Numerous evidences support the pathogenesis that M. pneumonias pneumonia is associated with cell-mediat-ed immune reaction. We report 2 cases of severe M. pneumoniae pneumonia in previously healthy sisters, who were both admitted during an epidemic of M. pneumoniae pneumonia. The elder sister, who was 16 years old, was admitted with M. pneumoniae pneumonia. She showed no response to treatment with clarithroycin and levo-floxacin, and eventually progressed to severe ARDS, requiring mechanical ventilation. After treatment with hydrocortisone (200 mg/day), there were rapid improvements in clinical manifestations and chest radiographic findings. The younger sister, who was 14 years old, was admitted 10 days later, Presenting with fever but no pneumonic lesions on chest radiograph. Just like her sister, the infection showed no response to clarithromycin. Fever persisted and pneumonic consolidation with mild pleural effusion was noticed in the left lower lobe on the 5th hospital-day. After treatment with oral prednisolone (1 mg/kg), she showed rapid defervescence and on the 8th hospital day, no pneumonic lesions were detectable on chest radiograph. Given the fact that the pathogenesis of pneumonia and extrapulmonary manifestations in M. pneumoniae infection is immune-mediated, an immuno-suppressive therapy would be validated for selected patients with M. pneumoniae infections.

<P><B>Abstract</B></P> <P>Potent anti-osteoporotic drugs have been developed over the past decades; however, the substantial need for treatments that can effectively and safely manage osteoporosis remains unmet. Barley leaf-derived products are one of best functional foods that can be used as nutritional supplements and detoxifiers in humans and are beneficial in improving bone disease. However, little information is available regarding the anti-osteoporotic effects of polysaccharides as the main component of barley leaf. This study aimed to clarify the beneficial effects of barley leaf (BLE0) polysaccharides on bone loss in ovariectomized mice and osteoclast differentiation in bone marrow-derived macrophages. BLE0 remarkably inhibited receptor activator of the nuclear factor-κB ligand (RANKL)-induced osteoclast differentiation in a dose-dependent manner. It also blocked RANKL-induced activation of osteoclastogenic signals including ERK and p38 and the expression of nuclear factor of activated T cells cytoplasmic 1, as a master regulator of osteoclast differentiation, leading to decreased expression of osteoclast-specific marker genes such as Atp6v0d2, DC-STAMP and cathepsin K. Micro-computed tomography revealed that a seven-week oral administration of BLE0 dramatically improved ovariectomy-induced trabecular bone loss. Anti-osteoporotic effects were confirmed using morphometric analysis. Taken together, BLE0 is a bioactive polysaccharide; it mitigates estrogen deficiency-induced bone loss by directly inhibiting osteoclast differentiation.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The crude polysaccharide BLE0 was isolated from barley leaf (<I>Hordeum vulgare</I> L.). </LI> <LI> We performed the first in vivo evaluation of the antiosteoporosis effect of BLE0. </LI> <LI> BLE0 protected ovariectomized (OVX) mice from trabecular microarchitecture deterioration. </LI> <LI> BLE0 suppressed osteoclast differentiation by inhibiting the activation of MAP kinases/NF-κB pathways of osteoclastogenesis. </LI> </UL> </P>
Signal Compensation for Analog Rotor Position Errors due to Nonideal Sinusoidal Encoder Signals

This paper proposes a compensation algorithm for the analog rotor position errors caused by nonideal sinusoidal encoder output signals including offset and gain errors. In order to achieve a much higher resolution, position sensors such as resolvers or incremental encoders can be replaced by sinusoidal encoders. In practice, however, the periodic ripples related to the analog rotor position are generated by the offset and gain errors between the sine and cosine output signals of sinusoidal encoders. In this paper, the effects of offset and gain errors are easily analyzed by applying the concept of a rotating coordinate system based on the dq transformation method. The synchronous d-axis signal component is used directly to detect the amplitude of the offset and gain errors for the proposed compensator. As a result, the offset and gain errors can be well corrected by three integrators located on the synchronous d-axis component. In addition, the proposed algorithm does not require any additional hardware and can be easily implemented by a simple integral operation. The effectiveness of the proposed algorithm is verified through several experimental results.

Background/Aims: In the Republic of Korea, an estimated 231,000 individuals have chronic hepatitis C virus (HCV) infection. The aim of the present analysis was to evaluate the safety and efficacy of elbasvir/grazoprevir (EBR/GZR) administered for 12 weeks in Korean patients who were enrolled in international clinical trial phase 3 studies. Methods: This was a retrospective, integrated analysis of data from patients with HCV genotype (GT) 1b infection enrolled at Korean study sites in four EBR/GZR phase 3 clinical trials. Patients were treatment-naive or had previously failed interferon-based HCV therapy, and included those with human immunodeficiency virus coinfection or Child- Pugh class A cirrhosis. All patients received EBR 50 mg/GZR 100 mg once daily for 12 weeks. The primary endpoint was sustained virologic response at 12 weeks after completion of therapy (SVR12, HCV RNA <15 IU/mL). Results: SVR12 was achieved by 73 of 74 (98.6%) patients. No patients had virologic failure and one discontinued from the study after withdrawing consent. SVR12 rates were uniformly high across all patient subgroups. A total of 16 patients had nonstructural protein 5A resistance-associated substitutions at baseline (16/73, 22%), all of whom achieved SVR12. Adverse events (AEs) reported in >5% of patients were fatigue (6.8%), upper respiratory tract infection (5.4%), headache (5.4%), and nausea (5.4%). Thirteen patients (17.6%) reported drug-related AEs, two serious AEs occurred, and two patients discontinued treatment owing to an AEs. Conclusions: In this retrospective analysis, EBR/GZR administered for 12 weeks was well-tolerated and highly effective in Korean patients with HCV GT1b infection.
Nodular Fasciitis of the Parotid Gland, Masquerading as Pleomorphic Adenoma

<P>It is difficult to distinguish nodular fasciitis (NF) from other neoplasm of the parotid gland, especially pleomorphic adenoma (PA) by fine needle aspiration cytology. A 39-year-old female noticed a mass in the parotid region. The aspirate material showed cohesive parts composed of the cells that had oval or spindle-shaped nuclei and relatively abundant cytoplasm and some cells with plasmacytoid features. The background substance was fibromyxoid. PA was diagnosed based on the cytologic findings. Subsequently, parotidectomy was performed and NF was diagnosed based on histologic and immunohistochemical findings. NF in the parotid region is rare and may be misdiagnosed as other benign or malignant tumors of the parotid gland. The clinical history of rapid growth and the presence of mitoses and inflammatory cells help to distinguish NF from PA. In addition, immunohistochemical stains for smooth muscle actin and CD68 are useful to confirm the diagnosis of NF.</P>

외안 이중검을 지닌 한국인에 있어 반대측 일중검을 비절개 봉합법을 이용하여 교정하므로써 보다 자연스럽고 대칭적인 이중검을 얻을 수 있었다. Double-eyelid operation is one of the most common cosmetic surgical procedures performed in the Orient. There have been numerous studies about the double-eyelid operation. The Non-incision technique is the procedure of choice in appropriately choosen patients who don't have excessive amounts of subcutaneous fat and who have no redundant upper lid skin and when patient do not desire a wide double eyelid line and scars. From January, 1990 to December, 1994, We have experienced 48 cases of correction of the unilateral double-eyelid patients with Single-stitch method or Continuous buried suture method. We have found that our method allows the construction of a more even, smooth and symmetrical fold. We present that correction of a unilateral double fold can be a indication of non-incisional suture method, too.
