      • Gas Chromatography를 이용한 Methoxime tert.-butyldimethylsilyl 유도체로서 α-Keto Acids의 분석에 관한 연구

        Abstract-Thirteen α-keto acids which are biochemically important were converted to methoxime(MO) derivatives with MOX and then to tert.-butyldimethylsilyl(TBDMS) derivatives using MTBS-TFA in isooctance prior to dual capillary column GC analysis. Single symmetric peaks were obtained for each MO TBDMS α-keto acids on the dual chromatograms. Optimum methoximation required heating at 60℃ for 2 hours and TBDMS derivatization required heating for 30 minutes. MO α-keto acids were stable during the tert.-butyldimethylsilylation and the methoximation of carbonyl group prevented silylation of enols. Mass spectrometric analysis of MO TBDMS derivatives was performed for structure identification. Base peaks at [M-57], intense ions at m/z=89, and weak ions at [M-15] were present in all spectra. The ions at m/z=89 were characteristic for α-keto acids.

        일렬 자가조혈모세포이식을 받은 다발성 골수종 환자에서 발생한 뇌 톡소포자충증 1례

        Toxoplasmosis is a rare but fatal complication in hematopoietic stem cell transplant recipients, usually associated with allogeneic hematopoietic stem cell transplantation (HSCT). We report a case of cerebral toxoplasmosis in a patient with multiple myeloma, following tandem autologous stem cell transplantation. A 55-year-old Korean male presented with weakness in both legs that had progressed to both arms. A magnetic resonance imaging scan of the brain revealed multiple, variablesized ring-enhancing lesions with surrounding edema in the cerebral hemispheres and brain stem. Stereotactic biopsy revealed bradyzoites of Toxoplasma gondii in the brain tissue. The patient received trimethoprim-sulfamethoxazole, followed by pyrimethamine and sulfadiazine, accompanying treatment for progressive multiple myeloma. Cerebral toxoplasmosis should be considered as one of the differential diagnoses in patients with neurologic signs following autologous HSCT.

        단기교육으로 시행가능한 심실기능평가법

        Background: The initial history, physical examination, and ECG assessment should focus on identification of potentially serious noncardiac or cardiac disorders, including coronary artery disease, congestive heart failure, and electrical instability at the emergency room. additionally, it is essential to define disease severity, stability and need for emergency therapy. echocardiography is a useful tool for this purpose. especially Doppler echocardiography may be more sensitive and time-saving diagnostic tool for the evaluation of patients presenting with cardiogenic symptoms. So we evaluate the feasibility of the echocardiographic measurement by emergency physicain after short-term course. Method and Results: Twenty volunteers(10 male, 38.8±9.3 years) were included in the study for measurement of myocardial performance index and established parameters of ventricular function using conventional echo-Doppler methods. Myocardial performance index: (ICT+IRT)/ET, was obtained by subtracting ejection time(ET) from the interval between cessation and onset of the mitral inflow velocity to give the sum of isovolumic contraction time(ICT) and isovolumic relaxation time(IRT). The most of mean values of echocardiogrphic parameters were not significantly different between those of cardiologist and those of emergency physicians(p<0.01). The duration for measuring myocardial performance index was shortest among echocardiographic parameters. the validity of echocardiographic parameters measured by emergency physicians was proved relatively good. Conclusion: It is proved to be feasible for emergency physician to perform echocardiographic evaluation of ventricular function after short-term course

        반복적인 부비동염으로 내원한 Good 증후군 1예

        Good 증후군은 흉선종과 면역결핍이 동반되는 질환으로, B세포의 감소로 인한 범저감마글로불린혈증과 CD4^(+)T세포의 감소로 인한 세포성 면역저하가 동시에 나타나는 것을 특징으로 한다. 본 증례는 18개월 전부터 발생한 반복적인 부비동염을 주소로 내원한 43세 남자 환자로, 내원 1년 전 흉선종절제수술을 시행받았으며, 면역학적 검사상 범저감마글로불린혈증과 T세포의 감소 등의 이상 소견이 발견되었다. 정주 면역글로불린과 경험적 항생제(amoxicillin/clavulanic acid) 사용 후 부비동염이 호전되었으며, 예방접종과 정기적인 면역글로불린 투여 후 특이 합병증없이 경과 관찰 중이다. Good's syndrome is the association of thymoma with immunodeficiency, characterized by hypogammaglobulinemia, B-cell lymphopenia and variably defects in cellular immunity with CD4^(+) T-cell lymphopenia and an inverted CD4^(+):CD8^(+) T-cell ratio. We report a 43-year-old male patient who presented with a 18-month history of productive cough and postnasal drip. One year ago, he underwent the operation for resection of a thymoma. Despite of appropriate management, sinusitis relapsed multiple times. He was found to have hypogammaglobulinemia with nearly absent B cells(4/μL). The CD4^(+) T-cell count was 554/μL with an inverted CD4^(+):CD8^(+) T-cell ratio of 0.6. His symptoms and signs improved with antibiotic treatment and monthly administration of intravenous immunoglobulin (IVIG, 400 mg/kg).

