        Safety and efficacy of dietary supplement (gintonin-enriched fraction from ginseng) in subjective memory impairment: A randomized placebo-controlled trial

        Background: Gintonin inhibits β-amyloid production, increases acetylcholine level in the brain, and promotes neurogenesis. We evaluated the efficacy of gintonin-enriched fraction (GEF) in improving the cognitive performance in subjective memory impairment. Methods: In this 8-week, randomized, assessor and participant blinded, placebo–controlled study, participants with subjective memory impairment but preserved cognitive function (Korean Mini-Mental State Examination [K-MMSE] score ≥23) were assigned to GEF 300mg/day or placebo. K-MMSE, Korean versions of the Alzheimer's disease assessment scale, color-word stroop test (K-CWST), clinical dementia rating, and Beck depression inventory-II were evaluated along with the safety profiles. The primary outcome was set as the change in the K-MMSE. Results: Seventy-six participants complete the study protocol. After 8 weeks, there was no inter-group difference in the primary or secondary outcome score changes. However, GEF group showed an improvement in the K-MMSE scores (P= 0.026), and in the number of correct answers in both word reading (P= 0.008) and color reading (P= 0.005) of K-CWST, although only the improvement in the K-CWST scores were higher than the minimum clinically important difference. The frequency of adverse events was comparable between the groups and all were of mild severity. Conclusion: GEF is safe but might not be effective in treating subjective memory impairment within the current study setting. However, GEF showed a trend of improving the global cognition and the frontal executive function. Further large-sized studies with longer follow-up period are warranted. Clinical trial registration: This clinical trial was registered at Clinical Research Information Service of Korea Centers for Disease Control and Prevention: KCT0004636.         RCP 8.5 기후변화 시나리오에 따른 소나무림과 굴참나무림의 산림 탄소 동태 변화 추정 연구

        산림은 많은 양의 탄소를 저장하고 있으며, 산림 탄소 동태는 기후변화에 따라 변화할 것으로 예상된다. 본 연구는 우리나라 산림에서 가장 우점하는 침엽수종과 활엽수종인 소나무림과 참나무림을 대상으로 최근 개발 및 개선된 한국형산림토양탄소모델(Korean ForestSoil Carbon model; KFSC model)을 이용하여 두 가지 기후변화 시나리오(2012년 기온이 2100년까지 유지되는 시나리오(CT), Representative Concentration Pathway(RCP) 8.5 시나리오) 하에서의 산림 탄소 동태를 예측하였다. 5차 국가산림자원조사 자료로부터 소나무림과 굴참나무림 조사구들을 추출한 뒤, 이를 행정구역(9개 도, 7개 특별·광역시) 및 영급(1-5영급, 6영급이상)별로 분류하여 탄소 동태 모의 단위를 설정하였다. 탄소 저장고는 2012년을 기준으로 초기화하였으며, 모의 기간인 2012년부터 2100년까지 모든 교란은 고려하지 않았다. 모의 결과 산림 탄소 저장량은 시간이 경과함에 따라 전반적으로 증가하지만, CT 시나리오에 비하여 RCP 8.5 시나리오 하에서 산림 탄소 저장량이 낮게 나타났다. 소나무림의 탄소 저장량(Tg C)은 2012년에 260.4에서 2100년에는 각각 395.3(CT 시나리오) 및 384.1(RCP 8.5 시나리오)로 증가하였다. 굴 참나무림의 탄소 저장량(Tg C)은 2012년에 124.4에서 2100년에는 219.5(CT 시나리오) 및 204.7(RCP 8.5 시나리오)로 각각 증가하였다. 5차 국가산림자원조사 자료와 비교한 결과, 고사유기물 탄소 저장량의 초기값은 타당한 것으로 나타났다. 모의 기간 동안 소나무림과 굴참나무림의 연간 탄소 흡수율(g C m-2 yr-1)은 CT 시나리오 하에서 각각 71.1과 193.5, RCP8.5 시나리오 하에서 각각 65.8과 164.2로 추정된다. 따라서 우리나라 소나무림과 굴참나무림의 탄소 흡수잠재력은 지구 온난화에 의하여 감소할 것으로 예상된다. 비록 모델의 구조와 파라미터로부터 불확실성이 존재하지만 본 연구는 미래 산림 탄소 동태 파악에 기여할 것으로 기대된다. Forests contain a huge amount of carbon (C) and climate change could affect forest C dynamics. This study was conducted to predict the C dynamics of Pinus densiflora and Quercus variabilis forests, which are the most dominant needleleaf and broadleaf forests in Korea, using the Korean Forest Soil Carbon (KFSC) model under the two climate change scenarios (2012-2100; Constant Temperature (CT) scenario and Representative Concentration Pathway (RCP) 8.5 scenario). To construct simulation unit, the forest land areas for those two species in the 5th National Forest Inventory (NFI) data were sorted by administrative district and stand age class. The C pools were initialized at 2012, and any disturbance was not considered during the simulation period. Although the forest C stocks of two species generally increased over time, the forest C stocks under the RCP 8.5 scenario were less than those stocks under the CT scenario. The C stocks of P. densiflora forests increased from 260.4 Tg C in 2012 to 395.3 (CT scenario) or 384.1 Tg C (RCP 8.5 scenario) in 2100. For Q. variabilis forests, the C stocks increased from 124.4 Tg C in 2012 to 219.5 (CT scenario) or 204.7 (RCP 8.5 scenario) Tg C in 2100. Compared to 5th NFI data, the initial value of C stocks in dead organic matter C pools seemed valid. Accordingly, the annual C sequestration rates of the two species over the simulation period under the RCP 8.5 scenario (65.8 and 164.2 g C m-2 yr-1 for P. densiflora and Q. variabilis) were lower than those values under the CT scenario (71.1 and 193.5 g C m-2 yr-1 for P. densiflora and Q. variabilis). We concluded that the C sequestration potential of P. densiflora and Q. variabilis forests could be decreased by climate change. Although there were uncertainties from parameters and model structure, this study could contribute to elucidating the C dynamics of South Korean forests in future.

