        지루각화증의 임상 및 병리조직학적 고찰

        Background: Although seborrheic keratosis is one of the most common epidermal tumors, there are only rare studies on the clinicohistopathologic features of seborrheic keratosis in Korea. The majority of cases of typical seborrheic keratosis can be clinically differentiated with ease from other dermatoses such as melanocytic nevi, lentigo, basal cell carcinoma and melanoma. However, its differential diagnosis can sometimes be difficult. Objective: This study was designed to investigate the clincohistopathologic characteristics of seborrheic keratosis, the frequency of biopsy and the accuracy of making the clinical diagnosis, as well as to analyze the cases where there is a mismatch between the clinical diagnosis and the histological diagnosis. Methods: We conducted a retrospective study and we searched for patients who were clinically diagnosed as having seborrheic keratosis between January 2000 and December 2008. The search found 1,248 patients and the patients were divided into two groups: a non-biopsy group (n=891) and a biopsy group (n=357). The clinical features, including age, gender, the anatomic site of lesion and the treatment method, were investigated in each group. In addition, the frequency of biopsy, the accuracy of the clinical diagnosis and the histopathological features were studied in the biopsy group. Results: Between the two groups, we found that the clinical features, including the proportions between the involved body sites, such as trunk/face, and the treatment methods were different. However, these differences might be partly attributed to the clinical decisions such as the frequency of performing biopsy for making the differential diagnosis of skin malignancy. In both groups, the most frequent age period was in the seventh decade and the most common anatomical site was the face. Only one-third of patients who were clinically diagnosed as having seborrheic keratosis received biopsy for histopathologic confirmation and the lesions were mostly located on the face. The clinical diagnosis matched the histological diagnosis in 282/357(79.0%) patients. In 75 patients who revealed a mismatch between the clinical and histological diagnosis, the skin lesion most frequently involved the face(57.3%) and the most common final diagnosis was basal cell carcinoma (13.3%). The frequency of a mismatch for the sun-exposed lesion was significantly higher than that of the non-sun exposed lesion (p=0.035). Conclusion: We present the clinical and histopathological features of seborrheic keratosis. If the lesion is presented with an atypical appearance or it located on a sun-exposed area and so it requires a differential diagnosis from other premalignant diseases or cutaneous malignancies, then we should perform a biopsy to make the differential diagnosis.         사망진단서(사체검안서) 상의 선행사인으로부터 사망통계의 원사인이 선정되는 비율 : 3개 대학병원에서 교부된 사망진단서를 중심으로

        Background : To exatnine the problems intolved in writing practice of death certificates, we cotnpated the determination of underlying cause of death for wital statistics using recorded underlying cause of death in issued death statistics. Methods : We collected 688 rnortality certificates issue in year of 2,000 from 3 university hospitals. And we also collected vital statistics from ministry of statistics. The causes of death were coded by experienced medical record wpecialists. And causes of death determined at ministry of statistics for national vita statistics were mapped to causes of death recorded at each death certificates. The rate that underlying causes of death for vital statistics were derived from underlying causes of death recorded at issued death certificaties sere analysed. Results : 64.5% of underlying cause of death for could be derived from underlying cause of death recorded at issued death certificates, 8.6% derived from intermediate cause of death, and 3.9% derived from direct cause of death. In 23% of cases, underlying cause of death could not be derived using issued death certificates. The rate that underlying cause of death for vital statistics could be derived from underlying cause of death recorded at death certificates was different between 3 university hospitals. Ane the rate was also different between death certificates and postmortem certificates. We classified the causes of death using 21 major categories. The rate was different between diseases or conditions tha caused death too. Conclusion : When we examined the correctness of death certificate writing practice using abpve methods, cortectness of writing could not be told as satisfactory. There was difference in correctness of writing between hospotals, between death certificates and postmortem certificates, and between diseases and conditions that caused death. With this results, we suggested some strategy to improve the correctness of death certificate writing practice.

        구강 내 미생물에 대한 서양산 고추냉이(Horseradish, Armoracia rusticana) 뿌리 천연추출물과 합성 Allyl isothiocyanate의 항균활성 비교

