Background and Objectives This study aimed to evaluate whether Toll-like receptors(TLRs) messenger ribonucleic acid (mRNA) were expressed in the facial nerve after incurringa crush or an injury of the facial nerve. Materials and Method An adult Sprague-Dawley underwent a crush or injury of the unilateralfacial nerve. The crush or injury was incurred by a cutting at the midpoint between thefacial nerve trunk and its branch. The whisker movement of vibrissae muscle was examined 4and 14 days after injury. Real-time polymerase chain reaction was performed for the normalfacial nerve taken from the left side and the damaged nerve taken from the right side. Results On the 4th day of injury, the expression of TLR 9, 13 mRNA was significantly lowerin the crush and injured groups than in the control group (p<0.05). On the 14th day of injury,the expression of TLR 2 mRNA was significantly higher in the injured group than in thecontrol group (p<0.05). Conclusion The expressions of TLR 9, 13 mRNA in the distal facial nerve after injury inthe crush and injured groups were significantly lower than that in the control group, but theexpression of TLR 2 mRNA in the injured group was significantly higher. Therefore, TLRsmay be involved in facial nerve damage and regeneration.

Actinomycosis of paranasal sinus is a very rare disease that is caused by infection of Actinomyces species that were present inoral or nasal flora due to trauma or dental treatment. Actinomycosis shows a local calcified lesion associated with soft tissuedensity on computed tomography, which is similar to fungal sinusitis. Actinomycosis associated with fungal ball due to Aspergillusaffecting sinus ventilation has been rarely reported. We experienced two cases of actinomycosis associated with fungalball and successfully treated with endoscopic sinus surgery and oral antibiotics for 3 months without recurrence.

Background and Objectives Although patients with chronic rhinosinusitis (CRS) presenta similar degree of olfactory dysfunction, their impairments in threshold, discrimination, andidentification test results may vary. We investigated factors related to each of these componentsusing the Korean version of the Sniffin’ Sticks test II in CRS patients. Subjects and Method A total of 120 CRS patients with olfactory dysfunction were enrolledand assigned to hyposmia and anosmia groups. Correlation between the three components wereexamined in both groups. We also subdivided patients into higher- and lower-score groups accordingto the threshold, discrimination, and identification scores within the hyposmia andanosmia groups to determine associated factors among the demographic factors, CRS severityon computed tomography (CT) and endoscopic findings. Results Threshold, discrimination, and identification scores were significantly correlated inhyposmia patients. Age [odds ratio (OR), 0.94] was associated with the threshold score, and theanterior olfactory cleft opacification score (OR, 1.31) on CT was associated with identificationdifficulties in hyposmia patients. The posterior olfactory cleft opacification score was associatedwith threshold (OR, 2.76) and identification difficulties (OR, 1.68) in anosmia patients. However,we could not identify significant risk factors for discrimination in both groups. Conclusion We demonstrated that the three components of the olfactory function test forCRS are significantly correlated in patients with hyposmia. Age was associated with thresholdscore in hyposmia patients and CRS severity, and with discrimination scores in both hyposmiaand anosmia patients. These findings will help the understanding of pathophysiology of CRSrelatedolfactory dysfunction.

Background and Objectives: Patients with nonallergic rhinitis (Non-AR) or idiopathic rhinitis are common, with both conditionsbeing classified as local allergic rhinitis (LAR). However, the link between LAR and CRS has not been identified. Thisstudy aimed to investigate the association of patient-reported local allergic symptoms with the postoperative outcomes of CRS. Materials and Method: We reviewed the medical records of 64 patients who underwent endoscopic sinus surgery for CRS. All patients underwent skin prick test, Multiple Allergosorbent Test, and computed tomography. Sneezing and nasal itchingwere defined as local allergic symptoms (LAS). We evaluated the relationships between clinical characteristics and recurrencerate of CRS according to the presence or absence of LAS. Results: In Non-AR patients, there was no significant difference in age, sex, and TNSS between the LAS (+) and LAS (-)groups. However, in all patients, the CRS recurrence rate was higher in the LAS (+) group (46.7%) than in the LAS (-) group(15.8%; p=0.02). A similar trend was observed in the Non-AR patients, showing that the CRS recurrence rate was significantlyhigher in the LAS (+) group (56%) than in the LAS (-) group (16.7%; p=0.024). Conclusion: The CRS recurrence rate was higher in patients with LAS regardless of the presence of AR.