        치과 재료용 NiTi 합금의 특성에 대한 Ag 첨가의 영향

        Equiatomic and near-equiatomic nickel-titanium alloys have shape memory effect and superelasticity. However nickel-titanium alloys are extremely sensitive to the precise nickel-titanium ratio and alloying additions. There are many reports on the alloying additions such as Fe, Al, Cr, Co, V, Pt, Pd, Zr, Hf, Nb and Cu. The purpose of this study was to investigate the effect of silver addition to nickel-titanium alloy for dental and medical application.Arc melting process was used to fabricate nickel-titanium alloys. The casts were heat-treated in a vacuum furnace at 950℃ for 72 hours to homogenize their composition. Subsequently, they were hot-rolled at 950℃ to obtain the plate samples and annealed in a vacuum furnace at 950℃. To investigate the properties of nickel-titanium alloys, phases, transformation temperature, compositions, corrosion resistance and hardness were evaluated using X-ray diffractometer, differential scanning calorimeter, energy dispersive spectroscopy or atomic absorption spectroscopy, potentiostat and micro-vickers hardness tester, respectively. NiTiAg alloys showed the low silver recovery rate for the cast due to its low evaporation temperature, and showed low silver solubility to NiTi alloy. Silver addition to NiTi alloy increased transition temperature range (TTR) above 100℃ and stabilized martensitic phase(monoclinic structure) at room temperature because Ms temperature was above room temperature. Martensitic and austenitic phase existed in x-ray diffraction pattern of solution annealed NiTiAg alloys. The Silver addition is considered to improve corrosion resistance and change largely the mechanical properties depending upon the amount of alloying addition and types of addition elements.

      • 자일리톨과 탄수화물의 Streptococcus mutans에 대한 효과

        Xylitol is a 5-carbons carbohydrate, which can be replacedwith sucrose for preventing caries. The replication of Streptococcus mutans and its formation of artificial plaque were studied in the media containing xylitol. The combined effect of xylitol and other carbohydrates on Streptococcus mutans wasalso studied. The replication of Streptococcus mutans was inhibited according to the increased concentration of xylitol.Streptococcus mutans replicated at the initial stage ofincubation in the media containing glucose. fructose of loctose, while replicating from the beginning of incubation in the media containing fructose as combining with xylitol. The formation of artificial plaque by Streptococcus mutans was significantly reduced in the media containing with xylitol and fructose. These results indicated that the replication of Streptococcus mutans was inhibited according to the increased concentration of xylitol, and the formation fo artificial plaque by Streptococcus mutans was significantly inhibited in the media containing xylitol and fructose.

      • 3차원 CFD 모형을 이용한 원형 교각의 국부세굴 특성 해석

        Scour is a natural phenomenon, which occurs whenever a solid object is placed on loose sedimentary material, such as sand, in moving water. The flow of water is close to any such obstruction causes local increases in water velocity together with eddies and vortices. The flow phenomena are complex due to the presence of a boundary layer as well as and adverse pressure gradient set up by the bridge pier. Many researchers have conducted a vast number of experiments in laboratory flumes to investigate the local scour depth around a bridge pier. A few empirical formulas predicting the maximum scout depth have been developed under various experimental conditions. However, most of the experiments have been carried out in flumes under idealized conditions. such as steady flow, uniform sediment, simplified geometry, etc. The purpose of the study is to simulate accurately the flow field and scouring processes using a three-dimensional numerical flow model. In this study, for analyzing the mechanism of scouring process, CFD(computational fluid dynamics) model is used, based on the VOF(Volume of fluid) theory and finite differential equation. For the flow model, the simulated three-dimensional flatbed flow field(without bed slop) is compared with experimental data obtained by established scour equation and data. The results showed that 3D scouring flow field simulated by CFD model have almost the same phasis as experimental results.

        타이타늄 임플란트와 상부구조물의 갈바닉 쌍의 전기화학적 특성

        For the aesthetic and functional restoration of edentulous patients, recently dental implants have been used widely. Dental implants and restorative alloys are required with the high corrosion resistance. Suprastructure and implant with different compositions, which are in electrical contact, may develop galvanic or coupled corrosion problems. Besides galvanic corrosion, crevice and pitting corrosion may occur in the marginal gap or the gap between dental implant assemblies. In this study, Ti implants, gold, silver-palladium, cobalt-chromium and nickel-chromium suprastructures were used to investigate their galvanic and crevice corrosion of suprastructure/Ti implant couples. Potentiodynamic and potentiostaic testing were performed in artificial saliva at 37℃. Potentiodynamic testing was carried out at the potential scan rate of 1 ㎷/sec and in the range of -600∼1600 ㎷(SCE). Potentiostatic testing such as open circuit potential and current densities at -250, 0 and 250 ㎷(SCE) was performed for 5000 seconds in artificial saliva. After electrochemical testing, the surface and cross-sectional morphologies of the samples were observed using optical microscopy. Potentiodynamic test results indicated that suprastructure/Tim implant couples exhibited the passive current density range of 0.5∼12 ㎂/㎠, Ti abutment/Tim implant and gold/Tim implant couples exhibited relatively low passive current density, Co-Cr/Tim implant couples the highest. Co-Cr and Ni-Cr/Tim implant couples showed the breakdown potentials of 700 and 570 ㎷(SCE), respectively. Open circuit potentials of silver, Ti abutment, gold, Ni-Cr and Co-Cr/Ti implant couples were -93.2±93.9, -123.7±58.8, -140.0±80.6, -223.5±35.1, and -312.7±29.8, respectively and stable according to immersion time. The couples exhibited cathodic current densities at -250 ㎷(SCE), especially, gold and silver alloys showed the high cathodic current densities of ±3.18 and ±6.63 ㎂/㎠, respectively. At 250 ㎷(SCE), Ti abutment/Ti implant couples exhibited the lowest current density of 9.48×10^-2 ㎂/㎠, and gold, Ni-Cr, Co-Cr and silver/Ti implant couples exhibited 0.313, 1.27, 5.60 and 8.06 ㎂/㎠, respectively. The couples exhibited relatively low current densities at 0 ㎷(SCE). The photographs after electrochemical testing showed crevice or pitting corrosion in the marginal gap and suprastructure surface. Although Co-Cr/Ti implant couples among the tested samples had the possibility of the galvanic corrosion, its degree was not significant.
