Comparison of Perioperative Blood Loss in Primary Non-cemented Total Hip Arthroplasty for Rapidly Destructive Coxarthrosis and Osteonecrosis of the Femoral Head

Purpose: The purpose of this study is to compare the perioperative blood loss in primary non-cemented total hip arthroplasty (THA) performed for rapidly destructive coxarthrosis (RDC) with the perioperative blood loss in primary non-cemented THA for typical osteonecrosis of the femoral head (ONFH). Materials and Methods: From January 2000 to December 2013, 19 patients were diagnosed with RDC (group 1) and 40 patients were diagnosed typical Ficat stage IV ONFH (group 2), comparison of perioperative blood loss between group 1 and group 2 in primary noncemented THA was done. Patients with preoperative usage of steroid or anticoagulants medication and with hemodynamic abnormal blood test results were excluded. The blood loss was measured up to the fifth post operation day and calculated with formula proposed by Mercuriali, Inghilleri and Nadler. Results: Non-compensated blood loss calculated in milliliters of red blood cells was 362 mL (standard deviation [SD], 187; range, 77-675) in group 1 and 180 mL (SD, 145; range, 53-519) in group 2. Compensated blood loss was 630 mL (SD, 180; range, 380-760) in group 1 and 503 mL (SD, 260; range, 190-1, 505) in group 2. The total blood loss after primary non-cemented THA is greater when surgery is performed for RDC than for ONFH, with the volume of 992 mL (SD, 300; range, 457-1, 434) in group 1 and 683 mL (SD, 360; range, 226-1, 975) in group 2 respectively. Conclusion: Total perioperative blood loss was significantly greater in RDC than in ONFH in primary noncemented THA.

국가균형발전과 지속가능한 미래를 위한 도시재생4.0 솔루션 제안

오랜 시간동안 다양한 이유로 해결이 어려웠던 지역 불균형 문제는 코로나19, 지방소멸, 인구절벽, 고령화 추세 등의 복합적인 배경 하에 한층 심화되고 있다. 시대적 여건 변화에 대응해 나가기 위해 정부와 각 지자체에서 적극적인 균형발전정책을 마련하고 있지만 여전히 우리는 난제 속에서 살아간다. 본 연구진은 국가균형발전을 저해하는 수많은 요인 중 지역 쇠퇴의 주범이 되는 빈집 문제에 주안점을 두고 지속가능한 방향으로 재생하기 위한 방안을 마련하고자 한다. 빈집의 발생과 증가는 이미 필수적으로 해결해야 할 지역현안으로 대두되고 있는데, 전남이나 제주 지역 등은 무려 10%에 달하는 빈집이 발생하는 등 상당히 심각한 상황이다. 예방하거나 해결되기 어려운 과제라는 이유로 장기간 방치해 놓아 점차 노후화 되고 있는 빈집들은 각종 사회적인 문제와 지역 슬럼화를 야기할 수 있다. 이에 따라 국내외 공공과 민간 부문에서는 재생이나 활성화를 위해 저소득층의 주거안정을 위한 정책으로 연결하거나 창업지원 등을 독려하는 등 빈집을 정비하여 지역 경제 활성화에 기여할 방안들을 모색 중이다. 지역 내 빈집들은 적절한 용도로 활용만 된다면, 지역을 재생할 수 있는 자산이자 수단이 되므로 본 연구진은 보다 장기적인 관점에서 전 가치사슬을 고려한 도시재생4.0 솔루션을 제안하고자 한다. 이는 기존의 도시재생 개념을 확대 적용하는 방향으로, 4차 산업혁명 기술을 적용하여 취약 지역의 경제적 자립성 향상과 지속가능한 미래 균형발전에 기여하게 될 것이라 기대한다.

Leptin: A Multifunctional Role as an Immunomodulator in Mycobacterial Lung Disease

Leptin is a 16 kDa protein which consists of 167 amino acids. Leptin is considered as one of the adipokines, secretedby white adipocytes, and is the product of the obese (ob) gene. Recently, leptin is recognized as the immuno-stimulatorwhich belongs to the same class of long chain helical cytokines such as interleukin (IL)-6. Leptin is related to the immuneresponses evoked by Mycobacterium tuberculosis infection. Thus, studies of association between immunomoleculesincluding leptin and tuberculosis may contribute to provide an essential solution regulating adverse immune responsesin several mycobacterial diseases. Leptin has a multifunctional role in the secretion of acute-phase cytokines includingIL-1β and tumor-necrosis factor-alpha (TNF-α), and links to T helper 1 (Th1) immune response. Moreover, the bindingof leptin to leptin receptor (LepR) is important in that this binding involves janus kinase/signal transducer and activatorof transcription (JAK/STAT) pathway. In addition, the activation of LepR mediates extra-cellular signal-regulated kinase(ERK) and phosphoinositide 3 kinase (PI3K) pathways. Furthermore, many studies suggest that leptin may play a criticalrole in respiratory diseases including chronic obstructive pulmonary disease (COPD) and asthma as well as tuberculosis. These findings indicate that leptin is one of the important regulators for immune responses in respiratory diseases. Weherein discuss the multifunctional role of leptin in mycobacterial lung disease, especially focusing on the related pathwayto immune responses.

