        Nasality Changes With Age in Normal Korean-Speaking Adults

        Objectives. This study was performed to investigate the effects of aging on nasality and the influence of age-related changes in nasal cavity volume and nasal patency on nasality. Methods. A total of 180 healthy Korean-speaking adult volunteers, who had no nasal or voice-related complaints, were enrolled in this study. Nasometry, acoustic rhinometry, and rhinomanometry were performed to obtain the nasalance score, nasal cavity volume, and nasal resistance, respectively. Changes in these parameters with age were analyzed. Results. Nasal cavity volume increased significantly, and nasal resistance decreased significantly, with age. The nasalance scores for the nasal passage and oronasal passage decreased significantly with age, while there were no age-related changes in nasalance scores for the oral passage. Conclusion. Nasalance scores for the passages containing nasal consonants decreased with age although significant increases were observed in nasal cavity volume and nasal patency with age. Therefore, the age-related decreases in nasalance scores may result from factors other than changes in the nasal cavity.

        A New Method of Extracting the Filter Characteristics of the Nasal Cavity Using Homorganic Nasal-Stop Sequences: A Preliminary R eportHansang ParkThis study provides a new method of extracting the filter characteristics of the nasal cavity. Korean lenis stops are realized as voiced in the homorganic nasal-lenis stop sequences between vowels. Since the only difference between the two members of the homorganic nasal- lenis stop sequences, such as [mb], [nd], and [ g], is whether the passage to the nasal cavity is open or not, the subtraction of the LPC spectrum of the voiced stop from that of the preceding nasal leads to the filter characteristics of the nasal cavity of an individual speaker regardless of place of articulation. The results suggest that various attempts should be made to extract a robust filter characteristics of the nasal cavity by giving variation to LPC coefficients and by paying particular attention to speech samples. This study is significant in that it provides a preliminary report about a new method of extracting the filter characteristics of the nasal cavity.

        만성 비부비동염 환자의 비음도 분석

        Background and Objectives: This study was designed to compare the nasalance between chronic rhinosinusitis (CRS) patients and normal controls and to determine the correlation of nasalance with nasal volume, nasal resistance, CT score, and polyp score in CRS patients. Materials and Methods: This study enrolled 150 CRS patients and 154 normal adults. All subjects underwent acoustic rhinometry and rhinomanometry. Nasalance scores were measured with the nasometer. All CRS patients were graded according to the Lund-Mackay CT staging system and the degree of nasal polyp. Results: Nasal volume was decreased and nasal resistance was increased in CRS patients compared with normal controls. However, the nasalance scores for oro-nasal and nasal passages were significantly higher in CRS patients compared with normal controls. In CRS patients, the nasalance score for the nasal passage showed positive correlation with nasal volume and negative correlation with nasal resistance and Lund-Mackay CT score. However, no significant correlation was found between nasalance score and endoscopic polyp score. Conclusion: Contrary to expectations, nasalance was increased in CRS patients despite decreased nasal volume and increased nasal resistance. Thus, nasal resonance likely depends on a multitude of factors other than changes in the sinonasal cavity.

        한국인에서 연령에 따른 비강면적 및 비저항의 변화

        Background and Objectives:It is often observed that the nasal cavity becomes atrophic with old age. However, there is no exact data that prove senile atrophic changes in the nasal cavity. Thus we evaluated the efect of aging on the dimension and the resistance of the nasal cavity. Subjects and Method:One hundred twelve healthy subjects free of nasal diseases were included in this study. Acoustic rhinometry and rhinomanometry were performed before and after nasal decongestion. The subjects were classified into 6 age groups and cros-sectional area (CSA) measured by acoustic rhinometry and the total nasal resistance were analyzed by rhinomanometry among each group. Results:with age. However, there was no significant diference before and after nasal decongestion. The average values of CSA at the attachment of the inferior turbinate before and after decongestion were on the increase with age. However, the diference of CSA between pre- and post-decongestion did not change significantly with age. Rhinomanometry showed that nasal resistance markedly decreased from twenties and thirties but did not change significantly after the thirty decade. Conclusion:The dimension of nasal attributed to the structural change such as bony atrophy rather than mucosal change.

