Background: The diabetic patients are at significantly increased risk of developing vascular disease. It's etiology may involve oxidative damage by free radiacals and protection againse such damage can be offered by antioxidants. We investigated that oxidative stress as assessed by measurement of total antioxidant status may play a role in development of diabetes mellitus. Method: We measured total antioxiant status using merchandised kit, glycated hemeglobin(HbA1c) in 46 type 2 diabetes mellitus patients and 50 healthy matched control subjects. Result: The total antioxidant status(TAS) was 2.10(±0.04) mmol/L in uncontrolled type 2 DM patients, 2.60(±0.03) in controlled type 2 DM patients and 2.70(±0.16) in healthy control subjects. TAS was significantly lower(P<0.05) in uncontrolled type 2 DM patients, but no significant association between in controlled type 2 DM patients and healthy control subjects. The TAS was 2.00(±0.17) mmol/L in complicated type 2 DM patients and 2.10(±0.29) uncomplicated type 2 DM patients. There was no significant associations between complicated type 2 DM patients and uncomplicated type 2 DM patients. Conclusion: Poor glycemic control is associated with reduced TAS in type 2 DM patients. TAS was thought indirect index that predict glycemic control of type 2 DM patients.

We simulate the light transmission through an extremely small nanoscale aperture having a 10 nm diameter punctured in a metal film positioned at the center of a plasmonic bull's eye grating. A considerable directive emission of transmitted light with a divergence angle of 5.7 degrees was ob- served at 10 m from the nanohole opening at the frequency of surface plasmon polariton excitation, an confirmed by measuring the distance dependent transmission amplitude. Observations of the electric field in cross-sectional, near-field, and far-field views near-field enhancement associated with the surface plasmon excitation, and the interference of the electric field light through the nanohole in the near-field region is responsible for such a considerable directive emission.

Pure red cell aplasia involves a selective failure in the production of erythroid elements in the bone marrow. The morphologies of megakaryocytes and granulocytes are not measurably changed, and they are present in normal numbers. A 48 year-old male was admitted to our hospital due to anorexia and dizziness, and he was taking diphenylhydantoin against epileptic features and schizophrenic behavior. The peripheral blood smear and bone marrow examination revealed normocytic normochromic anemia, reticulocytopenia, marked hypoplasia of the erythroid series, but the cellularity of the bone marrow, and megakaryocytic, granulocytic series remained no measurable abnormalities. the direct Coombs test was positive and other combined disease entities were not found. So we found that the diphenylhydatoin-induced pure red cell aplasia was presented and it strongly suggested the involvement of the immunologically mediated. This is the first case of the strongly suggested immune-mechanism cause of the diphenylhydantion-induce pure red cell aplasia, and so we report this case with brief review of the literatures.

To evaluate the therapeutic effectiveness of uro-kinas instillation for the resolution of loculated pleural effusion caused by bacterial or tuberculous empyema, we inserted one pig-tail catheter (Cook, Melbourne, Australia 8Fr.) into the loculated cavity under the guide of fluoroscopy or ultrasonography and compared the results with that of the control group who received closed thoracostomy. In 10 of 13 urokinse-instilled patients, complets l??sis of loculation and full expansion of lung parenchyme was revealed by the chest CT scan or ultrasonogram. The mean duration of pig-tail catheter insertion was 9.9±3.1 days and the mean dosage of instilled urokinase was 4.3±1.6 ×10^(5)units in the successfully treated patients. Any complication including localized or systemic bleeding was not observed in the urokinase-treated group. In the control group who received closed thoracostomy, only three of 14 patients showed improvement even after longer duration of chest tube insertion ranged from 20 to 50 days when compared with that of uro-kinase treated group. The other eleven patients should be managed by the rib resection and open drainage or pleurectomy or thoracoplasty because of retractable purulent drainage or persistence of septic condition in pleura and lung. In conclusion, the urokinase instillation into the loculated effusion was very effective in shortening the treatment duration of loculated empyema and it was considered to be desirable that urokinase instillation should be carried out in earlier or initial steps of therapeutic interventions.

Mitochondrial myopathies are diseases caused by defects in metabolic pathway of mitochondria. Mitochondrial myopathy is known as one of the causes of recurrent myoglobinuria, while clinically, rarely causes acute renal failure requiring medical treatments. We report a case of rhabdomyolysis and acute renal failure associated with mitochondrial myopathy. A 58-year-old male was presented with dyspnea and hypotensive shock. The patient had a history of recurrent dark colored urine and cramping leg pain after prolonged fasting. Laboratory findings showed hyperkalemia, azotemia, metabolic acidosis, and elevated AST, ALT, and creatinine kinase. He had no history of trauma or medication. Muscle biopsy showed ragged red fibers in modified Gomori staining. On electron microscope, increased number of mitochondria and abnormal mitochondria were seen. He received hemodialysis and his renal function recovered after 1 month. (Korean J Nephrol 2004;23(3):509-513)
