        The importance of psychological factors in dermatologic disorders has been recognized for several decades. However, there have been few reports on the management of psychodermatologic disorders. Psychodermatologic conditions can be divided into three broad subgroup in the dermatologic point of view ; first, psychiatric disorders manifesting as dermatological symptom, second, dermatoses mainly caused by emotional factors, i.e., stress, or determined by complex psychophysiological mechanism, and third, psychological symptoms secondary to dermatological conditions, i.e., anxiety or depression. This article describes first two major categories of psychodermatologic disorders and attempts to provide brief guideline for each psychodermatologic condition in view of the clinical interface between dermatology and psychiatry.

        압축력을 받는 고장력 볼트 이음부의 미끄러짐 거동

        본 논문에서는 압축력을 받는 고장력 볼트 마찰이음부의 미끄러짐 거동을 3차원 유한요소 해석 및 실험을 통하여 규명하였다. 모재의 두께가 고장력 불트이음부에 끼치는 영향을 미끄러짐 하중, 볼트의 변형 및 파괴하중과의 관계와 함께 파악하였다. 초기 미끄러짐 하중 이후의 볼트의 강성을 고려한 이음부의 거동 모델을 제시하고 유한요소해석 및 실험을 통하여 비교, 분석하였다. 범용 유한요소해석 프로그램인 ABAQUS에서 지원되는 고체요소를 사용하여 해석모델을 작성하였고 모재 사이의 마찰 및 미끄러짐이 발생한 이후 볼트와 모재 사이의 마찰 등을 고려하였다. 기존의 문헌에 제시된 여러 가지 강재의 응력-변형도 관계를 적용하였으며 미끄러짐 변위와 볼트 주변의 축응력들을 비교하였다. 모재의 두께가 볼트의 직경보다 작은 경우에는 압축력에 의한 휨좌굴에 시험체에 발생하였고 모재의 두께가 볼트 직경보다 두꺼운 경우에는 볼트의 전단파괴가 이음부의 극한강도를 나타냄을 파악하였다. In this study, the slip behavior of high-tension bolted joints subjected to compression force is investigated through 3D finite element analysis and experiments. The relation with sliding load, bolt deformation, and failure load are studied with the metal thickness affecting the bolted joint. The post-sliding behavior considering bolt stiffness is presented and compared with the results by finite element and experiments. The finite element model is constructed by solid elements in ABAQUS, in consideration of all the friction effects between metal plates and bolts. The stress-strain relations in the literature are used, and the sliding displacements and axial stresses around the bolt connection are investigated. The flexural buckling of species happened when the plate thickness is less than the bolt diameter. However, the shear failures of bolt occurred in the opposite case.

      • 한국어판 Alexithymia 척도의 개발에 관한 연구 - SSPS(Schalling-Sifneos Personality Scale)와 TAS(Toronto Alexithymia Scale)의 신뢰도와 타당도 -

        This paper describes the development of Korean version of two widely used self-report measure of alexithymia-Schalling-Sifneos Personality Scale (SSPS) and Toronto Alexithymia Scale(TAS), and present the preliminary results of their reliability and validity tested to an Korean college student sample(N=206). TAS showed moderate to acceptable internal consistancy with 0.26 of mean item-total correlation and 0.754 of Cronbach's alpha coefficient, while those parameters for SSPS turned out inconsistent. However, results of test-retest reliability for both scale were excellent. Factor analysis revealed tow factors for TAS and three factors for SSPS ; the four-factor structure of TAS was quite congruent with previously known alexithymaia construct, while, again, SSPS showed intermixed factor structure. TAS scores correlated fairly well with somatization scores enough to get concurrent validity while SSPS scores did not. Overall, Korean version of TAS demonstrated well matched findings in terms of internal consistency and factorial validity with original and Spanish version of TAS. The results suggest that Korean version of TAS could be a useful and reliable tool for the measurement in Korean population.

      • 자바 카드를 위한 통합 개발 환경

        이 논문에서는 자바 카드를 위한 통합 테스트 및 디버깅 환경에 대해 기술하고 있으며, 제안한 통합 개발 환경은 J-JCRE (Java language based Java Card Runtime Environment)와 Java Card API (Application Programming Interface)로 구성되어 있다. 또한, 개발 환경은 현재 두 가지 암호 알고리즘을 지원해주며, 스크립트 실행, 시스템 클래스의 소스 레벨 디버깅, 클라이언트/서버 skeleton 자동 생성 등 다양한 기능을 제공해준다. 제안한 통합 개발 환경을 이용함으로써 사용자들은 실제 자바 카드에 응용 프로그램을 탑재하기 전에 응용 프로그램 테스트 및 디버깅을 통해 프로그램상의 오류 및 에러를 미리 점검하여 수정할 수 있게 된다.

