        Accuracy of cardiac output measurements during off-pump coronary artery bypass grafting: according to the vessel anastomosis sites

        Background: During beating heart surgery, the accuracy of cardiac output (CO) measurement techniques may be influenced by several factors. This study was conducted to analyze the clinical agreement among stat CO mode (SCO), continuous CO mode (CCO), arterial pressure waveform-based CO estimation (APCO), and transesophageal Doppler ultrasound technique (UCCO) according to the vessel anastomosis sites. Methods: This study was prospectively performed in 25 patients who would be undergoing elective OPCAB. Hemodynamic variables were recorded at the following time points: during left anterior descending (LAD) anastomosis at 1 min and 5 min; during obtuse marginal (OM) anastomosis at 1 min and 5 min: and during right coronary artery (RCA) anastomosis at 1 min and 5 min. The variables measured including the SCO, CCO, APCO, and UCCO. Results: CO measurement techniques showed different correlations according to vessel anastomosis site. However, the percent error observed was higher than the value of 30% postulated by the criteria of Critchley and Critchley during all study periods for all CO measurement techniques. Conclusions: In the beating heart procedure, SCO, CCO and APCO showed different correlations according to the vessel anastomosis sites and did not agree with UCCO. CO values from the various measurement techniques should be interpreted with caution during OPCAB.         대장직장 종양에 대한 내시경 점막하 절제술의 연대순 임상 결과와 학습 곡선

        Background/Aims: Endoscopic submucosal dissection (ESD) is an effective procedure for en-bloc curative resection of the colorectal tumor. As it requires high technical skills and experience in therapeutic endoscopy, it is important to understand learning curve of ESD technique. The aim of this study was to retrospectively describe the clinical results of ESD and to evaluate learning curve for the ESD of colorectal tumors. Methods: A total of 90 patients with 90 colorectal neoplasms, who had undergone ESD at a tertiary referral hospital from July 2009 to December 2012, were enrolled. The ESD was performed by a single endoscopist. All ESD cases were divided into three periods: first, cases 1-30; second, cases 31-60; and third, cases 61-90. Results: The en-bloc resection rates in third period (100%) was significantly higher than that of the first (93.3%) and second period (80%) (p=0.025). The perforation rate in third period (0%) also significantly decreased compared with that of the first (13.3%) and second period (20%) (p=0.032). To calibrate the difference of tumor size among periods, proficiency was calculated, as the procedure time per specimen area (min/cm2). The proficiency in third period (4.3) was significantly shorter than that of the first (16.8) and second period (10.2) (p=0.004). Conclusions: The learning curve of colorectal ESD in our study shows that at least 60 cases of ESD have to be conducted to acquire sufficient skill of degree without perforation.       • Subcritical water extraction (SWE) of anthocyanin from blueberries (Vaccinium corymbosum)

        Subcritical water extraction (SWE) is an eco-friendly new extraction technology because it does not contain harmful organic solvent and has high extraction efficiency in a short time compared with conventional extraction methods. Blueberries (Vaccinium corymbosum) are widely known as superfood due to rich source of anthocyanin (malvidin-3-o-galgctoside) and antioxidant activity. In this study, optimal extraction condition of SWE from blueberries was determined and compared with the conventional extraction methods. SWE was carried out using a Dionex Accelerated Solvent Extractor (ASE, Model 350) under conditions of temperatures (110, 130, 150 and 170°C) and times (1, 3, 5 and 10 min). Total anthocyanin of SWE extracts was compared with hot water (60°C, 1 h) extract and pressed juice extract. The total anthocyanin content was determined by pH differential method. Considering both the extraction time and temperature conditions, the highest content of total anthocyanin content was 0.455 mg/g FW Vaccinium corymbosum at 130°C for 3 min. At high temperature and long extraction time, the anthocyanin in the blueberries will undergo thermal degradation due to low stability of anthocyanin at extreme condition. Besides, maximum yield of anthocyanin from blueberries using SWE was about 1.2 and 3.8 times more higher than hot water extract and pressed juice extract, respectively. Therefore, SWE is faster and more efficient method to extract anthocyanin from blueberries than conventional extraction methods. This study shows a possibility of SWE applied to food processing industry.

