Tracheobronchopathia osteoplastica is a rare disorder of unknown cause characterized by the projection of numerous bony or cartilaginous submucosal nodules into the tracheobronchial lumen with sparing of the posterior membranous portion of tracheobronchial tree. In the past, most of the cases were diagnosed as secondary finding at autopsy. Nowaday, the key component of the evaluation is flexible fiberoptic laryngoscopy and bronchoscopy, which typically demonstrates irregular spicules of subrnucosal bone and cartilage projecting into the tracheobronchial lumen and causing various degrees of airway obstruction. A 45 years old man was visited Sunchon St. Carollo hospital due to discomfort on throat. We saw several nodules just below vocal cord via laryngoscope. The computed tomography revealed multiple small calcified nqdules on trachea and both main bronchi. Pathologic finding of bronchoscopic biopsy showed abnormal proliferation of bony and cartilagious nodules in the tracheal submucosa. These fingdings were consistent with tracheobronchopathia osteoplastica. We report here on a case of tracheobronchopathia osteoplastica with review of literature.

A high-performance liquid chromatographic method for the simultaneous quantitative analysis of methylparaben (MP), ethylparaben (EP), propylparaben (PP), butylparaben (BP) and imidazolidinyl urea(IU) or diazolidinyl urea(DU) in cosmetics was studied by using a cyano-propyl column and 0.05M hexanesulfonic acid at 228 nm. Calibration curves were found to be linear in the 60-1000 μg/Ml range (parabens),100-1,250 μg/Ml range (IU) and the 120-2000μg/Ml range (DU). Linear regression analysis of the data demonstrates the efficacy of the method in terms of precision and accuracy. An extraction method is developed and validated in order to apply this chromatographic method to a commercial cosmetic cream. The precision of this method, calculated as the relative standard deviation (RSD) of the recoveries (0.46- 2.71%) was excellent for all compounds.

<P>Amorphous metal oxides are attractive materials for various sensor applications, because of high electrical performance and easy processing. However, low absorption coefficient, slow photoresponse, and persistent photoconductivity of amorphous metal oxide films from the origin of deep-level defects are obstacles to their use as photonic applications. Here, we demonstrate ultrahigh photoresponsivity of organic-inorganic hybrid phototransistors featuring bulk heterojunction polymers and low bandgap zinc oxynitride. Spontaneous formation of ultrathin zinc oxide on the surface of zinc oxynitride films could make an effective band-alignment for electron transfer from the dissociation of excitons in the bulk heterojunction, while holes were blocked by the deep highest occupied molecular orbital level of zinc oxide. These hybrid structure-based phototransistors are ultrasensitive to broad-bandwidth photons in ultraviolet to near infrared regions. The detectivity and a linear dynamic range exceeded 10(12) Jones and 122.3 dB, respectively.</P>

<P>Two ginsenoside derivatives (<B>9</B>, <B>10</B>) along with 10 known ginsenosides (<B>1</B>–<B>8</B>, <B>11</B>, and <B>12</B>) were isolated from BST204, which is a crude ginseng extract fermented by enzyme and acid hydrolysis. The two ginsenosides were determined as 12β,20(<I>S</I>),25-trihydroxydammara-3-<I>O</I>-β-<SMALL>d</SMALL>-glucopyranoside (<B>9</B>) and 12β,20(<I>R</I>),25-trihydroxydammara-3-<I>O</I>-β-<SMALL>d</SMALL>-glucopyranoside (<B>10</B>). Compounds <B>1</B>–<B>12</B> were categorized into stereoisomeric pairs differentiated by <I>R</I>- or <I>S</I>-configuration at C-20, the number or position of sugar residues at C-3 or C-6, and the type of derivative at C-21. Their structure–activity relationship was evaluated by the cell viability assay using HSC-T6 cells. Results showed that 20(<I>S</I>) (<B>3</B> > <B>4</B>, <B>7</B> > <B>8</B>, and <B>9</B> > <B>10</B>), a 2-hydroxy-2-methylbutyl moiety at C-21 (<B>3</B>, <B>7</B> > <B>9</B>), and the number of sugar residues at C-3 (<B>3</B> > <B>7</B>) significantly affected the antiproliferative activity on HSC-T6 cells. Prostacyclin 분무형과 정주형이 폐동맥 고혈압에 미치는 효과

