Characteristics and Management of Residual or Slowly Recurred Intracranial Aneurysms

Objective : Residual aneurysm from incomplete clipping or slowly recurrent aneurysm is associated with high risk of subarachnoid hemorrhage. We describe complete treatment of the lesions by surgical clipping or endovascular treatment. Methods : We analyzed 11 patients of residual or recurrent aneurysms who had undergone surgical clipping from 1998 to 2009. Among them, 5 cases were initially clipped at our hospital. The others were referred from other hospitals after clipping. The radiologic and medical records were retrospectively analyzed. Results : All patients presented with subarachnoid hemorrhage at first time, and the most frequent location of the ruptured residual or recurrent aneurysm was in the anterior communicating artery to posterior-superior direction. Distal anterior cerebral artery, posterior communicating artery, and middle cerebral artery was followed. Repositioning of clipping in eleven cases, and one endovascular treatment were performed. No residual aneurysm was found in postoperative angiography, and no complication was noted in related to the operations. Conclusion : These results indicate the importance of postoperative or follow up angiography and that reoperation of residual or slowly recurrent aneurysm should be tried if such lesions being found. Precise evaluation and appropriate planning including endovascular treatment should be performed for complete obliteration of the residual or recurrent aneurysm.

Objective: We aimed to evaluate the efficacy of EmboTrap II in terms of first-pass recanalization and to determine whether it could yield favorable outcomes. Materials and Methods: In this multicenter, prospective study, we consecutively enrolled patients who underwent mechanical thrombectomy using EmboTrap II as a front-line device. The primary outcome was the first pass effect (FPE) rate defined by modified Thrombolysis In Cerebral Infarction (mTICI) grade 2c or 3 by the first pass of EmboTrap II. In addition, modified FPE (mFPE; mTICI grade 2b–3 by the first pass of EmboTrap II), successful recanalization (final mTICI grade 2b–3), and clinical outcomes were assessed. We also analyzed the effect of FPE on a modified Rankin Scale (mRS) score of 0–2 at 3 months. Results: Two hundred-ten patients (mean age ± standard deviation, 73.3 ± 11.4 years; male, 55.7%) were included. Ninetynine patients (47.1%) had FPE, and mFPE was achieved in 150 (71.4%) patients. Successful recanalization was achieved in 191 (91.0%) patients. Among them, 164 (85.9%) patients underwent successful recanalization by exclusively using EmboTrap II. The time from groin puncture to FPE was 25.0 minutes (interquartile range, 17.0–35.0 minutes). Procedure-related complications were observed in seven (3.3%) patients. Symptomatic intracranial hemorrhage developed in 14 (6.7%) patients. One hundred twenty-three (58.9% of 209 completely followed) patients had an mRS score of 0–2. Sixteen (7.7% of 209) patients died during the follow-up period. Patients who had successful recanalization with FPE were four times more likely to have an mRS score of 0–2 than those who had successful recanalization without FPE (adjusted odds ratio, 4.13; 95% confidence interval, 1.59–10.8; p = 0.004). Conclusion: Mechanical thrombectomy using the front-line EmboTrap II is effective and safe. In particular, FPE rates were high. Achieving FPE was important for an mRS score of 0–2, even in patients with successful recanalization.

Background/Aims: The clinical characteristics of patients with masked uncontrolled hypertension (MUCH) have been poorly defined, and few studies have investigated the clinical predictors of MUCH. We investigated the demographic, clinical, and blood pressure (BP) characteristics of patients with MUCH and pro-posed a prediction model for MUCH in patients with hypertension. Methods: We analyzed 1,986 subjects who were enrolled in the Korean Ambulatory Blood Pressure Monitoring (Kor-ABP) Registry and taking antihypertensive drugs, and classified them into the controlled hypertension (n = 465) and MUCH (n = 389) groups. MUCH was defined as the presence of a 24-hour ambulatory mean systolic BP ≥ 130 mmHg and/or diastolic BP ≥ 80 mmHg in patients treated with antihypertensive drugs, having normal office BP. Results: Patients in the MUCH group had significantly worse metabolic profiles and higher office BP, and took significantly fewer antihypertensive drugs com-pared to those in the controlled hypertension group. Multivariate logistic regression analyses identified high office systolic BP and diastolic BP, prior stroke, dyslipidemia, left ventricular hypertrophy (LVH, ≥ 116 g/㎡ for men, and ≥ 96 g/㎡ for women), high heart rate (≥ 75 beats/min), and single antihypertensive drug use as independent predictors of MUCH. A prediction model using these predictors showed a high diagnostic accuracy (C-index of 0.839) and goodness-of-fit for the presence of MUCH. Conclusions: MUCH is associated with a high-normal increase in office BP and underuse of antihypertensive drugs, as well as dyslipidemia, prior stroke, and LVH, which could underscore achieving optimal BP control. The proposed model accurately predicts MUCH in patients with controlled office BP.

