        자연산 점몰개, Squalidus multimaculatus Hosoya et Jeon의 새인두흡충, Clinostomum complanatum (Rudolphi, 1814) 감염

        경상북도 영덕군의 하천에 서식하는 점몰개 spotted barbel gudgeon (Squalidus multimaculatus Hosoya et Jeon)의 근육에서 새인두흡충 Clinostomum complanatum (Rudolphi, 1814)의 피낭유충이 발견되었다. 감염된 점몰개는 "yellow grub"인 기생충의 피낭유충으로 인해 표면이 울퉁불퉁하게 보였으며, 수면위에 힘없이 유영하였고 일부 폐사한 개체도 발견되었다. 분리된 탈낭유충의 형태는 혓바닥 모양이었으며, 크기는 2.85~5.89 mm×0.8~1.99 mm였다. 어체당 1~98 개의 피낭유충을 분리하였으며 평균 감염수는 25개로 나타났다. 특히, 성어크기인 평균 7 cm 정도의 점몰개는 100% 감염되어 있었다. 본 연구는 한국의 자연수계에 서식하는 어류에서 C. complanatum 의해 질병이 발생하고 폐사가 일어난 첫 보고이다. The metacercariae of Clinostomum complanatum (Rudolphi, 1814) were isolated in the whole body muscle of wild freshwater fish, spotted barbel gudgeon (Squalidus multimaculatus Hosoya et Jeon) from the local river in Yeongdeok, Gyeongsangbuk-do, Korea. The infected fish with bumpy body surface by "yellow grub" showed erratic swimming behavior and some fish were died. The isolated excysted metacercariae were tongue shaped and 2.85~5.89 mm × 0.8~1.99 mm in size. 1~98 metacercariae were isolated in individual fish and mean infection intensity was 25. All examined adult spotted barbel gudgeon sizing 7 cm in body length were infected. This is the first report of disease outbreak and mortality caused by C. complanatum infection in wild freshwater fish in Korea.

        소아에서 간질발작의 임상양상과 뇌파검사의 관계

        목적:발작의 분류는 발작의 원인, 예후를 추정하거나 항경련제를 선택하는 데 있어 매우 중요하며 뇌파검사는 발작의 종류를 구분하고 적절한 치료를 하기 위해 시행하는 중요한 검사이다. 저자들은 소아에서 임상적 발작이 확실한 경우 간질발작의 임상양상과 뇌파검사 간의 일치 정도를 알아보고자 한다. 방법:2000년 1월 1일부터 2008년 9월 30일까지 1회 이상의 간질발작을 주소로 가천의대 길병원 소아청소년과에 방문한 환자 중 본원에서 처음으로 뇌파검사를 시행한 환자를 대상으로 하였다. 발작의 임상양상은 목격자와 본인의 설명을 기록한 의무기록지를 기준으로 하여 부분발작과 전신발작으로 분류하였고 뇌파검사는 발작간간질양방전을 기준으로 정상과 비정상으로 분류하였으며 각성과 수면상태 모두에서 시행하였다. 결과:총 461명의 환자의 뇌파검사 시의 평균 연령은 6.7세였고 남자는 247명, 여자는 214명이었다. 전체 환자에서 발작의 임상양상은 부분발작이 310명, 전신발작이 187명이었다. 각성뇌파검사에서 비정상은 158명으로 부분발작파가 118명, 전신발작파가 59명, 두 가지 발작파가 한 환자에서 나온 경우가 19명이었고, 수면뇌파검사에서는 비정상이 239명으로 부분발작파가 196명, 전신발작파가 77명, 두 가지 발작파가 한 환자에서 나온 경우가 34명이었으며 전체 환자에서는 비정상이 273명으로 부분발작파가 216명, 전신발작파가 97명, 두 가지 발작파가 한 환자에서 나온 경우가 40명이었다. 간질증후군은 전체 환자 중 90명에서 관찰되었다. 146명(41.5%)의 부분발작과 44명(23.4%)의 전신발작 및 79명(87.8 %)의 간질증후군이 뇌파소견과 일치하였다. 결론:영유아 및 소아에 있어 발작을 진단할 때에는 발작양상 분 아니라 정확한 뇌파소견을 세심하게 고려하여야 한다. Purpose:Electroencephalography(EEG) is an essential method carried out for classifying seizures and taking appropriate treatment. The aim of this study is to investigate the concordance between clinical findings of epileptic seizures and EEG in children. Methods:We enrolled 461 patients from those who visited Gil hospital from January 1, 2000 to September 30, 2008 with the chief complaint of epileptic seizure more than once and checked their first EEG at the same hospital. The clinical findings of seizure were based on the charts and interictal waking and sleep EEGs were done. Results:The mean age of the patients was 6.7 years old. 497 epileptic seizures occurred and its clinical finding included 310 of partial seizures and 187 of generalized seizures. In 315 waking EEG 158 were abnormal including 118 of partial seizures, 59 of generalized seizures and 19 of both seizures, in 431 sleep EEG 239 were abnormal including 196 of partial seizures, 77 of generalized seizures and 34 of both seizures, and in waking and/or sleep EEG 273 were abnormal including 216 of partial seizures, 97 of generalized seizures and 40 of both seizures. Epileptic syndromes were observed in 90 patients. 146(41.5%) of partial seizures, 44(23.4%) of generalized seizures, and 79(87.8%) of epileptic syndromes accorded with EEG. Conclusion:When diagnosing seizures in children, we must consider not only clinical findings but also accurate EEG findings.

