        전자부품제조업 근로자의 근골격계 자각증상과 관련요인

        The purpose of this study was to identify the observable symptoms of musculoskeletal disease from electronic components manufacturing workers who involved in many repetitive tasks and to provide the basic data for the prevention and management. The survey was conducted on 721 people from 15 April, 2013 to 17 May, 2013 by self­recording type. The results of the study are as follows. First, symptom complaints based on different body parts are in following order, waist was 12.9%, shoulder was 10.5%, neck was 7.4%, hand/wrist/finger was 7.4%, leg/foot was 4.4%, arm/elbow was 2.8%, and 21.9% of the respondents showed symptoms in more than one body part. Sex, age, marital status, work experience, work intensity, and past accident experience were statistically significant. Second, in the job stress evaluation, all male and female workers were below the median of Korean workers in all of 8 categories. the higher the scores for lack of job autonomy, the higher the symptom complaints of musculoskeletal disease. In the case of patients complain observable symptoms of musculorskeletal disease, they should receive proper outpatient treatment, various programs such as stretching by body parts, setting up a desirable working posture, switching to cyclical work, should b developed as much as possible.

        소아 IgA 신병증의 예후에 관한 고찰

        목적 : IgA 신병증은 다양한 임상 소견을 보일 수 있으며 초기에는 대부분 예후가 양호한 것으로 알려졌었으나 현재는 발병 20년후 20-30%가 말기 신부전증을 초래한다고 알려져 있다. IgA 신병증의 예후를 예측하기 위한 여러 조사에서 발병 초기의 심한 단백뇨, 고혈압, 조직 병리학 상의 심한 변화 등이 있을 경우 예후가 나쁘다고 하였다. 1996년 Yagame등은 IgA 신병증에서 새로운 조직 병리학적 분류를 하였고 WHO 분류 방법이 사구체 병변을 중심으로 한데 비해 세뇨관과 간질의 변화까지 포함하고 있어 이들의 분류 방법이 더 우수하며 추적관찰 결과 만성 신부전의 예후 인자로써 유용하다고 발표하였다. IgA 신병증에서 임상 증세, 검사 소견, WHO 및 Yagame 등의 조직 병리학적 분류 등과 추적 관찰시 만성 신부전과의 상관 관계를 알아보고자 하였다. 방법 : 1984년 1월부터 1996년 2월까지 소아과에 입원하여 신생검상 메산지움에 IgA가 현저히 침착되어 있어 IgA 신병증으로 진단된 환아 79명을 대상으로 평균 27개월의 추적관찰시 신 기능이 정상인군(73례)과 만성 신부전으로 진행한 군(6례)을 서로 비교하여 IgA 신병증에서의 예후 예측 인자에 대하여 알아보고자 하였다. 결과 : 1) 성별, 나이, 증상기간 등은 양군간 차이가 없었으나 내원시 고혈압이 있을 경우 만성 신부전으로의 진행이 많았다. 2) 내원시에 여러 임상 검사중 심한 단백뇨가 만성 신부전으로의 진행이 많았다. 3) WHO 및 Yagame 등의 조직 병리학적 분류 모두 소아 환자에서는 예후와의 관련을 찾을 수 없었다. 결론 : 이상의 결과로 보아 IgA 신병증 환아에서 내원시에 심한 단백뇨, 고혈압이 있을 경우 추적 관찰 결과 예후가 좋지 않았다. 그러나 본 연구의 경우 치료에 대한 평가가 이루어지지 않았고 대상 환아가 적었기에 향후 이를 포함하는 광범위한 연구가 진행되어야 할 것으로 사료된다. After the first description of IgA nephropathy by Berger in 1968, the prognosis of this disease was considered favourable. However recent studies have revealed that IgA nephropathy result in end stage renal desease in 25-30% by 20 years. Heavy proteinuria, hypertension, histological high class are regarded as poor prognostic factors. In 1996, Yagame et al reported the new histopathologic grading with a strong correlation between the grading, heavy proteinuria, high s-Cr level and renal survival. The aims of this study are to determine whether the pathological grading and other clinical parameters could contribute to predicting the outcome of this disease eventhough pediatric patients. Seventy nine patients (59 males, 20 females) with IgA nephropathy were examined. Patients were 2.08-15.17 years of age ($9.85{\pm}2.83$). The mean follow-up duration were $27{\pm}28$ months. Six of seventy nine patients progressed to chronic renal failure during the follow-up periods. High 24h urinary protein excretion at diagnosis were significantly higher in chronic renal failure patients (p<0.05). Hypertension at diagnosis were the significant associated factors in progression of chronic renal failure (p<0.05). Histological changes of IgA nephropathy in light microscopy were classified into five classes by WHO classification, four grades in Yagame's gradings. Among the seventy nine patients, 24 were as class 1, 30 as class 2, 23 as class 3; 4 as class 4, 0 as class 5 by WHO classification. 23 were classified grade 1, 31 as grade 2, 24 as grade 3, 1 as grade 4 by Yagame's grading. Among six patients who progressed to chronic renal failure, 1 clssified as class 1, 1 as class 2, 3 as class 3, 1 as class 4, 0 as class 5 by WHO Classification. 1 patients were classified as grade 1, 1 as grade 2, 3 as grade 3, 1 as grade 4 by Yagame's grading. (p>0.05) In conclusion, hypertension and heavy proteinuria at initial presentation were significantly associated with progression of chronic renal failure. The classification of WHO & Yagame's grading has no significant association with the progression of chronic renal failure in pediatric patients.