        서양산 고추냉이(Horseradish, Armoracia rusticana)의 항균 성분 중 주 성분인 Allyl isothiocyanate(AIT)는 천연물을 이용한 추출이나 유기합성을 통해서 얻을 수 있으며, 이제까지 사용되고 있는 항균제품 및 식품첨가물들은 주로 유기합성법에 의해서 얻어졌다. 이전 연구에서 서양산 고추냉이 천연추출물의 다양한 구강 내 미생물에 대한 항균활성은 보고된 바 있으나, 유기합성한 AIT용액의 구강 내 미생물에 대한 항균활성은 아직까지 연구된 바가 없다. 이에 본 연구에서는 유기합성을 통해 얻은 AIT 용액과 서양산 고추냉이 뿌리 천연추출물의 구강 내 미생물에 대한 항균활성을 평가하였다. 항균활성을 평가하기 위해 최소억제농도와 최소살균농도를 측정하였으며, 그 결과는 다음과 같다. 1. 본 연구에 사용된 7종의 구강 내 미생물에 대한 서양산 고추냉이 뿌리 천연추출물의 최소억제농도는 약 117~1,750 ppm(0.0117~0.175%)이었으며, 유기합성한 AIT용액의 최소억제농도는 약 344~3,000 ppm(0.0344~0.3%)으로 모든 균주에 대해 항균활성을 나타내었다. 2. 본 연구에 사용된 7종의 구강 내 미생물에 대한 서양산 고추냉이 뿌리 천연추출물의 최소살균농도는 약 625.2~6,000 ppm(0.06252~0.6%)이었으며, 유기합성한 AIT용액의 최소살균농도는 약 1,750~7,000 ppm(0.175~0.7%)이었다. Allyl isothiocyanate(AIT), the principle ingredient of antimicrobial ingredients from horseradish root, can be prepared from extracts of horseradish root or synthetic method. It is reported that the horseradish root extract has the antimicrobial effect against various oral microorganisms, while there is no further study about the antimicrobial effect against the oral microorganisms of synthetic AIT derived from synthetic method. The aim of the study is to compare the difference of the antimicrobial effect between horseradish root extracts and synthetic AIT. To evaluate the antimicrobial effect, we measured the minimum inhibitory concentration(MIC) and minimum bactericidal concentration (MBC), and the results are like following. 1. The MIC of horseradish root extract against 7 kinds of oral pathogenic microorganisms is about 117~1,750 ppm(0.0117~0.175%), and the MIC of the synthetic AIT is about 344~3,000 ppm(0.0344~0.3%), which have the antimicrobial effects against all kinds of microorganisms. 2. The MBC of the horseradish root extracts against the 7 kinds of oral microorganisms is about 625.2~6,000 ppm(0.06252~0.6%), and the MBC of the synthetic AIT is about 1,750~7,000 ppm(0.175~0.7%), which have the antimicrobial effects against all kinds of microorganisms.

        Alexander Hamilton's personality appears from all sources of information to indicate a mixture of aggressive force and infinite tenderness and amiability. The former led infinite tenderness and amiability. The former led him always to speak mind freely perhaps too freely for his own comfort when he knew he was right, and when he had a wrong to master or disclose, or an end to accomplish. This Hamilton did with an unselfishness and absolute fixity of purpose, and he often wondered why others did not think and act as he did, the righteous necessities of the case seemingly being so apparent. The energy of his nature is often shown in his letters, some of which are full of resentful impatience. He never hesitated to assail the corrupt wherever they were to be found, to quickly fetter out abuses and to publicly expose them. For this reason he made numerous bitter enemies, who did not hesitate on repeated occasion to try to ruin him. In a way he was at times tactless, but it cannot be denied that he rarely erred in judgment. No man whose object is personal glory will sacrifice his popularity to his opinions, and this was Hamilton's constant habit. At no great crisis of his life do we ever find him engaged in considering whether a certain course of action will or will not conduce to his personal aggrandizement. He belonged to the class of men with whom the accomplishment of their object is their most powerful motive. In the pursuit of renown he hardly rose above the average of public characters, but his desire for achievement was a passion.

      • Hydrodynamic analogue를 이용한 Compartment model의 분포속도 변화에 따른 생체 내 약물동태의 Simulation 연구

        In vitro pharmacokinetics were simulated using hydrodynamic analogue of the compartment models. The oral model and the intravenous model were simulated for one and two compartment models each and the rate of distribution were controlled in the hydrodynamic models. The drug concentration of central compartment and of peripheral compartment were measured simultaneously followed by the differentiation of the distribution rates. The measured central and the peripheral drug concentration were fitted simultaneously by computer program 'Boomer', and showed very good correlation between the measured drug concentration and the predicted concentration in both oral and intravenous models with different distribution rates.

