      • Laser-Assisted Turbinate Surgery for the Treatment of Chronic Rhinitis

        Patients with nasal turbinate dysfunction who have not responded to conservative treatment will have undergone surgical therapy. In 105 patients with allergic rhinistis or chronic hypertrophic rhinitis, the inferior turbinate were ablated using a diode laser equipped with specially-designed handpiece and suction tip under local anesthesia as an outpatient procedure. After 6 months of follow-up period, we analyzed the subjective symptoms, nasal airway resistance with a rhinomanometer, olfaction by the buthanol threshold test and nasal mucociliary transit time by the saccharin transit time. There were significant improvements in sneezing for allergic rhinitis and nasal obstruction for chronic hypertrophic rhinitis, and active anterior rhinomanometry showed significant decrease in nasal airway resistance after diode laser ablation of the inferior turbinate (p<0.05). These results suggest that ablation of the inferior turbinate with a diode laser can be an effective treatment modality for the patients with refractory allergic rhinitis or chronic hypertrophic rhinitis.

      • Effects of Saline Solution on Ciliary Movement in Human Nasal Epithelium in vitro

        Background and Objectives:Osmolarity is known to affect ciliary beat frequency (CBF);however, little is known aboutthe effects of saline spray or irrigation on ciliary activity in human nasal epithelium. The aim of this study was to assesswhether CBF is affected by hypertonic, isotonic or hypotonic saline solution in vitro and whether histologic changes areassociated with the alteration of ciliary movement. Materials and Methods:Normal turbinate mucosa was exposed to fivedifferent concentrations including 0.06%, 0.12%, 0.9%, 3.0%, and 7.0% of phosphate-buffered saline solutions. CBF wasmeasured up to 60 minutes after exposure to each concentration of saline solution and histologic changes were examined bytransmission electron microscopy. Results:Isotonic and hypotonic saline solutions produced no ciliary slowing;however,ciliostasis was observed within a few minutes in 3.0% or 7.0% saline solution. Immediately after complete ciliostasis in thehypertonic solutions, the solutions were replaced with 0.12% or 0.9% saline solution in order to observe the reversibility ofCBF. Only 0.12% hypotonic saline induced the recovery of ciliary movement. The ultrastructural changes demonstrated thatthe ciliary slowing might be attributed to epithelial damage by fluid transport toward the surrounding medium. Conclusion:Our results suggest that isotonic and hypotonic saline solutions may be more appropriate for nasal irrigation than hypertonicsaline solutions.

        Long-Term Outcomes of Acute Ischemic Stroke in Patients Aged 80 Years and Older

        Purpose: Short life expectancy influences decision-making when treating very old patients with acute ischemic stroke (AIS). We investigated mortality and survival duration in very old AIS patients (≥80 years) who received hospital care. Patients and Methods: Mortality data were obtained from medical records, structured telephone inquiries, death certificates from the Korean National Statistical Office, and social security data 5±1.9 years after stroke onset. Age, gender, vascular risk factors, and functional outcomes from modified Rankin scales (MRS) at discharge were analyzed as predictors of mortality. Results: Among 134 patients, 92 (68.7%) died. On Kaplan- Meier analysis, duration of survival of patients aged 80-84 years was longer than those aged 85-89 or 90-94 (24±6.4, 8±7.3, 7±2.0 months, respectively, p=0.002). Duration of survival of patients discharged in a state of MRS 0-1 was longer than the remaining groups at 47±4.8 months (p< 0.001). In Cox proportional hazard analysis, age and MRS at discharge were independent predictors of mortality. Conclusion: Long-term outcomes of very old patients with AIS are not uniformly grave, therefore predictors of mortality and estimated duration of survival should be considered during decision-making for treatment.

      • Unilateral Sinus Lesions

        Unilateral sinus have lesions a variety of causes which include congenital choanal atresia, foreign body of the nasal cavity, antrochoanal polyp, mycotic sinusitis, space occupying lesions and so on. Mycotic sinusitis has increased among the immunosuppressed patients or healthy persons. Some cases of facial trauma or tumors of the paranasal sinuses may cause unilateral sinusitis in terms of clinical manifestations or radiologic findings. To establish the differential diagnostic points for unilateral sinus lesions, we analyzed 978 cases of clinical features. The most common cause was nonspecific inflammation of the paranasal sinuses (535 cases, 54.2%). Other causes included inverted papilloma (90 cases, 9.2%), maxillary carcinoma (89 cases, 9.1%), fungal sinusitis (87 cases, 8.9%) and antrochoanal polyp (42 cases, 4.3%).

      • A Comparison of the Therapeutic Effects of Powder and Aerosolized Budesonide in the Treatment of Perennial Allergic Rhinitis

        The aims of this study are to compare the efficacy and side effects of intranasal budesonide as a dry powder and as a freonpropelledpressurized aerosol in the treatment of perennial allergic rhinitis and to assess the difference in compliance with thepowder and aerosol. The study design was a randomized, double-blind crossover between two treatment groups over a six-weekperiod. Twelve patients with symptomatic perennial allergic rhinitis received 400 μg of intranasal budesonide administereddaily as two puffs/nostril/day of the drug delivered by a freon-propelled aerosol for the first three weeks and as one inhalation/nostril/day of the pure drug powder for the next three weeks. Fourteen adults with allergic rhinitis received the drug in the reversesequence. Patients recorded daily symptoms on diaries, and, at each clinic visit, adverse effects were recorded. Compliance withthe powder and with the aerosol was also evaluated. During the six weeks, there were significant improvements in symptoms inboth treatment groups, and the differences in efficacy and compliance with the medication between the two groups were not significant. Adverse effects included headaches, epistaxis, and nasal dryness;however, they were not serious and equally distributedin the two groups. We conclude that powder and aerosolized budesonide improve the symptoms of perennial allergicrhinitis significantly with little difference in efficacy, side effects, and preference.

      • Th1 and Th2 Cytokine Expression in Allergic Mucosa of the Nasal Cavity

        In order to assess the role of cytokines in the pathophysiology of allergic rhinitis we observed the patterns of expression of cytokine mRNA in the nasal mucosa of 10 patients with perennial allergic rhinitis and 10 control subiects using reverse transcription-polymerase chain reaction (RT-PCR) for Th2 cytokine (IL-4, IL5) and Th1 cytokine (IFN- γ). Expression of IL-4 and IL5 mRNA’s detected by Southern blot of RT-PCR products was more often observed in patients with allergic rhinitis than in control subjects. When the level of expression of cytokines and β-actin mRNA was measured, the mean density ratios for IL-4 and IL-5 were higher in patients with allergic rhinitis than in control subjects, while there was no significant difference in the mean density ratio for IFN- γ between the two groups. This study demonstrates that both IL-4 and IL5 m-RNA’s are expressed significantly in the nasal mucosa of patients with perennial allergic rhinitis and these results suggest that Th2 cytokine may play an important role in the development of nasal allergy.
