        응급진료시 여성환자의 소변검체 채취방법

        Background: Urinalysis is a useful laboratory test in the diagnosis of various diseases. In sampling for the urinalysis, there is much concern about contamination that can lead to misdiagnosis in the mid-stream urine sampling method. We conducted this study to determine, in terms of concordance of results and contamination in culture, whether there was any superiority in nod-stream sampling methods with or without disinfection measures compared to the catheterization method. Materials and Methods: We used three kinds of urine sampling methods sequentially, mid-stream non-clean catch, mid-stream clean catch, and catheterization, for ambulatory, non-pregnant, non-men-struating female patients who visited NHIC Ilsan Hospital emergency department during a one-week period in September 2001. Each sample was electrophotometrically analyzed for leukocyte esterase, nitrite, and blood by using a reagent strip and was cultured immediately or after overnight refrigeration. Results: Of the 41 patients, 9 had culture-proven urinary tract infections. The concordance rates(kappa) for nitrite, blood, and leukocyte esterase were 0.875, 0.403, and 0.406 between non-clean catch and catheterized samples and 0.875, 0.481, and 0.560 between clean catch and catheterized samples, respectively. The contamination rate of the non-clean catch, the clean catch, and the catheterized samples were statistically different: 51.2%, 29.3% and 0%, respectively. Conclusion: We found that cleaning with disinfectant was effective for reducing the contamination rate, even though the best urine sampling method for zero contamination was catheterization. We recommend considering cost, patients' comfort, an acceptable threshold for contamination, and the necessity for a culture before choosing a urine sampling method for women who visit the emergency department.

      • 한국인 여성에서의 연령적 추이에 대한 체력변동에 따른 노쇄현상에 관한 연구

        The cardiopulmonary and physical functions were measured in young-aged and old-aged subjects aged between 10 and 60. The results are presented according to age group (1st, 2nd, 3rd, 4th, 5th and 6th decade) and are as follows. 1. The resting heart rates were 79±7.54, 73±11.2, 71±6.82, 76±8.40, 70±9.35, and 70±6.58 bpm in the 1st, 2nd, 3rd, 4th, 5th and 6th decade group, respectively. Aging was associated with a proportional increase in blood pressure 107/63±5.77/7.78, 114/72±10.50/9.49, 116/71±9.92/7.74, 120/76±14.60/10.00, 120/75±14.10/10.10 and 134/82±12.40/9.78mmHg in each respective group. 2. The vital capacity decreased 2.9% with aging from the 1st decade (2.11±0.53L), 2nd decade (2.91±0.38L), 3rd decade (2.66±0.37L), 4th decade (2.69±0.37L), and 5th and 6th decade (2.41±0.43 and 2.03±0.43L) respectively. 3. Gripping powers of the right/left hand were 15.1±2.14/13.6±1.87, 15.3±3.77/15.1±4.15, 22.3±5.92/21.0±6.27, 21.1±4.91/19.7±5.04, 20.2±6.10/18.7±5.58 and 20.5±4.20/17/5±3.99㎏, in the 1st, 2nd, 3rd, 4th, 5th and 6th decade group, respectively. Back muscle power decreased from 40.1±7.53㎏ in the 1st decade, 64.7±8.67㎏, 2nd decade 56.6±12.20㎏, 3rd decade 49.0±10.90㎏, 4th decade to 47.8±11.90㎏ in the 5th decade, and to 41.3±9.30㎏(16% decrease) in the 6th decade. 4. Jumping power decreased with aging. The maximum jumping heights were 34.7±7.36, 40.2±4.08, 31.4±4.82, 28.7±5.20, 27.3±5.02 and 24.5±4.48㎝ in the 1st, 2nd, 3rd, 4th, 5th, and 6th decade group, respectively. The above results indicate that aging beyond adolescence is associated with decreased cardiopulmonary and physical function.

        Testosterone-independent down-regulation of Oct2 in the kidney medulla from a uranyl nitrate-induced rat model of acute renal failure: Effects on distribution of a model organic cation, tetraethylammonium

        <P>Although acute renal failure (ARF) has been an area of extensive research in recent decades, our understanding of ARF is far from complete. Organic cations (OCs) are primarily excreted via vectorial transport by various renal organic cation transporters (OCTs). It is reasonable to assume that ARF may alter the expression profiles of these transporters. In a rat ARF model, induction of ARF by uranyl nitrate (UN) treatment significantly decreased the levels of Oct2 (slc22a2) mRNA and protein in the kidney medulla. mRNA expression of the other OCTs was not appreciably altered. The plasma level of testosterone, a well-known regulator of Oct2, was not changed, suggesting that the Oct2 down-regulation is testosterone-independent. The effect of reduced Oct2 expression on the distribution of a model OC, tetraethylammonium (TEA), in various rat tissues including kidney cortex and kidney medulla was investigated during steady state plasma TEA concentrations. The steady state tissue-to-plasma (T/P) TEA ratio was decreased in the kidney medulla (approximately 15-fold) during ARF. These results indicate that, in a rat model of ARF, reduced Oct2 expression in the kidney medulla results in decreased distribution of TEA to the kidney medulla, thereby reducing renal clearance of TEA in UN-ARF rats. © 2008 Wiley-Liss, Inc. and the American Pharmacists Association J Pharm Sci 98:739–747, 2009</P>

