      • 북한강 상류 천연자원의 종합관리 시스템 개발

        강원도 춘천시를 중심으로하는 북한강 상류는 수도권에 청정 상수원류를 제공하는 생활의 핏줄이다. 또한 수도권 지역 건설에 필수적인 석재 자원의 주요 공급 지역으로 알려져 있다. 본 연구에서는 이 지역의 보존과 체계적 개발을 위한 지리정보처리 기술을 이용한 천연자원 종합관리 시스템을 개발하였다. 이 시스템은 자원관리를 위한 막강한 도구와 기능 예를 들면 정보수집, 편집, 처리, 모델링, 분석 및 가시화기능을 제공한다. 결과적으로 이 시스템은 지하자원 분석, 천연자원의 기획 및 개발, 자연보호 등 자원관리와 연계된 문제를 효율적으로 해결할 수 있는 능력을 제공하고 있다. The upper stream area of the north Han river, which is located around the Chunchon city of the Kangwon province, is out essential resources, which supply clear drinking water to the capital city of out conutry, Seoul. This area has also produced stone and aggregate resources required for building and road constructions in the capital area For preservation and planned-development of this area, in this study, we have developed an integrated management system for natural resources, based upon geological information technologies. This system includes powerful tools and functions for resource management such as information collection, edition, processing, modeling, analysis, and visualization. As a result, this system may give us capabilities to efficiently handle a variety of resource management issues such as the analysis of underground resources, the planning and development of natural resources, the protection of environment and their related projects.

      • 강원지역의 자원관리를 위한 지리정보시스템 개발

        강원도는 오랫동안 금속, 비금속, 석재 및 석탄과 같은 지하자원들을 공급하는 국내 주요생산지로 알려져 왔으나, 장기적인 채광과 비 계획적인 생산으로 인하여 매장량이 고갈되고 있으며, 대외적으로 생산 경쟁력 마저 잃어가고 있다. 이러한 문제점에 대한 해결방안의 일환으로 본 연구에서는 강원도내에 산재되어 있는 지하자원의 분포, 자원 생산시설 및 생산량, 경제성, 자원개발, 환경문제 등 자원관리에 필요한 제반사항을 수행할 수 있는 강원지역 자원 종합관리 GIS 시스템을 개발하였다. 이 시스템의 개발은 방대한 양의 정보 조사 및 저장, 오랜 개발기간, 많은 소요인력 및 개발 비용이 요구되기 때문에 본 연구에서는 조기에 자원관리 GIS 시스템을 개발하여 이와 관련된 생산 및 연구 분야에 조기에 적용할 수 있도록, 일차년도에는 현재 석재복합 신소재제품 연구센터의 집중적 연구대상 지역인 태백지역에 대한 탄광지역을 중심으로 자원 종합관리 GIS 시스템을 개발하였다. The Kangwon province has been well-known to be one of major underground resource producing districts which are mainly supplying metals, non-metals, stones, and coals. However, due to the long-term mining and unplanned production, the natural resource reserves have been rapidly drained and oversea production competition has been losing. In this study, we has developed a GIS system for regional resource management in the Kangwon province, which can operate, maintain, and manage the distribution of underground resources, resource production facilities, production and economic development, and environmental issues. The development of this system requires the collection, analysis, evaluation, and storage for a huge amount of research data, and in addition, the long period of time, the big research man power, and big development cost. Accordingly, to shorten the development period of time and to early apply itself to production, this stage of study has been intensively developed a regional resource GIS system for managing the mines scattered in the Tabak province, which is one of hottest RRC's research areas.

