Background/Aims: The aim of this study was to investigate predictive factors for malignant transformation of colorectal adenomatous polyps in Korea. Methods: One thousand five hundred and eighty- nine sessions of colonoscopic polypectomy were performed from November 1994 to September 1998 for 1,375 patients with adenomatous polyps at Samsung Medical Center. We analyzed the 2,179 adenomatous polyps including 55 (2.5%) polyps with malignant transformation, retrospectively. Results: Six parameters showed significant differences in univariate (p$lt;0.01) and multivariate analysis. For each parameter, the proportions of malignant polyps were as follows: for number of polyps, 1.3% in cases of single polyp and 10.3% in cases of multiple polyps; for size, 1.4% of polyps less than 1 cm and 15.5% of polyps larger than 1 cm; for location, 4.6% of the polyps located in rectum or sigmoid colon and 1.1% of the polyps located in descending to ascending colon; for morphology, 0.9% in polyps of Yamada type I/II and 7.6% in polyps of Yamada type III/IV; for underlying histology, 1.4% in cases of tubular adenoma and 8.1% in cases of tubulovillous or villous adenoma; for degree of dysplasia, 1.2% in cases accompanied with low-grade dysplasia and 8.9% in cases accompanied with high-grade dysplasia. Conclusions: The malignant potential of colorectal adenomatous polyps in Korea is correlated with multiplicity, distal location, pedunculated shape as well as well-known factors like size, histologic type and degree of dysplasia.

<P>We evaluated the effect of CYP2C19 genotype over time on the antiplatelet response of clopidogrel in healthy subjects. Seventy subjects enrolled for a pharmacodynamic study and 22 subjects for a pharmacokinetic and pharmacodynamic study took 300?mg clopidogrel on the first day and 75?mg once daily for six consecutive days. The subjects with CYP2C19 poor metabolizers (PM, N?=?22) and intermediate metabolizers (IM, N?=?37) had significantly delayed time to inhibition of platelet aggregation (IPA) compared with CYP2C19 extensive metabolizers (EM, N?=?33) (12 vs. 9 vs. 2?hours as median Tmax , P?<?.05) after a 300?mg of clopidogrel. During maintenance doses of clopidogrel, IPA values of only CYP2C19 PM subjects were gradually decreased from 30.0??21.9% on day 2 to 23.7??16.6% on day 8 (P?>?.05 for time effect; P?<?.05 for time and genotype interaction effect). CYP2C19 PM had decreased Cmax and AUC of thiol metabolite compared with CYP2C19 EM (0.42- and 0.37-fold on day 1, P?<?.01; 0.39- and 0.34-fold on day 7, P?<?.01, respectively). Delayed time to reach maximal IPA as well as decreased IPA may influence the increased risk of the acute cardiac events in CYP2C19 PM and IM.</P>

<P>BACKGROUND/AIM: It has been reported that the Z-line appearance (the ZAP grade) wascorrelated with the prevalence of intestinal metaplasia. Therefore, we prospectively investigated the clinical usefulness of ZAP classification based on the histology in making a diagnosis of non-erosive reflux disease (NERD). METHODOLOGY: We studied 80 consecutive young male patients with heartburn and acid regurgitation. The symptom characteristics were collected by interviewing with a structured questionnaire. Upper GI endoscopy with biopsy was performed to identify the distal esophagitis. RESULTS: Correlation was significant between the ZAP score and the LA classification (Spearman cor relation factor: 0.463). There were no significant differences in symptom according to the ZAP score. There were no significant differences in microscopic esophagitis between the patients with NERD (ZAP 0) and those with ERD (Erosive Reflux Disease, ZAP I, II, III). No statistical significance was observed between the ZAP score and the histology. In the diagnosis of NERD, ZAP classificationwas more accurate than LA classification in the basis of histology at distal esophagus (p < 0.05). CONCLUSIONS: The ZAP classification could be applied to diagnosis of GERD, especially NERD. Biopsy taken at distal esophagus can be a limited but useful diagnostic tool, particularly in patients with NERD.</P>