        Background: Smoking is known to increase the risk of chronic disease. Understanding of factors that contribute to smoking cessation may help to develop strategies for smoking behavior change. Methods: Pooled cross-sectional data of 11,924 subjects from the Korea National Health and Nutrition Examination Survey II-V were analyzed. The stages of change in smoking cessation were categorized into pre-contemplation, contemplation, and preparation. Baseline characteristics, socioeconomic factors, quality of life, psychological characteristics, and tobacco-related factors were compared between the groups. Results: The study population consisted of 32.4% in pre-contemplation, 54.4% in contemplation, and 13.1% in preparation group. While the proportion for pre-contemplation group (37.4% to 28.4%) tended to decrease from 2001 to 2012, that of preparation group (6.4% to 18.1%) tended to increase. The mean age of pre-contemplation group (48.4±16.2) is higher than comtemplation and preparation group (42.2±13.9 and 44.4±15.3; P=0.02). As levels of education and income status increased, the proportion of pre-contemplation tended to decrease, and that of preparation tended to increase. Proportion of blue collar job in pre-contemplation group was higher than in preparation group (67.9% vs 58.1%; P<0.001). In pre-contemplation group, percentage of suicidal ideation within 1 year was higher than contemplation and preparation group (19.3% vs 16.0% and 16.1%). Upon adjusting analysis, the proportion of pre-contemplation group increased with increasing age (OR=1.01; 95%CI 1.01-1.02), and smoking amount (OR=1.04; 95%CI 1.03-1.05). People of pre-contemplation group (vs. preparation) are be less educated (OR=1.99; 95% CI 1.51-2.62), are in single marital status (OR=1.25; 95%CI 1.04-1.50), have lower prevalence of cardiovascular disease (OR=0.48; 95%CI 0.31-0.76), and do less exercise walking (OR=1.36; 95%CI 1.09-1.69) and weight training (OR=1.59; 95%CI 1.35-1.85). Conclusions: We demonstrated a significant association between the stages of change in smoking cessation and age, education, marital status, cardiovascular disease, and amount of exercise.

        Surgical Causes of Significant Intraoperative Neuromonitoring Signal Changes in Three-Column Spinal Surgery

        Study Design: Retrospective case series. Purpose: To evaluate the risks and causes of neurologic complications in three-column spinal surgery by analyzing intraoperative neurophysiological monitoring (IONM) data. Overview of Literature: Three-column spinal surgery, which may be required to correct complex spinal deformities or resection of spinal tumors, is known to carry a high risk of neurologic complications. However, few studies reported a specific surgical procedure related to a significant IONM signal change during surgery. Methods: Multimodality IONM data, including somatosensory-evoked potentials (SSEP) and motor-evoked potentials (MEP), were reviewed in 64 patients who underwent three-column spinal surgery from 2011 to 2015. Surgical procedures included posterior vertebral column resection, pedicle subtraction osteotomy, total en bloc spondylectomy, piecemeal spondylectomy, and corpectomy with laminectomy (n=27) in three cervical, 34 thoracic, and 31 lumbar procedures. Results: Significant IONM signal changes occurred in 11 of 64 (17.1%) patients. SSEP and MEP were changed in 11 patients. Postoperative neurologic deterioration occurred in 54.5% (6 of 11) of the patients, and two of them were permanent. There was no postoperative neurologic deterioration in patients without significant signal change. Suspected causes of IONM data changes are as follows: adhesion/tethering, translation, contusion, and perfusion. Conclusions: Based on the results of this study, to enhance neurologic safety in three-column spinal surgery, surgeons should pay attention to protect the spinal cord from mechanical insult, especially when the spinal column was totally destabilized during surgery, and not to compromise perfusion to the spinal cord in close cooperation with a neurologist and anesthesiologist.