      • Flavobacterium meningosepticum에 의한 균혈증 1예

        1996년 6월 카톨릭대학교 부천성가병원에 뇌경색으로 입원했던 61세 남자 환자의 혈액에서 Flavobacterium meningosepticum을 분리하였다. 환자는 폐렴을 동반한 패혈증의 예로 ceftriaxone과 metronidazole을 투여하였으나 입원 48시간만에 사망하였다. Flabovacterium meningosepticum is a rae cause of nosocomial infection which shows multi-drug resistance. It mainly invades patients with impaired immunity. Recently, we experienced a case of F. meningosepticum bacteremia in a patient with chronic debilitated state owing to stroke. The clinical progress was so rapid that he died within 48 hours. Blood culture revealed F. meningosepticum which showed characteristic yellow colony in blood agar plate.

        Purification and Characterization of Helicobacter pylori ${\gamma}$-Glutamyltranspeptidase

        Gamma-glutamyltranspeptidase (GGT) was purified to electrophoretic homogeneity from the cell extract of H. pylori. The purified enzyme consisted of heavy and light subunits with molecular weights of 38 kDa and 21 kDa, respectively. N-terminal amino acid sequence of heavy and light subunits revealed that H. pylori GGT was processed into 3 parts for a signal peptide of 27 amino acid residues, a heavy subunit of 352 residues, and a light subunit of 188 residues during translation. The reaction rate for hydrolysis of ${\gamma}$-GpNA was 84.4 ${\mu}mol/min$ per milligram of protein, and that for the ${\gamma}$-glutamyl transfer from ${\gamma}$-GpNA to gly-gly was 23.8 ${\mu}mol/min$ per milligram of protein. The apparent Km values of H. pylori GGT for ${\gamma}$-glutamyl compounds were on the order of $10^{-3}$ to $10^{-4}$ M and those for acceptor peptides and amino acids were on the order of $10^{-1}$ to $10^{-2}$ M. The GGT protein kept approximately 80% of the initial enzymatic activity on incubation at $60^{\circ}C$ for 15 min. The optimum temperature and pH for reactions of both hydrolysis and transpeptidation were $40^{\circ}C$ and 9.0, respectively. The transpeptidation and hydrolysis reactions catalyzed by H. pylori GGT were strongly inhibited by L-Gln and moderately inhibited by L-Ala, L-Ser, ${\beta}$-chloro-L-Ala, and L-Glu. These results demonstrated that the biochemical properties of H. pylori GGT are different from those of other bacterial GGTs. Further, H. pylori GGT might degrade glutathione in the gastric mucous layer of humans if the enzyme could be secreted in the bacterial niches.

        Lumbar epidural injections are used to treat chronic and acute pain involving lower extremities. We report a case of intraventricular pneumocephlaus occurring after lumbar puncture. He complains severe bifrontal headache followed by nausea. Brain CT reveals air in the lateral ventricle anterior horn, temporal horn and basal cistern. CSF study shows normal findings.