외상팀이 활성화된 성인 중증 외상 환자의 전산화 단층 촬영에 의한 방사선 조사량 에 대한 분석

Purpose: Emergency physicians utilize multiple computed tomography (CT) scans in acute evaluation of patients with multiple traumas. By its nature, CT involves larger radiation doses than conventional X-ray films do. A significant association was reported between radiation dose and increasing risk of cancer in atomic bomb survivors and radiation workers. The object of this study was to investigate the amount (dose) of CT radiation in patients who activated the trauma team during the first 24 hours of their stay in the emergency department. Methods: This study was designed as a retrospective analysis of radiologic and medical data. We reviewed 39 trauma patients who activated the trauma team of our emergency department between January 2008 and April 2009. Individual radiation dose reports calculated by the CT scanner were used to determine radiation dose from each CT scan. Radiation doses calculated by CT scanners were converted to effective dose by multiplying by a conversion coefficient. Results: A total of 33 patients were enrolled. Among these patients, 24(72.7%) were male. The mean age was 41.88±16.13 years. The mean Revised Trauma Score was 6.94±1.16. The mean Injury severity score was 22.85±10.34. The mean number of total CT scans was 3.61±1.22. The median effective dose of the total CT scans was 68.81mSv, with an intraquartile range of 56.30 to 88.41 mSv. Conclusion: Trauma patients in the emergency department following trauma team activation are exposed to clinically significant radiation doses from CT imaging during the first 24 hours of their stay.

국가비상사태 전후의 문화정치와 지식 전유의 한 양상 - 이병주 소설을 중심으로

이 논문은 1960년대로부터 1970년대에 이르기까지, 1971년 비상사태를 전후한 시기의 지식인의 분화와 존재양상을 고찰해보고자 하였다. 당시의 지식인 그룹은 사회와 역사의 변화에 명민하게 반응하며 체제에 저항하 고 성찰하는 방식으로 존재했으나 다른 한편으로는 국가의 문화정치전략 에 의해 전유되면서 지배체제에 순응하는 양상을 보이기도 했다. 이병주의 소설은 이러한 양상을 확인케 하는 하나의 지표가 된다. 이병 주는 당대의 전형적인 지식인의 양상을 보이고 있었는데, 그는 자신의 소 설을 통해 자기보존의 논리를 내장한 위장적 형태의 세계에 대한 저항을 보여주었다. 또한 나르시즘적 영웅주의에 경도된 그는 민중에 대한 불신과 체제에 대한 순응적 양상을 보였다. 그럼에도 불구하고 그는 당시 대중들의 상당한 호응을 받았는데 이는 당시의 보수적 대중들이 가지고 있 던 성공과 발전에 대한 열망, 지식을 통한 세계 해석의 열망과 조우한 결 과이다. 본 연구는 이병주 소설을 통해 당대 지배 권력의 문화통치 전략에 순 응하면서 굴절되어가는 당대 지식인, 작가의 한 양상을 살핀다. 이로써 한 시대의 지식인-작가-지성이 감당해 내야할 사회적인 책무, 혹은 역할이 무 엇인지 질문케 한다는 점에서 그 의의가 있다. This paper examines the patterns of differentiation and presence around the intellectuals at the time of a state of emergency in 1971. At the time of the intellectuals were sensitive to changes in society and history. They criticize and resist state power. But on the other hand it was used by the political culture of the country strategy. On the other hand, they not only submit to state power, were mobilized in the country. Lee Byeong-ju novel is one of the indicators of Kerala verify these aspects. Lee Byeong-ju was showing typical aspects of contemporary intellectuals. But he showed resistance to the stomach forms a built-in logic of self-preservation the world through their stories. And he was lost in narcissism ever heroism. He looked adaptive aspects of the system and distrust for the people. Nevertheless, he received a considerable response from the public at the time. This is the result of his meeting with conservatism of the masses at the time. This study examines one aspect of contemporary intellectuals, writers go is refracted while conforming to the cultural strategy of the ruling era dominated by the power Lee Byeong-ju novel. This social responsibility to have to deal with the intelligence of an era, or the question of what makes this role.

