LES is applied to analyze unsteady turbulent triple-jet water flows which are commonly found in thermal stripping phenomena. Current analyses performed to find effect of different SGS models on flow field with partition plates. LES tends to predict higher level of RMS temperatures compared to those of experiment indicating very active mixing effect among triple jets. The LES is found to be able to provide reliable frequency information of temperature fluctuation. The different SGS models show no significant difference in prediction ability, while Smagorinky-Lilly model yields slightly stronger mixing activity.

Analysis of ventilation fan flow has been carried out using CFX CFD code. The ventilation fan is applied widely to enhance air flow in open space. Selection of the ventilation fan is an important factor to design air ventilation facility to control air quality. Therefore, the accurate flow analysis is required in process of proper ventilation fan selection. Detailed analysis of fan flow in open space has been performed for 4 different commercially available ventilation fan. Computed air velocities show typical jet flow characteristics. The fan flow expands up to 10m in both of lengthwise and spanwise direction. The analysis also shows that turbulent flow enhances mixing of air in the space. From the analysis, flow analysis using CFD method is found to be an effective tool to select a proper ventilation fan. However, further detailed analysis which includes mass transfer for prediction pollutant behavior is required to complete air quality analysis.

Objective: Occupational medicine specialists in the Busan, Ulsan and Kyung-Nam areas established an area-based occupational disease surveillance system and used this system to collected case information for the purpose of preventing occupational diseases Methods: l l hospital participated in this system. The authors selected five main diseases, which were hand-arm vibration syndrome (HAVS), work-related musculoskeletal disorder (WRMSD), occupational asthma, occupational skin disease and occupational and occupational lung cancer and established their case definitions. All cases were reported on the web, and real time analysis was conducted. Results: Between April 2001 and April 2003, 192 cases of HAVS, 118 cases of WRMSD, 33 cases of occupational asthma, 17 cases of occupational skin disease and 17 cases of occupational lung cancer (for a total of 377 cases of these five main diseases) were reported. most of the HAVS cases came from shipbuilding, and 172 of them (92.7%) were associated with grinding. Of the four main types of WRMSD, carpal tunnel syndrome (CTS) was the most prevalent with 46 cases and shipbuilding was also the main industry involved (83.9%). The ergonomic risk factors involved mainly associated with the hand. In 19 (57.6%) and 4 (12.1%) cases of occupational asthma, the agents involved. The causative agents of occupational lung cancer included Cr, welding fume, PAH, Ni, etc. 10(58.8%) of the cases were diagnosed as squamous cell carcinoma and 5(29.4%) as adenoma. Conclusion: This result showed that an area-based occupational disease surveillance system might provide an effective method of evaluating the prevalence of such diseases, however the Busan, Ulsan and Kyung-Nam provinces are too large to be treated as individual areas. Therefore, the authors suggest that each province should establish its own surveillance system.
Management of Molar Pregnancy After Laparoscopic Radical Trachelectomy : A Case Report

We report a rare case of molar pregnancy after laparoscopic radical trachelectomy treated with hysterotomy and suction curettage. A 36-year-old unmarried woman (G0P0) was refererd for atypical endocervical cell on screening PAP smear. Colposcopy revealed a 1x1cm sized papillary growing polypoid mass with infiltration arising from the 12 Oclock direction of the cervix. Adenocarcinoma was diagnosed on punch biopsy, and HPV test was negative. No parametrial or pelvic organ invasion was suspected on pelvic exam and magnetic resonance imaging. The patient underwent laparoscopic radical trachelectomy, pelvic lymph node dissection and abdominal cerclage. Two years after surgery, she visited for positive result on her urine pregnancy test. Gestational sac like structure was seen on transvaginal ultrasound (TVS). But two weeks later, 9.1x6.2cm sized multiple cystic lesion filling the entire uterus was seen on TVS. Serum -HCG test measured 185,000mIU/mL. She was admitted for cervical dilatation and suction curettage. We used luminaria to dilate the cervix, but it was hard to dilate enough to introduce suction curette because of cervical stenosis and cerclage string, so we planned hysterotomy and suction curettage. Pfannenstiel skin incision was made, and molar tissue was thoroughly evacuated through hysterotomy site on the uterine fundus. Pathologic examination revealed partial mole, and the serum -HCG was normalized after 8 weeks. Now she is followed up for 5 years after radical trachelectomy without the evidence of recurrence. For patients who received radical trachelectomy and cerclage, hysterotomy can be a feasible alternative treatment option in situations where it is difficult to treat vaginally. Gestational trophoblastic neoplasia is a rare disease, but should not be put aside while preserving fertility in early stage cervical cancer patients.
Incidence of chemotherapy-induced neutropenic septic shock in patients with gynecologic malignancies