Background: Phototrichogram analysis of normal scalp hair characteristics in Asian populations is not well characterized. Objectives: To investigate the normal hair characteristics of South Koreans using a phototrichogram. Methods: We enrolled 674 males and 683 females between the ages of 10 and 69. Hair density, thickness and the white hair percentage were measured via phototrichogram. Six scalp sites, including front, top, the peak between top and back, back, and the right and left sides were targeted. Results: The mean hair density, hair thickness, and the white hair percentage were not significantly different between genders at any scalp site. The mean hair density and thickness were significantly different according to scalp sites. Hair density declined with age, especially at the peak between top and back. Males and females showed different patterns of aging in hair density. Hair thickness changed little with age. But, mean hair thickness generally tended to increase until the 20s, reach a plateau between the 20s and 50s, and decrease after the 50s. The mean white hair percentages were 1.84 ± 4.24% in males and 1.66 ± 4.21% in females. The mean white hair percentages were significantly increased in subjects greater than 40 years of age. Conclusion: We described the normal characteristics and aging patterns of scalp hair in a Korean population.

이 연구는 주목 행동을 노출과 관여의 두 차원으로 구분하여 모바일 콘텐츠에 대한 주목 행동의 특징을 살펴보고, 주목 행동들 간에 어떤 관계를 갖는지 피키캐스트의 콘텐츠 데이터를 이용하여 탐구하였다. 우선, 이 연구는 이중위험효과(double-jeopardy)가 모바일 콘텐츠에서도 나타나는지 알아보기 위해 노출과 관여의 상관관계를 검증했다. 또한 플랫폼별, 주제 속성별(연성 및 경성)로 주목 행동이 어떻게 다른 양상을 띠는지 살펴보았다. 그 결과, 모바일 플랫폼에서도 노출과 관여 간에 긍정적인 상관관계가 존재하는 것을 확인하였다. 또한 플랫폼별로 주목 행동, 특히 공유 행동이 다른 양상을 띠는 것을 발견할 수 있었다. 주제 속성별로는, 연성 콘텐츠에 대해서는 즉각적인 반응인 댓글 수, 공유 등의 주목 지표들이 연관되어 있는 반면, 인지적인 노력이 필요한 경성 콘텐츠에 대해서는 소비시간과 완독률 같은 주목 지표들이 연관되어 있었다. 본 연구는 이용자 중심의 관점에 치중했던 기존 연구에서 벗어나 콘텐츠 중심의 관점으로 모바일 환경에서의 주목 행동의 본질을 새롭게 이해했다는 데 의의가 있다. In this study, we divided attention to mobile contents into two dimensions (i.e., exposure and involvement), and explored the characteristics of and the relationship between the attention behaviors, using contents data from Pikicast. First, this study investigated the relationship between exposure and involvement in order to examine whether double-jeopardy effects appear in mobile contents as well. In addition, we examined how different attention behaviors differ according to platform and subject attributes(i.e., soft and hard). As a result, we found that there was a positive correlation between exposure and involvement in mobile platforms. Also, we found that the attention behaviors, especially sharing behaviors, were different in each platform. In terms of subject attributes, the attention indices such as the number of comments and sharing, which were immediate responses, were related to the soft content, whereas the attention indices such as the consumption time and the complete-read rate were related to the hard content requiring cognitive effort.This study is meaningful to understand the essence of the attention behavior in the mobile environment from a content - oriented perspective rather than the most existing research with a user – centered perspective.

The purpose for this study was to improve the thermal environment and lighting environment in classroom. There is close correlation between educational environment and learning efficiency. So, in order to manage suitable condition it is necessary to study the characteristic of the classroom. The study carried out with two different ways on the classrooms. The results were divided into three cases and evaluated by discomfort index and uniformity ratio. By this evaluation, we were able to verify the problems of existing classrooms. And we looked for improvement method based on active control & passive control in classrooms. The results have shown that there was a need for a suitable control method in these classrooms.

Purpose: Oxidative stress leads to an increased production of lipoxygenase derivatives in diabetic nephropathy. Thus, we hypothesized that lipoxygenase inhibitor, nordihydroguaiaretic acid (NDGA), ha the effects of decreasing proteinuria and preserving renal function in streptozotocin (STZ)-induced diabetic rats. Methods: 45 Sprague-Dawley rats were divided into three groups; (A) treatment with lipoxygenase inhibitor, NDGA in diabetic nephropathy rats, (B) treatment with dimethyl sulfoxide (DMSO) as a vehicle in STZ-induced diabetic rats, (C) normal control group with subcutaneous injection of normal saline. Diabetes was induced by a single intraperitoneal injection of STZ (65 mg/kg) in rats of group A and B. After the 4th week of STZ injection, NDGA (10 mg/kg) and DMSO were given subcutaneously for another 4 weeks in group A and B respectively. Results: The NDGA-treated diabetic rats exhibited significantly decreased urinary albumin excretion. Serum creatinine and blood urea nitrogen concentrations were increased in both group A and B, and tend to be higher in group B than group A. Twenty-four-hour urine creatinine clearances were increased in both group A and B after injection of STZ. Pathologic alterations of kidney were observed after injection of STZ, and then attenuated after administration of NDGA. Conclusion: These results suggest the potential of lipoxygenase inhibitor as a complementary therapy for the prevention and treatment of diabetic nephropathy.