        단순반복 작업자의 작업대 및 의자의 형태와 누적외상성질환 증상과의 관계

        목적 : 단순반복작업 생산직 근로자의 누적외상성질환에 대한 사업주의 건강관리규정 준수상태 및 근로자의 자각증상호소와 의자 및 작업대의 형태를 파악하고 크기를 측정하여 이들과 누적외상성질환 자각증상호소와의 관련요인을 알아보고 대책을 제공하고자 한다. 방법 : 서울 및 경기지역 일부 100인 미만 사업장중 전기, 전자부품 제조업체 7개를 선정하여 생산직 근로자 77명을 대상으로 누적외상성질환에 대한 사업주의 건강관리규정 준수상태와 및 근로자의 자각증상호소에 대한 설문조사를 실시하고 의자와 작업대의 크기를 측정하고 증상과의 관련요인을 알아보기 위하여 로지스틱회귀분석을 실시하였다. 결과 : 의자와 작업대가 작업자세에 영향을 준다고 인식하는 작업자는 85.7 %이였으며 단순반복작업 근로자 작업관리지침에 의한 사업주의 근로자에 대한 관심은 매우 낮았다. 누적외상성질환 자각증상 호소율은 63.6 %이였으며 자각증상 부위는 어깨 37.9%, 목 15.2 %, 등 6.1 %, 팔/팔꿈치 4.6 % 순이였다. 입사 후 초기발생시기는 평균 16.5개월이며 자각증상 호소는 작업대 두께, 작업대높이, 작업대 밑무릎공간, 등받이 높이, 좌석면 높이, 좌석면의 앞뒤길이, 좌석면의 좌우길이, 작업대끝에서 어깨까지의 거리에서 유의한 관련이 있었다. 자각증상호소의 관련요인은 등받이 높이, 좌석면의 높이, 좌석면의 앞뒤길이 이였다. 결 론 : 이상의 결과로 보아 정부는 단순반복작업 작업자의 누적외상성질환 예방을 위하여 사업주에 대 한 철저한 교육 및 관리감독을 해야한다. 사업주는 작업대 및 의자를 작업자의 신체치수와 작업내용에 맞게 높이를 조절할 수 있는 제품을 사용하고 작업전, 후에 정기적인 건강체조 및 휴식을 갖도록 하며 근로자에 대한 실질적인 보건교육이 이루어져야 한다. Objectives : The main purpose of this study was to investigate relationship between cumulative trauma disorders (CTDs) and the types of workstation and chair in workers with repetitive motion tasks. Methods : The study subjects were 77 repetitive motion tasks workers at small manufacturing industries in Seoul and Gyung-gi district area. A questionnaire was used to obtain about health practice for prevention and prevalence of CTDs by worker. The type of Workstations and chairs in workers were measured and analyzed with prevalence of CTDs by logistic regression. Results : Eighty-six percent of workers recognized that work posture was related to workstation and chair, Most employers and employees didn't follow the work practice guidelines for workers with repetitive motion tasks. The prevalence of CTDs was 64% by questionnaire. The prevalence of pain in shoulder, neck, back, and arm were 38%, 15%, 6%, and 5%, respectively, and the pain started after working an average duration of 16.5 months. The CTDs symptoms were significantly related to thickness of workstation, height of workstation, length of knee depth under workstation, height of back rest, height of seat pan, depth of sitting surface, width of sitting surface, and length from workstation surface edge to shoulder. By multiple regression analysis of each risk factor, prevalence of CTDs was highly correlated with height of back rest, height of seat pan, and depth of sitting surface. Conclusions : This study suggests that workstations should be made adjustable for height and chairs should be equipped with seat pan height control, enough sitting surface for depth and width, back rest height and angle control. The workers should use the right working postures, exorcise for health, occupational health education, and rest breaks and the manager of company should be educated for the preventing of CTDs.

