        In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

        Biodegradable retinal fixation devices obtain mechnical fixation of the retina with desirable chorioretinal scarring and with the potential for local, sustained release of antimetabolites and steroids to inhibit proliferative vitreoretinopathy. We manufactured a biodegradable retinal tack with barb that was designed in order to prevent intrusion from implantation of retinal tacks. This study was carried to evaluate the efficacy for retinal fixation and the capability for sustained release of drugs with a newly designed biodegradable retinal tack Biodegradable retinal tacks were made of polymers of glycolic acids and were designed with barbs in a shape to prevent the disinsertion. Biodegradale retinal tacks are divided into 3 parts, a conical portion that is inserted into the sclera, a cylinder portion that remains in the vitreous, and a neck portion between the pin and the cylinder. The tapered conical end was manufactured to allow easy insertion through the retina and choroid into the sclera. A cylinder portion was manufactured with a tapered angle that fixes firmly into the orifice of 19 gauge spinal needle. A neck portion, 0.4 mm in diameter, was designed to prevent disinsertion from following implantation of retinal tack. The applicator was a 19 gauge spinal needle and its orifice was prepared to 15°angle to accept the tapered cylinder portion of the retinal tack. The retinal tacks, secured in the needles, were passed through the formed vitreous and inserted into the retina, choroid, and sclera and were released by pushing the internal needle, usually within 2-3mm of the medullary ray of the posterior rabbit retina A retinal tack was placed in each of 8 pigmented rabbit eyes. Slit-lamp biomicroscopy, indirect ophthalmoscopy and fundus photography were performed periodically from 1 day to 8 weeks after surgery. Eight eyes were enucleated and studied by light microscopy at 8 weeks. Biomicroscopic evaluation of the animals revealed edemas adjacent to the retinal surfaces immediately after insertion of the biodegradable retinal tacks in all the animals. These edemas disappeared after 1 week. The first noticeable change in the size of retinal tacks was shown after 2weeks. The size of the retinal tacks gradually got smaller, decreasing to about one-half at 4 weeks and about one-third at 8 weeks. All retinal tacks remained in inserted places without any movement for an 8 week period. On light microscopy, epiretinal proliferations were seen to extend into the vitreous cavity. Cellular capsules that lined the inner aspect of the scleral defect caused by tack insertion were found. However the adjacent retina had a normal cytologic appearance and architecture in all specimens. We manufactured a biodegradable retinal tack that is designed to prevent intrusion from implantation of retinal tacks. All biodegradable retinal tacks reduce in size with time, but no retinal tacks extruded from the inserted place. The newly designed biodegradable retinal tack can be used for retinal fixation and may be used as a vehicle for the introduction of pharmacologic agents to prevent the cellular events that promote proliferative vitreoretinopathy.

        A new device, muscle clamping system was developed to facilitate exact quatifying technique A strabismus surgery, and reduce the risk of complications. The device is composed of a lower fixing body with three jaws and an upper supporting body. They are used to clip an extraocular muscle and fix it to the sclera with a single bite. Superior rectus recession on 16 rabbit eyes were performed with this new device. Conjunctival injection, muscle adhesion strength, and light microscopic findings were examined at 1, 2, 4 and 8 weeks postoperatively. The Conjunctival injection were minimal, adhesion power ranged from 420 to 600 gram gravity, which is sufficient in withstand the normal pull of human extraocular muscle. In microscopic exam, some Inflammatory cells and fibrosis were found. The new device was technically easy, fast, and accurate, so it may be useful in stabismus surgery.