교통사고로 인한 편타성 손상 환자의 어혈방(瘀血方) 치료 효능에 관한 임상적 연구

Objectives : The purpose of this study is to investigate the effect of eo-hyeol bang for patients with whiplash injury caused by a traffic accident. Methods : 60 patients were divided into two groups, Group A and Group B, of 30 patients each. Group A was treated with eo-hyeol bang twice per week for four weeks. Group B was treated with general acupuncture treatment twice per week for four weeks. Results : Between the two groups there was no significant difference in the VNRS score and the Neck disability index. Also, the study did not show significant difference between three variations of motion pattern induced pain and four stages based on the trauma duration of Group A. Conclusions : For patients with whiplash injury, eo-hyeol bang has not proven to be more effective than general acupuncture treatment.

성약시대에 천주청평수련원의 역사와 신약시대 예수그리스도의 신유은사 비교 고찰 : 하나님나라 운동을 목적으로

하나님 나라에 대해서는 구약에서도 여러 번 언급되어 있다. 이러한 전통은 예수 그리스도가 선포한 하나님 나라의 근간을 이루고 있다. 예수 그리스도가 지상에서 선포한 것은 하나님 나라의 도래이다. "하나님나라가 곧 온다, 오고 있다"라는 선언은 낡은 세계의 종말과 더불어 개벽의 때를 알려줌으로써 새로운 희망과 변화의 길을 열어주었다. 그리고 예수 그리스도는 기적적인 사역을 펼친다. 이 기적적인 사역은 하나님의 구원목적과 그에 대한 하나님의 계시이며, 종말론적인 사건이다. 예수는 무리들에게 절대적인 구원이 의지를 기적을 통하여 나타내 보이고 있다. '귀신을 내쫓는 일(exorcism)'은 예수님의 주된 임무며 사역이었다. 또한 이 사역은 그의 열 두 제자에게도 허락된 것으로서, 그는 그의 제자들을 파송 할 때 이 권세를 허락하여 이를 통해 만방에 하나님 나라를 선포케 했다. 이것은 하나님의 뜻이 무시되고 사탄의 세력을 발휘하는 어두운 세상에 대한 예수님의 사역이었다. 이렇게 예수 그리스도는 하나님 나라를 장님의 눈뜸으로, 앉은뱅이의 벌떡 일어섬으로 벙어리의 말함으로, 갇힌 자의 해방으로 또한 병자의 치유로 그리고 죄인들의 용서로 선포하셨다. 이러한 그의 선포는 소외되고 어려운 자들에게는 희망의 메시지, 구원의 목소리가 되었고, 그 희망은 2000년이 흐른 지금도 온 세계에 흩어져 있는 모든 그리스도인들의 영원한 염원이 되고 있다. 하나님나라 운동, 즉 지상천국의 도래는 그 나라의 도래를 원하셨던 예수시대는 물론이고, 이 땅에 다시금 메시아로 오신 참부모님의 꿈에도 소원이며, 더불어 참부모님과 이 시대를 살아가는 성약 성도들의 영원한 염원이기도 하다. 참부모님께서는 먹는 것 보다 입는 것 보다, 그 나라와 그 의를 구하는 것이 먼저라 하시며 80평생 당신의 삶을 통해 하나님의 한에 맺힌 소원을 풀어드리려 수고를 아끼지 않으신다. 참부모님께서는 세계적 승리의 기반을 쌓으시고, 오늘도 섭리의 고삐를 늦추지 않고 계신다. 그러기에 참부모님을 모시고 사는 부족한 우리 자녀들의 안타까운 염원도 역시 참하나님과 참부모님의 뜻을 하루속히 이 땅에 이루어 진정 하나님이참부모님이 바라시는 본연의 참된 본향세상을 어서 속히 되찾아드리는 것이다. 그러나 그러한 참부모님의 천주적 승리의 기반과 수고의 노정이 있음에도 불구하고 나는 부족한 자녀의 어린 모습으로 주위를 돌아 보메, 풀리지 않는 현실과 산적해 있는 우리의 과제에 많이 난감해 하고 고뇌하며 힘겨워했던 시간들이 있었다. 이상과 현실의 괴리에 혼란스러워 하며 방황했던 모습도 있었다. 신학도로서 이러한 나의 모습이었기에 때로는 분개하며 열정을 불태우기도 했고, 더러는 냉소적 시각으로 비판을 가한적도 있었으며, 또 때로는 마치 타인처럼 현실에 응대하기도 했었다. 성약시대 선포 이후, 우리는 뜻 성사가 머지 안았음을 느끼고 있다. 그러나 일면 작금의 현실 속에 답답함을 느껴본 일이 있을 것이다. 지상천국은 메시아인 참부모님 당대에 과연 이루어질 수 있을 것인가? 지난 날 신학도로써 지상천국의 도래에 대해 많이 고민하고 현실과 이상의 괴리에 혼란스러워했던 나는 대모님과 훈모님의 청평 역사에서 그 해답의 희망을 찾을 수 있었다. 따라서 이 논문은 청평의 역사를 지상천국 도래의 열쇠로 보고, 신약시대 예수님의 성령역사 즉, 신유은사를 통한 하나님 나라 운동과 비교해 봄으로써 대모님의 청평 역사를 참부모님의 하나님 나라 운동으로 고찰해 봄에 그 목적이 있다. 이러한 연구에 있어 내 자신의 부족함에 의한 학문적 뒷받침이나 이해 및 판단의 오류 등, 많은 허점이 있을지도 모른다. 그러나 이러한 부족함에도 불구하고 이 논문의 시도는 그 첫 출발로서 충분한 가치를 갖을 것이라고 생각한다. 다음으로 이어질 천주청평수련원에 관한 많은 연구들이 이 논문의 오류와 부족함을 채워주고 더욱 발전시켜줄 것으로 믿는다. 모쪼록 이러한 연구가 하나님과 참부모님의 섭리에 그리고 청평역사를 위해 진정 수고하시는 대모님과 훈모님의 노고와 은사에 조금이나마 보답이 되기를 바라며 이 논문을 작성하고자 한다.

