A clinical practice guideline for patients in the dying process in general wards and their families, developed through an evidence-based process, is presented herein. The purpose of this guideline is to enable a peaceful death based on an understanding of suitable management of patients’ physical and mental symptoms, psychological support, appropriate decision- making, family care, and clearly-defined team roles. Although there are limits to the available evidence regarding medical issues in patients facing death, the final recommendations were determined from expert advice and feedback, considering values and preferences related to medical treatment, benefits and harms, and applicability in the real world. This guideline should be applied in a way that takes into account specific health care environments, including the resources of medical staff and differences in the available resources of each institution. This guideline can be used by all medical institutions in South Korea.

IPTV systems adopting one of conditional access systems for contents security have a difficulty in changing the conditional access system embedded in a set-top box or on a removable hardware device. However, software downloading through an IP network enables an IPTV set-top box to change its existing conditional access system in a secure way. This paper proposes a synchronization mechanism for the dynamic conditional access system that securely downloads and seamlessly executes conditional access software. The proposed synchronization mechanism enables the dynamic conditional access system to pre-download the required software in advance and to lengthen the replacement delay.

To prohibit the unauthorized accesses on premium contents, IPTV service providers need to adopt Conditional Access System (CAS) services. Adopting an IP CAS enables the service provider to make profit from the IPTV and to persuade the content providers that they manage the contents securely. Although several standard specifications have been published for interoperable CAS models, the CAS models are based on the unidirectional or restricted bi-directional broadcasting network and do not utilize the characteristics of IP network. In this paper, we introduce the interoperable CAS services for the IPTV through software downloading. For providing the CAS services, we derive the necessary system components for an interoperable CAS and analyze their requirement.
Detecting user attention to video segments using interval EEG features

<P><B>Abstract</B></P> <P>To manage voluminous viewed videos, which US adults watch at a rate of more than five hours per day on average, an automatic method of detecting highly attended video segments during video viewing is required to access them for fine-grained sharing and rewatching. Most electroencephalography (EEG)-based studies of user state analysis have addressed the recognition of attention-related states in a specific task condition, such as drowsiness during driving, attention during learning, and mental fatigue during task execution. In contrast to attention in a specific task condition, both inattention and normal attention are meaningless to viewers in terms of managing viewed videos, while detecting high attention paid to video segments would make a valuable contribution to an automatic management system of viewed videos based on viewer attention. To the best of our knowledge, this is the first EEG-based study of detecting viewer attention paid to video segments. This study describes how to collect video-induced EEG and attention data for video segments from viewers without bias to specific genres and how to construct a subject-independent detection model for the top 20% of viewer attention. The attention detection model using the proposed interval EEG features from 14 channels achieved the best average F<SUB>1</SUB> score of 39.79% with an average accuracy of 52.96%. Additionally, this paper proposes a channel-based feature selection method that considers both the performances of single-channel models and their physical locations for investigating the group of channels relevant to attention detection. The attention detection models using the interval EEG features from all four or some of the channels located in the fronto-central, parietal, temporal, and occipital lobes of the left hemisphere achieved the best F<SUB>1</SUB> score of 39.60% with an average accuracy of 48.70%. It is shown that these models achieve better performance than models using the features from all four or some of their symmetric channels in the right hemisphere and models using the features from six channels located in the anterior-frontal and frontal lobes of the left and right hemispheres. This paper shows the feasibility of subject-independent and genre-independent attention detection models using a wireless EEG headset with optimized channels; these models can be applied to an intelligent video management system based on viewer attention in real-world scenarios.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A method of detecting the top 20% of viewer attention to video segments is proposed. </LI> <LI> This is the first study of detecting viewer attention during video viewing. </LI> <LI> All subject-independent models unbiased to specific genres are evaluated. </LI> <LI> The all-14-channel, single-channel, and selected multi-channel models are included. </LI> <LI> The interval band ratio features are the most suitable for all the types of models. </LI> </UL> </P>