Background: Prostacyclin administered intravenously has demonstrated intermediate pulmonary specificity and its aerosol form has an even greater pulmonary selectivity. There have been few systematic analyses of the difference in response according to the route of administration and the dose of administration of prostacyclin. So we have compared prostacyclin infusion versus inhalation in various concentrations in an animal model. Methods: Pulmonary hypertension was induced by continuous intravenous infusion of the vasoconstrictor U46619 and prostacyclin solutions of 10, 50, 100, 200 mcg/ml were inhaled using a jet nebulizer. Prostacyclin infusion was done at a rate of 100, 200, 400 ng/kg/min. Results: With inhalation of 10, 50, 100, 200 mcg/ml prostacyclin, PVR fell to values of 85%, 76%, 64%, 55% of the preinhalation value and SVR fell to values of 94%, 80%, 76%, 64% of the preinhalation value, respectively(p<0.05). PVR/SVR ratios decreased significantly in all inhalation doses(p<0.05). With infusion prostacyclin at a rate of 100, 200, 400 ng/kg/min, PVR fell to values of 73%, 60%, 50% of the preinfusion value and SVR fell to values of 68%, 54%, 38% of the preinfusion value, respectively(p<0.05). PVR/SVR ratios increased at an infusion rate of 400 ng/kg/min. Conclusion: Prostacyclin inhalation did not result in selective pulmonary vasodilation without causing any efects on the systemic vascular bed(absolute pulmonary selectivity). But it did cause more predominant vasodilation on the pulmonary vascular bed(relative pulmonary selectivity). By contrast, prostacyclin infusion caused more predominant vasodilation on the systemic vascular bed, creating the risk of severe systemic hypotension. (Korean J Anesthesiol 1998; 35: 413∼422)

The clinical review was done on 15 cases with the foreign body in the upper gastrointestinal tract who visited and underwent endoscopic removal of foreign body at department of lnternal Medicine of Chonnam University Hospital and Namkwang Hospital from June 1982 to August 1987. The results are as follows: 1) The age distribution varied from 32 yeara to 83 years of age, being most prevalent at 6th decade, The ratio between male and femaie was 4:1, more prevelant in male. 2} Among 15 eases, 8 cases were lodged in the esophagus and 7 cases in the stomach. The foreign bodies in the esophagus were meat, pressure-through package, food materials and bone in order of frequency and those in the stomach were bezoar, persimmon seed, brown seaweed, tooth brush and suture materials. 3) Cambined diseases of upper gastrointetinal passage disturbance were 1 case of esophageal cancer and 3 cases of gastric outlet obstruction caueed by duodenal ulcers and gastric malignancy. Other combined diseases were schizophrenia, chronic alcoholism, diabetes mellitus and gastrie polyp. 4) Complications by foreign body were retropharyngeal abscess and phlegmonous esophagitis with esophageal ulcer.

Congential bile duct cysts, the so called "choledochal cysts" occur at all levels of the biliary tree and may be of saccular, diverticular, or fusiform configuration. There are 6 types of choledochal cysts proposed by todani et al based on roentgenologic and operative findings. This disease is usually diagnosed at the childhood or early adult life and predilection for orientals. Clinical manifestations are usually jaundice, right upper quadrant abdominal pain and abdominal mass. Here we are reporting a case of choledochal cyst, type IV-A diagnosed by abdominal ultrasonogram. 99mTc-DISIDA hepatobiliary scan and endoscopic retrograde cholangiopancreatogram.

<P>Using various spectroscopic techniques such as UV-visible spectroscopy, circular dichroism spectroscopy, NMR spectroscopy, small-angle X-ray scattering, transient grating, and transient absorption techniques, we investigated how cell-mimetic environments made by crowding influence the photocycle of photoactive yellow protein (PYP) in terms of the molecular volume change and kinetics. Upon addition of molecular crowding agents, the ratio of the diffusion coefficient of the blue-shifted intermediate (pB) to that of the ground species (pG) significantly changes from 0.92 and approaches 1.0. This result indicates that the molecular volume change accompanied by the photocycle of PYP in molecularly crowded environments is much smaller than that which occurs in vitro and that the pB intermediate under crowded environments favors a compact conformation due to the excluded volume effect. The kinetics of the photocycle of PYP in cell-mimetic environments is greatly decelerated by the dehydration, owing to the interaction between the protein and small crowding agents, but is barely affected by the excluded volume effect. The results lead to the inference that the signaling transducer of PYP may not necessarily utilize the conformational change of PYP to sense the signaling state.</P>
