        Modified Free Wrap-around flap을 이용한 수무지 재건술

        The thumb is the most important part in the hand and its loss results in severely diminished function of the hand for pinch and grasp. There are several microsurgical and other methods to reconstruct and amputated or lost thumb such as toe-to-thumb transfer1)2)9) as well as several varieties of flaps. Among them, a free wrap around flap taken from the big toe, including the nail and an iliac bone graft as the cone of the reconstruction was a quite satisfactory procedure, but there were some problems. We performed a free wrap around flap on traumatically missing thumb with modification to include all part of the distal phalanx without iliac bone graft. The resorption and atrophy of a grafted iliac bone, a constant problem with Morrison's original procedure8), could be prevented by the modified method. The donor site defects were covered by remained medial flap for weight bearing area first, and full thickness skin graft for remained defected area. The results were good cosmetically and functionally without significant problems.

        MgO and Ba_(0.6)Sr_(0.4)Gd₂O₄films were deposited on Si(100), soda-lime glass and PDP's test panel substrates by using e-beam vapor deposition. Ba_(0.6)Sr_(0.4)Gd₂O₄ powder for e-beam evaporator source was prepared by sintering a mixture of BaO, SrO and Gd₂O₄ powder at a molar ratio of 0.6 : 0.4 : 1 at 1200℃ for 12 hours in air. The Crystalline and chemical shift of electron orbital binding energy of the MgO and Ba_(0.6)Sr_(0.4)Gd₂O₄ films are investigated by X-ray diffraction(XRD) and X-ray photoelectron spectroscopy(XPS). And we have analyzed the secondary electron emission coefficient (γ₄) of films. The secondary electron emission coefficient (γ₁) of Ba_(0.6)Sr_(0.4)Gd₂O₄ film is higher than that of MgO film. A firing and the sustain voltage of MgO film are lower than that of Ba_(0.6)Sr_(0.4)Gd₂O₄ by 7 V, 8.8 V respectively. The luminance efficiency of Ba_(0.6)Sr_(0.4)Gd₂O₄film is better than of MgO film with 0.66 lm/W. MgO and Ba_(0.6)Sr_(0.4)Gd₂O₄films showed good transmittance properties within the visual range.

        본 연구의 목적은 구조물의 최적화 설계에 있어서 평형선형계획법이 적합하다는 것을 보여주는 것이다. 본 연구에서는 수직 하중을 받고 있는 직선 보로 구성된 격자구조물에 평형선형계획법을 적용시켰다. 보 요소가 하나의 설계변수를 가지게 하기 위하여 단면 물성치간의 관계를 나타낸 경험식을 사용하였다. 모든 비선형 함수들은 제 1차 테일러 급수 전개에 의하여 근사 선형화하였다. 선형화 시킨 후 건형 프로그램중의 하나인 심플렉스법을 이용하여 최적해를 얻었다 그 결과 평형선형계획법이 구조물 최적화 문제에 있어서 최적해를 찾는데 만족스러운 방법임을 알 수 있었다. In this study, equilibrium linear programming technique which is suitable for optimization of structures is introduced. An application is presented for grillage made of straight beams loaded normally. Empirical relationships are used to relate beam section properties so that each beam element has only one design variable. All of the nonlinear functions are linearized by the first order Taylor series expansion. Optimization results are obtained by simplex method which is one of the linear programming method. As results, the equilibrium linear programming technique is a satisfactory method for seeking g1oba1 optimum of structural optimization problems

        급성 조증에서 기분안정제와 리스페리돈의 병합 투여 효과 : 다기관 개방연구

        Ohjective : The primary purpose of this study was a replication of the effectiveness and tolerability of rispehdone in the treatment of patients with acute mania in very larger cohon in naturalistic treatment setting to extend the data on the effect and tolerability of risperidone in the treatment of patients with acute mania to Asian population. Methods : A total of 909 patients with DSM-TV criteria of bipolar disorder current manic and hypomanic episode, entered this large, open, multicentre study. The Young Mania Rating Scale (YMRS), Clinical Global Impression (CGI) and Simpson-Angus Rating Scale (SARS) were measured at baseline and weeks 1, 3 and 6, for the assessment of effectiveness and extra-pyramidal symptom (EPS). Results : This study showed statistically significant reduction of scores on the YMRS and CGI-s (mean change=-23.5±11.8, p<0.0001 ; mean change=-2.7±1.5, p<0.0001, respectively) from the baseline to the endpoint (week 6). Number of patients with 50% reduction or more in the YMRS and CGI-s scores was 693 (77.8%) and 630 (70.7%) at endpoint, respec-tively. There were no statistically significant increments of scores on SARS. Risperidone was generally well tolerated. Conclusion : The present larger open study demonstrates that hsperidone add-on therapy is effective and tolerable in treat-ment of bipolar disorder, replicating results in various controlled and uncontrolled studies from Western countries.

        헬리코박터 파이로리 균 진단용 ^13C-요소 캅셀의 개발

        The purpose of this study was to develop a new ^13C-urea- containing capsule for diagnosis of H. pylori. The urea-containing capsules were prepared with various diluents such as polyethylene glycol (PEG), microcrystalline cellulose, sodium lauryl sulfate and citric acid. The dissolution test, ^13C-urea breath test and stability test were then performed on the capsules. Microcrystalline cellulose and sodium lauryl sulfate retarded the initial dissolution rates of urea. However, PEG increased the initial dissolution rates of urea. Furthermore, two formulae composed of PEG, [^13C-urea/PEG (38/1.9 mg/cap)] and [^13C-urea/PEG/citric acid (38/1.9/1.9 mg/cap)] had the maximum DOB value, about 16 at 20 mim, while the formula composed of only 38 mg ^13C-urea had the maximum DOB value at 30 min. The results indicated that PEG improved the sensitivity of ^13C-urea in the human volunteers. The capsule [^13C-urea/PEG (38/1.9 mg/cap)] was stable for at least six months in 25 and 37℃. Thus, a PEG-containing capsule, [^13C-urea/PEG (38/1.9 mg/cap)] would be a more economical, sensitive and stable preparation for diagnosis of H. pylori.

        Glycyrrhiza uralensis attenuates TNF‑α‑induced muscle atrophy in myoblast cells through the Nrf2 and MAFbx signaling cascades

        Muscle atrophy is associated with chronic diseases, such as heart failure diabetes, and aging-related diseases. Glycyrrhiza uralensis (GU) extract is widely used in traditional medicine. However, no studies have evaluated the effects of GU on muscle atrophy. Thus, in this study, we assessed the effects of GU on prevention of muscle atrophy. GU reduced the levels of the TNF-α-induced muscle atrophy markers, muscle RING-finger protein-1(Murf-1) and muscle atrophy F-box (MAFbx), and upregulated myosin heavy chain expression (MyHC). It also reduced the phosphorylation of nuclear factor kappa B, and downregulated Smad3 proteins, which are involved in protein ubiquitination. When we examined whether GU exhibits antioxidant activities. GU suppressed TNF-α-induced muscle atrophy by increasing the translocation of nuclear factor erythroid 2-related factor 2 (Nrf2), which regulates the expression of antioxidant factors such as heme oxygenase-1 (HO-1) as well as apoptosis-related factors, such as caspase-3/7. These results suggest that GU extract is potentially an important agent in the regulation of TNF-an induced muscle atrophy.
