STUDY ON ADAPTABILITY OF RUBBER DAM CLAMPS ON PRIMARY SECOND MOLARS IN KOREAN CHILDREN

The purpose of this study was to investigate the mesiodistal widths at the cervical level of primary second molars in Korean children, and to compare them with commercial rubber dam clamps commonly used in pediatric dentistry. Dental casts of 115 primary and mixed dentition children were studied. Cervical mesiodistal width (C-MD) was measured at the clinical cervical level of each primary second molar from buccal and lingual sides using a digital caliper (Absolute, Mitutoyo, Kawasaki, Japan). The data were compared with mesiodistal widths of rubber dam clamps #203/204, #10/11, and #205 (Dentech, Japan). C-MDs of primary mandibular second molars were larger than those of primary maxillary second molars, and C-MDs at buccal sides were larger than those of lingual sides. All C-MDs showed statistically significant discrepancies to corresponding widths on clamps (p < 0.05). However the amount of discrepancy was mild in maxillary teeth, while up to 1 mm of discrepancy was shown in mandibular teeth. In conclusion, C-MDs measured in this study imply a relatively fair fit of #10/#11 or #203/#204 clamps on primary maxillary second molars, while suggesting our need for a better clamp with proper size for primary mandibular second molars.
Dental Management in a Patient with Infantile Osteopetrosis : A Case Report with a 7-Year follow-up

Osteopetrosis is characterized by impaired osteoclast function and increased bone density. Infantile osteopetrosis is a severe form of the disease and has characteristics such as diffusely sclerotic skeleton, pancytopenia, cranial nerve entrapment, infection susceptibility, and abnormal craniofacial appearance. Patients with infantile osteopetrosis often experience developmental delay, and may have a short life span. A 14-month-old girl with osteopetrosis presented to the department of pediatric dentistry. Incipient caries on deciduous incisors were observed. The patient revisited 4 years of age. Besides medical problems, oral complications such as growth retardation, narrow upper arch, crowding, dental caries, and abnormal tooth development were observed. After consultation with her pediatrician, dental treatments were performed on the deciduous molars under sedation after a prophylactic antibiotic injection. At a periodic follow-up, multiple deciduous teeth were treated and extracted, and oralrehabilitation with a removable partial denture was initiated. Patient with osteopetrosis are highly susceptible to infection because of their compromised immune system and problems associated with wound healing that lead to osteomyelitis or sepsis development. Active participation in dental care for sugar intake management and proper oral hygiene are obligatory.
Distribution of Pediatric Malocclusion Patients in Seoul National University Dental Hospital

A total of 580 patients, who visited and received an orthodontic diagnosis in the Department of Pediatric Dentistry, Seoul National University Dental Hospital from 2017 to 2019, were investigated in this study. The aim of this study was to evaluate skeletal patterns of pediatric orthodontic patients determined with lateral cephalometric analysis and to analyze the relationship between skeletal pattern and probable associated clinical features. Also, the modality of orthodontic treatment for each skeletal classification was investigated to aid in therapeutic decisions. Patients aged 7 year accounted for the largest age group; 54.2% of patients showed a skeletal class I pattern, 22.2% showed a skeletal class II pattern, and 23.6% showed a skeletal class III pattern. Bi-maxillary retrusion for skeletal class I, retruded mandible with normal positioning of the maxilla for skeletal class II, and retrusion of the maxilla with protrusion of the mandible for skeletal class III were the largest subgroups by skeletal pattern. Brachyfacial type accounted for 55.0% of patients, followed by 31.9% of mesofacial type and 13.1% of dolichofacial type. The prevalence of anterior crossbite in the study was 43.3%, higher than that in previous studies.
Dental Management in a Patient with Turner Syndrome with Dental Anomalies : A Case Report

Turner syndrome (TS) is a chromosomal disorder caused by monosomy of the X chromosome, with complete or partial absence of the second sex chromosome. Anomalies of root morphology have been found to occur more often in patients with TS, which make endodontic treatment challenging and requires special handling. The patients with TS may also have systematic problems such as cardiac or renal malformations, so in treating these patients it is important for clinicians not only to be aware of the characteristic intraoral findings, but also to make the patients have regular dental check-ups to prevent oral complications in advance. An 12-year-old girl who had been diagnosed with TS at the age of 10 years was referred due to discomfort in the bilateral mandibular premolar regions. Dens evaginatus and taurodontism were detected in all the mandibular premolars characteristically. The bilateral mandibular first premolars had three roots and the bilateral mandibular second premolars had periapical lesion with two roots. Due to the complexity of the root canal anatomy, root canal treatment were completed with a dental microscope to ensure adequate visualization. After 2 years of regular follow-up examinations, there were no clinical sign or symptom associated with the teeth, and no periapical lesion, was found. This case report describes the characteristic oral features and dental management of TS patients.