      • Congenital cytomegalovirus infection: Variable prenatal ultrasonographic findings

        The objective of this study was to evaluate prenatal ultrasonograhpic findings and neonatal outcome in congenital cytomegalovirus (CMV) infection. Retrospective analysis was performed in 36 fetuses with congenital CMV infection between 1997 and December 2011. Among 36 fetuses, 12 were diagnosed prenatally and 24 after birth. Prenatal ultrasonographic abnormalities were observed in 35 of 36 cases. The median gestational age of ultrasonographic diagnosis were 28.4 weeks of gestation. Intracranial ultrasonographic abnormalities were detected in 24 (66%) fetuses. The most common finding was ventriculomegaly (n=23). The other findings were intracranial calcificataion (n=8), cerebellar hypoplasia(n=4), microcephaly (n=3), cerebral atrophy (n=3). extracranial findings were shown in 25 (69%) fetuses. Intrauterine growth restriction (IUGR) is most common finding (n=21) and the others were echogenic bowel (n=10), oligohydramnios (n=6), ascites (n=6), hepatosplenomegaly (n=2), fetal hydrops (n=2). Amomg 12 fetuses with prenatally diagnosed, 8 pregnancies were terminated and 4 cases were lost to follow-up. 24 liveborn neonates were delivered at mean gestational age of 38.0 weeks. Among these, 12 were doing well without any sequelae and 12 had adverse outcome. 41.7% (5/12) of neonates with adverse outcomes had more than two intracranial lesion in prenatal ultrasound. In contrast among 12 normally developing neonates, no one had multiple intracranial lesion in prenatal ultrasound. The spectrum of prenatal ultrasonographic findings are diverse in fetuses with congenital CMV infection. When the aforementioned abnormal ultrasonographic findings were identified, clinician have to rule out the congenital CMV infection and according to degree and number of ultrasonographic abnormalities, especially intracranial lesion, clinician can predict neonatal outcome and provide appropriate prenatal counseling to parents

        A Global Gene Expression Analysis of the Peripheral Blood Mononuclear Cells Reveals the Gene Expression Signature in Psoriasis

        Background: Psoriasis is a chronic inflammatory skin disease that affects approximately 1~3% of the general population. Objective: We performed cDNA microarray analysis with using the dendrimer labelling method to investigate the gene expression profile in the peripheral blood mononuclear cells (PBMCs) of psoriatic patients. Methods: The peripheral blood mononuclear cells of 5 patients with psoriasis and 8 control subjects were used in the gene expression analyses of psoriasis. Results: We identified 212 differentially expressed genes that showed at least a two-fold induction and/or reduction in psoriatic patients. Among those, 63 genes, including CD44, CD56 and IL7R, were induced, while 139 genes, including the sphingosine kinase 1 and p16-INK genes, were reduced in the psoriatic patients. Conclusion: We can speculate that these genes may have a role for the pathogenesis of psoriasis via their affecting different cellular functions. Our results suggest a possible mechanism by which activated immune cells migrate from the blood to the skin in psoriatic patients, and we provide novel putative targets for developing drugs to treat psoriasis.         Updated Trans-Ethnic Meta-Analysis of Associations between Inflammation-Related Genes and Intracranial Aneurysm

        Objective : We performed an expanded multi-ethnic meta-analysis to identify associations between inflammation-related loci with intracranial aneurysm (IA) susceptibility. This meta-analysis possesses increased statistical power as it is based on the most data ever evaluated. Methods : We searched and reviewed relevant literature through electronic search engines up to August 2022. Overall estimates were calculated under the fixed- or random-effect models using pooled odds ratio (OR) and 95% confidence intervals (CIs). Subgroup analyses were performed according to ethnicity. Results : Our meta-analysis enrolled 15 studies and involved 3070 patients and 5528 controls including European, Asian, Hispanic, and mixed ethnic populations. Of 17 inflammation-related variants, the rs1800796 locus (interleukin [IL]-6) showed the most significant genome-wide association with IA in East-Asian populations, including 1276 IA patients and 1322 controls (OR, 0.65; 95% CI, 0.56–0.75; p=3.24×10-9) under a fixed-effect model. However, this association was not observed in the European population (OR, 1.09; 95% CI, 0.80–1.47; p=0.5929). Three other variants, rs16944 (IL-1β), rs2195940 (IL-12B), and rs1800629 (tumor necrosis factor-α) showed a statistically nominal association with IA in both the overall, as well as East-Asian populations (0.01<p<0.05). Conclusion : Our updated meta-analysis with increased statistical power highlights that rs1800796 which maps on the IL-6 gene is associated with IA, and in particular confers a protective effect against occurrence of IA in the East-Asian population.

