      • 지역 사회 감염성 폐렴에서 Cefodizime의 효능 및 안전성을 Ceftriaxone과 비교한 연구

        목적: Cefodizime은 새로운 주사용 제 3세대 항생제이다. 저자들은 감염성 폐렴 환자들에서 이 약물을 하루 1번 정맥주사하였을 때 관찰되는 임상적및 세균학적 효과와 안전성을 ceftriazone과 비교하기 위하여 본 연구를 시행하였다. 방법: 1992년 6월부터 1993년 11월까지 서울대학교 병원에 지역사회 감염성 폐렴으로 입원하였던 환자 32명을 무작위로 3군으로 나누고 제 1군 : Cefodizime 1g(n=11), 제2군 : Cefodizime 2g(n=10), 제3군 : Ceftrizxone 1g(n=11)을 각각 하루 1회 정맥주사로 투여하여 그 치료율및 안전성을 비교하였다. 결과: 1)Cefodizime은 제1군 100%, 제2군 80.0%의 임상적 반응율을 보였다. Ceftriaxone을 투여하였던 제 3군에서는 81.8%의 임상적 반응율을 보였다. 이러한 임상적 반응율의 결과는 세 군간에 통계적으로 유의한 차이를 보이지 않았다(p>0.05). 2)세균학적 반응율도 세 군간에 유의한 차이를 보이지 않았다. 3)Cefodizime의 안전성의 평가에서 제1군 및 제2군에서 구토 및 오심이 2예(9.5%)에서 관찰되었고 일시적인 간효소치의 상승이 1예(4.8%)에서 관찰되었다. 경미한 공복시 위통이 1예(4.8%)에서 관찰되었다. Ceftriaxone으로 치료한 군에서는 특별한 부작용은 관찰되지 않았다. 결론: 이상의 결과로 지역사회 감염성 폐렴의 치료에 있어서 cefodizime 1내지 2g을 정맥 주사로 하루 1회 치료할 경우 적어도 ceftriaxone과 유사하게 효과적이며 안전한 것으로 사료된다. Background:Cefodizime(Modivid) is a new parenteral form of cephalosporin showing the antibacterial specturm of the third generation. The clinical efficacy and safety of cefodizime have been well proven by some previous clinical studies performed in European countries and Japan. The objective of this study was to evaluate the clinical, radiological and antimicrobial efficacy of this drug in patients with community-acquired pneumonia compared to that of ceftriaxone. Methods: We performed an open, controlled, randomized study in 32 patients with community-acquired pneumonia at Seoul national University hospital from June 1992 to November 1993. In the first group(n=11) 1g of cefodizime, and in the second group(n=10) 2g of cefodizime were respectively administered intravenously once a day. and in the third group(n=11), 1g of ceftriaxone was administered intravenously. Results: The results were as follows. 1) Cefodizime showed the clinical response rate of 100% in group I, and 80.0% in group Ⅱ. The clinical response rate of ceftriaxone in group Ⅲ was 81.8%. There was no significant statistical difference(p>0.05). 2) The rates of bacteriological efficacy wee not statistically different among three groups. 3) In the cefodizime treatment group(group Ⅰ, group Ⅱ) nausea and vomiting were observed in 2 cases(9.5%) and transient elevations of hepatic transaminases in 1 case(4.8%). Mild abdominal pain occurred in 1 case(4.8%). No significant adverse reactions were observed in the ceftriaxone treatment group. Conclusion: We could conclude that once a day intravenous cefodizime therapy was as effective and well tolerated as ceftriaxone in the treatment of community-acquired pneumonia.

      • 마이크로 모빌리티 네트워크에서의 안전한 멀티캐스트

        To reduce the control message overhead due to frequent handoffs in mobile IP, micro-mobility protocols have been proposed. In this paper we present an approach for providing secure multicasting services in micro-mobility environments. We first introduce a multicast routing protocol in a micro-mobility environment and then present protocols to add security services to the bare multicasting. The proposed multicast routing protocol builds a shared multicast tree and does not assume any unicast micro-mobility protocols. The added security services include authentication, authorization, confidentiality, and integrity and are based on both symmetric and asymmetric cryptosystems. The security protocols also use key hierarchy mechanism in a paging area to efficiently handle frequent group membership changes and handoffs.

      • Mobile IPv6 환경에서 신뢰성 있는 TCP 프로토콜의 성능향상에 관한 연구

        There are three methods for improving performance of TCP Protocol in the Mobile IPv6 Environment. Those methods are link layer protocol method, end-to-end protocol method and split-connection method. It's a link layer protocol method among these to be applied without any changes of wired network environment. Snoop module is one of link layer protocol methods for improving performance of TCP Protocol. We apply snoop module to Mobile IPv6 and improve Mobile IPv6 with snoop module by changing some feature of Mobile IPv6 which reduces its performance. Based on these improvement of Mobile IPv6, We simulate it by ns2 and mobiwan comparing original Mobile IPv6 and our Mobile IPv6 with snoop module. Simulation of ours is better than original.

