/w microemulsion droplets measured by dynamic light scattering showed a narrow monodisperse size distribution with an average diameter less than 50nm. The microemulsion existing range is increased proportional to the ration of S/CoS, however, it decreased remarkably as the oil content was more than 20%. In vitro dissolution study of SMEDDS showed a significantly increased dissolution rate of DDB in water (>12 fold over DDB powder), and SMEDDS also had significantly greater permeability of DDB in Caco-2 cell compared to powders.

Symplastic glomus tumor 1예

Glomus tumor is a benign lesion characterized by vessels and glomus cells in the varying proportion and it classically presents as a solitary, painful papule or nodule. Occasionally it displays unusual histopathologic features diagnosed as "atypical" glomus tumor, including malignant glomus tumor, glomus tumor of uncertain malignant potential, glomangiomatosis, and symplastic glomus tumor. Glomangiomatosis and symplastic glomus tumor reveal benign biologic behavior. We report herein a typical case of symplastic glomus tumor showing marked nuclear atypia, its only unusual feature comparing with classical one, in addition to the typical clinical and pathological features. 응급실 내원 환자의 만족도 평가 및 관련 요인에 대한 연구

Background: To examine the influencing factors on patients' satisfaction in the emergency department(ED) for quality assurance. Methods: Patients who visited to the ED were prospectively investigated from November 1 to December 31, 1997. Authors developed questionnaire to investigate influencing factors on patients' satisfaction. A Chi-square test and LISREL 7.0 were applied for statistical analysis. Results: Patients' satisfaction was significantly related to physical environment variables, accessibility variables, kindness of hospital personnel variables, and patient's trust for doctors variables. In path analysis, willingness for revisit was influenced by patients' satisfation, accessibility, physical environment, patients' trust for doctors in order, and willingness for recommendation was influenced by accessibility, patients' satisfaction, and kindness of hospital personnel in order. Conclusions: The influencing factors on patients' satisfaction are physical environment, accessibility, kindness of hospital personnel, and patient's trust for doctors. Willingness for revisit and willingness for recommendation are influenced by patients' satisfation. In spite of some limitations, the results of this study can be used as a baseline information for exploring the influencing factors on patients' satisfaction. Further comprehensive research efforts should be made on the study of patients' satisfactoin in the ED.

Diagnostic Performance of Rectal CT for Staging Rectal Cancer: Comparison with Rectal MRI and Histopathology

Purpose To compare the diagnostic performance of rectal CT with that of high-resolution rectal MRI and histopathology in assessing rectal cancer. Materials and Methods Sixty-seven patients with rectal cancer who underwent rectal CT with rectal distension using sonographic gel and high-resolution MRI were enrolled in this study. The distance from the anal verge/anorectal junction, distance to the mesorectal fascia (MRF), extramural depth (EMD), extramesorectal lymph node (LN) involvement, extramural venous invasion (EMVI), and T/N stages in rectal CT/MRI were analyzed by two gastrointestinal radiologists. The CT findings of 20 patients who underwent radical surgery without concurrent chemoradiotherapy were compared using histopathology. Interclass correlations and kappa statistics were used. Results The distance from the anal verge/anorectal junction showed an excellent intraclass correlation between CT and MRI for both reviewers. For EMD, the distance to the MRF, presence of LNs, extramesorectal LN metastasis, EMVI, T stage, and intermodality kappa or weighted kappa values between CT and MRI showed excellent agreement. Among the 20 patients who underwent radical surgery, T staging, circumferential resection margin involvement, EMVI, and LN metastasis on rectal CT showed acceptable concordance rates with histopathology. Conclusion Dedicated rectal CT may be on par with rectal MRI in providing critical information to patients with rectal cancer.

딸기 재배시설의 온실환경시스템 연구

본 논문은 딸기 재배 시설에서의 이루어지고 있는 온실 환경에서 생장 관리 기술의 생장 상태 및 환경 변화를 인지하는 과정에서 수집되는 환경데이터와 제어장치 간의 상관관계를 비교 및 분석하고, 이를 통해 전력소비량에 대한 에너지효율을 높이면서 최적의 생장 환경을 제공할 수 있는 시스템을 제안한다. 제안하는 시스템은 딸기 재배 시설의 품질 향상 및 생산량 증대를 지원하는 온실 생장 환경모니터링 시스템 개발에 대한 연구이다. 향후에는 다양한 환경데이터와 제어장치 간의 상관관계의 정밀 분석을 통해 온실환경제어 시스템을 개선할 수 있는 연구가 될 것으로 기대된다.

유동상촉매분해설비 통합시스템에 대한 구조건전성 평가

Fluid catalytic cracking (FCC) is one of the most important conversion process used in petroleum refineries. It is widely used to convert the high-boiling and high-molecular weight hydrocarbon fractions of petroleum crude oils to more valuable oil products. The purpose of this study is to verify the structural integrity of integrated system including reactor, regenerator and catalyst standpipes which are the core facilities of FCC. The fundamental process design of FCC is provided by licensor and the details of reactor and regenerator are designed by using an in-house program based on the ASME Sec.Ⅷ, Div.1. To get more conservative results, a variety of load conditions such as dead weight, pressure, thermal expansion, seismic and wind loads are applied in the structural strength assessment. The structural piping analysis considering the latest design specification of the specific vendor items like expansion joint, slide valve and spring hanger is performed and its result is provided to the related vendors to confirm their performance once more. From the comprehensive analysis results, it is concluded that the analyzed system is designed to have sufficient flexibility to prevent pipe movements from the elastic failure due to the overstress of the pipe components or anchors. The analysis results meet the requirement of all applicable design codes and licensor’s specification.
