        인삼 추출물 함유 한방화장품이 건강한 성인 여성의 삶의 질에 미치는 영향에 관한 예비 연구; Skindex-16을 중심으로

        본 연구에서는 건강한 성인 여성을 대상으로 인삼 추출물을 함유한 스킨케어 제품의 시료 제시 방법과 여성들의 삶의 질 개선 효과 간의 상관 관계를 분석하였다. 삶의 질이란 질병이나 건강 상태가 일상생활에 미치는 신체적, 정신적 그리고 사회적 영향에 대한 개인적인 반응을 나타내는 개념이다. 이에 저자들은 45명의 건강한 성인 여성을 두 그룹으로 나누어 한 그룹(Group A)에는 브랜드 고유의 패키지를 유지한 인삼 크림 시료를 제공하였고, 다른 그룹(Group B)은 브랜드와 제품명을 알 수 없는 하얀색 통에 담은 동일한 시료를 제공하여 8주간 사용하게 하였다. 피부 관련 삶의 질 평가는 Skindex-16 설문지를 사용하여 시험 초기와 4주 후, 8주 후에 걸쳐 시행되었다. 시험 참여를 중지한 2명의 피험자를 제외한 43명의 Skindex-16 초기값의 평균은 22.70 ± 4.82점이었다. 8주 후, 그룹 A의 Skindex-16 지표는 초기값 23.30 ± 5.14에서 20.20 ± 4.83으로, 그룹 B는 22.17 ± 4.58에서 20.52 ± 3.60로 통계적으로 유의한 정도로 개선되었다. 세부 항목에서는 그룹 A의 ‘증상’ 항목이 4주 후, ‘감정’ 항목이 8주 후에 통계적으로 유의하게 개선되었으며, ‘기능’ 항목은 어느 그룹에서도 통계적으로 유의한 변화가 관찰되지 않았다. 또한, 총합과 세부 항목 모두 측정 시기 및 그룹에 따른 교호작용은 통계적으로 유의한 차이가 확인되지 않았다. 본 실험을 통해 인삼 크림이 포함된 스킨케어가 건강한 성인 여성의 삶의 질에 긍정적인 영향을 줄 수 있음을 확인하였으며, 스킨케어 행위 자체가 패키지에 대한 영향보다 삶의 질의 개선에 주는 영향이 큼을 추측할 수 있다. 장기적으로 ‘삶의 질’이란 기존의 피부과학 지표의 개선과 주관적 만족도의 개념을 넘어 신규한 화장품 효능 영역으로 추가적인 연구가 필요할 것으로 생각된다. This study is designed to analyze the effect of skincare using cosmetic containing ginseng extract, on improving quality of life (QOL) of healthy women, with blind testing. QOL is a concept that represents how one’s disease or health condition can physically, psychologically, and socially influence his or her daily life. The study was conducted to assess the effect of a ginseng cosmetic preparation on quality of life (QOL) using the Skindex-16 score, stratified by blind versus non-blinded option. 45 healthy women aged between 30 and 49 years with no skin disease were recruited for this study. Volunteers were divided into two groups. Group A (n = 22) received anti-aging cream with ginseng extract in the original packaging, which included the brand name and logo. Group B (n = 23) received the same cream in a plain white jar without any package decoration or logo. Both groups used the cream for 8 weeks. For the skin-related QOL assessment, Skindex-16 was used at baseline, forth, and eighth week. All volunteers except two dropouts in Group A completed the dermatology-specific QOL measure, Skindex-16, at baseline, after 4 weeks, and after 8 weeks of treatment with the provided samples. As a result, the mean score of 43 participants at baseline was 22.70 ± 4.82. There was a significant difference between the baseline score and the score after 8 weeks in both groups: The scores changed from 23.30 ± 5.14 to 20.20 ± 4.83 in Group A, from 22.17 ± 4.58 to 20.52 ± 3.60 in Group B. The “Symptom” subscale of Skindex-16 improved after 4 weeks and the “Emotion” subscale improved after 8 weeks in Group A. The “Function” subscale did not show improvement in either groups. Both groups showed no interaction effect between follow up time and groups in Skindex-16 and subscale. This research opens up the possibility of skincare using ginseng cream having a positive effect on QOL in healthy women. Moreover, one can predict that skincare ritual itself may have greater impact on the improvement of QOL, compared to the product packaging.