This study evaluated the fluoride release of alkasite restorative material (ARM) and giomer penetrating the dentin adhesive layer. Twenty specimens were prepared for each restorative material, and dentin adhesive with uniform thickness was applied to half of them. The prepared specimens were placed in a polyethylene tube containing 2.0 mL of deionized water and deposited in a 37.0℃ water bath for the study duration. The amount of fluoride release was measured on the 1<sup>st</sup>, 3<sup>rd</sup>, 7<sup>th</sup>, 14<sup>th</sup>, 21<sup>st</sup>, and 28<sup>th</sup> days after deposition. The dentin adhesive applied to the ARM and giomer could not completely block the fluoride release; however, it significantly reduced its amount. The cumulative amount of fluoride release of the ARM after 28 days was higher than that of the giomer regardless of the application of dentin adhesive.
Traumatic Bone Cyst in the Mandibular Symphysis : Case Reports

Traumatic bone cyst (TBC) is an asymptomatic lesion seen most in adolescents. TBC is found incidentally on routine panoramic examinations and appears as a relatively well-demarcated unilocular radiolucency with scalloped margins. Histological examination reveals a vacant cavity of cancellous bone usually unlined or very occasionally lined with a thin connective tissue layer. The lack of lining epithelial membrane is common histological feature. The most affected site is between the mandibular canine and third molar. The involvement of the mandibular symphysis is rare. The etiopathogenesis of the TBC is unclear. TBC is treated with surgical exploration and curettage; new bone is formed in place of the lesion within 6 - 12 months of surgery. Diagnosis of TBC prior to surgical intervention has limitations in most of the cases. Both of our patients were diagnosed through radiological examination and biopsy. Neither patient had a history of trauma. After surgery, the panoramic radiograph and CBCT were used to confirm bone healing.
Effects of Essential Oil Containing Mouth Rinse in Children with Black Staining : Two Case Reports

Black staining of the teeth in children and adolescents does not cause pain or serious illness, but it can be socially debilitating for esthetic reasons. Black staining is easily removed through periodic professional mechanical tooth cleaning and ultrasonic scaling, but it can easily recur within few months. Using essential oil-containing mouth rinses diluted at 50% twice per day could prevent the black staining from returning after it is removed, reducing the need for mechanical treatments and improving esthetics.

이 후향적 연구의 목적은 한국 성장기 아동의 수직적인 안모 형태와 성별에 따라 골격적 성숙도와 치아의 성숙도를 평가하는 것이다. 총 184명의 8 - 14세 소아∙청소년을 대상으로 진행하였다. 하악평면각을 기준으로 3개의 군으로 나누었으며 성별 간 비교를 위해 3개의 군을 각각 남아와 여아의 하위 군으로 나누었다. 골격적 성숙도와 치아 성숙도는 측모두부 방사선 사진, 수완부 방사선 사진, 파노라마 방사선 사진으로 평가하였다. 수직적 성장군은 수평적 성장군보다 경추골 및 수완부 골의 높은 성숙도를 보였다. 치아 성숙도는 수직적 성장군에서 가장 높았다. 남아보다 여아에서 골격적 성숙도가 더 높았으며 치아 성숙도는 성별 간 유의한 차이는 관찰되지 않았다. 환자의 수직적 안모 형태 분석은 교정치료 시작 시기 결정에 도움이 되는 보조지표를 제공할 수 있으며, 수직적 안모 형태를 보이는 환자의 경우 교정치료를 조기에 시작하는 것을 고려할 수 있다.
Long Term Follow-up of Children with Facial Asymmetry: A Case Report

Mandibular fractures occur with high incidence among various fractures in maxillofacial region in children. Jaw fractures in children should be approached differently than in adults because bone growth continues throughout childhood. As far as displacement of the fragment is not severe, or if it is condyle that is fractured, closed reduction and additional intermaxillary fixation can be considered. Functional exercise is also required to prevent ankylosis of temporomandibular joint. Several complications, particularly malocclusion and facial asymmetry due to growth disturbances, can occur after condylar fractures. If growth disturbances take place after mandibular fractures, catch-up growth may occur in some patients, thus, periodic observation is necessary. In case of persistent growth disturbances, functional devices may be used to prevent severe facial asymmetry. This case report describes the long-term follow-up of two patients with facial asymmetry after mandibular fracture.

치아종은 유치나 영구치배의 지속적인 치배 형성이나 법랑기 세포들의 비정상적인 증식의 결과로 발생하는 방사선 불투과성 병소로, 비교적 흔한 치원성 종양이며 인접한 치아의 맹출을 방해 할 수 있다. 치아종은 영구치열기에서 주로 관찰되고, 유치열기에서는 매우 드문 것으로 보고되고 있다. 본 증례 보고에서는 치아종에 의해 매복된 유치가 치아종의 외과적 발거 후 자연 맹출된 증례들을 보고하고자 한다. 매복유치는 계승 영구치의 발육과 맹출 문제를 야기할 수 있기 때문에 치아종이 제거된 후에도 계승 영구치가 맹출할 때까지 장기적인 검사 및 관찰이 필요하다.