        Behcet 증후군의 분류아형에 따른 체액면역에 관한 고찰

        Various immunologic studies were performed for the investigation of humoral immunity in 30 patients with Behcet's syndrome who had been registered in Behcet Special Clinic of Severance Hospital: direct immunofluorescent staining, quantitation of serurn Ig, complement by immunoelectrophoresis and B-cell by EAC-rosette method. The results can be summerized as follows: 1. Direct immunofluorescent staining occurs in biopsies from patients with Behcets syndrome: 11 of the 30 patients(36. 7%). Among them, vascular fluorescene with C3 was noted in 10 of 11 patients (90. 9%), in addition of IgG, IgA, IgM, and fibrinogen on dermoepidermal junction or vessel. 2. The results of direct immunofluorescence staining showed different tendency according to biopsy sites and clinical types: 5 of 10(50%,) in the specimens from oral mucosa, 5 of 13(38.5%) from leg, 1 of 3(33.3%,) from genitalia, and 0 of 7(0%) from other sites: higher in complete and incomplete types than in suspected and possible types. 3. Immunofluorescent staining rates were statistically not significant according to sex, age and Lehner's classification. 4. Serum IgG, IgA, IgM, C3, C4 and B-cell were statistically not significant between DIF positive and negative groups. Therefore it is considered that vascular deposits of immune complexes and activation of complements by immune omplexes may be a role of pathogenetic mechanisms of Behcet's syndrorne.

        악성 횡문근 육종의 방사선 치료

        Rhabdomyosarcoma is a highly malignant soft tissue sarcoma that can arise in any site of the body containing striated muscle of its mesenchymal analgae. It is the most common childhood sarcoma with two peak age frequencies, one at ages 2 to 6, and one in the adolescence. The site, stage and extent of disease, and pathologic characteristics of the tumor contribute to prognostic factors that influence therapeutic decisions. The results of treatment of 52 patients with rdomyosarcoma, who were treated at Department of Radiation Oncology, Yonsei University College of Medicine, Yosei Cancer Center from 1976 to 1987 were retrospectively analyzed. The most frequent clinical group and primary sites were IRS group III(57.7%) and head and neck (42.3%) including orbit (11.5%) and parameningeal region (13.5%). The overall and disease free 5 year survival rate of eligible 41 patients were 31.7%, 29.3%, respectively. The complete remission (CR) rate was 50% in clinical group III and 0% in IV. Primary tumors of the orbit, clinical group I and embryonal subtype had the best prognosis. The survival rate was improved by addition of chemotherapy to operation and radiation therapy.

        면역세포화학 염색법과 전자 현미경검사를 통한 뇌하수체 선종의 특징

        N/A Background: The development of immunocytochemistry and the application of electron microscopy improved the knowledge of the pathology and classification of the pitutiary adenomas. We investigated the characteristics of the pituitary adenomas examined by the immunocytochemistry and electron microscopy. Methods: 44 patients with the pituitary adenomas were investigated, using immunocytochemical techniques in paraffin-embedded tissue, and electron microscopy in fresh tissue. Results: The mean age was 38±13 years, and the ratio of male to female was 1: 2.4 and 18 were prolactinomas, 8 acromegalies, 3 acromegalies with hyperprolactinemia, 3 Cushing`s diaseases, 3 FSH secreting adenomas, 1 TSH secreting adenomas, and 6 nonfunctioning adenomas among 44 patients. The agreement between serum prolactin and prolactin immunocytochemistry was 77.3% if the serum prolaetin level to predict lactotrope adenoma was above 50ng/ml; however, the agreement was 93,2% if the serum prolatin level above 100ng/ml. The serum levels of prolactin and growth hormone in sparsely granulated lactotrope and somatotrope adenomas were 1149.6, 109.5ng/ml, respectively, which were higher than those (129.6, 35.9ng/ml) of densely granulated ones. Immunocytochemical and electron microscopic analysis revealed that thirteen were lactotrope adenomas (12 sparsely granulated, and 1 densely granulated), 9 somatotrope adenomas (6 sparsely granulated and 3 densely granulated), 6 gonadotrope, 4 corticotrope (3 symtomatic and 1 silent), 1 mixed somatotrope and lactotrope, 1 mammosomatotrope, 1 thyrotrope, and 9 null cell (6 nononcocytic, and 3 oncocytic) adenomas. Conclusion: The immunocytochemistry, permitting the visualization of intracellular hormones in the tissue of the pituitary adenoma, made exact histologic diagnosis. So, we have known whether the cause of hyperprolactinemia was lactotrope adenoma or functional hyperprolactinemia due to PIF inhibiton. And it was also possible to diagnose silent corticotrope adenoma and gonadotrope adenoma which had no apparent clinical manifestation. Electron microscopic examination of the pituitary adenoma diagnosed by immunocytchemistry showed ultrastructrural characteristics of the adenoma cells containing each hormones, Accordingly, we could subclassify the pituitary adenoma. However, the identification of the adenomas with somewhat similar ultrastrutural features such as null cell, gonadotrope, and thyrotrope adenoma, may be facilitated by immunocytochemistry. Accordingly, both immunocytochemistry and electron microscopy are necessary for the identification and characterization of the pituitary adenomas.

