      • 호흡동조방사선치료 시 Real Time Monitor와 Ventilator의 유용성 평가

        목 적: 호흡동조방사선치료 시 호흡에 따른 종양의 위치를 정확하게 파악하여 치료하기 위해서는 안정되고 일정한 호흡유지가 중요하다. 이에 본 연구에서 Real time monitor와 Ventilator를 이용하여 측정한 호흡유지방법을 소개하고 그 유용성을 평가하고자 한다. 대상 및 방법: 7명의 실험자를 대상으로 하여 Gate on time (Beam on time) 600초를 기준으로 실험자를 총 5개의 그룹 -자연 호흡, Monitor를 이용하여 유도한 호흡, Ventilator를 이용하여 유도한 호흡, Monitor와 Ventilator를 동시에 이용하여 유도한 호흡, 실험자 스스로 Real time monitor를 이용하여 주기적 Breath-hold 상태를 유지하는 호흡- 으로 분류 하였다. 분류 후 ANZAI 4D system을 이용하여 각각의 호흡신호를 획득 하였다. 호흡신호별 총 소요 시간을 비교하고 호흡주기 상의 Gate on/off time 평균값, 표준편차, 분산, 평균 호흡 주기의 횟수와 변화 등을 각각 분석하여 호흡의 안정성을 평가 하였다. 동시에 실험자들을 대상으로 설문조사를 실시하여 각 장치들의 의존성을 정성적으로 조사하였다. 결 과: 5개 그룹의 호흡신호 편차변화를 비교하여 실험자의 각 호흡신호 안정성을 평가하였다. 그 결과 모든 실험자는 Gate on time 600초 동안 소요되는 시간이 Monitor와 Ventilator를 동시에 사용 했을 경우에 실험시간이 가장 짧게 걸렸고, 가장 안정된 호흡을 유지한 것으로 나타났다. 결 론: 본 연구에서 Real time monitor와 Ventilator를 사용하여 호흡유도를 진행해본 결과 실험대상자들의 안정적인 호흡유지에 유용성이 있는 것으로 나타났다. 2개의 장비를 독립적으로 사용하였을 경우 보다 동시에 사용할 경우 호흡유지 안정도가 더 우수한 것으로 나타났다. Real time monitor와 Ventilator를 이용하여 환자의 호흡조건을 설정하여 환자에게 적용시킨다면 호흡조절방사선치료 시 치료의 정확도를 높일 수 있으리라 예상되며 동시에 치료시간도 단축될 것으로 사료된다. 특히 규칙적인 호흡조절이 가능한 환자의 경우 Real time monitor를 활용한 Breath-hold 방법의 활용은 치료시간 단축의 효과를 현저히 높일 수 있을 것이라 사료된다. Purpose: The purpose of this study was to evaluate the possibility to optimize the gated treatment delivery time and maintenance of stable respiratory by the introduction of breath with the assistance of auditory-visual-tactile sense. Materials and Methods: The experimenter's respiration were measured by ANZAI 4D system. We obtained natural breathing signal, monitor-induced breathing signal, monitor & ventilator-induced breathing signal, and breath-hold signal using real time monitor during 10 minutes beam-on-time. In order to check the stability of respiratory signals distributed in each group were compared with means, standard deviation, variation value, beam_time of the respiratory signal. Results: The stability of each respiratory was measured in consideration of deviation change studied in each respiratory time lapse. As a result of an analysis of respiratory signal, all experimenters has showed that breathing signal used both Real time monitor and Ventilator was the most stable and shortest time. Conclusion: In this study, it was evaluated that respiratory gated radiation therapy with auditory-visual-tactual sense and without auditory-visual-tactual sense feedback. The study showed that respiratory gated radiation therapy delivery time could significantly be improved by the application of video feedback when this is combined with audio-tactual sense assistance. This delivery technique did prove its feasibility to limit the tumor motion during treatment delivery for all patients to a defined value while maintaining the accuracy and proved the applicability of the technique in a conventional clinical schedule.

        전립선비대증을 동반한 발기부전 환자에서 Mirodenafil 50mg 매일 복용요법과 100mg 필요 시 복용요법의 유효성 비교

        Purpose: To Compare the improvement of erectile dysfunction (ED) and lower urinary tract symptoms (LUTS) as well as the efficacy of mirodenalfil 50mg once daily and 100mg on-demand in patients with benign prostatic hyperplasia (BPH) and erectile dysfunction (ED). Materials and Methods: Prospective study was done with 220 patients who had BPH and ED from June 2013 to October 2014. Out of 220 individuals, 260 met inclusion criteria and 204 finished the research. Patients were divided into two groups. Group 1 had mirodenafil 50mg once daily and Group 2 had mirodenafil 100mg on-demand. The five-item version of the International Index of Erectile Function (IIEF-5), International Prostate Symptom Score (IPSS), Qmax, and residual urine volume(PVR) were assessed immediately before initiation of treatment (V1) and after four (V2) and twelve weeks of treatment (V3). Results: No difference of IIEF-5, IPSS, Qmax, and PVR between two groups in V1. At V3, both groups had improvements of IPSS and group 1 had better improvements (-5.1±4.4 vs. -3.1±3.9 p<0.001). And Group 1 had larger improvements than Group 2 in Qmax significantly. No difference in PVR in both groups comparing V1 vs. V2 and V1 vs V3. Group 1 had better improved IIEF-5 than Group 2 (V1 vs. V3 : 4.8±5.8 vs. 4.4±5.1; p=0.032). There was no drop out patients due to cardiovascular problems. Conclusions: Once daily mirodenafil 50mg was more efficacious in treating both ED and LUTS than on-demand dosing mirodenafil 100mg without any complication of cardiovascular problems.

