Stenosis of central vein is a common complication arising after percutaneous subclavian vein catheter insertion performed for temporary vascular access in chronic renal failure patients undergoing hemodialysis. There are several treatment methods for the condition like percutaneous angioplasty(PTA), stent insertion, and surgery, but recent trend is toward PTA and stents. Among the patients diagnosed with chronic renal failure from March 1993 to May 2002 and undergoing hemodialysis through AV fistula, the 14 Patients in whom central vein stenosis arose were selected for the study. A total of 28 percutaneous interventions(5 PTA and 23 stent placement) were performed, and restenosis rate and the time taken till the restenosis in de novo lesions and instant lesions were compared. All 28 cases were operated successfully. The 14 cases that received both anigioplasty and stent placement initially. (de novo lesion : 14 cases), Among the 10 cases with de novo lesion that followed up more than 1 year, 3 cases are currently undergoing hemodialysis without restenosis, and the remaining 7 cases have recurred stenosis with the mean time to restenosis of 10.9 months. In the 7 cases in whom stenosis recurred, 11 interventions were done(instent lesion: 11 cases). 4 of these were using only ballon angioplasty with 100% restenosis rate and the mean time of 3 months until restenosis. The remaining 7 cases were using both balloon angioplasty and stent placement, also with 100% restenosis rate but with the mean time of 12 months until restenosis, which was later than the group receiving only balloon angioplasty. In treating the patients with central vein stenosis, stent placement seems to be more advantageous over PTA in terms of restenosis rate and the mean duration of patency. In the case of instent lesion, inserting the stent for the second time after stenosis recurred lengthened the duration of patency compared to performing balloon angioplasty alone.

털진드기 유충 (Acari: Trombiculidae)은 쯔쯔가무시증을 전파하는 주요 매개체이다. 털진드기 유충의 발생량 은 가을철에 증가하는 것으로 알려져 있지만, 환경 및 시기에 따라 발생 패턴이 다르게 나타날 수 있어 각 지역에 대한 조사가 필요하다. 이 연구는 충남 예산의 털진드기 발생 양상을 확인하기 위해 2017년부터 2023년까지 36-51 주차 (9-12월)에 걸쳐 현장 조사를 수행하였다. 논, 밭, 수로, 초지에 5m 간격으로 털진드기 트랩을 환경별로 5개씩 설치하여 채집하였다. 그 결과 총 3,257개체로 2017년 1,104마리, 2018년 785마리, 2019년 650마리, 2020년 160마 리, 2021년 139마리, 2022년 233마리, 2023년 186마리 채집되었다. 동정 결과 5속 12종이 확인되었으며 둥근혀털 진드기(Neotrombicula tamiyai)가 1,882개체(57.78%)로 우점도가 가장 높게 나타났다. 이러한 발생 양상에 관한 연구는 매개 질환의 예방 및 관리 전략 수립에 있어 중요한 기초 자료로 활용될 수 있으므로 지속적인 연구와 모니터링이 필요할 것으로 사료된다.

The correct identification of filamentous fungi is challenging. We evaluated the performance of the VITEK MS v3.0 system (bioMérieux, Marcy-l’Étoile, France) for the identification of a wide spectrum of clinically relevant filamentous fungi using a Korean collection. Strains that were added to the upgraded v3.2 database were additionally identified by the VITEK MS v3.2 system. Of the 105 tested isolates, including 37 Aspergillus (nine species), 41 dermatophytes (seven species), and 27 other molds (17 species), 43 (41.0%) showed “no identification” or “multiple species identification” results at the initial VITEK MS testing; these isolates were retested using the same method. Compared with sequence-based identification, the correct identification rate using VITEK MS for Aspergillus, dermatophytes, other molds, and total mold isolates was 67.6%, 56.1%, 48.1%, and 58.1% at the initial testing and 94.6%, 78.0%, 55.6%, and 78.1% with retesting, respectively. Following retesting, 19 (18.1%) and two (1.9%) isolates showed “no identification” and “misidentification” results, respectively. VITEK MS reliably identified various filamentous fungi recovered in Korea, with a very low rate of misidentification.
Mechanism of Knee Adduction Moment Reduction Through Contralateral Cane Use in Healthy Subjects