      • 만성 호중구성 백혈병 1예

        Chronic neutrophilic leukemia(CNL) is a very rare myeloproliferative disease, characterized by sustained mature neutrophilic leukocytosis with granulocytic bone marrow infiltration, high NAP(neutrophilic alkaline phosphatase) score and absence of philadelphia chromosome, It is frequently accompanied by hepatosplenomegaly, elevated serum vitamin B_12 and uric acid level. For the diagnosis of CNL, the leukemoid reaction, especially secondary to neoplasia, infection and autoimmune diseases, should be excluded. Since Tuohy's first description in 1920, more than 50 cases fullfilling the above criteria have been reported worldwide, and 4 cases in Korea. Several authors have demonstrated the defect of intracellualr killing in the mature neutrophil and this finding correlates well with the very high incidence of fatal infection, The hemorrhagic diasthesis in CNL is caused by functional abnormality of the platelet. This disease has tendency to transform to blastic crisis and acute leukemia as in other myeloproliferative disease, but characteristically shows frequent coexistence with multiple myeloma. Until now , the therapeutic trials in CNL have been disappointing. Hydroxyrea and busulfan can control hyperleukocytosis. On the basis of functional defect in neutrophil, alpha-2b-interferon has been tried and several reports have demonstrated the clinical and functional effect of interferon on CNL. CNL is very rate hematologic disease and there are few report about general aspect of disorder. We report here a typical CNL case presenting with splenomegaly and leukocytosis with a review of the literature.

      • Elevated Response to Type I IFN Enhances RANKL-Mediated Osteoclastogenesis in Usp18-Knockout Mice

        <P>A balance between bone formation and bone resorption is critical for the maintenance of bone mass. In many pathological conditions, including chronic inflammation, uncontrolled activation of osteoclast differentiation often causes excessive bone resorption that results in osteoporosis. In this study, we identified the osteopenia phenotype of mice lacking Usp18 (also called Ubp43), which is a deISGylating enzyme and is known as a negative regulator of type I IFN signaling. The expression of Usp18 was induced in preosteoclasts upon receptor activator of NF-kappa B ligand (RANKL) treatment. In an in vitro osteoclast-differentiation assay, bone marrow macrophages from Usp18-deficient mice exhibited an enhanced differentiation to multinucleated cells, elevated activation of NFATc1, and an increased expression of osteoclast marker genes upon RANKL treatment. Furthermore, in vitro quantification of bone resorption revealed a great increase in osteoclastic activities in Usp18-deficient cells. Interestingly, proinflammatory cytokine genes, such as IP-10 (CXCL10), were highly expressed in Usp18-deficient bone marrow macrophages upon RANKL treatment compared with wild-type cells. In addition, serum cytokine levels, especially IP-10, were significantly high in Usp18-knockout mice. In sum, we suggest that, although type I IFN is known to restrict osteoclast differentiation, the exaggerated activation of the type I IFN response in Usp18-knockout mice causes an osteopenia phenotype in mice.</P>

        20대 이하 간세포암종의 증례 : 20대의 젊은 나이에 늑골전이로 발견된 간세포암종 1예

        Helatocellular carcinoma (HCC) is uncommon in young adults, and young HCC patients is known to show poor prognosis than older HCC patients because they have a more advanced tumor stage at diagnosis. We describe a case of HCC in a 28-year old chronic hepatitis B virus carrier who showed multiple nodular HCC with bone metastasis at diagnosis. In spite of multidisciplinary treatment including transarterial chemoembolization (TACE) for liver mass and radiotherapy for metastatic bone lesion, the patient died of cancer progression and weakened general condition 15 months after diagnosis. Therefore, we need to consider periodic surveillance in young chronic hepatitis B virus carriers.

      • Mycoplasma Pneumoniae 감염 후에 발생한 발작성한냉 혈색소뇨증 1례

        Paroxysmal cold hemoglobinuria (PCH) is one of the rare autoimmune hemolytic syndromes. It is characterized by acute intermittent hemolysis, hemoglobinuria, abdominal pain and periodic attacks of fever, followed by local or general exposure to cold, and positive Donath-Landsteiner test. PCH shows the Donath-Landsteiner antibody (D-L antibody) that is a complement binding immunoglobin G, anti-P biphasic hemolysin, causing significant red cell destruction. We report a case of PCH following mycoplasma infection which was confirmed by increased mycoplasma antibody titer, cold agglutinin and D-L antibody. The patient complained about dry cough, headache, high fever, jaundice and hematuria. In the antibody identification test the reactions with all panel cells were positive.