      • A multi-functional polyhydroxybutyrate nanoparticle for theranostic applications

        <P>Biopolymer-based multi-functional nanoparticles have been developed through a one-step enzymatic polymerization reaction using engineered polyhydroxyalkanoate (PHA) synthase fused with green fluorescent protein (GFP) and a single chain variable fragment antibody (A33scFv) specific to colon cancer. PHA synthase possesses unique catalytic characteristics, namely covalent catalysis, by which the synthesized polyhydroxybutyrate chain remains covalently attached to the enzyme. The amphiphilic nature of the resulting protein-polymer hybrid gives rise to spontaneous self-assembly into a micellar structure with GFP and A33scFv displayed on the surface (AGPHB nanoparticle). A model compound, Nile red, was loaded into the hydrophobic core of the AGPHB nanoparticle during the polymerization and self-assembly process. The specificity of the fluorescent multi-functional AGPHB nanoparticle towards the colon cancer cell lines SW1222 (A33+) and HT29 (A33−) was confirmed and analysed quantitatively <I>in vitro</I>. This new biological approach provides a simple means of producing nanocarriers with a range of surface functionality and the sizes desired for imaging and targeted drug delivery.</P>

      • S-443 Hyperosmolar hyperglycemic state developed after chemotherapy in a patient with diabetes

        A 73-year-old male patient visited our center due to abdominal discomfort at June 2014. He was diagnosed with colon cancer with multiple liver metastases, peritoneal seeding and malignant ascites. He had been diagnosed with diabetes 8 years before but had been off medication for 5 month at the time of diagnosis due to fair glycemic control. His hemoglobin A1c was 6.2% at the time of diagnosis. We started palliative chemotherapy at July 2014 with FOLFOX regimen. From the 4th cycle, dexamethasone was given in addition to serotonin antagonist for better emesis control at 8mg per day for 3 days. After discharge from completion of 5th cycle of FOLFOX regimen, the patient visited emergency room due to profound general weakness and poor oral intake. He was mentally confused and severely dehydrated. Blood chemistry showed serum glucose 965 mg/dL, serum osmolarity 410 mOsmol/kgH2O, blood urea nitrogen 55.6 mg/dL, serum creatinine 1.60 mg/dL, sodium 159 mmol/L (corrected sodium 173 mmol/L), potassium 5.2 mmol/L, chloride 123 mmol/L, and C- reactive protein 2.88 mg/dL. Urine ketone was negative and arterial blood gas showed pH 7.382, pCO2 37.5 mmHg, pO2 79.8 mmHg, and HCO3- 21.8 mmol/L. With the impression of hyperosmolar hyperglycemic state, we started treatment with intravenous fluid and insulin. The patient’s condition improved. Over following 2 weeks, the patient slowly recovered without any complications. There were no evidences of infection or cardiovascular events proven afterwards. He is on regular follow-up now. In addition to steroid, chemotherapy itself might have contributed this complication. They had not been diagnosed with diabetes before. Therefore, it was argued that chemotherapy itself or chemotherapy induced poor oral intake might have somehow caused HHS in these cases. And it is also known that malignant tumor itself could disturb glucose metabolism and cause impaired glucose tolerance. HHS in our case might be resulted from a combinatorial effect of these factors in addition to dexamethasone use. It would be prudent, therefore, that we physicians should be cautious about hyperglycemia and its rare but serious complications especially in the old aged diabetic cancer patients under chemotherapy. .

        A spick-and-span approach to the immobilization of horseradish peroxidase on Au nanospheres incorporated with a methionine/graphene biomatrix for the determination of endocrine disruptor bisphenol A

        <P><B>Abstract</B></P> <P>In the present study, we employ a straightforward, benign strategy to prepare thiol-functionalized reduced graphene oxide (S-RGO) using methionine as the sulphur source and reducing agent. The immobilization of horseradish peroxidase (HRP) over the AuNPs/S-RGO was developed by incorporating AuNPs on the S-RGO surface. The fabricated HRP/AuNPs/S-RGO electrode exhibits a remarkable decrease in the overpotential and a significantly increased oxidation peak current of bisphenol A (BPA) compared with the bare glassy carbon electrode (GCE) and AuNPs/S-RGO electrode. The biosensor shows an excellent amperometric analytical performance with a low detection limit of 2.6×10<SUP>−12</SUP> M and a linear range from 2.0×10<SUP>−11</SUP> to 1.18×10<SUP>−9</SUP> M, with the response time <2s for BPA. From the results, the apparent Michaelis-Menten constant was calculated as 8.14nM. The HRP/AuNPs/S-RGO biosensor exhibited faster response, adequate storage stability, inexpensive, simple fabrication with disposability, satisfactory reproducibility and repeatability, and outstanding selectivity.       • 심한 낙타혹(Hump)의 형성을 동반한 제 2 형 막증식성 사구체신염(Dense Deposit Disease) 1예

