        Evaluation of the Efficacy and Safety of DA-9601 versus Its New Formulation, DA-5204, in Patients with Gastritis: Phase III, Randomized, Double-Blind, Non-Inferiority Study

        <P>This study compared the efficacy of DA-9601 (Dong-A ST Co., Seoul, Korea) and its new formulation, DA-5204 (Dong-A ST Co.), for treating erosive gastritis. This phase III, randomized, multicenter, double-blind, non-inferiority trial randomly assigned 434 patients with endoscopically proven gastric mucosal erosions into two groups: DA-9601 3 times daily or DA-5,204 twice daily for 2 weeks. The final analysis included 421 patients (DA-5204, 209; DA-9601, 212). The primary endpoint (rate of effective gastric erosion healing) and secondary endpoints (cure rate of endoscopic erosion and gastrointestinal [GI] symptom relief) were assessed using endoscopy after the treatment. Drug-related adverse events (AEs), including GI symptoms, were also compared. At week 2, gastric healing rates with DA-5204 and DA-9601 were 42.1% (88/209) and 42.5% (90/212), respectively. The difference between the groups was −0.4% (95% confidence interval, −9.8% to 9.1%), which was above the non-inferiority margin of −14%. The cure rate of gastric erosion in both groups was 37.3%. The improvement rates of GI symptoms with DA-5204 and DA-9601 were 40.4% and 40.8%, respectively. There were no statistically significant differences between the two groups in both secondary endpoints. AEs were reported in 18 (8.4%) patients in the DA-5204 group and 19 (8.8%) in the DA-9601 group. Rates of AE were not different between the two groups. No serious AE or adverse drug reaction (ADR) occurred. These results demonstrate the non-inferiority of DA-5204 compared to DA-9601.       • P164 : A clinical and statistical study of cutaneous premalignant lesions in Busan city and the eastern Gyeongnam province over 19 years

        Background: Premalignant lesions has recently continuously increased in Koreans. However, there has been little research into the distribution and incidence of cutaneous premalignant lesions in the south-eastern part of korea Objectives: To analyze recent trends in incidence and clinical patterns of cutaneous premalignant lesions in Busan city and the eastern Gyeongnam province Methods: We reviewed 1,292 cases (3,651 lesions) of cutaneous premalignant lesions that include actinic keratosis (AK) and bowen’s disease (BD) at the Department of Dermatology in Dong-A University Hospital Results: The average annual incidence of cutaneous premalignant lesions among total outpatients was 1.82% and has continuously increased from 0.70% to 4.25% over the last 19 years. The most common cutaneous premalignant lesionwas AK (75.85%), followed by BD (24.15%). The mean age of onset was 68.76 years. A major skin cancer was found in 15.79% of patients with cutaneous premalignant lesions, including SCC (11.54%), BCC (6.09%), and malignant melanoma (0.64%). Additionally, 59.13% of the patients had a single lesion, while 40.87% had multiple lesions. Age, occupation-related exposure to ultraviolet radiation, and a past history of skin cancer were higher in the patients with multiple lesions than single lesion Conclusion: The incidence of cutaneous premalignant lesions has gradually increased in Busan city and the eastern Gyeongnam Province, which was relatively higher than incidence in other areas

