        Effects of Danggwieumja on the Healing of Full-Thickness Skin Injury in Rat

        The purpose of this study was to investigate the wound healing effects of Danggwieumja (DG), which is commonly used for skin inflammation, skin wound, skin pruritus, and chronic hives etc. The 1.5 cm × 1.5 cm full-thickness skin wound was induced to two groups, DG (n=16) and Saline (n=16) group. The DG extract and Saline were orally administrated daily for 15 days after skin wound induction. Then, the body weight of rats and the congestion indices were daily measured for 15 days after skin wound induction. The wound contractions and epithelizations were also measured. The wound contractions were daily measured for 15 days after wound induction and wound epithelizations were measured for 8 days from day 7 after wound induction. For evaluating angiogenesis, the immunoreactivities of vWF and VEGF protein were measured immunohistochemistrically on day 15. In results, although the percentage increases in mean body weight of rats in the DG and Saline groups hve no significant differences, DG extract decreased the time of wound healing and congestion around wound, and improved wound contraction and epithelization. The contraction percentage of DG group was significantly increased on day 5 (P<0.05) and day 7 (P<0.01) than that of Saline group. DG group showed significant increase of wound epithelization on day 7 (P<0.05) as compared to Saline group. Moreover, DG extract reduced the inflammation of skin dermis and promoted the growth of vascular vessels of dermis by accelerating vascular endothelial growth factor (VEGF) protein. These results suggest that DG has the beneficial effects on skin incision wound and can be the suitable wound healing agent for various surgical wounds.

        Healing Effect of Bee Venom Cream to Full-Thickness Skin Wound in Rabbits

        Experiments were undertaken to assess the healing effect of bee venom (BV) cream to full-thickness skin wound in rabbits. BV cream was compared with silver sulfadiazine (SS) as a topical medicament against a control on experimentally created full-thickness wounds. Two wounds measuring 2 × 2 cm were created bilaterally (4 wounds/rabbit) on the dorsolateral aspect of the trunk of 7 New Zealand white. Wound treatments were evenly distributed 4 sites, using a Latin square design. The contact layer of wounds was treated with physiological saline (control), SS cream and BV cream during 28 days. Wound healing was assessed on the basis of scab hardness, wound exudates, wound area, unepithelialized granulation tissue and histopathological findings. Topical application of BV and SS creams to wound reduced inflammation, debrided necrotic tissue and promoted granulation and epithelialization. Wound healing was statistically significantly faster in BV and SS treatments compared to the control (P<0.05). BV treatment evoked anti-inflammation effect in a rabbit model. BV cream produced wound healing effect similar to commercially available SS cream. Anti-inflammation effect as a topical treatment of BV cream seems to be better than SS cream. These results suggest that the topical application of BV cream may be an alternative treatment on full-thickness skin wound.

        소양막이식편이 개의 전층 피부 창상치유에 미치는 효과

        The purpose of the present study was to investigate the effect of bovine amniotic membrane grafts on healing of full-thickness skin wound in dogs. Two $3cm{\times}3cm$ area-matched full-thickness skin wounds were induced bilaterally on the dorsolateral aspect of the trunk of 15 dogs. Chlorhexidine-treated amnion, dried amnion, silver sulfadiazine and 0.9% sterile saline solution were applied on the wound area and examined grossly and histopathologically. Begining 14 days after wounding, amnion applied group had appreciably less amount of inflammatory exudate and hemorrhage than sulfadiazine and saline treated groups. From 14 days after wounding, the degree of wound contraction in amnion groups, especially in the dried amnion group was greater than that of the sulfadiazine and saline treated groups. The percentages of wounds completely healed on 28 days after wounding in saline treated group, chlorhexidine-treated amnion group, dried amnion group and sulfadiazine treated group were 33%, 50%, 83% and 50%, respectively. Microscopically neovascularization and fibrosis were first noticed on 5 days after wounding in the dried amnion group and sulfadiazine treated group, on 7 days in the chlorhexidine-treated amnion group and on 14 days in the saline treated group. Epithelialization in the dried amnion and sulfadiazine treated groups was first noticed on 9 days after wounding, which was faster than that in the other groups. The present study suggests that bovine amniotic membrane, especially dried bovine amnion is effective on healing of full-thickness skin wound in dogs through both wound contraction and epithelialization.