Objective. The increase in mortality in rheumatoid arthritis (RA) patients with interstitial lung disease (ILD) is well known. However, there are few studies on serum markers that can evaluate acute exacerbation or prognosis in RA-ILD patients. The purpose of this study was to identify the association between biomarkers and lung lesions in patients with RA-ILD. Methods. We analyzed 153 patients with serum samples in a prospective, multicenter cohort of Korean RA-ILD patients. The serum levels of biomarkers, matrix metalloproteinase (MMP-7), surfactant protein-D (SP-D), and Krebs von den Lungen-6 (KL-6) were measured and correlated with forced vital capacity (FVC), diffusing capacity for carbon monoxide (DLCO) and the results of computed tomography (CT). CT results were interpreted semi-quantitatively according to the extent of lung lesions (grade 1, 0%∼25%; grade 2, 26%∼50%; grade 3, 51%∼75%; grade 4, 76%∼100%). Results. MMP-7, SP-D, and KL-6 were negatively correlated with FVC (MMP-7, r=-0.267, p=0.001; SP-D, r=-0.250, p=0.002; KL-6, r=-0.223, p=0.006) and DLCO (MMP-7, r=-0.404, p<0.001; SP-D, r=-0.286, p=0.001; KL-6, r=-0.226, p=0.007). In addition, MMP-7, SP-D, and KL-6 tended to increase with higher grades of lung lesions on CT (MMP-7, p=0.013; SP-D, p<0.001; KL-6, p<0.001). Conclusion. MMP-7, SP-D, and KL-6 can be used to evaluate the functional and anatomical status of lung involvement in the RA-ILD patients. (J Rheum Dis 2021;28:234-241)

Nowadays, tumor necrosis factor-α (TNF-α) blockers are used for treatment of rheumatoid arthritis, inflammatory bowel diseases, ankylosing spondylitis, psoriatic arthritis, and psoriasis. Paradoxically, there are some reports on the appearance of psoriasis after administration of TNF-α blockers. Here, we report on a patient with monoarthritis in a knee joint who experienced psoriasis after TNF-α blocker therapy (adalimumab and etanercept). Oral medication was not a treatment option due to patient intolerance; thus, we tried ustekinumab, an anti-interleukin (IL)-12/23 monoclonal antibody used for treatment of psoriasis. Following ustekinumab injection, psoriatic skin lesions and joint symptoms were much improved. However, in the following period, joint pain and swelling became aggravated and synovial fluid cytokine levels including IL-6 and IL-17 were elevated. The treatment was changed to tocilizumab, a humanized monoclonal antibody against IL-6 receptor. After injection, knee joint swelling rapidly subsided without worsening of psoriatic skin lesions. Are cellulosic feedstocks for biofuel production a promising alternative considering economic and environmental aspects? A regional model of agricultural production in the watershed is constructed to analyze the economic and environmental impacts of cellulosic feedstock production. The model has a multi-region structure with various crop production activities and a high level of spatial details that previous studies do not consider. SWAT, a biophysical simulation model, is used to simulate the effects of feedstock production on soil and water quality in the Le Sueur River Watershed (LRW) in south-central Minnesota. Findings indicate that intensive corn production expands to whole watershed and more regions are covered with high levels of nutrient losses as corn stover production increases.
추상 비즈니스 프로세스를 위한 WSCI 와 BPEL4WS 간의 상호 변환 알고리즘에 관한 연구

웹서비스란 XML 포맷의 벤더 중립적인 표준을 따라 웹에 산재한 웹 애플리케이션들을 통합하는 새로운 분산 컴퓨팅 기술이다. 비즈니스 로직을 바탕으로 유용한 서비스를 제공하는 새로운 비즈니스 프로세스를 웹서비스를 이용하여 구현하기 위해서, 기존의 웹서비스를 조합하여 비즈니스 프로세스를 기술하는 웹서비스 컴포지션 스펙이 제안되었다. IBM과 Microsoft에서는 BPEL4WS, Sun에서는 WSCI, BPMI.org에서는 BPML을 제안하였는데, 그 중에 WSCI와 BPEL4WS는 기업간 협업에 의한추상 비즈니스 프로세스를 기술한다. 본 논문에서는 추상 비즈니스 프로세스를 위한 두 스펙간의 상호 운영성을 증대하기 위해서 WSCI와 BPEL4WS간의 상호 변환 알고리즘을 제안한다. 예를 들어 BPEL4WS 구현 시스템에서 WSCI에서 BPEL4WS로의 변환 알고리즘을 이용하여 BPEL4WS 뿐만 아니라 WSCI로 기술된 비즈니스 프로세스를 참조할 수 있고, BPEL4WS에서 WSCI로의 변환 알고리즘을 이용하여 BPEL4WS 구현 시스템에서 사용 중인 비즈니스 프로세스 인스턴스를 WSCI 인터페이스로 출력할 수 있다.