      • Streptomyces albulus 세정균체에 의한 polylysine의 대량생산

        Streptomyces albulus 세정균체를 이용하여 항세균물질인 폴리리신의 대량생산 가능성에 대하여 조사하였다. 발효기 배양에서 배양액 pH는 6.8에서 3.2로 감소하는 동안에 pH 4.0에서 4.5사이에서 폴리리신이 최대로 축적되므로 대수말기의 균체를 멸균증류수로 세척하여 마련한 세정균체를 아미노산 생산을 향상할수 있는 구연산을 2%첨가하고 pH를 최적인 4.2로 조정한 배지로 옮겨 배양하였다. 이러한 2단계공정으로 세정균체를 배양한 경우 배양액내의 폴리리신은 일반 1단계 단순 배양에 비하여 최대 6배의 축적량을 나타내었다. The feasibility of mass production of polylysine, an antibacterial substance, employing a washed mycelium of Streptomyces albulus has been investigated. Since the maximum accumulation of polylysine had appeared between 4.0 to 4.5 of pH during fermentor cultivation of which the pH of the broth has declined to 3.2 from 6.8, the strain growing in the broth on late log phase had been washed with sterilized water, then the sole mycelium had been transferred to another medium containing 2% citric acid having pH adjusted with NaOH to optimum of 4.2 to improve the production of the amino acid. The total accumulation of polylysine by washed mycelium of Streptomyces albulus from this two step procedure has exhibited 6 times more than the normal production of the simple one step fermentation.

      • 폐병변의 경피적 흡인생검술

        Over a period' of recent 2 years and 10 months, 47 patients who had localzied pulmonary lesions, but were not diagnosed pathologically through bronchoscopy, transbronchial biopsy, and sputum examination, underwent percutaneous aspiration biopsy using 22G Wescott needle under the fluoroscopy. The diagnostic accuracy was 85%. Malignant lesions were 32 cases, benign were 8 cases, and 7 cases were negative. Eleven of all 47 patients developed pneumothorax and one of them required treatment. Two of 11 pneumothorax patients also showed pleural effusion. Small amount of hemoptysis developed in 7 petients. The method, problems, and complications are discussed. Nowadays, percutaneus aspiration biopsy appears to be the procedure of choice in the pulmonary lesions, especially peripherally localized.

      • 열처리된 AgInS₂박막의 defect 연구

        AgInS₂단결정 박막은 수평 전기로에서 합성한 AgInS₂다결정을 증발원으로하여, hot wall epitaxy(HWE) 방법으로 증발원과 기판(반절연성-GaAs(100))의 온도를 각각 680℃, 410℃로 고정하여 단결정 박막을 성장하였다. 이때 단결정 박막의 결정성은 광발광 스펙트럼과 이중결정 X-선 요동곡선(DCRC)으로부터 구하였다. Hall 효과는 van der Pauw 방법에 의해 측정되었으며, 293K에서 운반자 농도와 이동도는 각각 9.35×10^(16)/㎤, 294 ㎠/V·s였다. 또한 광흡수 spectra에서 energy band gap의 온도 의존성 E_(g)(T)는 2.3165eV-(9.89×10_(-3)eV/K)T² / (T+2930K)임을 확인하였다. 성장된 AgInS₂단결정 박막을 Ag, S, In 분위기에 열처리하여 10K에서 photoluminescience(PL) spectra를 측정하여 점 결함의 기원을 알아보았다. PL 측정으로부터 얻어진 V_(Ag), Vs, Ag_(int), 그리고 S_(int)는 주개와 받개로 분류되어졌다. AgInS₂단결정 박막을 S분위기에서 열처리하면 p형으로 변환됨을 알 수 있었고, In 분위기에서 열처리하면 열처리 이전의 PL spectra를 보이고 있어서 I₂, I₁ 및 S, A emission에 의한 PL peak에는 영향을 주지 않는다고 보았다. A stoichiometric mixture of evaporating materials for AgInS₂single crystal thin films was prepared from horizontal furnace. To obtain the single crystal thin films, AgInS₂ mixed crystal was deposited on thoroughly etched semi-insulating GaAs(100) substrate by the Hot Wall Epitaxy(HWE) system. Temperature of the source and substrate were 680℃ and 410℃, respectively. The crystalline structure of the single crystal thin films was investigated by the photoluminescence and double crystal X-ray diffraction (DCXD). The carrier density and mobility of AgInS₂single crystal thin films measured from Hall effect by van der Pauw method are 9.35×10^(16)cm^(-3) and 294 cm²/V·s at 293 K, respectively. From the optical absorption measurement, the temperature dependence of the energy band gap bn-AgInSz single crystal thin films was found to be E_(g)(T) = 2.1365 eV - (9.89 x10^(-3) eV) T²/(2930 + T). After the as-grown AgInS₂single crystal thin films was annealed in Ag-, S-, and In-atmospheres, the origin of point defects of AgInS₂single crystal thin films has been investigated by using the photoluminescence(PL) at 10 K. The native defects of V_(Ag), Vs, Ag_(int), and S_(int) obtained from PL measurements were classified as a donors or acceptors type. And we concluded that the heat-treatment in the S-atmosphere converted AgInS₂single crystal thin films to an optical p-type. Also, we confirmed that In in AgInS₂/GaAs did not form the native defects because In in AgInS₂single crystal thin films did exist in the form of stable bonds.
