        NiTi scissors-bite corrector의 와이어 굵기에 따른 3차원적 치아 이동 양상

        Objective: The purpose of this study was to compare the difference in three dimensional tooth movement using three different wire sizes (0.018 × 0.025-in, 0.016 × 0.022-in, 0.016-in) on a NiTi scissors-bite corrector. Methods: Computed tomography (CT) images of the experimental model before and after tooth movement were taken and reconstructed into three dimensional models for superimposition. The direction and the amount of tooth movement were measured and analyzed statistically. Results: The lingual and intrusive movements of the crown of the maxillary second molar were increased as the size of the NiTi wire increased. The roots of the maxillary second molars moved buccally except for the 0.016-in group. The intrusive movement of the roots of the maxillary second molars was increased as the size of the NiTi wire increased. Due to the use of orthodontic mini-implants, anchorage loss was under 0.2 mm on average. Conclusions: The 0.018 × 0.025-in NiTi wire was most effective in lingual and intrusive movement of the maxillary second molar which was in scissors-bite position. Indirect skeletal anchorage with a single orthodontic mini-implant was rigid enough to prevent anchorage loss. 본 연구에서는 NiTi scissors-bite corrector를 세 가지 서로 다른 굵기 0.018" × 0.025", 0.016" × 0.022", 0.016" 의 NiTi 와이어로 제작하였을 때 각각의 치아 이동 양상을 치아 이동 시뮬레이션 장치인 Calorific machine을 이용하여 3차원적으로 분석하고자 하였다. 가위교합에 이환된 상악 제2대구치 모형을 제작하고 제2소구치와 제1대구치 사이에 교정용 미니 임플랜트(orthodontic mini-implant)를 식립하여 제1대구치에 간접골성고정원을 설계하였다. 세 가지 굵기의 NiTi scissors-bite corrector를 부착하여 실험하였다. 치아의 이동 전후에 실험 모형은 computed tomography (CT)로 촬영하고 3차원 데이터로 변환하여 중첩하였다. 치아 이동 방향과 이동량을 계측하여, NiTi scissors-bite corrector 와이어 굵기에 따른 치아 이동 양상에 관한 통계적 유의성을 검정한 결과, 제2대구치 치관의 설측 이동량은 0.018" × 0.025"군(2.65 mm)이 가장 많았으며, 0.016"군(1.96 mm)과 통계적으로 유의한 차이를 보였다(p < 0.05). 제2대구치 치관의 압하량은 0.018" × 0.025"군(2.35 mm), 0.016" × 0.022"군(1.18 mm), 0.016"군(1.00 mm)으로 0.018" × 0.025"군이 나머지 두 군과 유의한 차이를 보였고 (p < 0.05), 치근의 압하량은 0.018" × 0.025"군(4.19 mm), 0.016" × 0.022"군(3.29 mm), 0.016"군(2.24 mm)으로 세 군 간 모두 통계적 유의차를 보였다 (p < 0.05). 치아의 이동 양상을 관찰한 결과, 0.016"군에서는 제2대구치 치근의 협측 이동이 나타나지 않았다. 간접골성고정원으로 사용된 제1대구치의 고정원 소실은 0.2 mm 이하로 나타났다. 이상의 결과를 토대로 0.018" × 0.025" NiTi 와이어로 제작한 NiTi scissors-bite corrector를 교정용 미니 임플랜트를 이용한 간접골성고정원과 함께 이용하였을때, 최소한의 고정원의 소실과 함께 가장 큰 상악 제2대구치의 설측 이동 및 압하가 일어났다고 할 수 있다.