      • Co-Cr-Mo 합금의 선삭 가공 특성에 관한 연구

        In this study, researches were conducted as follows. First, as the basic experiment, the cutting speed, feedrate, and the depth of cut were set as the process parameters, and by setting the surface roughness as the factor of measurement for each of the combinations, and the analysis about cutting tendency of the material was conducted by proceeding the turning process of Co-Cr-Mo alloy. Second, by setting the feature of the surface roughness according to the 'turning processing condition' that was confirmed in the previous experiment, and by applying the Taguchi Method, the conditions that influence the features of the surface roughness according to the 'turning processing condition' of Co-Cr-Mo was analyzed, and also by measuring the surface roughness according to each of the 'cutting conditions', the optimal processing condition was generated. As the result of analysis, it was possible to understand that the factor that mostly affects the surface roughness was the cutting speed, followed by the dept of cutting and transfer speed, and as for the optimal processing condition, it was possible to find that the cutting speed was 5,000rpm, and the depth of cut was 0.1mm, and the feedrate was 0.003mm/rev, and the value of the surface roughness at this point is $0.197{\mu}m$.

        막성증식성 사구체신염 제 II형 (Dense Deposit Disease, DDD) 1례

        제2형 막증식성 사구체 신염은 전자현미경 검사 상 기저막 내에 리본과 같은 형태의 전자고밀도물질이 참착되어 지속적인 보체의 감소와 혈뇨, 단백뇨등을 나타내는 원발성 사구체 질환이다. 저자들은 내원 4년 전에 신장조직검사상 연쇄상구균 감염 후 급성사구체 신염을 진단 받은 후 4년 이상 지속되는 혈뇨와 보체의 감소로 재차 시행한 신장조직검사상 확진된 제 2형 막증식성 사구체신염 1례를 경험하였기에 국내외 문헌 고찰과 함께 보고하는 바이다. Type II membranoproliferative glomerulonephritis (Dense deposit disease) is an acquired primary glomerular disease characterized by electron microscopic evidence of a continuous dense membrane deposition replacing the lamina densa. It is a subtype of idiopathic membra- noproliferative glomerulonephritis, and was described as a separate entity by Berger and Galle in 1963. It frequently occurs in older chilren and young adults and the clinical course is variable, but is generally progressive. The presenting feature is nephrotic syndrome in many patients, and proteinuria and hematuria are also seen frequently. The purpose of this paper is to present a case of DDD (Dense deposit disease) from a 10 year old boy who was diagnosed as a acute poststreptococcal glomurulonephritis with protenuria, hematuria, and facial edema by renal biopsy 4 years ago.         OECD 주요 국가에서는 과학교육을 위하여 오래 전부터 마이크로컴퓨터 기반 실험교육을 실시해 왔다. 우리나라에서도 2005년부터 현재에 이르기까지 이를 시행해 왔는데, 일부 과학고등학교에서만 실시되고 있다. 일반 고등학교에서 실시되지 못하는 이유는, 마이크로컴퓨터 기반 실험교육이 갖는 비용이 크고, 일반 고등학교에서 교육 장비를 구비하기 어려운데다가 관련 전문지식을 갖춘 교육자들이 부족하기 때문이다. 본 연구에서는 아두이노(Arduino)를 이용하여 일선 학교에서 쉽게 접근할 수 있도록 했다. 아두이노와 연결되어 사용하는 센서들도 교육 목표에 도달하기에 부족함이 없었다. 학습자들이 직접 아두이노와 센서를 결합하여 회로를 구성할 수 있었으며, 아두이노를 제어할 수 있는 C언어 기반의 PROCESSING 프로그래밍 도구를 이용하여 결과를 얻을 수 있는 프로그래밍을 하였고, 그 결과를 그래프로 가공하여 보았다. 교사의 도움 없이도 쉽게 접근하여 문제를 처리할 수 있었다. 이와 같은 아두이노 기반의 피지컬 컴퓨팅의 효과를 알아보기 위하여, 본 연구는 아두이노를 이용하여 구성된 온도 및 빛의 양을 측정하는 모듈에 대한 성능을 평가할 수 있는 실험을 함으로써, 피지컬 컴퓨팅이 갖는 효과를 알아보았다.