Adiponectin as a new paradigm for approaching Alzheimer’s disease

Adiponectin is an adipocytokine released by the adipose tissue and has multiple roles in the immune system and in the metabolic syndromes such as cardiovascular disease, Type 2 diabetes, obesity and also in the neurodegenerative disorders including Alzheimer’s disease. Adiponectin regulates the sensitivity of insulin, fatty acid catabolism, glucose homeostasis and anti-inflammatory system through various mechanisms. Previous studies demonstrated that adiponectin modulates memory and cognitive impairment and contributes to the deregulated glucose metabolism and mitochondrial dysfunction observed in Alzheimer’s disease. Here, we aim to summarize recent studies that suggest the potential correlation between adiponectin and Alzheimer’s disease.

근대의 외부, 동양적인 것에 대한 사유 : 김동리와 박상륭의 1960년대 소설을 중심으로

본 논문은 소위 ‘비(非)근대주의적’ 작가로 규정지어지고 폄하되어온 김동리와 박상륭 소설의 당대적 의의를 탐구한다. 이들이 가진 동양적 세계관과 그것을 나타내는 방식은 소위 ‘반(反) 혹은 전(前)근대적’이라는 이유로 부정적 대상으로 오인을 받아오거나 평가절하 되어왔다. 혹은 아예 비평계의 침묵으로 가리워져오기도 했다. 본고는 1960년대 우리 문단에서 김동리와 박상륭의 위치와 의미를 살핀다. 김동리의 경우, 서구의 세계와 가치관에 대결·대항하는 것으로서 동양적 세계관·샤머니즘을 전면적으로 배치한다. 그의 작품이 구사하는 방법론은 접신(接神), 즉 대상과 인식주체의 완전한 몰입이다. 박상륭의 경우, 김동리가 보여준 샤머니즘의 세계를 논리화·현대화하면서 화합과 조화의 세계로 이끈다. 또한 그것은 정서와 정취의 언어인 남도 언어로 표현되었다. 이들이 보여준 몰입과 화해의 세계는 근대의 부정성과 한계를 넘어서는 ‘탈주’의 방식이었다. 1960년대 문단에서 김동리와 박상륭은 근대성의 ‘외부’에 존재함으로써 역설적으로 당대의 미적 근대성의 한 영역을 확보할 수 있었다. This study aims at finding out the novels of Kim Dong-Li and Park Sang-Ryung who regarded as Non-modern writers. Their oriental world view and the way to reveal their view were misconceived as negative target or devaluated because of their pre-modernity or anti-modernity. Or many modern critics kept silence about them. This study search their position and meaning in 1960’s literary world. Kim Dong-Li arranged in a wholesale way oriental world view and shamanism. His methodology was to be possessed of a spirit, it is to immerse between complete subject and object. Park Sang-Ryung try reasoning modernization the shamanism. And he used a the regional dialect very effectively. The southern provinces’ language was very affective and atmospheric. They secure some of esthetic modernity paradoxically as existing the outside of modernity. The world of immersion and harmony of Kim & Park can be the way to escape that overcome negativity limitation of modern society. With this, we can confirm that they extended field of esthetic modernity.