        壓挫시킨 家兎 軟骨의 生存

        The deviated nose regardless whether it is due to traumatic or congenital origin results in troublesome symptoms coming from airway obstruction. Numerous methods for straightening the severely deviated septum have been described by many authors but none is wholly satisfactory. A new method that the crooked septal cartilage is being flattened by crushing and reimplanting into the septal pocket is introduced. The authors are desirous of knowing about the fate of the crushed cartilage graft. Forty rabbits weighing 2 to 3 kg were divided into 2 groups, septal group and conchal group. The cartilages were obtained from the nasal septum and ear; these cartilages were placed under the paraspinal subcutaneous tissue. Prior to paraspinal iplantation the obtained cartilage was halved; one piece of cartilage being crushed by Cottle's cartilage crusher before its placement into the recipient site and the other was placed without crushing. The cartilage specimens were removed at 6 and 12 weeks postoperatively and these were examined by the naked eyes and light microscope. The results were as follows: 1. The fissures and cleavages of cartilage developed during crushing process are restored to normal by 6 weeks postoperatively. 2. All of the ujncrushed cartilages remained viable in the subcutaneous pockets. The crushed cartilages not only appeared viable but also induced a new cartilage formation beneath the perihondrium at 12 weeks postoperatively and the thickness being increased compaired to the cartilage before crushing. 3. Crushed cartilage remained flat at 12 weeks postoperatively. 4. The degree of strength of the crushed cartilage is increased due to new cartilage formation and fibrosis of perichondrium at 12 weeks postoperatively. These findings support the clinical impression that the crushed autogenous cartilage is viable and can be reshaped according to the contour of the recipient site.

        압좌시킨 이개 연골을 이용한 안저 골결손의 재건

        A numerous materials have been used for reestablishing continuity of the orbital floor in orbital floor fracture. Traditionally autogeneous bone has been utilized for reconstructing orbital floor defect but autogeneous bone has criticized for unpredictable resorption, donor site morbidity and difficulty in shaping and fixation. Alloplastic materials have gained some popularity because of their availability and ease of use but they disadvantages like infection. extrusion and implant displacement. There is an alternative to bone grafts and inorganic implants fir reconstruction of orbital floor defect; auricular conchal cartilage, however, has convolution so the authors have made the conchal cartilage flat and malleable by crusher. Crushed autogenuous conchal cartilage has proved to be a material that is easy to obtain and shape and can reliably support the orbit without evidence of resorption. Crushed autogeneous conchal cartilage offers an attractive alternative to inorganic implants and bone grafts in properly selected patient for orbital floor reconstruction.

        하악 제3대구치가 하악 우각부골절 정복술후 감염에 미치는 영향에 관한 연구

        Any fracture passing through the socket of a teeth is compounded intraorally, even if the fracture is not displaced and the tooth is firm in its socket. Before the advent of antibiotic therapy the danger of infection in a compounded fracture posed severe problems in treatment. Infection is reduced by antibiotic therapy but prolonged use of antibiotics is not justified in an attempt to save a tooth which might eventually be sacrificed. There is still controversy in the management of third molar in mandibualr angle fracture, particularly in regard to their retention or removal at the time of fracture treatment. So we surveyed the 159 patients who were treated with open reduction of mandibular angle fracture containing third molar in fracture line, and compared with the postoperative infection rate depending on time intervals between injury and operation, eruption state of third molar, non-extraction or extraction of third molar related to eruption state, non-extraction or extraction of third molar related to condiition of third molar and its surrounding periodontium and were to propose treatment guidline of third molar in mandibular angle fracture The results obtained were as follows : 1.There were no statistical significance between the time from injury to operation and postoperative infection. 2.There were no statistical significance between eruption state of third molar and postoperative infection. 3.In case of retention of the third molar, there were no statistical significance between eruption state of third molar and postoperative infection, but in case of extraction, postoperative infection was high rate in complete impacted cases. 4.There were no statistical significance between non-extraction or extraction of third molar and postoperative infection depending on condition of third molar. There are no difference in infection rate statistically according to the time from injury to operation, eruption state and condition of third molar, but retention of third molar revealed lowered infection rate in completely impacted cases. By terms of the manegement of third molar, we should extract or preserve third molar in the line of the mandibular angle fracture according to possibility of infection.

        4세 유아에 발생한 몰입성 하마종(plunging ranula); 증례보고

        Whereas oral ranula is relatively common and presents as a cyst in the mouth, the plunging ranula is rare and manifests itself as a mass in the neck with or without an associated oral lesion. When there is a soft anterior neck swelling without oral swelling, diagnosis is still difficult. In such a case, clinical suspicion may be low and the pathologist may be misled by the histologic appearance. Recognition of the diagnosis of plunging ranula is essential for the correct treatment of these lesions We report a case of a 4-year-old girl and review the literatures to discuss the differential diagnosis and treatment modalities.