      • Risk Factor of Post-Polypectomy Bleeding in Early Liver Cirrhosis

        Aims: Bleeding is the most common colonoscopic polypectomy complication. However, the risk of post-polypectomy bleeding in liver cirrhosis is unknown. We aimed to investigate the incidence and risk factors of post-polypectomy bleeding after a colonoscopic polypectomy in patients with early liver cirrhosis(LC). Methods: We performed a retrospective study of patients with early LC who underwent colonoscopic polypectomy at a single center between January 2006 and December 2015. In total, 41 patients with early LC were enrolled. We investigated the incidence of immediate PPB (IPPB) and delayed PPB (DPPB) in these patients. Results: Among 41 patients, 36 (87.8%) were Child-Turcotte-Pugh class A, 5 (12.2%) were class B. The mean prothrombin time was 1.26 ± 0.33, and the mean platelet count was 124.87 ± 71.32 × 103/L. A total of 78 polyps in 41 patients were removed. IPPB was observed 5 (6.41 %) of the 78 removed polyps presented with mild oozing and were controlled by hemostatic procedures using endoscopic hemoclips. Both IPPB and non IPPB group, during the observation period there were no DPPB. Conclusions: The risk of hemorrhage after polypectomy in the case of early cirrhosis of the liver did not increase significantly. However, when the size of the polyps are large, it is necessary to caution about bleeding after polypectomy.

        Barrier Characteristics of TaN Films Deposited by Remote Plasma Enhanced Atomic Layer Deposition (PEALD) Method

        Remote plasma-enhanced atomic-layer deposition with tertbutyllimidotris(diethylamido)tantalum as the Ta precursor and NH3 gas or NH3 plasma as the reactant was used to deposit TaN lms, the TaN lms deposited at 250 C showed a lower carbon impurity contents when NH3 plasma was used as the reactant. The resistivities of the TaN lms processed with NH3 gas and NH3 plasma were approximately 3,000 and 1,000 cm, respectively. The growth rates of the TaN lm processed with NH3 gas and NH3 plasma were about 1.3 and 1.4 per cycle, respectively. The TaN lms showed a relatively smooth interface and amorphous characteristics exhibited excellent step coverage on 0.25-m-wide by 2.5-m-deep contact hole structure. The barrier characteristics of Cu/TaN/Si samples were observed after annealing at various temperatures, and the TaN lms failed after annealing at 700 ℃.

      • Identification of lipopolysaccharide-binding peptide regions within HMGB1 and their effects on subclinical endotoxemia in a mouse model

        <P>Lipopolysaccharide (LPS) triggers deleterious systemic inflammatory responses when released into the circulation. LPS-binding protein (LBP) in the serum plays an important role in modifying LPS toxicity by facilitating its interaction with LPS signaling receptors, which are expressed on the surface of LPS-responsive cells. We have previously demonstrated that high mobility group box 1 (HMGB1) can bind to and transfer LPS, consequently increasing LPS-induced TNF-α production in human peripheral blood mononuclear cells (PBMCs). We report here on the identification of two LPS-binding domains within HMGB1. Furthermore, using 12 synthetic HMGB1 peptides, we define the LPS-binding regions within each domain. Among them, synthetic peptides HPep1 and HPep6, which are located in the A and B box domains of HMGB1, bind to the polysaccharide and lipid A moieties of LPS respectively. Both HPep1 and HPep6 peptides inhibited binding of LPS to LBP and HMGB1, LBP-mediated LPS transfer to CD14, and cellular uptake of LPS in RAW264.7 cells. These peptides also inhibited LPS-induced TNF-α release in human PBMCs and induced lower levels of TNF-α in the serum in a subclinical endotoxemia mouse model. These results indicate that HMGB1 has two LPS-binding peptide regions that can be utilized to design anti-sepsis or LPS-neutralizing therapeutics.</P>

        Pattern analysis of patients with temporomandibular disorders resulting from unilateral mastication due to chronic periodontitis