        Purpose: Pathogenically, both erectile dysfunction (ED) and benign prostatic hyperplasia (BPH) are closely related to vascular problems, and vascular problems are closely associated with obesity. This study evaluated the relationships between obesity, BPH, and ED. Materials and Methods: The medical history of male patients, aged ≥40 years, evaluated at urology outpatient clinics of 11 university hospitals in Korea with chief complaints of lower urinary tract symptoms (LUTS)/BPH and ED were retrospectively evaluated. The correlations of medical history, body mass index (BMI), international prostate symptom score (IPSS), prostate volume, International Index of Erectile Function -5 (IIEF-5), and serum testosterone level were assessed. Results: Body mass index showed significant correlations with IPSS (r=0.087, p=0.003), prostate volume (r=0.384, p<0.001), IIEF (r=-0.072, p=0.015), and serum testosterone concentration (r=-0.303, p<0.001). Conclusions: Body mass index correlates with testosterone concentration, prostate volume, and markers of LUTS and ED in men with the latter conditions.

        Development and Validation of an Analytical Method for Glucuronolactone in Energy Drinks by Hydrophilic Interaction Liquid Chromatography-electrospray Tandem Mass Spectrometry

        A rapid, sensitive analytical method for glucuronolactone in beverages was developed and validated using hydrophilic interaction liquid chromatography coupled to electrospray ionization tandem mass spectrometry (HILIC-ESI-MS/MS). To determine the optimum analytical conditions for glucuronolactone, three different kinds of HILIC columns and two mobile phases with different pH values were examined. An amide-bonded stationary phase with a pH 9 acetonitrile-rich mobile phase was the best condition in terms of column retention, ESI-MS/MS response area, and signal-to-noise ratio. After extraction, glucuronolactone was separated through the HILIC amide column and detected by negative ESI-MS/MS in selected reaction monitoring (SRM) mode. Nine energy drinks sold in Korea were spiked with glucuronolactone at a concentration of 5 ng/mL; the Monster EnergyTM sample showed the smallest peak area and its signal-to-noise ratio was used for method validation. Good linearity was obtained in the concentration range from 20 to 1500 ng/mL with a correlation coefficient > 0.998. The developed method had a limit of detection (LOD) of 6 ng/mL and a limit of quantitation (LOQ) of 20 ng/mL. The recovery of this method at concentration of 20, 100, 500, and 1000 ng/mL was 96.3%-99.2% with relative standard deviations (RSD) of 1.6%- 14.0%. A reproducibility precision assessment at concentration of 100 and 500 ng/mL was carried out among three laboratories. The recovery of that evaluation was 95.1%-102.3% with RSD of 2.7%-7.0%. An analysis of variance indicated that there was no difference between the recovery results of the three laboratories at the 5% significance level. The validated method is applicable to inspecting beverages adulterated with glucuronolactone in Korea.

        팬데믹 이후 회복기에 접어든 현재 관광산업의 효율적인 데이터관리와 여행객을 대상으로 맞춤형 관광지 추천을 위한 서비스 기반의 관광플랫폼 개발이 활발히 진행되고 있다. 이러한 관광플랫폼에는 여행객들을 관광지로 찾아오게 만드는 다양하고 새로운 시도와 높은 질의 서비스가 필요하다. 이에 관광플랫폼들이 개인화된 관광지 제안을 위해 기본 관광지 정보데이터를 활용하고 있으나, 사용자의 구체적인 요구와 선호를 완벽히 반영하는 데에는 한계가 있다. 이러한 요구에 관광지 이미지 데이터의 효율적 분류와 관리를 통해 서비스의 개인화와 맞춤화 그리고 효율적인 데이터관리에 도움이 될 수 있는 기술을 연구하고자 한국관광공사의 OpenAPI 데이터를 활용하여 EfficientNet 모델을 통해 관광지 이미지를 카테고리별로 분류하는 연구를 수행했다. 연구는 EfficientNet 모델의 B7버전(Version)을 사용하여 36개의 소분류 카테고리로 관광지 이미지를 분류하였으며 모델의 분류 정확도는 85.15%로 나타났다. 하지만, 데이터의 학습이 진행될수록 모델이 정확도 성능이 저하되었으며, 클래스별 데이터 추가 확보를 통한 정확도 개선의 필요성이 있었다. In the post-pandemic recovery period, service-based tourism platforms are being actively developed for efficient data management in the tourism industry and customized tourism recommendations for travelers. These tourism platforms require various new attempts and high-quality services to attract travelers to tourist destinations. Therefore, tourism platforms are using basic tourist information data for personalized tourist suggestions, but there are limitations in fully reflecting the specific needs and preferences of users. To study technologies that can help personalize and customize services through efficient classification and management of tourist image data and efficient data management, we conducted a study to classify tourist images into categories using the EfficientNet model using OpenAPI data from the Korea Tourism Organization. The study used Version B7 of the EfficientNet model to classify tourist images into 36 subcategories, and the classification accuracy of the model was 85.15%. However, the accuracy performance of the model decreased as the data was trained, and there was a need to improve the accuracy by securing additional data for each class.