      • 대망에 발생한 방선균증 1례

        Actinomycosis is an uncommon chronic suppurative disease characterized by multiple abscess, draining sinuses, abundant granulation, and dense fibrous tissue, caused by the actinomyces organisms. It occurs in three major sites: cervicofacial, abdominal, and thoracic. The abdomen is the second most common involved site in previous studies, and there are three ways this organism can affect the gastrointestinal tract: through a blood-borne infection, by swallowing, or by proliferation at a more distal intestinal area. Abdominal actinomycosis is an unusual disease but remains an important differential diagnosis in patients with abdominal mass. Obtaining the diagnosis is difficult and rarely made preoperatively, but provide the key to its management. Medical therapy is effetive in eradicating the disease; however, because of the large amount of reactive fibrosis formed by the infection, it requires long-term therapy. Surgical treatment may be essential in many cases, primarily when malignancy cannot be excluded or if extensive necrotic tissue exists. There are a few description for actinomycosis involving omentum. We report a case of patient with omental actinomycosis, suspected to have a malignancy requiring an exploratory laparotomy with resection.

        21세기의 정보화 산업사회와 지식기반 사회에서 우리의 아이들이 자신의 목소리로 세계 속에서 살아가게 하기 위해서는 다른 사람과 다른 아이디어를 낼 수 있도록 교육의 틀을 바꾸지 않으면 안된다. 다라서 21세기의 미래 사회를 살아갈 학생들을 가르치는 교육의 현장 역시 창의적인 인간을 길러낼 수 있도록 변해야 한다. 즉, 미래 사회를 대비하기 위해서는 각 개인의 개성을 존중하는 분위기에서 창의적인 능력을 지닌 인간을 양성해야 한다는 것을 의미한다. 따라서, 학교 안에서의 교육뿐만이 아니라 학교 밖에서도 다양한 학습 경험을 통해서 교육 과정이 추구하는 인간상을 구현할 수 있도록 활동할 수 있는 기회가 학생들에게 제공되어야 한다. 창의력을 발휘하여 문제를 해결하는 능력을 기르기 위해서는 개별적이 아닌 여러 명의 동료가 주어진 과제를 해결할 수 있는 기회를 갖고 짧은 시간이 아닌 장시간 동안 고민하고 그 해결책을 찾는 활동을 가질 기회가 필요하다. In order to educate our students with their special talents for the informational and knowledge based age expected in the 21st century, we must improve our educational programs such that students are able to create their own fresh ideas. Hence the present educational environment should be changed such that they are willing to respect others' characters and personality with their own original ideas. Therefore, we must make them have the opportunity that they can acquire many experiences through the school education and the social education program, so that they may grow up to the person with the humanity and creativity. In this study we have suggested new educational program where students engage themselves in long term research projects with collaboration of the team and creativity of individuals.

        Design of Metal Oxide Hollow Structures Using Soft-templating Method for High-Performance Gas Sensors

        Semiconductor gas sensors based on metal oxide are widely used in a number of applications, from health and safety to energy efficiency and emission control. Nanomaterials including nanowires, nanorods, and nanoparticles have dominated the research focus in this field owing to their large number of surface sites that facilitate surface reactions. Recently, metal oxide hollow structures using soft templates have been developed owing to their high sensing properties with large-area uniformity. Here, we provide a brief overview of metal oxide hollow structures and their gas-sensing properties from the aspects of template size, morphology, and additives. In addition, a gassensing mechanism and perspectives are presented.

      • F-103 Lessons learned from continued TB outbreaks in a high school

        Objective: Contact investigation with Tuberculin Skin Test (TST) and delayed QuantiFERON testing (QFT) had been conducted against continued tuberculosis (TB) outbreaks in a high school before Korea latent TB infection (LTBI) policy establishment. We investigated the aftereffect of confirmatory QFT added to positive TST. Methods: We reviewed the outbreaks courses, and identified the TB development in relation to initial TST and QFT with the aid of Korean national claims database. Results: Continued sequential 43 active TB cases were diagnosed in a high school for one year before LTBI investigation. After contact investigation, a total of 925 contacts were divided into three groups: TST- (n=632), TST+/QFT+ (n=25), TST+/QFT- (n=257). The mean duration of follow-up was 3.9 ± 0.9 years. QFT- was more prevalent compared with QFT+ in contacts with 10mm ≤ TST <15mm (157, 61.1%) than in contacts with TST ≥15mm (100, 38.9%) among the TST positive reactors (Odds ratio 4.97, C.I: 1.80 - 14.46, P < 0.001). Of the 257 TST+/QFT- subjects, 255 received no LTBI treatment, and 7 contacts developed TB during follow-up. Among these 7 patients, four had initial TST ≥15mm, and three 10mm ≤ TST <15mm. Conclusions: The delayed contact investigation for LTBI in a high school TB outbreak brought upon the continuous successive TB outbreaks. False negative QFT performed lately among the TST positive reactors should not be considered as a criteria for LTBI treatment. Additionally, the contacts only with strong induration size of TST (≥ 15mm) should be considered for LTBI treatment.