        흰쥐의 파라쿼트 중독모델에서 폐손상에 대한 N-acetylcysteine과 Methylprednisolone의 효과

        Background: This study evaluated the inhibitory effects of N-acetylcysteine(NAC) and methylprednisolone on lung injury in the paraquat-poisoned rat model. Methods: Sixty rats were divided into four groups(n=15 in each group) accordingly to the drug administered : group I, only intraperitoneally injected paraquat (20 mg/kg); group II, intraperitoneally injected paraquat and NAC(300 mg/kg); group III, intraperitoneally injected paraquat and methylprednisolone(60 mg/kg); and group IV, intraperitoneally injected paraquat, NAC(300 mg/kg), and methylprednisolone(60 mg/kg). On the 7th day after injection, the survival rate of experimental rats and the positive area of collagen fiber in the injured lung stained by Masson's trichroms were evaluated. Results: 1. There were no differences in the 7-day survival rates for the four groups. 2, The percent of collagen fiber for group II(6.3 ± 4.7%) was significantly decreased in comparison with that for group I (14.4±9.7%). 3. The percent of collagen fiber for Group III(13.2±5.9%) was not significantly different from that for group I(14.4±9.7%). 4. The percent of collagen fiber for Group IV(6.9±4.6%) was significantly decreased in comparison with that for group I, but was not different from that for group II. Conclusion: These results suggest that NAC protects against pulmonary fibrosis in paraquat-poisoned rats whereas methylprednisolone does not protect against pulmonary fibrosis.

      • 슬관절에 히야루론산 주입후 발생한 활막성 연골종증 : 증례 보고

        활막성 연골종증은 비교적 드문 양성 종양으로 관절내 활액막 결체조직의 양성 반응성 이형성에 의해 여러 개의 연골성 결절을 형성하고 이것이 관절내로 유리되어 유리체를 형성하는 질환으로 주로 슬관절에 발생한다. 활막성 연골종증의 정확한 발생 기전은 아직 밝혀지지 않았으나 외상, 감염 등이 원인으로 제시되고 있다. 저자들은 슬관절 내 히야루론산 주입후 발생한 슬관절내 활막성 연골종증을 1 례 경험하였으며, 관절경적 제거술을 통해 만족스런 결과를 얻었다. As a relatively rare benign tumor, synovial chondromatosis forms several cartilaginous nodules by the benign reactive metaplasia of synovial connective tissues within joints, which are loosed into the joints and cause a disease forming loose body mainly in knee joint. Accurate mechanism of synovial chondromatosis has not been clarified yet; however, trauma, infections and others have been suggested as its possible causes. The authors of this study experienced an example of a patient with synovial chondromatosis in knee joint occurred after the injection of hyaluronic acid in the joints and had a satisfactory outcome through the arthroscopic resection.

        In this study, examined the cutting characteristics of alumuminum alloy AC8A-T6 that is used to present car piston materials. And in been holding materials machining empirically escape as result that experiment comparison changing the cutting speed and feed on various condition to choose efficient machining condition. The following results can be summarized from this research. 1.As the cutting speed decreased, principal cutting force and thrust cutting force is increased, and reason that cutting force interacts greatly in the low cutting speed is thought by result by BUE's stabilization. 2.The feed speed and cutting speed increase, friction factor is decrescent and the cause appeared the thrust cutting force is fallen than cutting force relatively because chip flow according to increase of the feed rate is constraint. 3.Though specific cutting resistance grows cutting area and the feed rate are few, the cause was expose that shear angle decreases by rake face of tool gets into negative angle remarkably as wear of a cutting tool or defect part of workpiece is cut. 4.Cutting speed do greatly depth of cut is slow, surface roughness examined closely through an experiment that becomes bad, and know that it can get good surface that process cutting speed because do feed rate by 0.1㎜/rev low more than 250m/min to get good surface roughness can.