The use of a contralateral cane has demonstrated a reduction in the external knee adduction moment (KAM) in patients with knee osteoarthritis. However, the underlying mechanism remains unclear. This study aimed to investigate the specific mechanism in healthy young subjects. Sixteen healthy young participants were included in this study. They walked on a floor under three loading conditions: no cane and with cane loads equivalent to 5% and 10% of body weight. Change in posture and kinetic features of ipsilateral leg were examined at two instants of ground reaction force (GRF) peaks. The results revealed a substantial decrease (40–55%) in both KAM and moment arm, whereas only 12% decrease was observed in GRF with 10% cane loading. Furthermore, KAM was strongly correlated with moment arm (r = 0.97, p < 0.01), but not with GRF (p > 0.05). The reduction in moment arm could be attributed to a medial shift of knee joint and/or a lateral shift of GRF. At the maximum cane loading, medial shift in the knee joint center was significant (p < 0.001), while any shift in GRF was insignificant (p > 0.80). The results suggest that the primary factor contributing to the reduction in KAM is the decrease in moment arm among healthy young subjects. Importantly, the reduction in moment arm was primarily induced by a medial shift of the knee joint center, emphasizing the significance of postural changes.
시각장애인을 위한 병원 접수 및 길 안내 애플리케이션 연구

최근 키오스크 도입의 증가와 더불어 키오스크를 사용하는데 불편함을 겪는 시각장애인이 늘고 있다. 그 원인 중에 가장 큰 원인은 앞이 보이지 않는다는 것이다. 병원 또한 키오스크의 사용량이 늘어가고 있고 이로 인해 진료 신청과 수납을 하는 등의 어려움이 있으며, 점자 블록과 점자 글씨의 부제로 진료과까지 찾아가는 것이 매우 어렵다. 이에 본 논문은 시각장애인도 타인의 도움 없이 혼자서 진료 신청을 하고 진료과까지 혼자서 찾아갈 수 있는 애플리케이션 개발을 목적으로 한다. 이를 위해 핸드폰에 내장되어 있는 음성 안내, 음성인식, 블루투스, 카메라, Talk back 기능을 활용해야 하며 음성으로 진료 접수를 하고 카메라 기능으로 병원내 길 안내를 하여 도움을 줄 수 있는 시스템을 설계 구현한다. 본 논문에서 제시하는 애플리케이션으로 시각장애인이 타인의 도움에 의지하지 않고 혼자서 해결해 나갈 수 있을 것이라 기대한다. Along with the recent increase in the introduction of Kiosks, a growing number of blind people are having difficulty using Kiosks. One of the main causes is the inability to see ahead. The use of Kiosks is also increasing in hospitals, which makes it difficult to apply for and receive medical treatment, and it is very difficult to visit the clinic due to the subtitle of Braille blocks and Braille letters. Therefore, this paper aims to develop an application that allows blind people to apply for medical treatment alone and visit the clinic alone without the help of others. To this end, the built-in voice guidance, voice recognition, Bluetooth, camera, and Talk back functions should be used, and a system that can help by receiving medical treatment with voice and guiding the hospital with camera functions is designed and implemented. It is expected that the application presented in this paper will allow the blind to solve it alone without relying on the help of others.

본 논문에서는 WCDMA, LTE에 이르기까지 USIM을 사용한 국내 이동통신서비스 시장에서 단말기정보를 사용하여 USIM에 의한 사용자 인증을 제한하여 이용자의 정상적인 통신서비스의 이용을 제한하는 현상이 벌어지고 있는것과 관련하여 국내에서 USIM을 사용하기 시작한 뒤부터 최근에 이르기까지 어떻게 이용자의 USIM 사용을 제한하여 장치 활용성을 저해하여 왔는지에 대한 방법과, 이로 인하여 유비쿼터스 컴퓨팅의 5-Any 中 하나인 Any-Device性을 저해하는 현상과 영향에 대하여 논한다.