        Evaluation of the Antioxidant Activity of the Fruiting Body of Phellinus linteus Using the On-line HPLC-DPPH Method

        Phellinus linteus has traditionally been used as a medicinal mushroom in Oriental countries. Bioactive phytochemicals from the fruiting body of P. linteus were investigated using the on-line HPLC-DPPH system. The system was used to separate 10 antioxidants: protocatechuic acid (1) protocatechualdehyde, (2) caffeic acid (3), ellagic acid (4), hispidin (5), davalliallactone (6), interfungins A (7), hypholomine B (8), inoscavin A (9), and methyldavallialactone (10). Of these, compounds 6 and 7 showed the strongest inhibitory effect against the DPPH radical and superoxide anion radical-scavenging capacity. On comparison with quercetin ($IC_{50}$, 44.0 ${\mu}M$), the $IC_{50}$ value of compounds 6 and 7 for DPPH radical-scavenging capacity were 19.6 ${\mu}M$ and 18.5 ${\mu}M$. Moreover, the antioxidative capacity of compound 6 at 40mM was 2.95 nmol Trolox equivalents per mg dry wt. These results indicated that hispidin dimers such as hispidin with hispidin or hispolon had a stronger effect on radical-scavenging capacity than hispidin itself.

        Anti-Cancer Effects of High-Purity Edible Sulfur on Immortalized and Malignant Human Oral Keratinocytes

        Sulfur is commonly used in Asia as a n herba l medicine to treat infl ammation and cancel‘. and potent chemopreventive effects have been demonstrated in various in vivo and in vitromodels for sulfur-containing compounds found in naturally occun‘ ing products. Here, we report the growth inhibitory and apoptosis-related effects of a newly developedhigh- purity eclible sulfur (ES) on immortali zecl human oral keratinocytes (IHOKs) and on oral cancer cells representing two stages of oral can cer (HN4‘ HN12) basecl on an 3-(4. 5-Dimethylt hiazol-2-yl)-2.5-cliphenyl tetrazolium bromide (MTT) a ssay, Western blotting, cell cycle analysis, ancl nuclear staining. The puri ty of the ES used in th is s tucly was verified by high performance liquid chromat ography (HPLC) , amino acid analysis and energy di spersive spectroscopy (EDS). ES inhibitecl the proliferation of immor talized and malignant oral kerati nocytes in a dose- and time-dependent manner FITC-Annexin V staining. DNA fragmentation testing. and Hoechst 33258 staining revealed that ES inhibits cell growth via apoptosis .       • OB-20 : Prognostic factors and perinatal outcomes of fetuses with congenital diaphragmatic hernia (CDH)

        The aim of this study is to describe the prenatal prognostic factors and the perinatal outcomes of fetuses with antenatally diagnosed with congenital diaphragmatic hernia (CDH). A single-center retrospective analysis of fetuses with CDH detected antenatally between January 1997 and December 2012 was conducted. We evaluated perinatal outcomes and prenatal factors associated with poor neonatal outcomes in a setting which before imposing fetal intervention for CDH. One hundred ninety-eight cases of CDH were identified. Of these, 138 (69.7%) infants were born alive, 22 cases underwent termination of pregnancy, 37 cases were lost to follow-up and 1 case were transferred to other hospital at term for delivery. Among live-born infants, 94 babies survived (68.1% of live-born infants, 47.5% of all cases of CDH). 23 (16.7%) infants were died prior to corrective surgery and 21 (15.2%) infants were died after operation. The presence of liver herniation were more commonly found in non-surviving infants compared to surviving infants (48.8% vs 14.%, p<0.001). Survival rate in fetuses with isolated bowel herniation was 96.7% whereas fetuses with liver herniation was 39.3%. Variables associated with death were liver herniation (OR 7.69), presence of an additional anomalies (OR 14.15) and polyhydramnios (OR 3.28). This study provides data for counselling parents regarding perinatal outcome and prognosis of CDH. Although the survival rate of CDH has improved but in some case the mortality still remains high. So in the future, more accurate prenatal assessment and imposing fetal intervention in fetuses with predicted poor prognosis should be considered in order to improve survival rate