        Purpose: The purpose of the present study was to perform a pattern analysis in patients with temporomandibular disorder (TMD) resulting from unilateral mastication due to chronic periodontitis. Methods: Thirty participants with signs or symptoms of TMD who engaged in unilateral mastication due to periodontitis-related discomfort (test group) were selected. Another 30 subjects exhibiting signs or symptoms of TMD resulting from unilateral mastication not due to chronic periodontitis (control group) were also recruited. An interview-based questionnaire was administered, and an examination of the temporomandibular joint (TMJ) with determination of periodontal status was performed. Results: The duration of unilateral mastication was significantly longer in the control group than in the test group. There was a significant negative correlation between the duration of unilateral mastication and the Community Periodontal Index score. Using the Research Diagnostic Criteria for TMD (RDC/TMD) axis I algorithms, all the subjects were assigned to 3 main groups. The test group exhibited significantly a higher diagnostic distribution of group III (arthralgia, osteoarthritis, or osteoarthrosis), and in both the test and control groups, the number of diagnoses was larger for the non-chewing side. The control group showed a significantly higher diagnostic distribution of group I (myofacial pain), and in both the test and control groups, the number of diagnoses was larger for the chewing side. Conclusions: The results of the present study indicate that unilateral mastication due to chronic periodontitis could induce not only pain but also structural TMJ changes if adequate treatment is not administered and supported within a short time from the onset of the condition. Therefore, immediate treatment of chronic periodontitis is recommended to prevent not only the primary progress of periodontal disease, but also secondary TMJ-related problems. Furthermore, subjects who have suffered chronic long-term periodontitis without treatment should be urged to undergo a TMJ examination.

      • S1 domain‐containing STF modulates plastid transcription and chloroplast biogenesis in <i>Nicotiana benthamiana</i>

        <P><B>Summary</B></P><P><P>In this study, we examined the biochemical and physiological functions of <I>Nicotiana benthamiana</I> S1 domain‐containing Transcription‐Stimulating Factor (STF) using virus‐induced gene silencing (VIGS), cosuppression, and overexpression strategies.</P><P>STF : green fluorescent protein (GFP) fusion protein colocalized with sulfite reductase (SiR), a chloroplast nucleoid‐associated protein also present in the stroma. Full‐length STF and its S1 domain preferentially bound to RNA, probably in a sequence‐nonspecific manner.</P><P><I>STF</I> silencing by VIGS or cosuppression resulted in severe leaf yellowing caused by disrupted chloroplast development. STF deficiency significantly perturbed plastid‐encoded multimeric RNA polymerase (PEP)‐dependent transcript accumulation. Chloroplast transcription run‐on assays revealed that the transcription rate of PEP‐dependent plastid genes was reduced in the <I>STF</I>‐silenced leaves. Conversely, the exogenously added recombinant STF protein increased the transcription rate, suggesting a direct role of STF in plastid transcription. Etiolated seedlings of <I>STF</I> cosuppression lines showed defects in the light‐triggered transition from etioplasts to chloroplasts, accompanied by reduced light‐induced expression of plastid‐encoded genes.</P><P>These results suggest that STF plays a critical role as an auxiliary factor of the PEP transcription complex in the regulation of plastid transcription and chloroplast biogenesis in higher plants.</P></P>

      • 처음 발견된 고혈압 환자들의 관리의도와 실천에 영향을 미치는 요인

        This study will provide early effective intervention strategy through a predicting behavioral intention or behaviors of hypertension patients, and improve the compliance via early removal of barriers. Study subjects are new case of hypertension with above stage 2 by JNC-Ⅵ from first community survey including blood pressure measurement, sociodemographic questionnaires and physical examination in study region. Second survey was applied to the new case of hypertension(541 persons) for estimation of behavioral intention. Subject of this survey was 383 persons. Follow-up survey performed about 5 months later after regular telephone visiting by trained nurse previously. Final study subjects were 222 persons with above stage 2 hypertension by JNC-Ⅵ criteria. This results were as follows ; 1. State of hypertension management 18.9% of subjects managed with drug medication, 19.7% with exercise, 29.8% with quitting or reducing of smoking, 37.0% with quitting or reducing of alcohol. 2. Sociodemographic factors Drug medication was higher in old age, lower education level, and rural residents. Its difference is statistically significant. In gender difference, female was higher than male. In aspect of life style modification management, exercise rate was higher in male, high educational level, 5th and 4th decade age group, but it was not statistically significant. Reducing or quitting of smoking was higher in male, rural area residents, above high school educational level, but was not significant with the exception of habitation area. 3. Life style modification It was revealed statistically significant that male and rural area residents were higher in life style modification. In education level, contrary to the intention for life style modification, the lower education level is, the higher life style modification occurs. 4. Factors associated with intention The former or abstinence smoker and alcohol drinker revealed higher intention score than current in intention for life style modification. And the other variables were not statistically significant.
