Primary endometrial squamous cell carcinoma is extremely rare, with only 64 cases repo- rted in the literature. In 1928 Fluhmann proposed three criteria for differentiating primary from secondary endometrial squamous cell carcinoma:(1) no coexisting endometrial adenocarcinoma (2) no connection between the endometrial tumor and the squamous epithelium of the cervix, and (3) no squamous cell carcinoma of the cervix present. Several authors have modified these criteria in their case presentations. Kay accepts the presence of an in situ cervical carcinoma, if there is no connection between cervical carcinoma in situ and the endometrial tumors. We have experienced a case of simultaneous carcinoma in situ of the cervix and endomet- rial squamous cell careinoma. This case was treated with chemotherapy(cisplatin, 5-fluoruracil) and external radiation following extended hysterectomy and bilateral salpingooophorectomy We report this case with a review of the concerned literatures.

Abdominal pregnancy is a rare and life threatening variant of ectopic pregnancy with high maternal mortality and perinatal mortality. The clinical characteristics of abdominal pregnancy is extremely variable, so early diagnosis is very difficult. We have experienced a case of early abdominal pregnancy diagnosed at emergency laparotomy. We report a case of early abdominal pregnancy with a review of literatures.
자궁적출술을 시행한 폐경 후 여성에서 혈청지질 및 지단백질치에 미치는 에스트로겐 보충요법의 영향

Estrogen replacement therapy in postmenopausal women has been known to reduce the risk of cardiovascular disease, and this effect is thought to be mediated in part by favorable changes in serum lipids and lipoproteins. The other cardiovascular protective effects of estrog-ens are vasodilation, anticoagulation and an antiatherosclerosis effects on blood vessels. To elucidate the main effects of estrogen on the levels of serum lipids and lipoproteins, we conducted the randomized trials of oral and transdermal estrogens in healthy hysterectomized postmenopausal women. In this study, the first group(57 patients) received Premarin(conjugated estrogen) 0.625 mg per day. And the second group(53 patients) received transdermal estradiol patches(Estraderm TTS 50) twice weekly. Serum total cholesterol, triglyceride, HDL cholesterol, and LDL cholesterol levels in fasting state were measured in all subjects before treatment and 6 months after treatment. In both groups, serum total cholesterol levels and serum LDL cholesterol levels were decreased significantly after 6 months of treatment. Also, serum HDL cholesterol levels were increased significantly after treatment. Serum triglyceride level was increased in group 1 and decreased in group 2, but there were no statistical significances. The degrees of change were not differ significantly in both groups. These data suggests that the postmenopausal use of estrogens in hysterectomized women, administered by either oral or transdermal route favorably alters serum LDL and HDL levels that may protect women against cardiovascular disease and there are no significant differences statistically between two regimens on the serum lipids and lipoprotein levels. Considering the costs, the side effects and the compliance of patients on the estrogen replacement therapy, oral forms of estrogen are thought to be the first line of therapy in hysterectomized postmenopausal women.

저자들은 연세대학교 원주의과대학 산부인과학교실에서 수술후 병리학적으로 확진된 자궁경부의 유암종 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Carcinoid tumor of the uterine cervix is uncommon neoplasm and clinical behavior of this tumor was extremely aggressive. It`s diagnosis was based on histological and immunohistochemical findings. Because carcinoid tumor of the cervix is so uncommon, it`s natural history has not been well documented. Despite the early stage in these patients, systemic metastasises are common and the dismal outcome indicates the traditional modes of therapy for cervical carcinoid tumor are not effective. We report a case of carcinoid tumor of the uterine cervix with review of the literature of carcinoid tumor.

자궁경부 세포검사에서 비정상소견을 보인 환자에서 원추생검술을 시행하여 그 검체의 심부에서 중신관 잔유물(mesonephric duct remnant)이 확인되어 자궁적출술을 고려하였던 환자에게 더이상의 수술을 진행하지 않았던 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. The mesonephron is regressed and the uterus is formed by fusion of the Mullerian ducts by the 8th gestational weeks. The mesonephric ducts are occasionally involved in the lower corpus and cervix of the uterus. Mesonephric duct remnants in the uterine cervix is an unusual condition that is often misdiagnosed as adenocarcinoma. Mesonephric hyperplasia does not appear to require therapy, even if it is found at the resection margin of a conization or hyterectomy specimen. It is important to distinguish benign mesonephric lesion from malignancy, to avoid overtreatment.

Carcinoid tumors, producing biogenic amines and various polypeptides, arise from the diffuse peripheral endocrine system. They are most frequently localized in different parts of the gastrointestinal tract and less frequently in bronchi, biliary tract and ovaries. Carcinoid tumors of the ovary are classified into two categories: primary and metastatic. The primary carcinoid tumors are classified as follows: insular or islet carcinoid(carcinoid tumors of the midgut type); trabecular carcinoid (carcinoid tumors of the foregut or hindgut type); struma carcinoid (struma ovarii and carcinoid or carcinoid tumor combined with thyroid tissue). Primary trabecular carcicnoid of the ovary is an uncommon ovarian tumor, begaving like a tumor of low malignant potential. In the younger patient the treatment is unilateral salpingo-oophorectomy, while in the postmenopausal paient bilateral salpingo-oophorectomy and hysterectomy is the treatment of choice. Long term follow up is advisable. Authors have experienced a case of primary trabecular carcinoid tumor in mature ovarian teratoma and reported with a brief literature review.