        Dense deposit disease (DDD), known as type Ⅱ MPGN, is an uncommon form of glomerulonephritis. It is an acquired primary glomerular disease, characterised by electron microscopic evidence of a continous, dense membrane deposition replacing the lamina densa. There is associated alternative pathway complement activation and a C3 nephritic factor maybe present. Patients with dense deposit disease tend to be young at clincal onset and more commonly have persistent nephrotic syndrome, gross hematuria, and acute nephritis episode and persistent depression of the serum complement level. The prognosis of dense deposit disease is worse than that of typeⅠMPGN. We experienced a patient with dense deposit disease that presenting persistent nephrotic syndrome, gross hematuria and having subepithelial hump. It is a first report of dense deposit disease having subepithelial hump in Korea, therefore we report this case with the review of relevant literatures.

        기억력 저하를 호소하는 노인들을 대상으로 INM176를 3개월간 임상시험한 후의 치료성적 및 부작용 평가 보고

        Objectives : We examined the effects of INM 176 (K-l107) compared with placebo on the cognitive functions of 92 old aged subjects with cognitive impairment. Methods : This was a prospective, 12 week, double-blind, Placebo-controlled clinical trial. The elderly who achieved a score of less than 25 points on the K-MMSE or showed a high risk of Alzheimer's disease from the 7-Minute Neurocognitive Screening Battery were considered to have objective impairment and were selected as subjects for this study. The subjects were randomized to placebo or INM 176 group. The outcome measures were from the Alzheimer's Disease Assessment Scale-cognitive subscale (ADAS-Cog), the Instrumental Activities ofDaily Living (IADL) and the Korean Geriatric Depression Scale (KGDS) and two kinds of computerized priming tests. After setting the total error score in the Alzheimer's Disease Assessment Scale-Cognitive section (ADAS-cog) as the repeated measurement factor, an analysis of variance of the combined factor design was done between the placebo and INM 176 group. Results : The interaction effect of time (pre- and post-trial) and group (placebo and INM 176 group) was significant in the analysis of the ADAS-cog's total error score. The INM 176 group's total error score in the ADAS-cog decreased significantly (p<0.01), whereas the placebo group showed a slight increase. The mean changes in IADL and GDS from baseline scores favored in the INM 176 group than in the placebo group. Outcome changes ofADAS-cog, IADL, KGDS scores during the 12 week clinical trail ofINM 176 and placebo demonstrated favorable responses in the INM176 administered group. Conclusions : This is a preliminary clinical trial result of INM176 as a memory pill. Based on these results, INM176 may be a candidate molecule for the improvement of cognitive functions, including memory, Further clinical trial should demonstrate its efficacy.

      • Clinical Impact of Exosomal microRNA as a Novel Biomarker of Liver Fibrosis

        Aims: Many approaches have been suggested for the non-invasive diagnosis of liver fibrosis, including the use of serum biomarkers and ultrasound-based elastography, but none has yet replaced liver biopsy. MicroRNAs (miRNAs) have been suggested as potential diagnostic tools for liver diseases. We investigated alterations in the expression of serum exosomal miRNAs with the progression of liver fibrosis and evaluated their clinical applicability as biomarkers. Methods: This study prospectively enrolled 71 patients who underwent liver biopsy at a large-volume academic hospital in Korea. Exosomes were extracted from serum samples, and next-generation sequencing (NGS) of miRNAs was conducted in patients from different stages of liver fibrosis. Differential expression of miRNAs was quantified using targeted real-time quantitative polymerase chain reaction (RT-qPCR). A model was derived to discriminate advanced fibrosis based on miRNA levels using multivariate logistic regression. The performance of this model was evaluated and compared using area under the receiver operator characteristic (ROC) curve (AUC) and De- Long’s test. Results: NGS data revealed the relationship between exosomal miR-122 expression and liver fibrosis progression. The level of miR-122 decreased as the pathologic fibrosis grade progressed from stage 0 to 4. Patients with biopsy-proven advanced fibrosis had significantly lower levels of exosomal miR-122 (P<0.001) than those without advanced fibrosis. Exosomal miR-122 exhibited a fair performance in discriminating advanced fibrosis with an AUC of 0.77, which improved to 0.86 in combination with fibrosis-4 score (FIB-4) and transient elastography (TE). This value was higher than that reported for any other non-invasive modalities, including TE (AUC of 0.80) or FIB-4 (AUC of 0.57) alone. In a subgroup of patients with a non-viral etiology of liver disease, the performance of exosomal miR-122 as a biomarker improved, evident from the increase in the AUC value to 0.87. In this subpopulation, the combination model of miR- 122, FIB-4, and TE showed the best discrimination ability (AUC of 0.90), which was significantly higher than that of TE alone (AUC of 0.83; DeLong’s test P=0.046). Inhibition of miR-122 expression increased the proliferation of the human hepatic stellate cell line, LX-2, and upregulated the expression of collagen- 1A, a-smooth muscle actin, fibronectin, and transforming growth factor-ß. Conclusions: Exosomal miR-122 may serve as a novel biomarker for discriminating advanced liver fibrosis, and its accuracy may enhanced in combination with other non-invasive tests such as FIB-4 and TE.