Background/Aims: Little is known about the effect of Helicobacter pylori (HP) infection on proximal gastric motor function in functional dyspepsia (FD). We evaluated total gastric emptying, initial proximal gastric distribution of meal (IPGD), proximal gastric emptying, basal tone, compliance and postprandial accommodation in the patients with FD relating to HP status. Methods: Of 25 subjects, 18 were HP positive and 7 were HP negative by the result of 13C urea breath test. A solid-phase scintigraphic gastric emptying and gastric barostat were performed. Results: In both HP-positive and HP-negative patients, sex, age, symptom score and body mass index were not different. Total gastric half-emptying time (101±13.9 min vs. 107±25.9 min), retention rate at 30 min (80.4±1.2% vs. 81.6±3.2%) and at 120 min (47.3±4.1% vs. 49.9±7.7%) were similar between the two groups. IPGD (69.4±4.1% vs. 64.6±11.1%) and proximal half-emptying time (37.9±5.2 min vs. 48.4±13.2 min) were not different between the two groups. basal tone (60.8±3.4 ml vs. 65.0±7.0 ml), compliance (49.2±2.4 ml/mmHg vs. 37.6±5.7 ml/mmHg), and accommodation (207±46% vs. 137±75%) were not different between the two groups. Conclusions: There was no association between HP infection and proximal gastric motor function in FD.
Relationship Between Gastroesophageal Reflux Symptoms and Dietary Factors in Korea

Background/Aims The incidence of gastroesophageal reflux disease (GERD) is increasing in Korea. The aim of this study was to evaluate the relationship between GERD symptoms and dietary factors in Korea. Methods From January 2007 to April 2008, 162 subjects were enrolled (81 in GERD group and 81 in control group). They were asked to complete the questionnaires about GERD symptoms and dietary habits. The symptom severity score was recorded by visual analogue scale. Results Subjects with overweight or obesity had an increased risk for GERD (OR, 2.52; 95% CI, 1.18-5.39). Irregular dietary intake was one of the risk factors for GERD (OR, 2.33; 95% CI, 1.11-4.89). Acid regurgitation was the most suffering (2.85±2.95 by visual analogue scale) and frequent reflux-related symptom (57.5%) in GERD. Noodles (OR, 1.22; 95% CI, 1.12-1.34), spicy foods (OR, 1.09; 95% CI, 1.02-1.16), fatty meals (OR, 1.20; 95% CI, 1.09-1.33), sweets (OR, 1.42; 95% CI, 1.00-2.02), alcohol (OR, 1.16; 95% CI, 1.03-1.31), breads (OR, 1.17; 95% CI, 1.01-1.34), carbonated drinks (OR, 1.69; 95% CI, 1.04-2.74) and caffeinated drinks (OR,1.41; 95% CI, 1.15-1.73) were associated with symptom aggravation in GERD. Among the investigated noodles, ramen (instant noodle) caused reflux-related symptoms most frequently (52.4%). Conclusions We found that noodles, spicy foods, fatty meals, sweets, alcohol, breads, carbonated drinks and caffeinated drinks were associated with reflux-related symptoms. Because castration-resistant prostate cancer (CRPC) does not respond to androgen deprivation therapy and has a very poor prognosis, it is critical to identify a prognostic indicator for predicting high-risk patients who will develop CRPC. Here, we report a dataset of whole genomes from four pairs of primary prostate cancer (PC) and CRPC samples. The analysis of the paired PC and CRPC samples in the whole-genome data showed that the average number of somatic mutations per patients was 7,927 in CRPC tissues compared with primary PC tissues (range, 1,691 to 21,705). Our whole-genome sequencing data of primary PC and CRPC may be useful for understanding the genomic changes and molecular mechanisms that occur during the progression from PC to CRPC.

Background/Aims: We aimed to clarify clinical factors which suggest the possibility of neoplastic polyps of gallbladder. Methods: Ninety-five resected gallbladders with polyps were included. Size and number of the polyps were obtained by preoperative ultrasonography. Histollogically, the polyps were classified into non-neoplastic (cholesterol and inflammatory or hyperplastic polyp) and neoplastic lesions (adenoma and adenocarcinoma). Clinical features, maximum diameter and number of the polyps were compared between the two groups. Results: Of 95 patients, non-neoplastic polyps were observed in 65.3% and neoplastic polyps were observed in 34.7%. We found cholesterol polyp in 47.4%, inflammatory or hyperplastic polyp in 17.9%, adenoma in 25.2% and adenocarcinoma in 9.5%. Two groups showed no difference in age, sex, body mass index, cholesterol, triglyceride, glucose, ALT, HBsAg carriage and the number of polyps. Among the patients with non-neoplastic polyps, 70.2% was under 60 years, while 72.7% of the patients with neoplastic polyps was over 60 years (p$lt;0.05). The size of polyps was under 10 mm in 88.9% of non-neoplastic polyps, and between 10 and 15 mm in 40% of neoplastic leisions. Moreover, 68.4% of neoplastic polyps exceeds 15 mm in diameter (p$lt;0.05). Conclusions: The size of polyp (≥10 mm) and patient's age (≥60 years) could be indicators for neoplastic polyps.