      • 진동삼축시험을 이용한 실트질 모래의 액상화강도 산정

        국내 해안에 퇴적한 실트질모래와 hydraulic fill한 지반에서 채취한 시료의 액상화강도 측정을 위해 응력제어 반복삼축압축시험을 수행하였다. 이를 위해 1)불교란시료와 교란시료의 액상화강도 산정과 2)등방압밀된 시료와 비등방압밀된 시료의 반복삼축시험결과 비교, 3)본 시험결과와 국내 다른 시험자료 및 외국의 결과를 비교하였다. 불교란시료의 경우 반복횟수 N이 10회 일 때 액상화강도는 교란시료에 비해 약 두 배 정도 크게 나왔다. 비등방압밀된 시료의 액상화강도는 수직응력을 구속응력으로 나눈 유효압밀비에 영향을 받는다. 유효압밀비가 증가함에 따라 액상화 강도는 증가하여, 유효압밀비가 1.2와 1.5인 경우 유효압밀비가 1인 경우보다 각각 24%와 56% 정도 증가되었다. 본 시험결과는 국내 다른 연구자들의 시험과 비교했을 때 같은 상대밀도에서 약간 큰 경향을 나타냈으며, Seed and Idress(1971)가 제안한 입경별 액상화강도 추세선으로 비교하였을 때도 약간 큰 강도를 보이고 있다. Dynamic triaxial tests were performed in order to evaluate the liquefaction potentials of silty sands and hydraulic fill ground. In the tests, (1) undisturbed and disturbed samples were prepared for the tests, (2) dynamic shear strengths were measured under isotropic and anisotropic condition and (3) the test results were compared with the other results which were tested by domestic and foreign researchers. The test results of undisturbed samples showed the dynamic shear strength twice larger than those of disturbed samples at the 10 cycles loading. The dynamic shear strength of anisotropically consolidated samples were affected by the effective consolidation ratio which is the rate of vertical effective stress divided by confining stress. The dynamic shear strength at the effective consolidation ratio of 1.2 and 1.5 increased by 24% and 56%, respectively, comparing to the strength at the ratio of 1. These test results showed that the dynamic strengths were a little larger than those of other domestic researchers, and Seed and Idress(1971).

      • Pentaerythritol Tetrabenzenesulfonate 와 (1R, 2R)-Diaminocyclohexane을 이용한 광학활성 Polyamine 의 합성과 특성

        광학활성 polyamine을 합성하기 위하여 pentaerythritol[2,2-bis(hydroxymethyl)propane -1, 3-diol]tetrabenzenesulfonate와(1R, 2R)-diaminocyclo hexan을 반응시켰다. 그리고 polyamine 생성물을 분리하기 위하여 코발트(Ⅲ)착물을 합성하고 Dowex 50W-X2 양이온 교환 수지를 이용하여 크로마토그래피를 행하였다. 코발트(Ⅲ)착물의 결정구조 해석에 의하여 3,3-Bis(2-aza- (1'R, 2'R)-2'-arninocyclohexyl)-heptahydro- (1"R, 2"R)-1,4-cyclohexanodi azepine (L¹)이 확인되었으며, 착물 내에서 고리 내의 질소원자는 배위되지 않고 양성자화 되어있으며, 초록색의 단핵분자인 trans- dichlorotetramine 착물, trans-[CoC1₂ (H2L1)]Cl₃·4.5H₂O이 결정되었다 Co(Ⅲ) 주위의 내부 배위권은 네자리의 선형 tetramine이 적도면을 점유하며, 두 개의 chloro 리간드가 꼭지점을 점유하는 대략적인 괄면체이다. Co-Cl 결합길이는 2.261(1)Å 및 2.249(2)Å 이다. 금속이온 주위의 결합각은 비교적 스트레인을 받지 않으며 모든 trans각도는 176˚ 보다 크다: 또한 Co-N 결합길이 1.941(4)∼1.986(4)Å는 일반적으로 스트레인 을 받지 않는 trans-(CoCl₂N₄)^(+) 종의 각과 비슷하다. 두 개의 5원 킬레이트고리의 배치는 gauche이며 절대배치는 λ 이다. 한 개의 6원 킬레이트 고리는 skew-boat 배치이다. 비대칭 2차 질소의 배치는 모두 S이다. In order to synthesise the optically-active polyamine, pentaerythritol [2,2-bis(hydroxymethyl)propane-l,3-diol] tetrabenzenesulfonate and (1R, 2R)-diamino cyclohexane have been treated. And in order to separate the polyamine product, cobalt(Ⅲ) complex has been synthesised and chromatographied by Dowex 50W-X2 cation exchange resin. 3,3-Bis{2-aza- (1'R, 2'R)-2'-aminocyclohexyl}-heptahydro-(l"R, 2"R)-l,4-cyclohexanodiazepine(L¹ ) has been confirmed by crystal-structure determination on its cobalt(Ⅲ) complex, where the ring nitrogen atoms are uncoordinated and protonated, as a green, mononuclear trans- dichlorotertramine complex, trans-[CoCl₂(H₂L¹) ]Cl₃·4.5H₂O. The inner coordination spher around Co(Ⅲ) is approximately octahedral, with the tetradentate linear tetramine occupying the equatorial plane and two chloro ligands the apices. Co-Cl bond lengths are 2.26KDA and 2.249(2)Å. The angular environments about the metal relatively unstrained, all trans angles being greater than 176˚: the Co-N bond lengths, 1.941(4)-1.986(4) A, also are closely clustered around values typical of unstrained trans-[CoCl₂N₄]^(+) species. The conformation of two five-membered chelate rings are gauche, with absolute configuration A. The one six-membered chelate ring assume the skew-boat conformation. The configuration of the asymmetric secondary nitrogen atoms are both S.