      • CT검사시 조영제의 혈관외유출 발생률 감소를 위한 방안 모색

        Purpose The purpose of this study was that we made a guiding principle in the CT scan with contrast media and have applied it in order to improve patient satisfaction and job satisfaction among the radiological technologists and nurses through decreasing the incidence of extravasation of contrast media during CT scan. Materials and methods To get data of extravasation we had analyzed the factor of extravasation by comparing the cases(January 2006~December 2007) with the cases (January~February 2008). Also we found out the decrease of extravasation when the guiding principle was applied(May~April 2008) to the CT scanning. Results Before we applied these guiding principle(January~February 2008), the frequency of getting extravasation was ten. However after we applied this(May~April 2008), It happened only twice in the total of 1,893 CT scan with contrast media which means the incidence of extravasation is only 0.1%. Therefore it was decreased by 80%. We prevented eleven incidences of extravasation before the patient had been given the CT scan with contrast media because of following the guiding principle during that time. Conclusion It is needed to follow the guiding principle especially for high risk patients who is over than 75 aged person, or has got anti-cancer treatment including radiation therapy, or has clinical renal failure or has difficulty of the injection due to regular hemodialysis. Also prompt treatment and well organized system will be needed for unexpected accident. It is considered to make a check list and to follow the guiding principle thoroughly for constant analysis and monitoring. To perform guiding principle nurses, radiological technologists and policlinic should be able to cooperate very well together.

        노인의 기능 체력과 경도 인지 장애와의 연관성

        In a cross-sectional design, this study examined the relationship between functional physical fitness and mild cognitive impairment (MCI) in older adults. A total of 306 older adults (91 men and 215 women) aged 65 years or older were recruited from our local community. A senior fitness test (SFT) was used to assess functional domains of physical fitness consisting of upper and lower body strength, endurance, upper and lower body flexibility, and agility/dynamic balance. Korean version of mini-mental state examine for dementia screening and short-form geriatric depression scale were used to assess global cognitive performance and depression, respectively. Age, sex, education, body mass index, percent body fat, blood lipids, glucose, insulin, and homeostasis model assessment of insulin resistance (HOMA-IR) were assessed. A composite score of physical fitness was calculated as a sum of z scores for each domain of the SFT parameters. Based on the composited score of physical fitness, subjects were classified as low fit (lower 25 percentile), moderate fit (middle 50 percentile), and high fit (high 25 percentile). Linear contrast analysis using one-way analysis of variance showed significant linear trends for age (p<0.001), education (p<0.001), cognitive function (p<0.001), and depression scale (p=0.006) across incremental composite score of physical fitness. Functional physical fitness was positively associated with age, years of education and global cognitive performance and negatively with depression scale. Logistic regression analyses showed that compared to the low fit group, the moderate (p=0.004) and high fit (p=0.010) groups had significantly lower odds ratios for having MCI even after adjusted for age, sex, education, and body fatness parameters. However, the odds ratios of the moderate (p=0.101) and high fit (p=0.191) groups were not significant when additionally controlling for depression scale and HOMA-IR. The current findings suggest that physical fitness promotion along with treatments of depression and HOMA-IR should be key components of interventions to prevent and/or treat MCI with normal aging.

        Achondroplasia is the most common non-lethal skeletal dysplasia characterized by rhizomeric disproportionate short stature, enlarged head, midface hypoplasia, short hand, lordotic lumbar spine. Occurrence of narrow upper airway and obstructive sleep apnea is common due to relative adenoid and tonsil hypertrophy caused by midfacial hypoplasia. Restrictive respiratory insufficiency may be frequently accompanied due to characteristically small chest. We have experienced emergency tracheal intubation due to respiratory insufficiency occurred after extubation during emergence of a 50- year-old man with achondroplasia who underwent laparoscopic appendectomy under general anesthesia. Full recoverys of self respiration and mental function were checked prior to the extubation. We also review the literature on the anesthetic management of achondroplasia and discuss methods for avoiding complication.