        A Comparative Proteomics Survey of Proteins Responsive to Phosphorous Starvation in Roots of Hydroponically-grown Rice Seedlings

        Rice takes up phosphorous (P) as major nutrient source for its growth and development when grown under anaerobic water-logged soil conditions. To better understand the underlying mechanisms and to develop potential protein biomarkers of P-starvation, hydroponically-grown rice seedlings in the complete media and phosphorus absence (P-starvation) of phosphorous nutrient solutions were investigated for physiological and proteome changes. The P-starvation manifested significant reduction in root growth in three-week-old seedlings compared to respective complete media. Furthermore, P-starvation also showed increased activity of acid phosphatase in roots of one- and three-week-old seedlings, suggesting that experimental design is suitable for proteomics survey of P-starvation responsive proteins. Two-dimensional gel electrophoresis coupled with matrix-assisted laser desorption/ionization-time of flight (MALDI-TOF) mass spectrometry analysis of total root protein from three-week-old seedlings identified 10 P-starvation responsive protein spots out of 140 high-quality protein spots. Identified 10 proteins were involved in metabolism and defense/stress response. Out of 10, 2 and 8 protein spots were found to be up- and down-regulated, respectively. Semi-quantitative reverse transcription polymerase chain reaction (RT-PCR) analysis of corresponding genes of four randomly selected proteins, including putative glyceraldehydes-3-phophate dehydrogenase (G3PDH, spot R1), S-adenosyl-L-methionine synthetase (SAMS, spot R4), ATP synthase subunit alpha (spot R6), and root-specific pathogenesis-related protein 10 (PR-10, spot R8), showed that just as protein abundance, these proteins are also regulated at the transcript level. Results suggest identified P-starvation responsive proteins are involved in maintaining nutrient homeostasis and/or associated with changes in root physiology under the absence of P.

      • P2P 캐싱을 기반으로 한 효율적인 브로드캐스트 스트리밍 기법

        Video-on-Demand is undoubtedly a promising technology for many important applications. Several periodic broadcast techniques have been proposed for the cost-effective implementation of such systems. However, there is a server delay for all of them. To address the shortcoming, we present a new scheme improving the staggered broadcasting by jointly using prefix caching scheme of VOD system in P2P environments. In thins paper, each client plays a role in the first segment staggered broadcasting server to serve the prefix stream to other clients that request the same video stream. The buffer can be empty after the client has served the following client. As a result, we are able to achieve no service latency time and the low buffer space.

      • 효율적인 모바일 서비스를 위한 하이브리드 스트리밍 아키텍처

        To provide the seamless multimedia streaming service for mobile users, both performance of the mobile device and bandwidth constraint of the wireless network should be considered. Recently, in order to solve these problems, the QoS adaptive architectures as well as additional features are being devised. In this paper, we designed and implemented a hybrid architecture-based mobile media streaming system under the Linux environment. The implemented system features as follows. First, a hybrid architecture which complements shortcomings of the static and dynamic architectures is presented. The hybrid architecture is able to maximize the proxy utilization rate by dividing the proxy into the related functions according to computation and disk access rate. Second, the new resource monitoring scheme that efficiently manages resource of the proxy is proposed. Third, the work prediction module is added to choose exactly the proxy which finish the task earlier than others. The experimental results demonstrate that our system decreases not only the user-perceived latency, but also the proxy overload. Furthermore, it has an advantage of reducing the traffic between the proxy and the system manager.

      • PC 클러스터를 이용한 수치최적의설계의 병렬처리

        3차원 천음속 날개의 공력형상 설계최적화를 위하여 저렴한 비용의 PC 클러스터를 제작하였다. 클러스터는 3개의 노드로 구성되었으며, 리눅스 0S를 설치하고 메시지 전달방식의 통신 라이브러리인 MPI(Message Passing Interface)를 사용하였다. 이 시스템을 이용하여 24개의 설계변수를 가진 3차원 천음속 날개의 항력최소화 문제를 해석하였다. 유동장 해석을 위해 오일러 코드를 사용하였고, 일반적인 영역분할방식의 유동장 병렬처리를 택하는 대신에 수치최적화 알고리즘 중 민감도를 계산하는 부분을 병렬처리 하였다. 설계결과는 단일 노드를 사용한 순차계산의 결과와 동일하였으나, 설계 소요시간은 큰 폭으로 감소하였다. 본 연구의 결과로 저렴한 비용으로 우수한 성능을 갖춘 3차원 항공기 날개의 형상최 적설계 시스템을 구성할 수 있었으며, 향 후 이를 이용하여 보다 복잡하고 정교한 전산원용설계시스템으로 발전할 수 있는 기초를 마련하였다. A cheap FC-Cluster was assembled for the aerodynamic design optimization of three dimensional transonic wing. The cluster has 3 nodes in which Linux Operating System and MPI as message passing library are installed. The system was applied to solve a wave drag minimization problem for transonic wing with 24 design variables. The Euler method was used to solve the flow around the wing, The process of sensitivity-derivative computation was parallelized in the numerical optimization algorithm, instead of using domain decomposition technique as usual parallel processing in the computational fluid dynamics. The design result with the parallel processing is exactly same as the one with the serial computation, while the parallel computation time was considerably reduced. The cheap shape optimization system based on the PC-cluster developed here could be a starting of the computer-aided design system with more sophisticated geometry generating module and flow analysis methods in the future.
