The epidemiology, diagnostic criteria, clinical features, symptoms and signs laboratory findings, kidney pathology, and clinicopathologic correlation of systemic lupus erythematosus(SLE) were analyzed. The 63 patients studied were managed at the Department of Internal medicine, CNUH, from January 1983 to December 1997. Kidney biopsy was performed in 53 patients out of 63 patients with SLE. The results were as follows: 1. The ratio of male to female was 1:26. They were 12 to 71 years old and mean age was 32.1 years old. The peak age incidence was 4th decades(30%). 2. The most frequent chief complaint on admission was generalized edema. Most patients complain two or more symptoms. 3. Immunologic and renal disorders were the most frequently observed in the ARA criteria of SLE. And the positive ANA, hematologic disorder, malar rash, and arthritis, were observed in order of frequency. 4. Among the 53 patients with renal biopsy, 30 patients revealed class Ⅳ lupus nephritis(56%), class II in 12 patients(23%), class V in 8 patients(15%) and class III in 2 patients(6%). 5. The cases of lupus nephritis represented as nephrotic syndrome were high in the class IV with 68% and class V with 86%. 6. Of 30 patients who can be followed up, 5 patients resulted in death(17 % of mortality). They "were 14 to 57 years old and mean age was 28 years old. Follow up duration were from 1 month to 6 years and 3 months, and mean duration was 1 year and 1 month, but 3 cases 7. The causes of death were as follows: Two cases were due to seizure and coma resulted from CNS involvement. One was sepsis due to miliary the & pneumonia. One was dilated cardiomyopathy and heart failure due to cardiac ac involvement. And the other one was sepsis and acute renal failure resulted from cellulitis. 8. Prednisolone was used in all patients basically, and methyl-prednisolone pulse therapy, antimalarials and alkylating agents were used in some cases. In the cases of combined therapy, the activity of SLE was well controlled.

Acute renal failure is a frequent complication in hospitalized patients and is strongly related to the mortality. The Clinical outcome and prognostic factors of acute renal failure(ARF) have been analyzed by many authors. The present study describes the etiologic and clinical aspects, as well as other factors related to mortality. All the patients suffering from acute renal failure admitted during the period of January 1993 - August 1998 were included in the study. The average age of the patients was 52±17 years and mortality rate was 24%. The causes of acute renal failure were hemorrhagic fever with renal syndrome(HFRS), sepsis, renal hypoperfusion, urinary tract obstruction, acute tubular necrosis, etc. The etiology of ARF was a significant prognostic factor on mortality in ARF. Other significant prognostic factors were oliguria, organ failure, use of vasoconstrictors, hypotension, serum bicarbonate, premorbid conditions, sepsis, neurologic complications, gastrointestinal bleeding. On the other hands, operation, sex, anemia, thrombocytopenia, hypoalbuminemia, BUN, serum creatinine, and hyperkalemia were not significant factors for the mortality. We conclude that major prognostic factors of acute renal failure arc premorbid conditions, sepsis and multiorgan failure, and they are responsible for persistent high mortality of acute renal failure despite of advances of medical care.

The reaction of 6-chloro-2-hydrazinoquinoxalines with acetylacetone or dibenzoylmethane gave pyrazoles via dehydration of the intermediate hydrazone and no triazolo compound was obtained. The probable mechanism for the formation of the pyrazole is discussed.

Major cardiovascular complications of diabetes are coronary atherosclerosis, diabetic dilated cardiomyopathy, autonomic neuropathy and those are major causes of morbidity and mortality in diabetic patients. Gated blood pool heart scan is noninvasive and useful method for evaluation of functional status of heart in diabetics. We evaluated 52 patients with diabetes and divided 3 groups. Group 1 were 11 patients without proteinuria or with proteinuria less than 550mng during 24 hours. Group 2 were 9 patients with proteinuria more than 550mg during 24 hours and group 3 were 32 patients with endstage renal diasese due to diabetes. We performed 99mTc-HSA cardiac gated blood pool scan and used left ventricular ejection fraction(LVEF), peak ejection rate(PEF) to indices of LV systolic function and peak filling rate(PER) to index of LV diastolic function. The results were follows : 1) LVEF, PER were significantly lower in diabetics with ESRD than diabetics without ESRD, but there were no significant difference between normal controls and diabetics without ESRD 2) PFR was significantly lower in diabetics than normal controls, but there were no significant differences in diabetics with or without nephropathy. 3) There were negative correlation between PER, PFR and duration of diabetes. On the basis of results, PFR is a LV functional index of GBPS which can disclose early change of LV dysfunction in patients with diabetes.
새로운 Quinoxalinedione계 신약 KU-4162의 NMDA수용체 선택성

Antagonists for glycine binding site of NMDA receptors have advantage over those of NMDA receptors in terms of their side effects. In this study, the receptor binding profile of KJ-4162, a newly synthesized quinoxalinedione-derived antagonist for glycine binding site of NMDA receptors, was characterized using radioreceptor binding assays. The experimental condition of ligand binding assay of [³H]MK-80] for NMDA channel, [³H]kinate for kinate receptors, amino-3-hydroxy-5-methylisoxazole-4-propinc acid, DL-[5-methy]- ³H] ([³H]AMPA) for AMPA receptors, and [³H]MDL 105,519 for glycine-binding sites of NMDA receptors were established in rat cerebral cortex membranes. The range of specific binding is appeared to acceptable (65-98%). MDL 105,519, ACEA-1,021, glycine, 5,7-dichlorokynurenic acid (DCKY), and 6,7-dichloroquinoxalline-2,3-dione (DCQX) displaced the specific binding of [³H]MDL 105,519 with different binding affinity. KJ-4162 displaced the specific binding of [³H]MDL 105.519, and its apparent affinity constant (76.2±1.0 nM) was lower than that of MDL 105,519, ACEA-1,021, DCKY or glycine, but higher than that of DCQX and L-695,902. KJ-4162 (1 цΜ) displaced specific binding of [³H]MDL 105,519 by about 90%, but showed little influence on the specific bindings of [³H]kinate and [³H]AMPA. These results indicate that KJ-4162 is a novel and potent antagonist for glycine binding site of NMDA receptor complex. As well, this compound could possess neuroprotective effect.

This study was investigated the environmental activities of the international and domestic situations for environmental education to specify the systematic analysis on the research tools for its education related to science fields. The concepts of environmental education were chosen on water, air and energy which they are referred to biological concentration of pollutants in water system, air quality diffusion by means of Gaussian plume model and solar fuels in the experimental model. The results of this study are as follows: 1. Mathematical model of the biological concentrationof pollutants in the water ecosystem is modified, and accumulation of pollutant can be calculated by the equation C_i=MC(l-e^-1/Ti) and shows the practical data of the DDT concentration. 2. The calculation of amount of pollutants in the air corresponding to the stable atmospheric index and topograghic levels is reviewed by the Gaussian plume model on air quality diffusion, and practical application of SO_2 gas concentration in Jeonju area is expressed by Le Blancs' IAP.(index of atmospheric purity). 3. Solar fuel model related to energy is rearranged and calculated solar power(E), maximum photochemical power yield (Y), their efficiencies, and efficiency of solar cells, which is made by Institute of Science education of Jeonbug N. University. 4. Questionnaire method is used to collection of data related to knowledges, inquiry skills and values on environmental education of secondary school teachers. The scores of them are measured by path analysis.