      • Molecular mechanisms of pruritus in dry skin conditions

        We all take it for granted that dry skin causes itch, but it is surprisingly unclear how dry skin evokes the unpleasant sensation. The underlying mechanism of itch was once thought to be mainly mediated by histamine, an endogenous itch-inducing agent (pruritogen) released from the mast cell. Once histamine is released from the mast cell, it binds to the histamine receptor, leading to the activation ion channel in the sensory neurons to relay the itch signal. Indeed, antihistamine agents are based on this mechanism, which interferes with the binding of histamine to its receptors. However, antihistamines are not fully effective in many diseases characterized by dry skin, implying the symptom may rely on histamine-independent mechanisms. Fortunately, animal models for dry skin, which is achieved by applying a mixture of acetone and ether followed by water (AEW), significantly helped researchers to reveal underlying mechanisms related to the symptom. It was found that sensory neurons, keratinocytes, and spinal cords in AEW mice undergo significant changes at molecular levels. The present talk will summarize findings related to pruritus in dry skin conditions.

        Decreased Secretory Transport of a Quarternary Ammonium, TBuMA, across LLC-PK1 Cells by the Anionic Kidney Extract

        The effect of organic anionic (OA) fractions of various rat organ tissues on the apparent partition coefficients (APC) of quarternary ammoniums (QAs) between n-octanol and phosphate buffer (pH 7.4) and QAs transport across the LLC-PK1 cell monolayer was examined. The OA fraction was prepared by filtering the aqueous extract of each tissue through an ion-exchange cartridge (Bond Elut C18). In the presence of OA fractions of liver and kidney extracts, substantial increase in APC was observed for tributylmethylammonium (TBuMA, Mw 200) and berberine (Mw 335), but not for triethylmethylammonium (TEMA, Mw 116) and tetraethylammonium (TEA, Mw 130). Because only QAs with higher Mw than a threshold (i.e., > 200) are known to form lipophilic ion-pair complexes with certain organic anions (e.g., bile salts such as taurodeoxycholate), above results are consistent with the hypothesis that only larger Mw QAs form lipophilic ion-pair complexes with endogenous organic anionic components of the liver and kidney extracts. Considering the comparable effect between the liver and kidney extracts on the APC of TBuMA regardless of far less (1/5) content of bile salts in the kidney extract, OAs other than bile salts in the kidney appear to contribute to the formation of lipophilic ion-pair complexes. Most interestingly, the secretory (i.e., basolateral to apical direction) transport of TBuMA and berberine across the LLC-PK1 cell monolayer was decreased by the cis-presence of the kidney extract, while remained unchanged for the transport of TEMA and TEA. The kidney extract had no effect on the absorptive (i.e., apical to basolateral direction) transport and cellular (LLC-PK1) accumulation of all of these QAs. Regardless of underlying mechanisms,it is notable that OA components of liver and kidney extracts influence the APC and secretory transport of QAs with Mw >200.

        약물유인성 식도궤양

        Over 26 kinds of tablets and capsules, such as Tetracycline, Quinidine and Potassium preparations were reported to cause esopbageal ulcerations, eharacterized in various size, shape and number, ind sually in mid-esophagus. Recently authors experienced 10 cases of drug-induced esophageal ulcerations due to antibiotics and antiinflammatory agents such as Tetracycline, Aspirin, etc. Four cases were men and six were women. Four cases were in third dades, three in fourth cleeades, two in fifth decades and one in sixth decades. Presenting symptoras were odynophagia(4/10), dysphagia(3/10), substernal pain(7/10) and epigastric pain(3/10). Endoscopic examination of the esophgus showed single or multiple, small and shallow ulcers on the mid-esophgeal mucosa at the level of 30cm from the ineisor in eight cases, one Iarge and deep ulcer at the level of 40cm from incisor in one patient and one amall, shallow and one large, deep ulcers at the same time. in one patient The shape of alcers were various from a round to a large horseshoe shaped one. The clinical course was mild without complications. It was suggested that drug-induced esophsgeal ulcers with antibiotics and antiinflammatory agents could be found frequently and they had benign and mild clinicalc ourse,
