        상악 편악 소구치발치와 양악 소구치발치에 의한 교정치료 전후의 측모 두부방사선계측법적 비교 연구

        상악 편악 소구치발치에 의한 교정치료는 임상적으로 상당히 많이 행해지고 있는 현실이지만 임상적 활용도에 비하여 그 보고는 희귀하다. 본 연구는 선택적 교정치료로 행해지는 상악 편악 소구치발치에 의한 교정치료 시 치료전후의 상, 하악 절치위치 및 입술위치 변화를 양악 소구치발치에 의한 교정치료의 치료 전후와 비교, 분석하여 편악발치에 의한 교정치료의 진단적 지침을 제시하고 치료 종료 후의 치료결과를 알아보고자 시행되었다. 최근에 교정치료가 종료된 환자 중에서 초진 시 수평피개량이 7mm 이상을 보이는 증례 중 성공적으로 치료되었다고 판단되는 40증례를 선정하여, 교정치료의 목적으로 상악 제1소구치만을 발치하여 치료한 군(이하 편악발치군; Group 1, 20증례)과 상, 하악 제1소구치를 발치하여 치료한 군(이하 양악발치군; Group 2, 20증례)으로 나누어 치료 전후의 측모 두부방사선 사진을 채득하여 연구재료로 사용하였다. 연구결과 초진 시 편악발치군에서 양악발치군과 비교하여 하악 절치의 위치는 순측경사의 정도가 덜하였으며 하순의 돌출도도 심하지 않았다. 치료종료 시 편악발치군에서 양악발치군과 비교하여 하악 절치의 위치를 제외한 모든 항목에서 두 군과의 차이는 관찰되지 않았다. 치료 전후의 변화량 비교에서 상, 하악 절치의 이동량은 양악발치군에서 많게 관찰되었으며 하악 절치의 견인에 따라 하순의 돌출도 감소의 양도 양악발치군에서 크게 관찰되었다. 상순의 위치는 두 군간의 차이가 관찰되지 않았다. 큰 수평피개를 갖는 증례에서 적절한 발치 선택을 위한 진단정보의 수집에 있어 상악 절치의 위치 및 상순의 돌출도는 충분한 정보를 제공하지 않았다. 하악 절치의 위치와 하순의 돌출량은 큰 수평피개를 갖는 증례에 있어서 편악발치 혹은 양악발치를 결정하기 위한 적절한 진단적 근거를 제시하였다. 큰 수평피개를 갖고 있는 환자 중 하악 절치의 순측경사가 심하지 않으며 하순의 돌출이 심하지 않은 안모를 갖고 있는 증례의 경우 편악발치에 의해 좋은 치료결과를 얻을 수 있었다. Objective: The objective of this study was to provide guidelines for the diagnosis and successful treatment of orthodontic treatment with upper premolar extraction only. Methods: The sample group consisted of 40 patients (20 with upper premolar extraction only, 20 with both upper and lower premolar extractions) who showed an overjet of more than 7mm and were finished successfully. Lateral cephalographs were taken before and after orthodontic treatment. Landmarks showing the position of the upper and lower incisors and the position of the upper and lower lip were determined and angular measurement of these values were obtained for statistical analysis (Mann-Whitney test). Results: At pre-treatment, the position of the lower incisor was less labially inclined and the convexity of the lower lip was smaller in the upper premolar extraction only group than in the upper/lower premolar extraction group. At post-treatment, there was no difference in all measurements except for the position of the lower incisor. A comparison of changes between pre- and post-treatment showed that the retraction of upper & lower incisors and the decrease in convexity of the lower lip were greater in the upper/lower premolar extraction group. Conclusions: Treatment by upper premolar extraction can be of benefit for patients whose lower incisor proclination and lower lip protrusion are not excessive.

      • 경포호의 어류상 및 어류군집 특성

        The characteristics of ichthyofauna and fish community in the Lagoon Gyeongpo, Korea were investigated from May to October 2005. The total number of fish caught during the period was representing 26 specis and 18 families. Most dominant species was T. hakonensis(50.65%) and subdominant specis was A. flavimaus(25.41%). K. punctatatus(11.07%), M. cephalus(6.53%), E. japonicus(1.46%), C. castaneus(1.01%) were also numerous. The total biomass of collected fish showed 147.00kg, 135.83kg, 85.60kg,23.76kg, repectively. A pattern of the fish community according to seasonal veriations appear to be related tp spawning periods of species. Comparison of the ichthyofauna in present study with the references from early years suggests that there has been a gradual decrease, in composition, of primary freshwater specis accompanying with the increase of peripheral freshwater and seawater specis.