        흰쥐의 전층피부손상에서 온청음(溫淸飮)이 신생혈관형성에 미치는 영향

        The wound healing process can be categorized as follows : inflammation, fibroplasia, neovascularization, collagen deposition, epithelialization, and wound contraction. During the healing process, various growth factors are secreted to accelerate wound healing. Previous studies have demonstrated that endogenous growth factors, such as vascular endothelial growth factor(VEGF) are the important regulatory polypeptides for coordinating the healing process. They are released from macrophages, fibroblasts, and keratinocytes at the site of injury and participate in the regulation of reepithelization, granulation tissue formation, collagen synthesis and neovascularization. Onchung-Um has been used clinically to treat various skin diseases. In addition, Onchung-Um has been also used for congestive inflammations. In the present study, we evaluated the effects of Onchung-Um on wound healing process and wound size reduction in rats. Full-thickness skin wounds ($15mm\;{\times}\;15mm$) were created on the back of rats. Rats were then divided into 2 groups : The Onchung-Um treated group that was orally administered with a dose of 193.9mg/100g of Onchung-Um extract per day for 15 days and Control group without Onchung-Um administration. Moreover, the histological changes and VEGF immunoexpressions of two groups were estimated. In results, wound closures were significantly accelerated by oral administration of Onchung-Um extract. Furthermore, in Onchung-Um treated group, there were significant increases in fibroblast migration, epithelialization compared with the Control group. VEGF expressions were also increased in Onchung-Um treated group. This study has therefore demonstrated the Onchung-Um can significantly improve the quality of wound healing and scar formation and the oral administration of Onchung-Um extract may increase early tissue angiogenesis in the incisional wound of an experimental animal model.

        황기약침이 전층피부손상 마우스의 치유에 미치는 영향

        Objectives : In this study, we investigated whether Astragali Radix Pharmacocupuncture (ARP) has an effective on the full thickness defect wound healing process of mouse. Methods : A total of 50 mice (ICR mouse, 7 week-old male) were divided into control group and ARP group. A single full thickness skin defect was made on the dorsal side of the each mouse using an 8㎜ diameter biopsy punch. Control group were treated with 0.2㎖ saline and ARP group were treat with 0.2㎖ ARP at 8 points around the wound every three days total 4 times during the experimental period. The change in wound size, contraction rate, healing rate, and epithelization rate was measured by digital images taken on days 3, 6, 9, and 13, and evaluated using a digital image analysis program. Tissues were collected for histological analysis, RT-PCR, and Western blot on days 3, 6, 9, and 15. Results : The results are as follows. ARP group accelerated the rate of wound contraction, wound healing and epithelization compared to the control group. ARP group showed the decrease of inflammatory cells in early inflammatory phase compared to the control group. ARP group upregulated PECAM-1 mRNA and protein expression compared to the control group. ARP group inhibited the scar width and area compared to the control group. Conclusions: ARP showed positive effects on wound healing through the inhibition of inflammatory reaction and increase of PECAM-1 expression related to the wound healing process.

        Silicone Wound Contact Layer를 이용한 전층 피부이식편의 고정

        Background: The tie-over dressing is the classic dressing method for a full thickness skin graft (FTSG) to secure the graft on the skin defect. Although many methods to secure the graft have currently been employed, they have some disadvantages such as stretching discomforts and irregular pressure delivery to the graft, resulting in partial necrosis or surface irregularity. Objective: The aim of this study was to evaluate the usefulness of a porous, flexible and distensible silicone wound contact layer (Mepitel(R)) for securing a graft as a bolster fixation in FTSG reconstruction. Methods: Sixty seven patients with surgical defects following mainly malignant tumor excision were included in this study. After suturing and bolstering the graft in place, a piece of silicone contact layer was used to cover the graft, and then second sutures were placed to apply uniform pressure. Results: Thirty three cases of basal cell carcinoma, 14 cases of malignant melanoma and 10 cases of squamous cell carcinoma were included as a cause of the defects. Forty six defects were located on the head and neck, and the rest were on the trunk and extremities. The size of the surgical defects ranged from 0.5 to 14.0 cm at the greatest diameter (mean: 4.0 cm). One major and 1 minor loss of graft developed. Mild to moderate scar formation, mainly marginal elevation, occurred in 9 patients. Sixty five percents of the patients who had facial defects were satisfied with their results. Conclusion: We suggested that a silicone wound contact layer for bolster fixation might be a useful material to secure a FTSG, and this is easy to apply and the resulting cosmesis is good and there are few problems with using it.