        추출용매 및 발아시기에 따른 무순 추출물의 항산화 활성 비교

        본 연구에서는 무순을 발아시기별로 70% 에탄올, 80% 메탄올, 75% 아세톤, 물 등 용매종류에 따라 추출한 후 폴리페놀 함량과 전자 공여능, TBARS, 산화안정도 측정, 과산화물가 등 항산화특성을 확인하는 실험을 하였다. 폴리페놀 함량은 발아 4일째에서 에탄올추출물이 296.51 mg/g으로 가장 높은 것으로 나타났으며 다음으로 메탄올추출물이 219.39 mg/g으로 많았다. 발아 8일째에서는 에탄올추출물의 함량이 가장 높았으며 메탄올추출물과 아세톤추출물은 각각 197.72 mg/g, 200.45 mg/g으로 유의차가 없는 것으로 나타났다. DPPH 라디칼 소거능 측정에서 발아 4일째에 물추출물이 86.67%로 가장 높은 소거능을 보였으며 아세톤추출물에서 77.23%로 낮은 소거능을 보였다. 발아 8일째 라디칼 소거능은 아세톤추출물이 89.18%로 가장 높았으며 에탄올추출물이 70.14%로 가장 낮은 소거능을 보였다. 또 발아 12일째의 라디칼 소거능은 4일째와 비슷한 양상을 보였다. TBARS를 통한 지질과산화 억제효과 측정에서 발아 4일째 무순추출물의 TBARS값은 70% 에탄올추출물이 71.48%로 가장 높은 값을 보였으며 발아 8일째가 되면서 TBARS값이 상승하여 메탄올추출물이 78.99%로 높은 값을 보였고, 아세톤 추출물의 값이 가장 낮게 나타났다. 발아 12일이 되면서 TBARS값은 상대적으로 낮아졌으며, 발아 4일째와 유사한 경향의 값을 나타내었다. Rancimat에 의한 산화안정도 측정에서 발아 4일째의 산화유도기간은 메탄올추출물이 6.07시간으로 가장 높게 나타났고 항산화지표는 1.16의 수준을 보였다. 발아 8일째의 산화유도기간은 발아 4일째 산화유도기간보다 전반적으로 낮게 나타났으며, 메탄올추출물과 에탄올추출물, 아세톤추출물은 비슷한 경향을 보이며 유의차가 없었으나 물추출물의 산화유도기간이 낮게 나타나는 것으로 보였다. 발아 12일이 되면서 전반적인 산화유도기간이 5.25~5.91시간으로 낮게 나타나는 경향을 보였으며 메탄올추출물을 제외한 나머지 추출물은 발아 4일째와 유사한 패턴을 보였다. 항산화지표 또한 1.00~1.13의 수준을 나타냈다. 과산화물가 측정에서 무순추출물의 과산화물가는 저장기간이 길어질수록 유의적으로 증가하였다. 발아 4일째 저장기간 0일에서 각 추출물들이 3.02~4.21 meq/kg oil로 차이를 보이지 않았으며, 저장기간 60일에서 물추출물이 43.83 meq/kg oil과 메탄올추출물에서 45.42 meq/kg oil을 나타내어 두 추출물이 가장 낮은 값을 나타내 황산화 효과가 높았으며 발아 8일째와 발아 12일째의 무순추출물에서도 모두 비슷한 경향을 보이며 저장기간이 늘어날수록 과산화물가도 증가하는 것으로 나타났다. 본 연구 결과 무순추출물은 총 페놀 함량이 높고 항산화활성도 높게 나타나 천연항산화제와 같은 식품첨가물 및 식품보존제 등 기능성 소재로써 활용될 수 있을 것으로 사료된다. The purpose of this study is to investigate the total polyphenol and antioxidant activities of radish buds (Raphanus sativus L.) based on sprouting periods and extraction solvents in order to present basic data that are needed for using the radish buds as functional food material. The antioxidant activities were assessed by using various antioxidant models (DPPH, TBARS, Rancimat method, POV). The total polyphenol contents according to the extraction solvents were 84.11 and 296.51 mg/g, and the ethanol extract on day 4 of showed the highest value as 296.51 mg/g. As for DPPH radical-scavenging activity, on the day 4 of sprouting, water extracts indicated the highest scavenging activity by 86.67%, and the acetone extracts indicated a rather low scavenging activity as 77.23%. As for TBARS measurement of the radish bud extracts on day 4 of sprouting the extract of 70% ethanol was highest (71.48%). On day 8 of sprouting the TBARS value was increased and the methanol extract was highest (78.99%). As for the oxidative induction period on day 4 of sprouting in Rancimat measurement, the methanol extract was highest (6.07 hours) on day 4 and the antioxidant index was 1.16. On day 12 of sprouting, the general oxidative induction period tended to be reduced to 5.25 to 5.91 hours. In the peroxide value measurement on day 4 of sprouting and beginning of the storage, the extracts showed no difference between 3.02 meq/kg oil and 4.12 meq/kg oil, and on the day 60 of storage, the water extract (43.83 meq/kg oil) and the methanol extract (45.42 meq/kg oil) were lowest with higher antioxidant effect. In conclusion, the radish bud extract with higher total polyphenol contents and antioxidant activities may serve as functional material for food additives, such as natural antioxidants and food preserving agents.