      • 사람 비만세포주에서 Clozapine과 Fluoxetine에 의한 케모카인 및 케모카인 수용체의 상이한 발현

        Objectives : It has been suggested that the immune system has a role in the pathophsiology of neuropsychiatric diseases. There are some reports that clozapine and fluoxetine affect on cytokine networks. Recently, it has been known that chemokines have chemotactic effect, and modulate a number of biological responses including the process of inflammation and the maintenance of CNS homeostasis. In the present study, we investigated the effect of clozapine or fluoxetine on the expression of chemokines and their receptors. Methods : Human mast cells(HMC-1) was incubated with phorbol myristate acetate(PMA, 100ng/㎖) and calcium ionophore A23187(350ng/㎖) for 3 hours. Colzapine(10^-7M) or fluoxetine(10^-7M) were pretreated for 1 hour. RT-PCR was performed for the detection of expression of various chemokines and chemokine receptors. Results : after treatment of clozapine, the expression of MCP-1, MIP-1β, CCR3,CXCR2, CXCR3 and CXCR4 were lower than the PMA/Ca ionophore-treated group, while the expression of MIP-1α, RANTES and IL-8 were not changed. However, fluoxetine was not affected the changes of expression of various chemokines and chemokine receptors. Conclusion : This result indicates that clozapine may affect on the expression of chemokines and chemokine receptors. Therefore, this study could, in part, provide the important basic data on the explanation of side effects of clozapine, such as fever and pancreatitis.

        Background: The proinflammatory cytokine, IFN-y has been shown to exert pleiotropic effects in a variety of pathophysiologic conditions in autoimmune thyroid disease. The thyrocyte response to IFN-y is mediated two distinct classes of proteins, Janus kinases(Jakl and Jak2) and Signal Transducers and Activation of Transcription(STATl). The activation of STAT 1 is involved in the regulation of many interferon stimulated genes, such as MHC class II, intercellular adhesion molecules-1(ICAM-1) and MHC class II transactivator(CIITA) after the binding to the GASgFN- pactivated site) of the gene promoters. Recently we found TSH/forskolin inhibits IFN-y stimulated maximal expression of ICAM-1 in FRTL-5 cell. IFN-y action is localized between -175 bp and -97 bp from the start of translation of ICAM-1 gene which contains regulatory elements known to be involved in IFN-y action in other eukaryotic cells, palindromic IFN-y activated site(GAS)(5-TTTCCGGGAAA-3) which could bind STAT1, STAT3, STAT5, STAT6. Furthermore, the addition of TSH and forskolin causes a decrease in ICAM-1 promoter activity and its action was localized in GAS. These findings suggested TSH/cAMP signaling pathways downregulate IFN-y activated Janus kinase-STAT signaling path. We wanted to explore the possible involvement of elevated cAMP in the negative regulation of IFN-y induced STAT1 activation in thyroid cells. Method: We made several 5-deletion constructs of rat ICAM-1 promoter and analyzed the promoter activities by measuring the luciferase activity after tranfection into FRTL-5 cells. The protein/DNA complex was measured by electrophoretic mobility shift analysis using labeled oligonucleotide. We checked the level of total and phosphorylated STATl protein by immunoblot analysis using specific antibodies. Results: Stimulation of IFN-y in FRTL-5 cells resulted in rapid activation of STATl/DNA binding activity, which was apparent after several minute of stimulation, maintains its activity until 48 h. Incubation of cells with TSH result in suppression of IFN-p mediated STAT1/DNA binding activity throughout the time course of activation by IFN-y. Addition of TSH into 5H maintained FRTL-5 cells did not change the total amount of latent STAT1 amount and also not affect IFN-y mediated production of total STAT1 until 4 h. IFN-y(100 U/mL) rapidly induced phosphorylation of STAT1 within 30 min. and maintained its level without significant change until 48 hours. Cells treated with TSH dramatically lowered the level of IFN-y induced production and phosphorylation of STAT1 after 12 h, 24 h, 36 h, and 48 h but TSH had no effect on the level of phosphorylated STATl within 4 h after IFN-y stimulation. The proteasome inhibitor, MG132 and phosphatase inhibitor, sodium orthovanadate did not block the TSH or forskolin mediated downregulation of phosphorylated STAT1. Conclusion: These results indicate a regulatory mechanism which TSH signaling can modulate the prolonged activation of Jak/Stat by IFN-y. We identified one of mechanisms related to TSH mediated negative suppression of the ICAM-1 gene; TSH/cAMP signaling pathways downregulate the cytokine activated Janus kinase-STAT signaling path (J Kor Soc Endocrinol 13:536-553, 1998).