      • Sheehan 증후군 발생한 Torsades de Pointes

        재발하는 심실 빈맥으로 내원한 환자에서 Sheehan 증후군을 진단하였고, 적절한 호르몬 보충 치료 후 심전도 이상이 정상으로 회복된 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Torsades de Pointes is a form of polymorphic ventricular tachycardia that is associated with prolongation of QT intervals, characterized by QRS complexes of progressively changing ampulitude and cyclic length, giving the appearance of oscillations around the baseline. A 77 years-old woman was presented with hypotention and arrhythmia. Twenty four-hour ECG showed typical episodes of torsades de pointes with QT prolongation. This arrhythmia was controlled by isoproterenol and magnesium infusion, but it was not disappeared. She was diagnosed as Sheehan's syndrome by past history, anterior pituitary stimulation test and empty sella on CT scan. After hormone replacement treatment, Prolonged QT interval was normalized and polymorphic ventricular tachycardia has not recurred. We report a case of torsades de pointes associated with Sheehan's syndrome

        蓄血證과 蓄水證의 病理에 關한 文獻的 考察

        연구배경 : 축혈증과 축수증은 태양표증이 풀리지 않아 邪熱이 안으로 들어가 형성된 양대병증으로 이들의 부위 및 병리에 대한 설명이 명확치 못한 점이 많다. 이에 축혈증과 축수증의 본래 의미를 검토하고 그것의 병리에 대해서 부위, 증상, 개체별 차이 및 측수ㆍ축혈증의 실질에 대해서 연구하고자 함. 연구방법 : 축혈증과 축수증에 관련된 조문 및 유사증에 관한 조문들을 가려서 역대 주가들의 주를 참고로 연구하였다. 축수ㆍ축혈의 실질에 관해서는 처방을 중심으로 사용된 약재들의 현대적 약리작용을 통해 연구하였다. 결과 : 축혈증은 하초소복부의 혈액순환계통의 질환과 관련된 것으로 나타났으며, 축수증은 방광자체의 문제이기 보다는 기화분리로 인한 수액대사 장애와 관련된 병증으로 볼 수 있다. Through the philological study on syndrome of stagnated blood & syndrome of stagnated water, the results are obtained as following; 1. It is resonable to understand that the location of Syndrome of Stagnated Blood(蓄血證) is the lower abdomen and Syndrome of Stagnated Waste (蓄水證) is not exactly the disorder of Gall Bladder itself, but the disturbance of waterbolism derived from the disturbance of Qi (氣) transformation. 2. Syndrome of Stagnated Blood is that, being uncured Taiyang Channel Syndrom (太陽病), pathogen enters inside and pathogenic heat combines with blood at the lower abdomen; its main signs are spasmatic distention or muscular rigidity of the lower abdomen, mental derangement and normal urination. 3. Syndrome of Stagnated Water is that, being uncured Taiyang Channel Syndrom, pathogenic heat enters inside and combines with water and leads to disturbance of waterbolism; its main signs are thirsty, dysphoria, and difficulty in urination. 4. In the view of Western Medicine, Syndrome of Stagnated Blood is related with the disease of the blood circulation-especially in microcirculation- of the lower abdomen; Syndrome of Stagnated Water is similar to the disease which is related with the abnomal state of osmotic density derived from the unblance of electrolytes. 5. Lee Je-Ma regared the Syndrome of Stagnated Blood as SinSooYeolPyoYeolByung(腎受熱表熱病) of ShaoYin-Ren (少陰人), so explained it as the disease derived from the lower abdominal accumulation of Yang-Qi (陽氣) which coudn't have ascended; and regarded the Syndrome of Stagnated Water as PiSooHanPyoHanByung (脾受寒表寒病) of ShaoYang-Ren (少陽人), so explained it as the disease derived from the accumulation of internal heat because of discending disturbance of Yin-Qi (陰氣).

        응급실 자의퇴원환자의 요인분석

        Discharge against medical advice(AMA) in the emergency department is a frustrating, painful event to both patients and emergency physicians and emergency physicians. A retrospective chart review of 347 consecutive AMA patient from our emergency department from December 1992 to May 1993. Among them, 185 patients were interviewed with our researcher about primary cause of AMA through telephone contact. The results were as follows : 1. Total 9,705 patients visited our emergency department during 6-months period and there was 347 AMA patients(3.6%) 2. The main reasons for 185 AMA were ; 1) patients' or family's own problem in 114 cases(61.6%) 2) changes in patient condition(terminal case, improvement) in 56 cases(30.3%). 3) dissatisfaction to medical services in 15 cases(8.1%) 3. During same period, there was 117 cases of drug intoxication mainly due to suicidal attempt and 18 AMA was noted. Among 18 AMA cases, 16 patients discharged against medical advice due to patients' own problem(mainly due to economical problem) but none of them revisited OPD or emergency department. 4. Eight of 40 AMA patients with symptom improvement were managed in OPD, emergency department or other hospital but none of them admitted. In conclusion, we noticed that the main cause of AMA is non-medical problem in Korea and there should be continuous effert to improve our medical insurance system, emergency medical services system. Only through the judicious use of AMA, it can be used as a tool of determining patient satisfaction and quality improvement in emergency medical field.