      • 방사선 촬영실내 세균 오염도에 관한 연구

        As a result of measuring the bacterial contamination level of radiographic room, It was examined 27 of Gram (+) cocci, 1 of Gram (-) Bacillus was found in the air of a cubic meter (㎥) radiographic room. As a result of measuring the radiographic table, a large number of Gram (+) cocci was detected in the B, F and Gram (+) Bacillus was detected in the F, G hospital. as a result of measuring the radiographic cassette, a large number of Gram (+) cocci was detected in the B, G hospital and Gram (+) Bacillus was detected in the B, G and D, E hospital. After independent culturing and monitoring, it was clear up the D, E, F, G hospital Gram (+) cocci was the simple Gram (+) Bacillus. On the contrary, the D, G hospital appeared saprophytic Staphylococcus epidermis. This study verifies that a number of saprophytic bacteria can survive in the air of the radliographic room, radiogrphic table and radiograhic cassette. There was the hospital which is uncountable, and was very detected. These results notified all visiting person and patients could be infected from the baterial contamination in radiographic room. Also, it means that all visitors could be changed baterial transmitter. It is provides comfortable environment and better medical service to the disinfection that minor filming is indoor and personal hygiene in order to prevent this bacillary infection and must listen to the safe department of radiology on bacillary infection in ten thousand. This study verifies that common potentially pathogenic bacteria can survive in radiographic processing room. Therefore, effective cleaning regimens are of fundamental importance in controlling and preventing potential nosocomial transmission in the diagnostic imaging department

      • 뇌 수막종의 Volume Rendering 영상과 고식적 혈관 조영술의 영상 비교

        This study showed following results. The time interval for maximal enhancement of meningioma was 120∼180 sec after injection of contrast material. All 12 cases showed good relationship between the tumor and surrounding vessels. A case of aneurysm associated with tumor was also well visualized. Three-dimensional volume rendering imaging of meningiomas obtained by using double contrast-enhancement technique was effective in the evaluation of relationship between the tumor and adjacent vascular structure. This technique was considered to be useful in making the preoperative plan.

      • Multiple myeloma 환자에서 Bortezomib치료 후 발생한 마비성 장폐색증 1례

        Bortezomib is the first clinically available proteasome inhibitor that is clinically effective in the treatment of multiple myeloma. A proteasome inhibitor acts through multiple mechanisms to arrest tumor growth, tumor spread, and angiogenesis. The main adverse effects of bortezomib are gastrointestinal symptoms, cytopenia, fatigue, and peripheral neuropathy. To date, severe paralytic ileus has not been reported as a toxic effect of bortezomib treatment in multiple myeloma. Bortezomib is a novel agent that has only been used clinically for 30 months, so a need exists to further evaluate its toxicity. We report a case of grade Ⅲ (NCI CTCAE v3.0) or grade Ⅳ(SWOG toxicity criteria) paralytic ileus in a 65-year-old man with relapsed multiple myeloma who underwent one cycle of single-agent bortezomib treatment scheduled in a 21-day cycle (1.3 mg/㎥ as a single i.v. bolus on days 1, 4, 8, and 11).

      • 용혈성 요독 증후군이 합병된 출혈성 장염 1예

        저자들은 쇠고기에 의해 매개된 것으로 추정되는 출혈성 장염 및 이에 합병된 용혈성 요독 증후군 환자 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. We report case of hemolytic uremic syndrome(HUS) associated with hemorrhagic colitis. A 35-year0old man was admitted to a local hospital because of abdominal pain and watery diarrhea which had developed about 6 hours after taking roast beef. He was treated with intravenous fluids and antibiotics, but watery diarrhea changed to bloody in nature from the next day. He was transferred to our hospital due to progressive ascites and jaundice on his 8th day of illness. Examinations revealed ascites, jaundice, microangiopathic hemolytic anemia, thrombocytopenia and uremia. Sorbitol-negative Escherichia coli was isolated from his stool, which proved as Shiga-like toxin-negative E. coli, serotype O25. His conditions improved markedly after three times of plasmapheresis and intravenous fluids therapy, and the organism was not isolated from the follow-up stool culture.