      • Gynecologic manifestation and identification of genetic causes of Klippel-Trenaunay syndrome

        Klippel-Trenaunay syndrome (KTS) is a congenital disorder characterized by malformations of the arteries, capillaries, veins and lymphatic system, surrounding soft tissue and bone hypertrophy. A. Timur et al. (2005) and others have reported that AGGF1 gene causes excessive angiogenesis in KTS. In addition, Fabio C. Vicentini (2006) and others introduced cases involving bladder and vulva. However, there has been no genetic study of KTS patients who have invaded the clitoris. In this study, whole genome sequencing of KTS patients and their families involving vulva were performed to identify KTS - related genes and to confirm genetic variants in KTS. A 26 year old woman visited OBGY clinic for protruding vaginal mass. On examination, the purplish protruding anterior vaginal wall was noted and a 1.5 x 1.5 cm sized clitoral mass was also noted. A sessile type elevated skin lesion causing itching and irritation symptom was noted on both buttock. She had suffered heavy menstrual bleeding and on pelvic MRI, a upward displacement of uterus was noted near umbilicus level. NSAIDs prescribed onthe first 3 days of menstruation decreased the menstrual blood loss upto 40percent. In this study, whole genome sequencing of a KTS patient involving vulva was performed to identify KTS - related genes and to confirm genetic variation. The patient's peripheral blood and a sessile mass around buttock was sampled and whole genome sequencing was performed. Most KTS has known to be a sporadic one, however some genetic variants has been reported.

        간호실무에서 신봉이론(Espoused theories)에 대한 탐색적 연구

        As a nursing practice involves nurses' actions in a specific context of health care, this study has focused on exploring the espoused theories in nursing practice within the action science perspectives, Espoused theories are the belief, principles, and rationale expressed by the practitioner as guiding her/his actions in a situation of practice. The data were analysed qualitatively and 25 elements of espoused theories of nursing action were identified and clustered into 6 categories. The 25 elements of espoused theories are as follows: The clinical nurse worked in wholistic and individual nursing, focussed on the patient's needed, comfort and supportive nursing (5 theories of nursing goal) ; excellent skills, knowledge based, assessment and data collection, explaining, educating or a scientific basis(6 theories of nursing intervention): advocacy, value oriented, treatment, account- ability and commitment(4 theories of nursing ethics) ; human respect. partnership, trust(3 theories of patient-nurse relationship) : knowledgable, accumulated clinical experiences and personally lived experiences. positive perspectives(4 theories of nurse), role of intervention. rewarding peer relationship(3 theories of situations) The above mentioned espoused theories are similar to that of nursing textbooks which students learned through basic nursing education and almost the same as the Acts ofa Nurse in Korean. However, we are doubtful whether nurses actually do as they think. Therefore. it is recommended to review the theories-in-use in order to and any discrepancies between the espoused theories and the reality of nursing actions

        Oxidative lipid modification of nicastrin enhances amyloidogenic γ‐secretase activity in Alzheimer’s disease

        <P><B>Summary</B></P><P>The cause of elevated level of amyloid β‐peptide (Aβ42) in common late‐onset sporadic [Alzheimer’s disease (AD)] has not been established. Here, we show that the membrane lipid peroxidation product 4‐hydroxynonenal (HNE) is associated with amyloid and neurodegenerative pathologies in AD and that it enhances γ‐secretase activity and Aβ42 production in neurons. The γ‐secretase substrate receptor, nicastrin, was found to be modified by HNE in cultured neurons and in brain specimens from patients with AD, in which HNE–nicastrin levels were found to be correlated with increased γ‐secretase activity and Aβ plaque burden. Furthermore, HNE modification of nicastrin enhanced its binding to the γ‐secretase substrate, amyloid precursor protein (APP) C99. In addition, the stimulation of γ‐secretase activity and Aβ42 production by HNE were blocked by an HNE‐scavenging histidine analog in a 3xTgAD mouse model of AD. These findings suggest a specific molecular mechanism by which oxidative stress increases Aβ42 production in AD and identify HNE as a novel therapeutic target upstream of the γ‐secretase cleavage of APP.</P>