Objective: Neutropenic septic shock, which is associated with a mortality rate of 40%, is the most lethal complication for patients who receiving chemotherapy. This study is to assess the incidence of chemotherapy-induced neutropenic septic shock, according to cancer types and chemotherapeutic agents. Methods: We reviewed medical records of patients who had diagnosed with gynecologic cancer and treated with chemotherapeutic agents between Mar. 2009 and Mar. 2018. Neutropenic septic shock is defined as neutropenia <1g/L with septic shock that requires inotropics to maintain a mean arterial pressure of 65 mmHg. Statistical analysis was performed by Pearson’s chi-squared test and Mann-Whitney U test. Results: Total 1,010 patients were included in this study. Among them, 29 (2.87%) patients experienced 33 events of chemotherapy-induced neutropenic septic shock. Eleven events of deaths were occurred, and the mortality rate was 33.3%. Patients with septic shock were significantly older than those who did not (58.2 vs. 53.5, p=0.027). According to the cancer types, 2.65% of ovarian, 2.01% of cervical, 5.65% of endometrial, and 4.62% of peritoneal cancer patients experienced chemotherapy-induced neutropenic septic shock. There was no statistical significance between the types of cancer. According to the chemotherapeutic regimens, 1.28% of patients who treated with carboplatin-paclitaxel chemotherapy, 2.91% of carboplatin-docetaxel, and 4.55% of cisplatin-adriamycin experienced neutropenic septic shock induced by chemotherapy. Cisplatin-adriamycin chemotherapy was associated with significantly higher incidence of neutropenic septic shock compared to carboplatin-paclitaxel chemotherapy (p=0.049, OR 3.685, 95% CI 1.09-12.50). Conclusion: Through this study, we could identify the incidence of chemotherapy-induced neutropenic septic shock in gynecologic malignancy. The findings of this study will aid clinicians to select the most appropriate treatment modality.
Malignant female adnexal tumor of Wolffian origin (FATWO)

Female adnexal tumors of Wolffian origin (FATWOs) are rare tumors arising in the leaves of the broad ligament from the remnants of the mesonephric duct. Most FATWOs behave in a benign fashion, and there are only 11 cases reported in the literature showing malignant FATWOs. In this study, the authors report a 50-year-old woman with a malignant FATWO, positive for calretinin, CD34, CD56, focal positive for CK, and CD117 (c-Kit). The tumor cells were negative for alpha-inhibin, actin, S-100, CK 19, and EMA. The tumor showed a high mitotic count (17/10HPF), necrosis, capsular invasion, high cellularity, and moderate cytologic atypia. Review of literature showed that about 10% of FATWOs pursued an aggressive course and were generally regarded to be at least low malignant potential. Adverse prognosis factors include large side, capsular invasion and rupture, hypercellularity, nuclear pleomorphism, and increased mitotic activity. Because of the few reported cases, there are no clear recommendations regarding initial evaluation, treatment, and follow- up. A larger retrospective study is needed to support to establish diagnostic criteria and the scale of malignant potential.
발효미생물에 따른 산삼배양근의 대사물질 분석 및 프로파일링

Background : The study about cultured wild ginseng root (Panax ginseng C. A. Meyer) have been reported mainly ginsenosides in saponins family. However metabolites of fermented wild ginseng roots by microorganisms was not reported yet. Methods and Results : Cultured wild ginseng roots were used for fermentation of ginseng roots using Pediococcus pentosaceus and other bacterial strains. We analyzed different types of ginsenoside contents, metabolite and enzyme contents, and gene expression by using microorganisms. Results showed considerable differences in ginseonoside contents specially Rk1 and Rg5. The highest enzyme activity level was by Glutathione reductase (GR) and Glutathione S transferase (GST) in fermented ginseng roots than control (non-fermented), whereas Glutathione peroxidase (GPX) and Peroxidase (POD) contents were reduced. Score plots and loading plots of principal components 1 of the PCA result obtained from the data on 43 metabolites in fermented wild ginseng root of five conditions. The concentration of metabolite such as β-alanin and 4-aminobutyric acid (GABA), which is used to improve memory were increased in fermented ginseng roots than control. We found functional gene in wild ginseng root related with metabolic process. The APX gene expression gradually increased in fermented ginseng root with respect to fermentation times. Conclusion : In this study, accumulation of functional metabolite in cultured ginseng r
