상완신경총 손상의 상부 신경간 손상 환자의 감각신경전도검사

Objectives: Localizing or evaluating lesions of upper trunk brachial plexopathy using conventional nerve conduction studies is not accurate. This study examined whether sensory nerve conduction studies are useful for detecting upper trunk brachial plexopathy. Methods: Sixty eight patients with upper trunk brachial plexopathy based on the intra-operative findings were selected among 162 cases of brachial plexopathy revealed by electrodiagnostic tests. All subjects were evaluated by extensive sensory nerve conduction studies including the lateral antebrachial cutaneous nerve, median nerve recorded at the 1st, 2nd and 3rd finger and superficial radial nerve, bilaterally. The following abnormalities were identified: lower amplitude or prolonged onset latency compared to the normal value or a decrease in amplitude more than 50% compared to the contralateral uninjured limb in the sensory nerve action potentials (SNAP). This study analyzed the percentage of abnormal latency and amplitude in the SNAP of lateral antebrachial cutaneous nerve, median nerve and superficial radial nerve. Results: The percentage of cases who showed abnormal findings of the nerve conduction studies in the lateral antebrachial cutaneous nerve, median nerve recorded from the 1st, 2nd and 3rd finger and superficial radial nerve group was 88.2%, 69.2%, 48.5%, 44.4% and 66.2%, respectively. Compared to the uninjured limb, the abnormal findings of nerve conduction studies in the lateral antebrachial cutaneous nerve, median nerve recorded from the 1st, 2nd and 3rd finger and superficial radial nerve group was 92.6%, 88.4%, 64.7%, 55.5% and 82.4%, respectively. Conclusion: Various nerve conduction studies, including the lateral antebrachial cutaneous nerve, median nerve recording at 1st digit and superficial radial nerve, are needed to diagnose upper trunk brachial plexopathy. Because most brachial plexopathy is axon loss, a comparison of the injured and uninjured limb is recommended even if the amplitude of SNAP is within normal limits.

Desensitization of Triggers and Urge Reduction for Paruresis: A Case Report

Paruresis is a special type of non-generalized social phobia that involves fear and avoidance of urination in public restrooms. We administered eight 60-minute sessions of desensitization of triggers and urge reduction (DeTUR), an addiction protocol of eye movement desensitization and reprocessing (EMDR) therapy, to a 29-year old man with paruresis of 10 year duration. Because phobic avoidance is the hallmark of any anxiety disorder, we applied DeTUR targeting the urge to avoid each anxiety-provoking situation in succession. After treatment, the participant no longer met the requirements for a diagnosis of social anxiety disorder, and the self-reported symptoms of social anxiety had decreased to non-clinical levels; furthermore, these treatment gains were maintained at the one-year follow-up. Further clinical studies are needed to generalize this finding.

중학교 교사가 인식하는 학습조직문화, 학교장의 수업리더십, 학교만족도, 수업개선활동 간 구조적 관계

본 연구의 목적은 중학교 교사가 인식하는 학습조직문화와 학교장의 수업리더십이 학교만족도를 매개하여 수업활동개선에 영향을 미치는 구조적 관계를 검증하는 것이다. 이를 위하여 서울교육종단연구 6차 패널자료에서 중학교 교사 데이터를 추출하였고, AMOS 24.0을 활용하여 구조방정식모형을 분석하였다. 그 결과는 다음과 같다. 첫째, 중학교 교사의 학습조직문화가 활발하고, 학교장의 수업리더십이 강하다고 인식할수록 학교만족도는 향상되며, 학교만족도가 높을수록 수업개선활동이 활발히 이루어지는 것으로 나타났다. 둘째, 중학교 교사들이 인식하는 학교만족도는 학습조직문화와 수업개선활동의 관계를 매개하는 것으로 나타났다. 셋째, 중학교 교사들이 인식하는 학교장의 수업리더십과 수업개선활동 간 관계에서 학교만족도는 매개하는 것으로 나타났다. 이러한 결과를 바탕으로 교사의 수업개선활동을 촉진시킬 수 있는 방안과 후속연구에 대하여 제언하였다. The purpose of this study is to explore the relationship among learning organization culture, principals’instructional leadership, school satisfaction teachers percieved culture, and the participation of instructional school effectiveness. To pursue it, we used the panel data and analyzed the relationship between intercept and slope of principals’ leadership, teachers’ culture, and school effectiveness through the latent growth model analysis method. The results of the study are as follow. First, teachers’ learning organizational culture and instructional leadership of principal influenced school satisfaction, and also the school satisfaction improve positively the teachers’ participation of instructional improvement activities in the middle school in Seoul. Second, the school satisfaction was significant mediator between school teachers perceived and their participation in instructional improvement activities. Third, the school satisfaction was also a mediator between instructional leadership of principal and the activities of instructional improvement. Based on the findings, the study suggests several implications for practical interventions and future limitation were outlined.

본 연구는 배달노동자의 행복에 직무환경과 직무태도가 어떤 영향을 미치는가를 탐색하는 데 목적이 있다. 이를 위하여 한국노동패널 제23차 패널데이터에서 배달노동자 데이터를 추출하여 독립표본 t-검정 및 위계적 회귀분석을 통해 분석하였고 그 결과는 다음과 같다. 첫째, 정규직이 비정규직 배달노동자에 비해 고용안정성과 임금, 그리고 행복에 있어 더 높게 인식하였다. 둘째, 고용상태와 상관없이 배달노동자들의 조직몰입이 높을수록 행복도 더 높게 인식하였다. 셋째, 정규직 배달노동자는 고용안정성과 조직몰입이 영향을 주었으며 비정규직 배달노동자의 경우 임금이 행복에 영향을 주었다. 본 연구는 배달노동자의 행복을 향상시키기 위하여 플랫폼 업체의 사회적 책무를 강조하며 정규직 전환을 위한 경력사다리 기회제공, 조직몰입을 높일 수 있는 외·내재적 보상 및 직무자율성 부여 등을 제언하였다.

Combined Control Charts for the Mean and Variance using Likelihood Method

In this study, we consider to propose combined control charts for the process mean and variance to control simultaneously assuming that the process distribution is normal. For this, first of all, we review the existing combined control charts and then propose a new chart based on the union-intersection test with the jointly likelihood ratio statistics for the mean and variance. Then we apply the Liptak combining function to propose another combined control chart. For the construction of those combined control charts, we use the fact that a series of the hypothesis tests is equivalent to the maintenance of a control chart. In order to combine the two individual tests for the respective mean and variance, we utilize the p-values for both individual tests since the two likelihood ratio statistics have different forms. Then we compare the performance of the combined charts reviewed and proposed in this study by obtaining the empirical probabilities detecting the out-of-control state when the process becomes out-of-control under the five different scenarios through a simulation study. Finally, we discuss some interesting aspects which are related with the combined charts as concluding remarks.