        비출혈을 주소로 내원한 환자에서 확인된 비부비동 IgG4-연관 질환 1예

        Immunoglobulin (Ig) G4-related disease is a systemic chronic inflammatory disease characterized by sclerosing lesions by IgG4 positive plasma cells and T lymphocytes. It is rarely described in the sinonasal cavity and the paranasal sinus. Here we report a case involving a 69-year-old woman whose chief complaint was recurrent nasal bleeding for 2 years. Her nasal cavity showed erosion of nasal septum as well as the synechia in between the nasal septum and the lateral nasal wall with severe edematous change in the left inferior turbinate. Imaging finding showed bilateral soft tissue density in the maxillary and ethmoid sinuses and mucosal swelling of left nasal cavity. Her IgG4 level was elevated and proteinase-3 Ab was positive. Histopathological examination from the left inferior turbinate showed dense lymphoplasma cell infiltration and storiform fibrosis. Immunostaining for IgG4 and IgG revealed numerous plasma cells positive for IgG4 (more than 90 cells per high power field) and IgG4/IgG ratio of 0.7. Otorhinolaryngologist should consider IgG4 related disease as one of the differential diagnosis in case of bloody rhinorrhea with mucosal erosion or synechia inside the nasal cavity.

        Vegetable or Pulse Granuloma in the Nasal Cavity

        Vegetable granuloma or pulse granuloma results from the implantation of food particles of vegetable origin. Pulse granulo- mas have mainly been reported in association with lung aspirations, the oral cavity with a history of oral procedures and less frequently in gastrointestinal tracks. We report a 31-year-old woman who presented with right nasal obstruction and was found to have a firm mass in the right nasal cavity. Paranasal sinus computerized tomography scans identified a calci- fied ring lesion in her right nasal cavity. Endoscopic sinus surgery was performed, and pathology examination revealed a lesion consistent with a pulse granuloma that contains starch granules with cellulose envelopes appearing as hyaline rings surrounded by inflammation cells and concentrically arranged delicate connective tissue. Pulse granuloma is a well de- scribed entity with distinct histopathology. However, pulse granulomas are rare, and especially extraoral pulse granulomas are extremely rare. We found that pulse granuloma can be occurred in the nasal cavity through regurgitation.

        비강 내에 발생한 혈관근육지방종 1예

        Angiomyolipoma (AML) is a generally benign tumor that usually occurs in the kidney. Nasal AML is extremely rare, with eight cases of AML in the nasal cavity have been reported in the English-language literature and only one case has been reported in Korea. Our case involved a 55-year-old male who presented with a history of frequent nasal bleeding on the right side of nasal cavity. Physical examination revealed a mass of about 1.2-cm in the right nasal cavity, which was removed by an endoscopic approach. Microscopically, it consisted of bundles of smooth muscle and blood vessels, mature fat cells in various proportions. We suggest that the cause of the nasal bleeding should be considered this case.

        Paragangliomas arising from the nasal cavity and paranasal sinuses are extremely rare. Sinonasal paraganglioma causes repeated sal sinus and the roof of nasal cavity. Histopathologically, chief cels composing tumors are arranged in the zellballen pattern. Sinonasal paraganglioma is usually treated through radical surgical excision. We experienced a rare case of paraganglioma of the nasal cavity and treated it without recurrence through nasal endoscopic excision. (Korean J Otorhinolaryngol-Head Neck ;50 :1054-7)

        비강내 발생한 해면상 림프관종 1예

        Cavernous lymphangioma is a benign congenital malformation that usually appears in the early childhood. It may arise in any anatomical location, including the head and neck, but is rarely found in the nasal cavity. Recently, we experienced a case of a cavernous lymphangioma originating from the nasal cavity, which was successfully treated by endoscopic approach without recurrence. Cavernous lymphangioma can occur in the nasal cavity, and therefore, it should be included in the differential diagnosis of mass on the nasal cavity and paranasal sinuses.         영아에 발생한 비강내 미성숙 기형종 1예: 10년간 장기추적 결과

        Teratomas are rare congenital neoplasms that arise from pluripotent cells. Teratomas of the head and neck are exceedingly rare and are usually present in the neonatal period. Immature teratomas account for less than 1% of all germ cell malignancies and the incidence in the nasal cavity is very rare. We report, with a review of literature, of an immature teratoma in the nasal cavity. A 5- month-old female presented with a right nasal cavity tumor associated with nasal obstruction and recurrent epistaxis. Endoscopic removal was done by minimally external alotomy approach. Tumor had originated from the roof of the nasal cavity & ethmoid sinus, and the nasal septum was deviated to the left side due to expansile mass. At follow-up, 10 years after operation, there was no evidence of recurrence;no severe nasal septum deviation nor atrophy of inferior turbinate was observed.
