<P><I>BACKGROUND:</I> Although pazopanib treatment has become the standard chemotherapy in salvage setting for metastatic sarcoma patients, most patients progress after pazopanib treatment in 4 to 6 months. After failure to pazopanib, patients have limited options for treatment. Therefore, subsequent therapy in patients who failed to pazopanib is urgently needed and the use of patient derived cells or patient derived tumors for accompanying testing with various pharmacological inhibitors could offer additional treatment options for these patients. <I>METHODS:</I> Patient derived tumor cells were collected from ascites at the time of progression to pazopanib and a 13-drug panel was tested for drug sensitivity. We confirmed the results using <I>in vitro</I> cell viability assay and immunoblot assay. We also performed the genomic profiling of PDX model. <I>RESULTS:</I> The growth of patient derived tumor cells was significantly reduced by exposure to 1.0 μM AZD2014 compared with control (control versus AZD2014, mean growth = 100.0% vs 16.04%, difference = 83.96%, 95% CI = 70.01% to 97.92%, <I>P</I> = .0435). Similarly, 1.0 μM BEZ235 profoundly inhibited tumor cell growth <I>in vitro</I> when compared to control (control versus BEZ235, mean growth = 100.0% vs 7.308%, difference = 92.69%, 95% CI = 78.87% to 106.5%, <I>P</I> < .0001). Despite the presence of CDK4 amplification in the patient-derived tumor cells, LEE011 did not considerably inhibit cell proliferation when compared with control (control vs LEE011, mean growth = 100.0% vs 80.23%, difference = 19.77%, 95% CI = 1.828% to 37.72%, <I>P</I> = .0377). The immunoblot analysis showed that BEZ235 treatment decreased pAKT, pmTOR and pERK whereas AZD2014 decreased only pmTOR. <I>CONCLUSION:</I> Taken together, upregulation of mTOR/AKT pathway in sarcoma patient derived cells was considerably inhibited by the treatment of AZD2014 and BEZ235 with downregulation of AKT pathway (greater extent for BEZ235). These molecules may be considered as treatment option in STS patient who have failed to pazopanib in the context of clinical trials.</P>
Discordance in Spirometric Interpretations Based on Korean and Non-Korean Reference Equations

Objectives: Korean regression models for spirometric reference values are different from those of other ethnic groups. The purpose of this study was to evaluate discordance in spirometric interpretations based on using Korean reference equations versus non-Korean reference equations. Methods: Spirometry was performed on 825 Korean male workers from April 2009 to November 2011. The spirometric patterns and disease severity were evaluated using two Korean equations (Choi’s and Lee’s) and three equations for Caucasians (NHANES III, Morris’s, and Knudson’s), and the results of Choi’s equation were compared with the non-Korean equations. The spirometric patterns were defined as normal, restrictive, and mild and moderate obstructive. Results: The mean differences in the FEV1% and FVC% between the two Korean equations were 2.0 ± 1.3% and 3.5 ± 2.2%, respectively. Morris’s equation had the greatest difference in the FEV1% from Choi’s equation: 32.9 ± 8.5%. Knudson’s equation had the greatest difference in the FVC% from Choi’s equation: 10.5 ± 6.5%. Conclusions: The two Korean equations shared similar characteristics of spirometric interpretation. The spirometric interpretations of Choi’s equation were significantly different from those of Morris’s equation and Knudson’s equation.
다공성 여재를 이용한 축산폐수의 영양물질 제거에 관한 연구

The fixed bio-reactor with the porous media was used to remove the nutrients in livestock wastewater. The dilution ratio and the reaction time for the removal efficiency of the nutrients in livestock wastewater were made a thorough investigation. The removal efficiency of total nitrogen(T-N) and total phosphorus(T-P) was 25 to 57% and 19 to 41% for treatment of 3 to 6 hour respectively, and >84.9% and >61.9% for treatment of 12 hour respectively. Therefore the reaction time for the effective removal of the nutrients in livestock wastewater must be more than 12 hour. Also the removal efficiency of total nitrogen(T-N) and total phosphorus(T-P) depends on the dilution ratio. As the dilution ratio increases, the reaction rate for the removal of nutrients also increases. By using the fixed bio-reactor with the porous media, it is possible to remove the nutrients in livestock waste water effectively and successively.
Primary Hepatosplenic Tuberculosis in an Immunocompetent Adult and Domestic Literature Review

Hepatic or splenic involvement of tuberculosis without other disseminated lesions is a very uncommon form of extrapulmonary tuberculosis, especially in an immunocompetent adult. We report a case of a 25 year-old-man who developed primary hepatosplenic tuberculosis not associated with the lung or other distant organs. He was initially diagnosed with esophageal candidiasis in local clinic. A computed tomographic scan of the abdomen showed hepatosplenomegaly and multiple microabscesses in the spleen. Our initial diagnosis, based on the clinical feature and radiologic findings, was hepatosplenic candidiasis. However, histopathology of the liver specimens revealed chronic granuloma with central caseous necrosis, strongly suggestive of tuberculosis. Although rare, splenic tuberculosis should be considered in the differential diagnosis of splenic abscess, especially in countries where tuberculosis is endemic.

When calculating the storm runoff from road surface, it is recommendable to introduce the critical duration of rainfall by means of the RUNOFF (SWMM) model, or the Rational method with the travel time values of the kinematic wave' or Kerby's equation modified by multiplying the correction factor of 2.3 or 1.1, respectively. But the travel time of 5-10 minutes suggested in the Guide Manual for Sewer Design is too long to use for calculating road surface runoff. The peak runoff rate increases as the road sectional slope increases, whereas it shows the opposite results for an increase in the longitudinal slope. Also, the effects of the width and length of the road on the surface runoff rates increases greatly, as their values increase.

The objective of this research is to develop a measurement error compensation method for On-Machine Measurement(OMM) process based on a closed-loop configuration. Geometric error of vertical machining center arc measured using ball-bar system, and probing errors are measured using master ball. The errors are represented using homogeneous trans-formation matrices and the closed-loop configuration method is applied to calculate 3-dimensional errors. To verify the effectiveness of the method proposed in this research, compensated results are compared to the data using CMM process, and the results are analyzed. The results show the proposed method can be applied in OMM process to make the measured data more reliable.
코일과 클립을 선택적으로 적용해서 성공적으로 치료했던 다발성뇌동맥류 1례 : A case report

The author report a case of multiple cerebral aneurysm successfully treated with an appropricate selection of surgical clipping and coil embolization. A 64 years old female patient was admitted with stuporous consciousness. Initial 3D CT angiography showed diffuse subrarachnoid hemorrhage on the basal cisterns, saccular aneurysms on the terminal basilar artery and right middle cerebral artery(MCA). Converntional angiography confirmed the ruptured aneurysm was basilar one. Coil embolization was perfromed on the basilar aneurysm with 6 Guglielmi detachable coils(GDC) on the 3 days after ictus. Sugical clipping was done on the unruptured right MCA aneurysm on 10 days after ictus without any morbidity. Each treatment modalities can be appropriate indicated for the coiling or clipping aneurysms.