        고압 커패시터의 고장 분석을 통한 신뢰도 예측

        본 논문은 고압 커패시터의 고장분석과 신뢰성 예측 결과를 다루고 있다. 부품의 수명과 고장률을 예측하기 위해서 두 가지 방법으로 고장 모드와 고장 메커니즘을 연구하였다. 에폭시 수지로 성형된 고압 커패시터가 절연내압 시험 하에서 저항이 제로로 되는 고장에 대하여, 근본원인 고장분석 체계를 효과적으로 수립함으로써 고장 메커니즘의 원인을 분석하였다. 특히 세라믹-에폭시 계면에서의 절연파괴 고장 현상이 강조되었으며, 본 연구에서 얻어진 결과의 타당성은 마그네트론에 장착된 고압 커패시터의 열사이클 시험 수행에 의한 가속시험 결과로부터 입증되었다. 시험 결과들은 결함이 있는 로트를 신속히 규명하고 B10수명을 결정하는데 유용하게 사용할 수 있다. 또한, 유전체의 절연파괴에 대해서 부하-강도 간섭모델을 이용하여 고장률을 예측하였다. This paper presents a result of failure analysis and reliability evaluation for high voltage ceramic capacitors. The failure modes and failure mechanisms were studied in two ways in order to estimate component life and failure rate. The causes of failure mechanisms for zero resistance phenomena under withstanding voltage test in high voltage ceramic capacitors molded by epoxy resin were studied y establishing an effective root cause failure analysis. Particular emphasis was placed on breakdown phenomena at the ceramic-epoxy interface. The validity of the results in this study was confirmed by the results of accelerated testing. Thermal cycling test for high voltage ceramic capacitor mounted on a magnetron were implemented. Delamination between ceramic and epoxy, which might cause electrical short in underlying circuitry, can occur during curing or thermal cycle. The results can be conveniently used to quickly identify defective lots, determine B10 life estimation each lot at the level of inspection, and detect major changes in the vendors processes. Also, thd condition for dielectric breakdown was investigated for the estimation of failure rate with load-strength interference model.

      • GIS와 유전자 Algorithm을 이용한 유지관리를 고려한 최적노선 선정

        Maintenance of roads is important thing and it should be considered in the plan of the road design process. Optimizing some road design characteristics in the plan may reduce road maintenance costs over the life of the road. Maintenance costs are relevent to sideslopes, cross section, and annual average daily traffic. Road can be chosen to minimize these costs, focusing on those conditions. Genetic algorithm, which is a good technic about optimization problem, is used to optimize those of characteristics. The purpose of this study is to choose a road by optimizing these characteristics. It was selected automatically by using GIS and GA and the result shows that sideslopes have influence on choosing optimal road relevent to maintenance.

        지형공간 특성자료를 이용한 하천유역의 강우-유출해석

        The subject basin of the research was the basin of Yeongcheon Dam located in the upper reaches of the Kumho River. The parameters of the model were derived from the results of abstracting topological properties out of rainfall-runoff observation data about heavy rains and Digital Elevation Modeling(DEM) materials. This research aimed at suggesting the applicability of the CELLMOD Model. a distribution-type model, in interpreting runoff based on the topological properties of a river basin, by carrying out runoff interpretation for heavy rains using the model. To examine the applicability of the model, the calculated peaking characteristics in the hydrograph was analyzed in comparison with observed values and interpretation results by the Clark Model. According to the result of analysis using the CELLMOD Model proposed in the present research for interpreting the rainfall-runoff process, the model reduced the physical uncertainty in the rainfall-runoff process, and consequently, generated improved results in forecasting river runoff. Therefore it was concluded that the algorithm is appropriate for interpreting rainfall-runoff in river basins. However, to enhance accuracy in interpreting rainfall-runoff. it is necessary to supplement heavy rain patterns in subject basins and to subdivide a basin into minor basins for analysis. In addition, it is necessary to apply the model to basins that have sufficient observation data, and to identify the correlation between model parameters and the basin characteristics(channel characteristics).

        In this paper present Autonomous Adaptive Digital Over Current Relay for distribution networks which acts autonomous setting using the short circuit impedance measured by relay of power systems. Automation of relay setting is one of the basic requirements for distribution automation, although manual relay setting is used at present. The short circuit impedance from a power source in distribution networks essential for the Autonomous Relay Setting changes frequently in distribution networks. In this paper the short circuit impedance is calculated with voltage and current measured in real time operation of digital relay using the Recursive Least Squares. A new method of digital relay setting in introduced using the the short circuit impedance and load current.
